4.A Study of Physical Disorder in a Geriatric Psychiatric Inpatients.

BACKGROUND: This study was designed to investigate the relationship between psychiatric disease and systemic disease in geriatric psychiatric inpatients who were consulted to department of internal medicine and were above 6th decade, and was directed to assess the physical problems and possible ways to resolve them in a closed psychiatric unit. METHOD: Through evaluation of medical records of 225 inpatients who had consulted in department of internal medicine in Seoul National Mental Hospital from January 1, 1998 to December 31, 1999. We classified the physical illness according to ICD-9-CM and psychiatric disease according to NTA. RESULTS: The result were as follows: 1) In age sex distribution, male was 62.7%, and 81.8% of the subjects was 6th decade. 2) The most common systemic diagnosis were disease of circulatory system, disease of digestive system, and disease of respiratory system. 3) The most systemic disease was disease of circulatory system in 6th and 8th decade. 4) Disease of circulatory system in all psychiatric disease was the most frequent. 5) Rate of circulatory disorder was the highest in organic mental disorder. Disease of digestive system and disease of endocrine system were the most frequent in alcoholics. Disease of respiratory system showed the highest rate in schizophrenea. CONCLUSION: The clinical characteristics of psychiatric diseases influenced to get systemic disease in geriatric patients.

5.Production of IFN-gamma by HBsAg - reactive T cells correlates with viral clearance in HBV infection.

The T cell responses to hepatitis B surface antigen (HBsAg) were analyzed in acute hepatitis patients, chronic active hepatitis (CAH) patients and asymptomatic carriers. Neither proliferative responses nor substantial cytokine production of peripheral blood mononuclear cells (PBMC) in response to HBsAg was detected. For further studies, HBsAg- reactive T cell lines were prepared from PBMC of the hepatitis patients and asymptomatic carriers. No proliferative response of the T cell lines was observed. Interestingly, however, T cell lines obtained from acute hepatitis patients were found to produce IFN-r, but not IL- 4, in response to HBsAg stimulation, whereas T cell lines obtained from CAH patients and carriers were not. Results of this study suggest that HBsAg-reactive T cells producing Thl type cytokines may play an important role in the viral clearance during acute infections, while defects in those T cells may be responsible for the viral persistency.

8.Effect of polyadenylic.polyuridylic acid on the proliferative responsiveness of mouse thymus and spleen cells.

The effects of polyadenylic.polyuridylic acid [poly(A).poly(U)] on in vitro proliferations of thymus and spleen cells from C57BL/6 mice were investigated. Mice were injected intravenously with 30 micrograms of poly(A).poly(U) or placebo. Two days later, thymus, spleen and peritoneal cells from these mice were prepared and cultured in pooled or non-pooled conditions. Cell proliferations were assessed by the technique of incorporation of tritiated thymidine. It has been revealed that the in vitro proliferations of thymus and spleen cells as well as the productions of interleukin-1 by peritoneal adhering cells and interleukin-2 by spleen cells were significantly enhanced in the cultures of cells from poly(A).poly(U)-treated mice. These enhancing effects were observed only in the cultures of pooled cells from mice whose genetic homogeneity is suspected. Furthermore, thymus cells from poly(A).poly(U)-treated mice acted as strong responder cells but not as stimulators in one way mixed cultures. Thus, the enhanced cellular responsiveness may be mediated by the increased production of cytokines and antigen recognitions of thymus-derived cells following activations via the adjuvant effect of poly(A).poly(U).

10.A Case of Actinic Reticuloid.

Actinic reticuloid as a manifestation of chronic actinic dermatitis (CAD) is a rare dermatosis whose clinical and histologic features resemble other types of pseudolymphomas including mycosis fungoides and Jessner's lymphocytic infiltration, and it is regarded as an eventual stage of various photodermatoses like photosensitive eczema or persistent light reaction or chronic photoallergic contact dermatitis and so on. Phototests in the patients with actinic reticuloid usually reveal hypersensitivity to UVB, UVA, and sometimes to visible light. We present a case of actinic reticuloid in a 65-year-old male, whose skin lesions developed as erythematous lichenified infiltrating plaques on the face at first, and then spread themselves onto the upper trunk later. Histologically those skin lesions showed the aggregation of atypical lymphocytes and photobiologically the results of phototests revealed photosensitivity to UVB and UVA.
