4.The Effect of Mitomycin on the Experimental Filtering Surgery.

In tiis study, an expenmental model of glaucoma filtering surgery in rabblts was used to evaluate the effect of single intraoperative application of Mitomycin Full-thickness filtering surgery, posterior lip sclerectomy, was performed after application of 0.5mg/ml Mitomycin-soaked Week-Cel(TM) sponge for 5 minutes in one eye of 15 pigmented rabbits and was irrigated copiously with balanced salt solution(BSS(TM)). The same procedure was done in the contralateral eye as a control, using BSS(TM). In Mitomycin-treated group, the filtering blebs were more prominent with sparse vascular tissues and were maintained through postoperative 8 weeks or more. Histologically, there loas was amount of inflammatory reactions in Mitomycin-treated group. The fibroblastic activity and proliferations of the collagen were also suppessed markedly in Mitomycin-treated group. These results suggest that a single intraoperative application of Mitomycin can be used adjunctively to improve the success rate of glaucoma filtering surgery.

5.Choriocarcinoma of the Colon.

Choriocarcinoma of the gastrointestinal tract is rare. Among them, that of the stomach is the most common. Six cases of choriocarcinoma of the colon were found in the review of the literature. All of these previously reported cases had multiple metastatic foci in the liver, lung, lymph nodes and the prognosis seemed to be very poor. Therefore we think that choriocarcinoma of the colon should be distinguished from conventional adenocarcinoma. A 66-year old female patient, described in this case, was operated on under the impression she was suffering from acute appendicitis. The resected ascending colon revealed extensive hemorrhagic necrosis and perforation with fibrous adhesion in the cecum. On the cut section, the mural tumorous thickening was not definite. Histologically, the tumor showed a focus of typical adenocarcinoma arising from glandular epithelial cells, which were transformed into highly anaplastic tumor cells. There were frequent vascular invasions of tumor cells, similar to syncytiotrophoblasts. In the immunohistochemical stains, both glandular and highly anaplastic tumor cells reacted with cytokeratin. The glandular cells were also reactive for carcinoembryonic antigen (CEA) and anaplastic tumor cells for human chorionic gonadotrophin (hCG). This is the first report of choriocarcinoma of the colon in Korea. We describe this case with a review of the literature.

6.Effects of Thera-Band(R) Resistance Training Including Isometric Exercise in Total Knee Replacement Patients.

BACKGROUND: The purpose of this study was to assess the effects of an exercise program using isometric and isotonic thera band exercises. METHODS: Data were collected from 30 total knee replacement patients hospitalized in the orthopedics department at 'B' hospital (control=15, experimental=15). The control group performed isometric exercises for one week while the experimental group performed three or more rounds of both isometric and isotonic exercises, including Thera-Band(R) exercises, everyday for four weeks. RESULTS: The experimental group improved significantly in muscular strength (Z=-1.99, P=0.047) and self-efficacy (Z=19.48, P=0.000), and showed lower levels of depression than the control group (Z=8.60, P=0.003). However, no difference was found in pain experience between the two groups. CONCLUSIONS: Exercise programs using the isometric and Thera-Band(R) resistance training improve muscular strength and self-efficacy and relieve depression in total knee replacement patients.

7.The Correlations between Cardiorespiratory Fitness Levels and Body Mass Index, Metabolic Syndrome Risk Factors, Homeostasis Model Assessment-Insulin Resistance and High Sensitivity C-Reactive Protein in Male High School Students.

BACKGROUND: The purpose of this study was to analyze the relationships between cardiorespiratory fitness and body mass index (BMI), metabolic syndrome risk factors, homeostasis model assessment-insulin resistance (HOMA-IR), and high sensitivity C-reactive protein (hsCRP) in male high school students in Korea. Furthermore, the effects of the cardiorespiratory fitness levels on these four factors were examined. METHODS: The level of cardiorespiratory fitness, BMI, percent body fat, waist circumference (WC), systolic blood pressure, glucose, triglycerides (TG), high-density lipoprotein-cholesterol (HDL-C), HOMA-IR, and hsCRP of 227 male high school students were measured. Analyses of multiple correlation and simple regression were applied. RESULTS: The results showed that the level of cardiorespiratory fitness correlated negatively with BMI, percent body fat, WC, TG, and HDL-C, in addition to HOMA-IR and hsCRP, and strongly influenced BMI, percent body fat, WC, TG, HDL-C, HOMA-IR, and hsCRP. Comparative analysis of our subjects showed that higher cardiorespiratory fitness levels had positive effects on BMI, metabolic syndrome risk factors, HOMA-IR, and hsCRP. CONCLUSIONS: This study found that there are close correlations between cardiorespiratory fitness and BMI, metabolic syndrome risk factors, HOMA-IR, and hsCRP. It also showed that, compared to lower levels of cardiorespiratory fitness, higher levels had beneficial effects on BMI, metabolic syndrome risk factors, diabetes, and cardiovascular disease risk factors.

10.The Relationship between Noise and Sleep Patterns in Intensive Care Units.

The environment in the ICU leads to negative changes in a patient's usual sleep pattern and so contributes negatively to the patient's health condition as compared to patients in general wards. Therefore, it is thought that an important nursing intervention would be to identify the relation between noise and sleep patterns which play an important role in illness recovery. The purpose of the present study was to explore the relationship between noise in the ICU and the sleep pattern of patients admitted to the ICU. A descriptive correlation design was used to examine the relationship. Thirty-four subjects were recruited from a Medical ICU (MICU), Surgical ICU (SICU) and Coronary Care Unit (CCU) at a large university hospital in Suwon. Data were collected from September 28 to October 31 in 1999. In the present study, noise was categorized into noise level and patients' perception of noise. The objective noise level was measured using the A-Weighted Sound Level Meter. The patients' preception of noise was measured using a self-reported questionnaire developed by the researcher. Sleep patterns in this study includes both quantity and quality of sleep. These were measured using open ended questionnaires and the 'Korean Sleep Scale A' developed by Oh, Song, Kim(1998). The data was analyzed using the SPSS-WIN to test the research question, Pearson product moment correlation coefficient was run. Ancillary analysis were conducted with demographic variables to determine their relation to the main study variables. For the ancillary analysis, t-test and one-way ANOVAs were performed. The results of the present study are summerized as follows: 1. The total mean of objective noise level (10pm-6am) was 56.2dB. The means for night time noise level in individual ICUs for the SICU, MICU and CCU, were 58.7dB, 58.6dB and 48.3dB, respectively. The total mean for patients' noise perception was 42.8 out of a maximum possible score of 76. For item means of noise perception, the one ranked highest was "conversations between doctors and nurses" (3.2). The one ranked lowest was "noise from the radio" (1.2). Regarding the degree of perception for each type of noise source, the one ranked highest was "equipment noise" (2.6), the second was "conversation between medical staff" (2.4), the third was "conversation between patients, caregivers and visitors" (2.3), and the one ranked lowest was "environment noise" (1.8). 2. Looking at quantity of sleep of ICU patients, the mean nocturnal sleep time was found to be 4.9 hours. The total mean of sleep quality for ICU patients was 21.0 out of a maximum possible score of 40. 3. The relationship between perception of noise and quantity of sleep was statistically significant(r= - .41, p<.05). The relationship between perception of noise and quality of sleep was also statistically significant(r= - .47, p<.01). The results of the study indicate that personal perception of noise is related to sleep patterns. Therefore, it is suggested that nursing interventions be developed to reduce the degree of personal perception of noise and, thus, decrease sleep pattern disturbances in patients in the ICU.
