1.The Study of Identification of Methicillin-Resistant Staphylococcus Aureus using Polymerase Chain Reaction.

BACKGROUND: Rapid and accurate identification of methicillin-resistant Staphylococcus (MRSA) is very important for patients because they are one of the most common etiologic agents of hospital infection. Conventional identification methods for MRSA are influenced by various factors such as pH, concentration of salt, conditions of media. METHODS: 53 methicillin resistant staphylococcus strains identified by ATB plus system (Biomerieux, France) were preformed the polymerase chain reaction (PCR), Southern blot hybridization fort the detection of mec A gene, and subcultured in Meuller-Hinton media containing 4 microgram/mL oxacillin for the comparison. RESULTS: The correlation of detection rate of mec A gene PCR and ATB plus systems was 81.6%. The correlation of mec A gene PCR and MRSA on Mueller-Hinton media containing 4 microgram/mL oxacillin was 80%. We confirmed by Southern blot hybridization the amplified mer A gene originated from chromosome of MRSA. As the results of oxacillin sensitivity test, minimal inhibitory concentrations of MRSA were distributed between 40 microgram/mL and 320 microgram/mL. When compared with executing time, ATB plus system took 24 hours, but PCR took 5 hours for identification. CONCLUSION: We concluded that mec A gone PCR techniques were simple and rapid for detection of MRSA comparative to conventional methods.

2.Two Cases of Partial Unilateral Lentiginosis.

Partial unilateral lentiginosis(PUL) is a rare pigmentary disorder charaeterized by lentigines limited to one side of the body with or without neurologic abnormalities. We report two cases of PUL unassociated with any other defect, One patient was 20-year-old female who had lentigines confined to the right side of the face and the other was 19-year-old male who had lentigines confined to the left side of the neck and upper trunk and left upper extremity. Histologic examination of the pigmented lesion revealed findings consistent with lentigo simplex.

3.A Case of Sebaceous Adenoma.

Sebaceous adenoma is a benign organoid tumor of proliferating, incornpletely differentiated sebaceous glands. We report a case of sebaceous adenoma in a 15-year-old male. He had a 1.7 x 2.2 cm sized, asymptomatic, srnooth, round, raised, firm, reddish tumor on the left quadrant of abdomen. Histopathologically, the tumor was composed of lobules of irregular size and shape, and sharply demarcated from the surrounding tissue, The most lobules showed undifferentiated germinative and mature sebaceous cells in about equal number. Sebaceous cells in lobules were stained for prekeratin by means of avidin-biotin-peroxidase complex method. Ten months after surgical excision there was no evidence of recurrence.

6.A Case of Treatment of the Drug-Resistant Auditory Hallucinations of a Schizophrenic Patient with Continuously Application of rTMS and ECT.

The Persistent auditory hallucinations may provoke the uncomfortable feeling and can be dangerous according to its contents in treatment-resistant schizophrenic patients. Previous functional brain imaging studies on auditory hallucinations reported that the abnormal activations of the normal auditory pathway and the activations of the left upper temporal lobe were found while patients were hallucinating voices. Base on the these findings, we treated a female schizophrenic patient with persistent auditory hallucinations that were treatment-refractory with rTMS and the ECT continuously while she was on the drug treatment. The total amount of pulses applied in each trial ranged from 240 to 1000pulses. Fourteen trials were done and resulted in decrease by 30% in the subject severity of auditory hallucinations and the improvement of the affect. Two weeks after the completion of rTMS trials, nine trials of ECT were administered which leaded to complete disappearance of auditory hallucinations. This result suggest that ECT is more effective than rTMS in treating persistent auditory hallucinations in treatment-resistant schizophrenic patients.

7.A Clinical Study of Congenital Syphilis (in the View of Bone Change)

Syphilis was known to be generalized disease and it had been decreased until 1960's. But, it has been increased again recently. The author analysed the 22 cases of congenital syphilis which were treated at department of orthopaedic surgery at Ewha Womans University Hospital from Jan. 1979 to July 1983 and the following results were obtained. 1. There were no significant changes of the number of patient by years. 2. Common symptom and sign were hepatosplenomegaly(84%), fever(62%) and skin changes(50%), and pseudoparalysis was observed in one case. 3. Radiologic bone changes were observed in 11 cases (50%) and the following results were obtained: a. Below the two week old, there was relatively low incidence of bone change with 22%. b. There was high incidence of bone change in prematurity with 75%. c. Common features of bone changes were periosteal reaction, osteochondritis and metaphysitis (91%). Pathologic fracture was observed. in one case. 4. Although we have observed the bone changes only in 50% of cases, all the other reports has reported more than 80% and we thought that radiologic bone change would aid the diagnosis of congenital syphilis.

8.Ultrasonographic findings of pelvic masses

Ultrasonography affords an accurate assessment of the presence, size, location and internal consistency of apelvic mass. We present our experience of 105 cases of pelvic masses in female patients by ultrasonography, andthe results are as described; 1. Ultrasonography is nonspecific, however, the possibility of mucinous cystadenomaof ovary, teratoma(dermoid cyst), uterine leiomyoma and trophoblastic disease can be suggested due to theircharacteristic internal consistency. 2. This smooth internal septation seen within mucinous cystadenoma, eccentricsolidmural component of dermoid cyst, enlarged uterine wall with lobulated outer margin of leiomyoma, andyesicular appearance of trophoblastic disease were the specific sonographic features. 3. Loculated ascites and hydronephrosis were identified with malignant ovarian neoplasm and pelvic metastasis. 4. Accurate ultrasonographicdiagnosis was made in 88 cases (83.7%) and it may be improved with clinical history of the mass in question.

9.Two Cases of Malignant Fibrous Histiocytoma.

Malignant fibrous histiocytoma(MFH) was first introduced referring to a group of soft tissue tumors characterized by a storiform or cartwheel-like growth pattern. MFH is an aggressive soft-tissue sarcoma that most commonly occurs in the skeletal muscle of the extremities or retroperitoneum of adults. We report 2 cases of MFH occurring in 59-year-old man and 57-year-old woman. In the first case, the tumor occurred in right upper arm and has been removed totally. It was consisted of storiform, pleomorphic and myxoid area. In the second case, a painful nodule developed in right shin where the tumor had been excised 1 year ago. A biopsy revealed plump spindle cells appearing in a storiform-pleomorphic pattern.

10.Reversibility of Endoscopic Features after Treatment for Eosinophilic Esophagitis

Purpose:
The prevalence and incidence of eosinophilic esophagitis (EoE) are increasing worldwide. Despite increased understanding of inflammatory pathogenesis, changes in endoscopic features after treatment of EoE have not been clearly described.We aimed to investigate the reversibility of endoscopic features of EoE after treatment.
Materials and Methods:
Out of 58 adult subjects who were diagnosed with EoE at the Yonsei University Health System from July 2006 to August 2019, we recruited 33 subjects (30 males; mean age: 42 years) whose pre-treatment and post-treatment endoscopic images were available. Endoscopic features included both inflammatory and fibrostenotic features. Exudate, edema, furrow, and crepe paper-like mucosa were classified as inflammatory features. Ring and stricture were classified as fibrostenotic features. We compared changes in endoscopic features after treatment for EoE.
Results:
After treatment, clinical symptoms improved in all patients. The following endoscopic features were observed before treatment: furrow (81.8%), edema (90.9%), exudate (42.4%), ring (27.3%), crepe paper-like mucosa (15.2%), and stricture (3.0%).Endoscopic remission was achieved in 21 patients (63.6%). Inflammatory features were reversible (72.7%, p<0.001), whereas fibrostenotic features were not (10%, p=0.160). Exudate had resolved in 92.9% of patients, edema in 70% and furrow in 88.9%. Ring and stricture persisted in almost all of the patients (9/10) who had these endoscopic features before treatment.
Conclusion
We outlined the reversibility of endoscopic inflammatory features of EoE. Fibrostenotic features were irreversible after esophageal remodeling in patients with EoE. However, further validation studies with long-term follow-up are needed.
