5.The Influence of Uncertainty and Social Support on General Well-being among Hemodialysis Patients.

PURPOSE: The purpose of this study was to explore factors affecting uncertainty and general well-being based on Uncertainty in Illness Theory. METHODS: Data were collected from 125 outpatients who had received hemodialysis. The path model among four concepts, such as period of hemodialysis, social support, uncertainty, and general well-being, was tested. Tangible support, positive social interaction, affectionate, and emotional/informational support were measured as social support. Adaptation in the model was operationalized as general well-being which consisted of anxiety, depression, positive well-being, self-control, and general health. RESULTS: All paths were statistically significant at the level of alpha=.05. The significant paths were the path from period of hemodialysis to uncertainty (t=-2.86), social support to uncertainty (t=-2.01), uncertainty to general wellbeing (t=-2.85), and social support to general well-being (t=3.55). CONCLUSION: Patients who perceived low uncertainty and high social support were likely to feel well-being. Therefore, nurses should give patients appropriate information according to their needs and have meaningful interaction with patients to reduce their uncertainty and render social support.

6.A Case of Sebaceous Adenoma.

Sebaceous adenoma is a benign organoid tumor of proliferating, incornpletely differentiated sebaceous glands. We report a case of sebaceous adenoma in a 15-year-old male. He had a 1.7 x 2.2 cm sized, asymptomatic, srnooth, round, raised, firm, reddish tumor on the left quadrant of abdomen. Histopathologically, the tumor was composed of lobules of irregular size and shape, and sharply demarcated from the surrounding tissue, The most lobules showed undifferentiated germinative and mature sebaceous cells in about equal number. Sebaceous cells in lobules were stained for prekeratin by means of avidin-biotin-peroxidase complex method. Ten months after surgical excision there was no evidence of recurrence.

7.Nutritional status and plasma amino acid profile in maintenance hemodialysis patients.

BACKGROUND: Many studies document the presence of abnormalities in amino acids metabolism in chronic uremia. These abnormalities have been attributed to low protein intake, deficiency of excretory and metabolic functions of the diseased kidneys, toxic effects of uremia on the intermediary metabolism of amino acids and in dialysis patients, loss of protein and amino acids by the dialytic procedure. METHODS: This study was designed to compare anthropometric measurement, biochemical characteristics and plasma amino acid concentration between patients with end stage renal disease on maintenance hemodialysis (HD) and normal controls. A cross sectional study of overnight fasting plasma amino acids and plasma albumin, prealbumin, triglyceride (TG), cholesterol, transferrin concentration were performed on 20 hemodialysis patients and 20 normal controls, matched by age and sex. RESULTS: The concentrations of prealbumin (25.60+/-7.05 mg/dL vs 35.08+/-8.11 mg/dL, p<0.005), transferrin (158.30+/-39.66 mg/dL vs 275.50+/-55.46 mg/dL, p<0.001) were found to be lower in HD patients. No differences in albumin, cholesterol and TG were observed between the two groups. Several amino acids (taurine, cystine, phosphoserine) were found to be higher in the HD patients, while the concentrations of other five amino acids (serine, alanine, valine, leucine, tyrosine) were lowered in HD patients. No differences in nine amino acids (asparagine, glutamine, proline, glycine, methionine, isoleucine, lysine, histidine, arginine) were observed between the two groups. CONCLUSION: Our results suggest that chronic renal failure patients have malnutrition and amino acids abnormalities. To correct the amino acids abnormalities and improve nitrogen utilization in hemodialysis patients, correction of acidosis and supplementation of the diet with serine should be considered.

8.Papillary Adenocarcinoma of Nonpigmented Ciliary Epithelium of the Eye.

Adenocarcinoma of the ciliary epithelium is a rare tumor, usually occuring in elderly patients who have a history of severe ocular trauma or chronic inflammation. We report an adenocarcinoma of the nonpigmented ciliary epithelium found within the phthisical globe of a 36-year-old female whose eye had been loss of vision since infancy. The mass, measured 4.0 x 4.0 cm, was relatively limited by sclera but had invasion to posterior portion. Histologically, the tumor was a compact mass which consisted of tubular and papillary structures with foci of the pleomorphic area. Strands of cells and individual cells were invested with thick basement membrane that have positivity for periodic acid-Schiff stain. Immunohistochemical staining showed strong reactivity for cytokeratin and epithelial membrane antigen, and focal for neuron-specific enolase and S-100 protein.

9.The Effects of Propofol Anesthesia or Prophylactic Droperidol on Postoperative Nausea and Vomiting in Patients Undergoing a Gynecologic Laparoscopy.

BACKGROUND: Postoperative nausea and vomiting (PONV) is a common complication of a gynecologic laparoscopy. This study was designed to assess the effect of prophylactic droperidol 1 mg or propofol as the induction and maintenance anesthetic agent for prophylaxis of PONV in women undergoing a gynecologic laparoscopy. METHODS: Eighty ASA physical status 1, 2 patients undergoing an elective gynecologic laparoscopy were randomly allocated into four groups. Group 1 (n = 20) recieved an intravenous placebo of noraml saline 1 ml prior to induction of anesthesia and N2O-enflurane general anesthesia. Group 2 (n = 20) recieved an intravenous placebo of noraml saline 1 ml prior to induction of anesthesia and N2O-propofol general anesthesia. Group 3 (n = 20) recieved intravenous prophylactic droperidol 1 mg prior to induction of anesthesia and N2O-enflurane general anesthesia. Group 4 (n = 20) recieved intravenous prophylactic droperidol 1 mg prior to induction of anesthesia and N2O-propofol general anesthesia. RESULTS: The incidence and severity of PONV and sedation scores were assessed at 0, 30 min, 1, 3, 6, 24 and 48 hours postoperatively. The incidence of PONV was 75% in group 1, 10% in group 2, 30% in group 3 and 20% in group 4. The incidence of PONV during the first 6 hours postoperatively was 70% in group 1, 0% in group 2, 10% in group 3 and 5% in group 4 and there were no statistical differences among the four groups in the 6 to 24 hour postoperative period. Sedation scores were significantly higher in group 3 and 4 than in 1 and 2 in the 3 to 6 hour postoperative period. CONCLUSIONS: Propofol anesthesia, prophylactic droperidol 1 mg and a combination to prevent PONV were highly effective during the first 6 hours postoperatively.

10.Two Cases of Partial Unilateral Lentiginosis.

Partial unilateral lentiginosis(PUL) is a rare pigmentary disorder charaeterized by lentigines limited to one side of the body with or without neurologic abnormalities. We report two cases of PUL unassociated with any other defect, One patient was 20-year-old female who had lentigines confined to the right side of the face and the other was 19-year-old male who had lentigines confined to the left side of the neck and upper trunk and left upper extremity. Histologic examination of the pigmented lesion revealed findings consistent with lentigo simplex.
