1.Two Cases of Atypical Angioleiomyoma.

We herein present two cases of angioleiomyoma with unusual location and number of cutaneous lesion. One was 58-year-old male who has two small peasized nodules on the left palm and volar aspect of the left fifth finger. The other was 63-year-old male with three small pea-sized nodules on the left palm, volar aspect of the left middle finger and the right sole. We confirmed these as angioleiomyoma by clinical and histopathological findings.

2.A Stastistical Survey of Cutaneous Epithelial Cysts.

Sixty-one cases in which the diagnosis of epithelial cysts had been made studied. The data were collected from our own clinical records and a review of all histopathologic slides of epithelial aysts that had been filed at Departrnent of Dermatology, Chosun University Hospita.l during the past g years from July, 1979 to June, 1984. The results were as follows: 1) Among the 61 cases of epithelial cysts, 41 cases were epidermal cysts, 10 cases of milia, 7 cases of steatocystoma. multiplex, 2 cases of pilar cysts and 1 case of eruptive vellus hair cyst. 2) The average age of these patients was third decade. 3) The commonest site of epidermal cyst and milium was face. Two cases of pilar cyst occurred only on the scalp. Fteatocystoma multiplex was developed on the several sites, but not on the scalp and scrotum, 4) The average diameter was 6. 5 mm for epidermal cyst, p 6 mm for milium, 3 1 mm for steatocystoma multiplex and 17 mm for pilar cyst.

3.Relationship of Family Function with Life Event Stress and Depression in Healthy Adult Males.

BACKGROUND: Many of the major life events that appear to affect biologic system and health occur within the context of the family and can have a profound psychosocial and biologic impact on family memebers. So, the present study attempted to examine the relations of family function with stressful life events, depression, and physical and psychological symptoms reported by patients. METHODS: Design : Cross sectional, observational study. Participants : 41 healthy adult males from a family medicine ambulatory care unit in a tertiary hospital located in Guro completed a series of psychometric instruments that include the Family APGAR(Adaptation, Partnership, Growth, Affection, and Resolve), FACESIII(Family Adaptability and Cohesion Evaluation Scale), Modified 46-item Stressful Life Events Scale, Lee's 98 items Stressful Life Event Scale, Korean version of BEPSI(Brief Encounter Psychosocial Instrument) and BDI(Beck's Depression Inventory Scale). RESULTS: Family APGAR was negatively correlated with stress related life events(both in total and negative events, -0.30, -0.37, respectively; p<0.05) and BEPSI(-0.56; p<0.0001). No significant correlation could be found between family function and positive life events. Family APGAR also revealed significant negative correlation with BDI(-0.61; p<0.0001). Among the three family types, severe dysfunctional family demonstrated highest level of stressful life events score, perceived stress, and depression(p<0.05). CONCLUSIONS: The higher the assessment of the family function(Family APGAR), the lower the level of stressful life events, perceived stress and depression. There, knowledge of the family function, stress, and coping could be useful to family physicians in the whole person approach of their patients.

4.Animal Experiment for the Analysis of Postmortem Inhalation Degree by Drowning Medium Containing Methylene Blue.

Drowning is a leading cause of accidental deaths worldwide, but its pathological diagnosis remains a challenge for forensic pathologists owing to a lack of pathognomonic findings in drowning deaths and inconclusive autopsy findings caused by postmortem changes. The aim of the present study was to investigate the pathway taken by inhaled drowning medium through the airway after death in a variety of experimental conditions, including underwater pressurization. We used methylene blue dye to monitor the spread of drowning medium to the lungs. Results of theses experiments demonstrated that it is possible for a significant amount of downing medium to enter the airway during immersion after death. Our results suggest that autopsies of immersed bodies and interpretation of these findings should be performed with special care.

5.Sudies About the Pressor Response to Intracerebroventricular Infusion of Hypertonic NaCl in Rabbits.

Changes of blood pressure, heart rate, ECG, respiration rate and pupil size by intracerebroventricular(ICV) infusion of hypertonic NaCl with 0.04 ml/min for 5 min(total 0.2ml) were observed in urethane-anesthetized rabbits. ICV infusion of 0.75M NaCl produced slight pressor effect (11mmHg) and did not affect other parameters. ICV infusion of 1.5M NaCl began to increase blood pressure from 2~3 min after the infusion and produced maximal increase(24mmHg) at 5~10 min. Then the pressor effect was recovered to the original level at 30~60 min. Change of heart rate by the infusion was not clear, but ST-segment of ECG was markedly depressed. Respiration rate increased about 1.5 times than the control in accordance with the pressor effect and the state was continued even after the recover of the pressor effect. Both pupils dilated markedly and light-reflex was lost. Changes of parameters by ICV infusion of 3.0M NaCl were similar to those by 1.5M NaCl and some rabbits caused severe arrhythmias and died. The purpose of present study is to investigate the mechansim(s) of the pressor effect induced by the ICV infusion of 1.5M NaCl. The pressor effect of 1.5M NaCl was attenuated by the continuous infusion of vasopressin antagonist(20microm/kg/min) but not affected by intravenous treatment with 2mg/kg phentolamine, 2mg/kg propranolol and 1mg/kg chlorisondamine. The pressor effect was not altered with ICV 0.12mg/kg phenoxybenzamine, 0.4mg diltiazem, 0.1mg/kg mecamylamine and 0.2mg/kg atropine. After ICV infusion of 25microg/kg/min of diazepam, however, the pressor effect was completely abolished and restored 3~4 hours after stopping diazepam infusion. The pressor effect was rather potentiated than inhibited in bilateral adrenalectomized or nephrectomized rabbits. Infusion of 2microg/kg/min of saralasin for 10 min in the bliateral adrenalectomized rabbit did not affect the pressor effect at all. These results suggest that hypertensive effect induced by ICV infusion of hypertonic NaCl is mediated by the increase of vasopressin secretion.

6.Immunohistochemical Identification of Keratin in Cutaneous Tumors.

Sixty cutaneous tumors of different types were investigated by means of immunoperoxidase technique(avidin-biotm-peroxidase complex method) with the use of antikeratin antibody. The results were as follows: 1. Squamous cell carcinomas, keratoacanthomas, and Bowen's disease exhibited strong reactivity with antikeratin antibody. 2. In squamous cell carcinomas and keratoacanthomas, the most heaviest staining patterns were detected in areas of keratinization and horn pearl formation. 3. Basal cell carcinomas and seborrheic keratosis were moderately stained, whereas Paget'a disease and metastatic carcinoma(oat cell carcinoma) did not contain keratin. 4. In some cases of basal cell carcinomas, the tumor cells were partially positive. 5. Malignant melanomas, nevi, malignant.lymphomas, neural tumors, hemangiomas, and dermatofibromas were uniformly negative.

8.A case of xanthoma showing unusual clinical manifestation.

We report an atypieal case of xanthoma without evidence of underlying disease, A 50-year-old asymptomatic, yellowish, flat plaques on the upper eyelids, trunk and thighs, and huge, deep-seated tumors on anterior aspect of the neck, back and ankle areas of 3 years duration. Biopsies were performed at the lesions of the neck and hack. Two biopsy specimens showed the same findings of xanthoma, respectively. The appearance of refrigerated serum was clear. Paper electrophoresis didn't show elevation of any lipoprotein, suggesting normolipoproteinemia.

9.A Case of Sebaceous Adenoma.

Sebaceous adenoma is a benign organoid tumor of proliferating, incornpletely differentiated sebaceous glands. We report a case of sebaceous adenoma in a 15-year-old male. He had a 1.7 x 2.2 cm sized, asymptomatic, srnooth, round, raised, firm, reddish tumor on the left quadrant of abdomen. Histopathologically, the tumor was composed of lobules of irregular size and shape, and sharply demarcated from the surrounding tissue, The most lobules showed undifferentiated germinative and mature sebaceous cells in about equal number. Sebaceous cells in lobules were stained for prekeratin by means of avidin-biotin-peroxidase complex method. Ten months after surgical excision there was no evidence of recurrence.

10.A Case of Subepidermal Calcified Nodules Showing An Unusual Clinical Manifestation.

Subepidermal calcified nodules, consisting of solitary circumscribed deposits of insoluble calcium within the subepidermis, have generally been assumed to be idiopathic. As the disease is not associated with any systemic or cutaneous diseases, or any biochemical abnormalities, the pathogenesis is still unexplained, and it is thought to be a definite entity with only a characteristic histopathology. We report a 36-year-old female who has had subepidermal calcified nodules on both upper eyelids symmetrically for about 20 years. It is interesting that the lesions have xanthelasma-like appearance and are associated with syringomas. Excisional biopsy showed typical histopathologic and histochemical features of the sub epidermal calcified nodules.
