3.Effect of Antenatal Magnesium Sulfate Administration on Neonatal Mortality and Morbidity in Very Low Birth Weight Infants.

PURPOSE: We investigated whether in utero exposure to magnesium sulfate is associated with lower incidence of neonatal mortality, morbidity, and neurodevelopmental abnormalities in very low birth weight infants. METHODS: We evaluated 172 infants with birth weight less than 1,500g. According to the maternal receipt of magnesium sulfate, we divided into two groups. We reviewed the medical records of mothers and their babies to evaluate clinical outcome. RESULTS: Of 172 infants, 58(GA 30.0+/-2.3weeks, BW 1,20+/-2221g) received magnesium sulfate prior to delivery for tocolysis or preeclampsia(study group). The remaining 114(GA 29.6+/-2.3weeks, BW 1,220 +/-198g) served as the control group. Neonatal mortality was not significantly different between the two groups(12.5% vs 26.0%). There was no significant difference between the two groups in neonatal complications' RDS(52.4% vs 67.6%), BPD(16.7% vs 29.6%), apnea(52.4% vs 67.6%), IVH(23.8% vs 31.0%), PVL(4.8% vs 11.3%), NEC(7.1% vs 7.0%), ROP(26.8% vs 41.2%), and neal infection(33.3% vs 54.9%). The duration of ventilator therapy(10.9+/-7.3 vs 14.2+/-10.6 days), oxygen administration(20.2+/-20.0 vs 24.3+/-19.9 days) and admission(51.5+/-16.9 vs 54.6+/-16.7 days) were not different. Among 64 patients in whom follow up evaluation was possible more than 1 year, the incidence of cerebral palsy or developmental delay was not different(12.0% vs 12.8%). CONCLUSION: Maternal receipt of magnesium sulfate does not seem to be associated with an appreciably reduced risk of neonatal mortality, morbidity, and neurodevelopmental abnormalities in very low birth weight infants. 7.Consideration of Birth Weight by Gestational Age.

This study was undertaken to make current fetal growth curve throughout later part of pregnancy. 5,110 normal singleton deliveries at Il Shin Christian Hospital were included. The 10th, 25th, 50th, 75th, 90th percentiles of birth weight were calculated from 21 to 42 weeks' gestation and the 10th, 50th, 90th percentiles of birth weight compared by fetal sex, maternal parity and height were graphed to know the potency of factors. There was a linear growth pattern between 28 and 38 weeks' gestation. During last month of prgnancy, three factors have significant effect on median birth weight but no significant differences in 10th percentiles. Maternal height & parity significantly affeect on 90th percentiles. So, these factors must be considered when diagnosing large for date.

8.Influence of Reward and Nursing Professional Pride on Nursing Intention in Patient with Emerging Infectious Disease

Purpose:
The purpose of this study was to investigate the influence of nurses on the nursing intentions in patients with emerging infectious diseases.
Methods:
Data were collected from 201 nurses with over one year of clinical experience in a general hospital located in Seoul, South Korea. A structured self-report questionnaire was utilized to measure belief factors, core factors, rewards, and nursing professional pride, as well as nursing intentions in patients with emerging infectious diseases.
Results:
The results revealed that nurses' intentions to provide care for patients with emerging infectious diseases were significantly influenced by perceived behavioral control (β=.26, p<.001), clinical experience (β=-.14, p=.008), behavioral beliefs (β=.31, p= .010), feeling of vocation (β=.29, p=.012) and attitudes toward behaviors (β=.08, p=.034).
Conclusion
Enhancing nurses' intentions to provide care for patients with emerging infectious diseases requires the implementation of systemic and educational strategies aimed at strengthening their confidence and beliefs.

BACKGROUND: To determine the molecular structure of type II collagen-specific T-cell receptors associated with rheumatoid arthritis (RA). METHODS: We generated CII-specific T-cell lines of 8 RA patient s by prolonged in vitro culture with bovine CII (bCII) and the immunogenic peptide (256-270) of human CII. The proliferation response towards CII stimulation was measured from the uptake of 3 H-thymidine. Changes in the secretion of Th 1 and Th2 cytokines in the culture supernatent were measured by ELISA. The TCR clonotypes of these T-cells were examined by RT-PCR/ SSCP analyses of all 22 V beta chains. RESULTS: T-cells from patients' tissue exhibited strong proliferation index upon CII stimulation, which was maintained up to 6 months in the culture. The secretion of INF-gamma from these T-cells increased along with the duration of culture time, while the amount of IL-4 production did not show significant changes. The SSCP band patterns of patients' T-cells appear as discrete bands unlike the smeary streak produced from normal samples. Some SSCP bands, each representing selected expansion of a TCR containing certain subtype of V beta peptides, appeared to be identical in more than one patients. Among these, the expansion of SSCP band representing the V beta 14 CDR3 region persisted after switching the antigen to the immunogenic human peptide (256-270). CONCLUSION: CII-reactive T-cells expressing distinct CDR3 motifs are selectively expanded in the peripheral blood and synovial fluid of RA patients, and their persistent proliferation upon CII stimulation, as well as the production Th 1-type cytokines, may play pivotal roles in RA pathogenesis.
