BACKGROUND: We evaluated 1) the sequential changes of angiotensin-converting enzyme (ACE) mRNA expression in lung (main site for circulating ACE synthesis) and left ventricle, and 2) whether such expression is modified by ACE inhibitor or angiotensin II receptor blocker treatment after acute myocardial infarction (MI) in rats. METHODS: Placebo, captopril (2 g/liter drinking water) or TCV-116 (10 mg/kg/day gavage) was administered 3 days before left coronary occlusion and continued for 6 weeks. At 1, 3 and 6 weeks after operation, hemodynamic measurement was performed and pulmonary and myocardial ACE mRNA expression was analyzed by quantitative RT-PCR using rcRNA as an internal standard. RESULTS: Mean BP and LVEDP increased in untreated rats compared with captopril- and TCV-116-treated rats (post-MI 6week, p<0.05). Pulmonary ACE mRNA increased in acute phase (post-MI 1 week, max. 1.4 fold, p<0.05 vs. pre-MI) and returned to pre-MI value thereafter. In contrast, cardiac ACE mRNA expression showed slightly decreasing tendency in acute phase, and was increased up to 1.6 fold in chronic phase after MI (post-MI 3 and 6weeks, p<0.05 vs. pre-MI). No changes in pulmonary ACE gene expression were found with RAS blockade. However, in acute phase after MI, cardiac ACE mRNA increased with both captopril and TCV-116 treatment (post-MI 24hour and 1week, max. 2 fold, p<0.05 vs. untreated group). CONCLUSION: 1) In contrast to the pulmonary ACE mRNA that is activated in acute phase, the cardiac ACE mRNA is activated in chronic phase after MI. 2) RAS blockade does not affect the change of pulmonary ACE expression, but modulate the change of cardiac ACE expression after MI.

Selected number of urinary E. coli isolates from patients were subjected to the tests to characterize in vitro properties, compared with the strains of normal flora in intestinal tract or other sources. Further studies were carried out for determination of the mechanism of pathogenesis by using rabbit as an experimental animal model. Both strains of 87E1 and 87E51 which had one type of pili, were eliminated rapidly 4 or 8 days after inoculation into renal pelvis of rabbit. On the contrary, 88E18 strain which had both types of pili successfully colonized in urinary tract and the rabbit died due to urosepsis after the 8th day of experiment. The result of bacterial count colonized in the tissue by colony forming unit (CFU)per gram were similar to those in the urine.

The incidence of acute urinary retention in females is very low but the causes are diverse. So, it is important to verify the causative diseases trough history taking, physical examination including neurologic tests and it is essential to find adequate therapeutic modalities. MATERIALS AND METHODS: We experienced 45 female patients who visited under the chief complaint of acute urinary retention and reviewed, evaluated retrospectively or prospectively during 4 years. The patients are divided 1) transient causes: immobilization after surgery, constipation, medications, herpes zoster, psychogenic causes 2) neurologic causes: cerebral concussion, peripheral neuropathy, brain tumor, herniated nucleus pulposus, spinal cord injury, previously undiagnosed diabetes 3) pelvic causes: difficult labor, uterine prolapse,, pelvic bone tumor, vaginal invasion of rectal cancer, imperforate hymen. RESULTS: Screening test were done through physical examination, history taking, routine urinalysis. Most patients recovered voiding after correction of underlying diseases. CONCLUSIONS: The patients who showed persistent retention were further evaluated through neurologic tests, urodynamics, pelvic ultrasonography and serious diseases as blain tumor, polyneuritis, metastaic pelvic bone tumor were diagnosed.

Although the rates of tuberculosis cases are continuing decline with advent of newer drugs, sporadic cases of urinary tuberculosis still occur. During last 5 years, we experienced 22 cases of reconstructive surgery and 18 cases of endourologic procedures for preservation of renal function. The following results were obtained: 1. Reconstructive surgeries including ureteroneocystostomy etc. were underwent in 22 cases and improvement were noted in 18 cases. 2. In 18 endourologic cases, there has been a substantial improvement in renal function, decrease in upper tract dilatation and relief of symptom. Average indwelling duration was 2.4 months and the mean number of dilatation per patient was 1.8 time. 3. Endopyelotomy and direct ureterotomy with ureteroscope are useful modalities in multiple ureteral stricture or upper ureter stricture. 4. It should be emphasized that percutaneous antegrade stent indwelling was more easier than retrograde technique especially in failure cases.

BACKGROUND: This study was designed to investigate the relationship between psychiatric disease and systemic disease in geriatric psychiatric inpatients who were consulted to department of internal medicine and were above 6th decade, and was directed to assess the physical problems and possible ways to resolve them in a closed psychiatric unit. METHOD: Through evaluation of medical records of 225 inpatients who had consulted in department of internal medicine in Seoul National Mental Hospital from January 1, 1998 to December 31, 1999. We classified the physical illness according to ICD-9-CM and psychiatric disease according to NTA. RESULTS: The result were as follows: 1) In age sex distribution, male was 62.7%, and 81.8% of the subjects was 6th decade. 2) The most common systemic diagnosis were disease of circulatory system, disease of digestive system, and disease of respiratory system. 3) The most systemic disease was disease of circulatory system in 6th and 8th decade. 4) Disease of circulatory system in all psychiatric disease was the most frequent. 5) Rate of circulatory disorder was the highest in organic mental disorder. Disease of digestive system and disease of endocrine system were the most frequent in alcoholics. Disease of respiratory system showed the highest rate in schizophrenea. CONCLUSION: The clinical characteristics of psychiatric diseases influenced to get systemic disease in geriatric patients.

Neurilemmoma reprsents a benign nerve sheath tumor derived from Schwann cells. Lesions almost always are solitary, although multiple lesions may occur in association with multiple neurofibromatosis. We report a case of multiple neurilemmomas not in asaociation with neurofibromatosis. The patient was a 28-year-old male who had multiple tender nodules along the course of peripheral nerves on the palmar aspect of the right hand. A local excision of all lesions was performed and the microscopic diagnosis of neurilernmoma was established.
