Ethmoid sinus foreign body can be divided into endogenous and exogenous, the clinically referred mostly belonging to the exogenous foreign body. The exogenous ethmoid sinus foreign body generally has a history of facial trauma, its clinical manifestations are associated with the foreign body size, nature, residence time and location. Using X-ray, CT scan can further confirm the diagnosis. Removing the ethmoid sinus foreign body may have a better therapeutic effect by endoscopic sinus surgery. Here we report a rare case of foreign body in the ethmoid sinus.

OBJECTIVE:To establish a rapid,sensitive,accurate and stable method for the determination of voriconazole in human plasma in order to monitor clinical use of voriconazole. METHODS:HPLC-UV detection method was applied using carba-mazepine as internal standard. Plasma samples were treated with acetonitrile protein precipitation. The determination was performed on Phecda C18 column with 20 mmol/L monopotassium phosphate buffer solution (pH 6.0)-acetonitrile (50∶50,V/V) as mobile phase at flow rate of 1 ml/min. The column temperature was 40℃ and detection wavelength was 255 nm. The injection volume was 50 μl. RESULTS:The retention time of voriconazole and internal standard were 8.34 and 6.24 min. The linear range of voricon-azole in plasma were 0.10-20.00 μg/ml(r=0.999 5). The lowest limit of quantitation was 0.05 μg/ml. Intra-day and inter-day RSD were below 1.57% and 1.45%,respectively. The extraction recovery of low,medium and high concentrations were between 81.40%to 128.29%. CONCLUSIONS:The method is simple and accurate for therapeutic drug monitoring(TDM)of voriconazole.

OBJECTIVE:To observe the clinical efficacy and safety of saxagliptin combined with metformin in the treatment of type 2 diabetes. METHODS:A total of 95 patients with type 2 diabetes were randomly divided into observation group(47 cases) and control group(48 cases). All of the patients received Metformin hydrochloride sustained-release tablets 0.5 g for continuous 4 weeks,orally,3 times a day;based on the treatment,control group was continuously given Metformin hydrochloride sustained-re-lease tablets 0.5 g for continuous 24 weeks,orally,3 times a day;observation group was given Saxagliptin tablets 5 mg continu-ous 24 weeks based on the treatment of control group,orally,once a day;the treatment course was 28 weeks. The clinic data was observed,including clinical efficacy,FPG,2 h PG,bed time glucose,HbA1c,BMI before and after treatment,incidence of hypoglyce-mia,severe hypoglycemia and adverse reactions. RESULTS:The total effective rate in observation group was higher than control group,the incidence of hypoglycemia in observation group was lower than control group(P<0.05). After treatment,the glucose in-dexes in 2 groups was significantly lower than before,and FPG and HbA1c in observation group were lower than control group(P<0.05). There were no significant differences in the BMI before and after treatment,incidence of severe hypoglycemia and adverse reactions between 2 groups(P>0.05). CONCLUSIONS:Saxagliptin combined with metformin has better efficacy and safety than metformin alone in the treatment of type 2 diabetes,and can significantly reduce the incidence of hypoglycemia.

Objective Abstract:Objective To explore the effects of arsernic trioxide(As2O3) on telomerase activity and cellular apoptosis of human colon cancer cell line CCL-187.Methods CCL-187 cells were divided into control group,1.0 ?mol?L-1As2O3 group and 2.5 ?mol?L-1As2O3 group,all groups were cultivated for 1 to 6 d.The inhitory rate of cell growth was detected by MTT assay.The morphologic changes of CCL-187 cells were observed under electron microscope.The expression of hTERT at mRNA level was analyzed by RT-PCR.Telomerase activity was determined by TRAP-PCR-ELISA.Results The differentiation of cells was found in 1.0 ?mol?L-1 As2O3 group,but apoptosis in 2.5 ?mol?L-1 As2O3 group,and at the same time,the telomerase activity was reduced and the hTERT-mRNA expression was downregulate in 1.0 ?mol?L-1 As2O3 group and 2.5 ?mol?L-1 As2O3 group in a time-dependent manner.There were significant differences compated with control group(P

Objective To replace the imported vacuum pump with a Chinese-made instrument using a micro-ELISA negative pressure pump,to further extend the life of the machine.Methods Electric aspirator(YB.Dx23D) was used as source of negative pressure to connect with barrels of waste and the vacuum-phase of imported enzyme immunoassay instrument.The circuit was transformed,and the host matched the load,so the host and negative pressure pumps worked simultaneously.Results Compared with other aspirators,the modified electric aspirator YB.Dx23D has low-noise ratio,high negative pressure,vast flow,small volume,and mobile function.Conclusion The modified aspirator YB.Dx23D electric instrument is well used in washing ELISA plates and the effective use of waste water.Meanwhile,it can be applied to other necessary suction negative pressure(vacuum) equipment.

Objective To study the efficacy of problem-based learning (PBL) in clinical teaching of gynecology and obstetrics.Methods Totally 170 students in our department for probationary were divided into two groups randomly.Students in the two groups were taught by theoretical lecture and case study in the ward.PBL (After the lectures,teachers choose clinical cases,students inquire medical history and do physical examination,students discuss on the case and propose treatment scheme,students look up references and consult specialist to determine treatment scheme,students do team report and teachers give comments) was used in experimental group (n=87) while traditional teaching method (teachers guide students to inquire medical history,do physical examination,analyze the cases and make diagnosis,do differential diagnosis and give treatment as well as answer the questions posed by students) in control group (n=83).Ability of students to solve the practical problems was assessed by final examination (theoretical exam and case analysis) and the efficacy of PBL was investigated by questionnaire survey.SPSS 11.5 soft ware was employed to do statistic analysis.Measurement data were analyzed by t test and manifested asand enumeration data were analyzed by chi-square test.P≤ 0.05 stands for statistical differences.Results There was no difference in theoretical score between experimental group and control group.Score of case analysis was higher in experimental group than in control group (t=2.236,P=0.017).Students' satisfaction degree of PBL was much higher than that of traditional teaching method.Conclusions PBL is better than traditional teaching method in clinical teaching of gynecology and obstetrics.It can improve students' ability of problem-analyzing and problem-solving,therefore should be promoted in clinie education.

Diversified clinical teaching methods which include theoretical teaching by multimedia,specific chapter explains with PBL,contacting with clinical diseases and integrating theoretical knowledge by case discussion,solving learning problems and introducing new progress by seminars,can improve the quality of teaching in the department of obstetrics and gynecology.

To develop a patient-reported outcome (PRO) scale for patients with stable chronic obstructive pulmonary disease (sCOPD) and to study its clinical applicability in traditional Chinese medicine (TCM).

Objective To investigate knowledge of cervical cancer,human papillomavirus (HPV) and HPV vaccine and attitude to HPV vaccine among employed urban women in China,and to assess willingness to vaccinate their children.Methods This study was conducted in women from 16 factories or companies in Beijing,Hangzhou,Changsha,Chcngdu and Guangzhou.A questionnaire on cervical cancer,HPV knowledge and HPV vaccine acceptability was administered to participants before and after educational intervention.Results A total of 1 146 women were surveyed from August to November,2011.The awareness rates of cervical cancer,HPV and HPV vaccine were 95.06 ％,27.98 ％ and 12.82 ％,respectively.Only 20.68 ％ knew that HPV was related to cervical cancer.After educatioual instruction,89.26 ％ understood the relationship between HPV and cervical cancer (x2 =93.414,P ＜ 0.001).Only 19.25 ％ women were unwilling to allow their children to be vaccinated.The main reasons were as follows:worried about vaccine safety (23.52 ％),belief that their child was not at risk (21.92 ％),lack of scientific evidence for the vaccine (13.01％),and worricd about vaccine efficacy (12.79 ％).Conclusions In general,there is a high acceptability of HPV vaccine to prevent cervical cancer among employed urban Chinese women after educational intervention.However,some women remain reluctant to vaccinate their children due to concern about vaccine safety and efficacy.

In this article,the authors analyzed the current status of the therapeutic effect evaluation of TCM and proposed some measures to improve the current therapeutic effect evaluation of TCM,quantificated messages from TCM four diagnostic methods,explored biological parameter reflected essence of diseases,introduced quality of life scale and PRO,in the hope of establishing a comprehensive system of therapeutic effect evaluation reflecting the superiority and characteristics of theory of body and spirit.
