1.Analysis of factors affecting diagnosis and morbidity of upper gastrointestinal cancer

Objective To analyze the factors affecting diagnosis and morbidity of upper gastrointestinal cancer.Methods Clinical data of 17 830 patients who were performed with upper gastrointestinal endoscopy between January 2020 and January 2022 was collected.Combined with questionnaire and pathological examination,the diagnosis and morbidity of upper gastrointestinal cancer was analyzed.Results There were 88,33 and 119 cases of carcinoma of esophagus,cardia and stomach respectively.Among them,57,27 and 91 cases of early carcinoma of esophagus,cardia and stomach were found.They were divided into the observation group(240 cases)and control group(17 590 cases).Univariate analysis showed significant differences between the two groups in education,drinking,pickled food,atrophic gastritis,intraepithelial neoplasia,painless gastroscopy,standard gastroscopy,and anxiety.Multivariate unconditional Logistic regression analysis and propensity score matching showed that drinking,atrophic gastritis,pickled food,intraepithelial neoplasia and anxiety were risk factors influencing the diagnosis and morbidity of upper gastrointestinal cancer.Conclusion Drinking,atrophic gastritis,pickled food,intraepithelial neoplasia and anxiety are risk factors influencing the diagnosis and morbidity of upper gastrointestinal cancer.Adjust the diet and life style,enhance the awareness of early cancer screening,can control the risk factors influencing the diagnosis and morbidity of upper gastrointestinal cancer.
2.Implications of Reciprocity Mechanism Based on Gift Exchange Game for Physicians'Altruistic Behavior Incentives

Objective:To propose ideas for designing an incentive system that aligns with the unique characteristics of the medi-cal industry while maintaining altruism.Methods:The concepts of reciprocal altruism theory and gift exchange game experiments are introduced from the field of economics to incentivize physicians'altruistic behavior.Results:It proposes the indirect reciprocal altru-ism theory,which focuses on the three-party principal-agent relationship between physicians,patients,and management in the medical field.The compensation incentive concept that measures health value is also introduced.Additionally,it constructs a model for the incentive mechanism of physicians'altruistic behavior based on the three-party gift exchange game.Conclusion:Based on the concept of value-based incentives for physicians'altruistic behavior,it develops an incentive system that caters to the unique characteristics of the medical industry and promotes its high-quality development.
3.Effect of lncRNA GAS5 on Lung Tissue Cell Pyroptosis in Rats with Acute Lung Injury via Regulation of the miR-452-5p/Mcl-1 Pathway

Objective To investigate the effect of long non-coding RNA growth arrest-specific transcript 5(lncRNA GAS5)on lung tissue cell apoptosis in acute lung injury(ALI)rats via regulation of the miR-452-5p/myeloid cell leukemia sequence 1(Mcl-1)pathway.Methods SD rats were randomly separated into 6 groups(10 in each group):control group,model group,pUC57 lncRNA GAS5 group,miR-452-5p antagonist group,pUC57 NC+miR-452-5p NC group,and pUC57 lncRNA GAS5+miR-452-5p agonist group.The ALI model was constructed via the intraperitoneal injection of lipopolysaccharide in the model group and drug intervention group,whereas the control group rats were intraperitoneally injected with an equal dose of physio-logical saline.At the same time,the intervention was performed,and the lung function of the rats was subsequently meas-ured.The forced vital capacity(FVC),forced expiratory volume in 0.3 second(FEV0.3)/FVC,and tidal volume(VT)were com-pared among the groups.HE staining was used to observe pathological damage in the lung tissue,and the degree of damage was evaluated via the Holfbauer score.An enzyme-linked immunosorbent assay(ELISA)was used to measure the levels of bronchoal-veolar lavage fluid(BALF)and the serum inflammatory factors interleukin-18 and IL-1β in the rats.Immunoblotting was used to detect the expression of pyroptosis-related indicator proteins[(GSDMD),nucleotide binding oligomeric domain-like receptor protein 3(NLRP3),cysteine-containing aspartate-specific protease(Caspase-1),and Caspase-11]and Mcl-1 in rat lung tissue.Real-time fluorescence quantitative PCR was used to detect the expression of lncRNA GAS5 and miR-452-5p in rat lung tissue.A dual-luciferase reporter gene experiment was performed to validate the targeted regulation of miR-452-5p by lncRNA GAS5 in rat alveolar epithelial cell L2.Results Compared with those in the control group,the FVC,FEV0.3/FVC,expression of lncRNA GAS5,expression of Mcl-1 protein and mRNA in the model group decreased(P<0.05),and the VT,Holfbauer score,IL-18 and IL-1β levels,GSDMD,NLRP3,Caspase-1 and Caspase-11 protein expression,and miR-452-5p expression were elevated(all P<0.05).Compared with those in the model group,the FVC,FEV0.3/FVC,expression of Mcl-1 protein and mR-NA in the pUC57 lncRNA GAS5 group and the miR-452-5p antagonist group were increased(all P<0.05),and the VT,Holf-bauer score,IL-18 and IL-1β levels,GSDMD,NLRP3,Caspase-1 and Caspase-11 protein expression,and miR-452-5p expression were reduced(all P<0.05).There was no obvious difference in various indicators in the pUC57-NC+miR-452-5p-NC group(all P>0.05).Compared with those in the pUC57 lncRNA GAS5 group,the FVC,FEV0.3/FVC,expression of Mcl-1 protein and mRNA in the pUC57 lncRNA GAS5+miR-452-5p agonist group decreased(all P<0.05),and the VT,Holfbauer score,IL-18 and IL-1β levels,GSDMD,NLRP3,Caspase-1 and Caspase-11 protein expression,and miR-452-5p expression were elevated(all P<0.05).Conclusion lncRNA GAS5 can promote Mcl-1 expression by down-regulating miR-452-5p,thereby inhibiting the in-flammatory response and lung tissue cell pyroptosis in ALI rats,ultimately reducing lung injury and improving lung function.
4.Cloning and expression profile of ZFP36L1 gene in goat.

The purpose of this study was to clone and characterize the ZFP36L1 (zinc finger protein 36-like 1) gene, clarify its expression characteristics, and elucidate its expression patterns in different tissues of goats. Samples of 15 tissues from Jianzhou big-eared goats, including heart, liver, spleen, lung and kidney were collected. Goat ZFP36L1 gene was amplified by reverse transcription-polymerase chain reaction (RT-PCR), then the gene and protein sequence were analyzed by online tools. Quantitative real-time polymerase chain reaction (qPCR) was used to detect the expression level of ZFP36L1 in intramuscular preadipocytes in different tissues and adipocytes of goat at different differentiation stages. The results showed that the length of ZFR36L1 gene was 1 224 bp, and the coding sequence (CDS) region was 1 017 bp, encoding 338 amino acids, which was a non-secretory unstable protein mainly located in nucleus and cytoplasm. Tissue expression profile showed that ZFP36L1 gene was expressed in all selected tissues. In visceral tissues, the small intestine showed the highest expression level (P < 0.01). In muscle tissue, the highest expression level was presented in longissimus dorsi muscle (P < 0.01), whereas the expression level in subcutaneous adipose tissue was significantly higher than that in other tissues (P < 0.01). The results of induced differentiation showed that the expression of this gene was up-regulated during adipogenic differentiation of intramuscular precursor adipocytes (P < 0.01). These data may help to clarify the biological function of the ZFP36L1 gene in goat.

5.miR-23b-3p regulates the differentiation of goat intramuscular preadipocytes by targeting the PDE4B gene.

This study aimed to explore the effect of miR-23b-3p on the differentiation of goat intramuscular preadipocytes, and to confirm whether miR-23b-3p plays its roles via targeting the PDE4B gene. Based on the pre-transcriptome sequencing data obtained previously, the miR-23b-3p, which was differentially expressed in goat intramuscular adipocytes before and after differentiation, was used as an entry point. real-time quantitative-polymerase chain reaction (qPCR) was used to detect the expression pattern of miR-23b-3p during the differentiation of goat intramuscular preadipocytes. The effects of miR-23b-3p on adipose differentiation and adipose differentiation marker genes were determined at the morphological and molecular levels. The downstream target genes of miR-23b-3p were determined using bioinformatics prediction as well as dual luciferase reporter assay to clarify the targeting relationship between miR-23b-3p and the predicted target genes. The results indicated that overexpression of miR-23b-3p reduced lipid droplet accumulation in goat intramuscular adipocytes, significantly down-regulated the expression levels of adipogenic marker genes AP2, C/EBPα, FASN, and LPL (P < 0.01). In addition, the expressions of C/EBPβ, DGAT2, GLUT4 and PPARγ were significantly downregulated (P < 0.05). After interfering with the expression of miR-23b-3p, lipid droplet accumulation was increased in goat intramuscular adipocytes. The expression levels of ACC, ATGL, AP2, DGAT2, GLUT4, FASN and SREBP1 were extremely significantly up-regulated (P < 0.01), and the expression levels of C/EBPβ, LPL and PPARγ were significantly up-regulated (P < 0.05). It was predicted that PDE4B might be a target gene of miR-23b-3p. The mRNA expression level of PDE4B was significantly decreased after overexpression of miR-23b-3p (P < 0.01), and the interference with miR-23b-3p significantly increased the mRNA level of PDE4B (P < 0.05). The dual luciferase reporter assay indicated that miR-23b-3p had a targeting relationship with PDE4B gene. MiR-23b-3p regulates the differentiation of goat intramuscular preadipocytes by targeting the PDE4B gene.

6.Legal risks of face recognition technology in medical field

Under the background of " Internet+ medical treatment" and the continuous deepening of face recognition technology research, the face recognition industry has continued to mature, and face recognition has been initially applied in medical fields such as hospital management, auxiliary medical care, and epidemic prevention and control. At the same time, face recognition technology brings problems including error risk, technical cracking risk, privacy risk, equality risk, abuse risk, and other issues in practice, which seriously threaten the personal and property rights and interests of the public. On the basis of summarizing the specific application direction of face recognition technology in hospitals, the authors sorted out the legal regulation of face recognition in China, and proposed that it should be based on technology research and development, strengthen the " gatekeeper" responsibility of medical institutions, improve legal system and recommendations for strengthening judicial leadership in order to improve the legal regulations of face recognition technology, reduce the risk of infringement by medical institutions in the application of face recognition technology, and protect the legitimate rights and interests of citizens.
7.Effects of major diagnostic choices on DRG grouping and weight changes

Objective:To study the influence of the choice of main diagnosis on diagnosis-related groups(DRG) grouping and weight change, by taking cerebral infarction as the entry point.Methods:From January 1, 2019 to March 31, 2019, 331 patients in three DRG groups(BR25, BR23, BR21) with cerebral infarction were selected. The original group was used as the control group; the main diagnosis was exchanged with the first other diagnosis, then DRG group was used as the experimental group. The difference of the number of cases and weight between the two groups was analyzed.Results:41.4%(137/331) of the patients in the experimental group were enrolled in BZ11 with other neurological disorders associated with important comorbidities and comorbidities, and 82.5%(273/331) patients′ weight increased after diagnostic conversion.Conclusions:Choosing other diagnosis as the main diagnosis may change the weight of the disease and affect the corresponding disease benefit. In order to prevent downcoding, DRG grouping should be detailed, and DRG payment should consider how to reflect the value of difficult cases; for upcoding behavior, medical insurance center and relevant medical institutions should check the correctness of coding, and monitor in place.
8. An analysis of status of personnel in occupational disease prevention and treatment institutions in Hunan Province, China, from 1996 to 2015

Objective:
To analyze the status of personnel in occupational disease prevention and treatment institutions in Hunan Province, China, from 1996 to 2015, to predict staff composition using grey model (GM) (1, 1) , and to provide a scientific basis and reference for optimizing human resource planning of occupational disease prevention and treatment in other provinces and regions and promoting the service capacity of the institutions.
Methods:
The data of the staff in occupational disease prevention and treatment institutions in Hunan Province, China, from 1996 to 2015 were obtained from the established basic information management system. The descriptive analysis method was used to analyze the dynamic changes in number and composition of the staff and the GM (1, 1) was used to predict the staff composition.
Results:
The numbers of the staff members in 1996 and 2015 in occupational disease prevention and treatment institutions in Hunan Province, China were 1591 and 1429, respectively. In the twenty years, the main education level of the staff transformed from "technical secondary school education and non-academic qualifications" to "bachelor degree or above and college degree"; the main major of the staff transformed from "other majors" to "public health and clinical medicine"; the proportion of the staff members without professional titles changed from >1/3 to 5%; and the proportions of the staff members with senior, intermediate, and junior professional titles were steadily rising. GM prediction showed that the proportions of highly educated staff members in 2018 and 2020 would be up to 41.00% and 45.61%, respectively; and the proportions of the staff members with a major in public health in 2018 and 2020 would be up to 44.15% and 46.60%, respectively.
Conclusion
The staff in occupational disease prevention and treatment institutions in Hunan Province, China, in the twenty years have slight changes in staff size and great improvement in staff quality, which is beneficial to sustainable development of the occupational disease prevention and treatment undertakings. The education level and major will be further optimized in the next five years.
9.Significance of the items for Dizziness Handicap Inventory in differential diagnosis of benign paroxysmal positional vertigo.

To explore the value of items for the Chinese version of Dizziness Handicap Inventory (DHI) in differential diagnosis of benign paroxysmal positional vertigo (BPPV) in patients with vertigo or dizziness first coming to the outpatient clinic.
Methods: A total of 322 patients with vertigo or dizziness, who came from Nanfang Hospital, Southern Medical University, were enrolled from April 2016 to February 2017. The Chinese version of DHI and Visual Analogue Scale (VAS) were completed by themselves. After detailed vestibular function examination, the patients were divided into a BPPV group, a normal vestibular group, and a abnormal vestibular group.
Results: The score of DHI-2 in the BPPV group was 5.52±2.10, which was higher than that in the normal vestibular group (3.94±2.91)(t=3.847, P<0.01) and the abnormal vestibular group (4.17±2.74)(t=4.149, P<0.01). There were significant differences in the DHI-2 among the 3 groups (F=9.870, t=4.515, P<0.01). The score of DHI-item 13 in the BPPV group was 3.09±1.39, which was higher than that in the normal vestibular group (1.97±1.63)(t=4.515, P<0.01) and the abnormal vestibular group (1.95±1.70)(t=5.305, P<0.01). There were significant differences in the DHI-item 13 among the 3 groups (F=16.402, P<0.01). There was significant difference in VAS scores among the 3 groups (P<0.05). However, the t-test analysis showed that there was significant difference between the BPPV group and the vestibular abnormal group (P<0.05), while there was no significant difference between the BPPV group and the vestibular normal group (P>0.05).
Conclusion: DHI-2 and DHI-item 13 should be included in the inquiry of disease history at the time of first diagnosis, which can be used to identify patients with BPPV quickly and effectively.

10.Application of double-low technique to aortic MSCTA

Objective To discuss the feasibility of double-low technique applied in aortic MSCTA.Methods Totally 70 patients with BMI not more than 25 kg/m2 were divided into a routine group and an experimental group randomly and equally,and then went through aortic MSCTA.Iohexol (350 mgI/ml) was injected with the velocity of 4 ml/s.The routine group had the parameters as 120 kV,120 mAs and 1.5 ml/kg contrast agent,and the experimental group had the parameters as 90 kV,120 mAs and 1.0 ml/kg contrast agent,while the other scanning parameters were kept the same in the two groups.The radiation doses,contrast agent doses and iodine contents were recorded in the two groups,and the image quality was assessed with double-blind method objectively and subjectively.Results The experimental group had the CT dose index volumes (CTDIvol),dose length product (DLP) and effect dose (ED) significantly lower than those in the routine group (P<0.05).The two groups had the consistency of subjective scores (Kappa>0.6) and image quality scores (not lower than 2) meet the desired requirements,and there were no significant differences between the scores (P>0.05).Objective evaluation showed that the signal values of aortic trunk and major branch vessels as well as image noise SD of the experimental group were obviously higher than those in the routine group (P<0.05).There were no statistical differences between the signal noise ratios (SNR) and contrast to noises (CNR) in the two groups (P>0.05),The iodine content in the contrast agent of the experimental group was significantly lower than that in the routine group (P<0.05).Conclusion Double-low technique applied in aortic MSCTA of the patient with BMI not more than 25 kg/m2 reduces the radiation dose and iodine content in the contrast agent,has the image quality meet the desired requirements,and thus is of great value for clinical application.
