Objective To investigate the regularity and significance of serum interleukin-6(IL-6) levels in the patients with sepsis.Methods Forty sepsis patients with APACHEⅡ scores over 12 and 30 normal controls were involved in the study.The IL-6 levels in serum were detected with radio-immunity analysis assay and the correlations among the IL-6 levels,the prognosis and APACHEⅡ score were investigated.Results In the patients with sepsis,the average level of IL-6 in serum was(152.02?55.77) pg/ml,which was significantly higher than that of the control group [(85.79?30.96) pg/ml](P
2.Protective effect of emodin on hepatic ischemia-reperfusion injury in the rat liver

Objective To study the protective effect of emodin on hepatic ischemia-reperfusion injury in the rat(liver). Methods Thirty Wistar rats were randomly devided into 3 groups:the sham-operatment group (Control group),the ischemia-reperfusion group (I/R group) and the emodin-treatment group (EMD group). By(clamping) the left and median lobes of the liver for 1 h and followed by 1 h of reperfusion,the(partial) (70%)(normothemic)(ischemia)-reperfusion injury model was established. The rats of emodin group received emodin [60mg/(kg?d)] by(oral) route for 3 days before ischemia. Blood and liver samples were obtained for(biochemical) and(histological) analysis 1 h after reperfusion. Objective To discuss surgical method and clinical curative effect of the headless compression cannulated screw percutaneous minimally invasive treatment of Evans -JensenⅠ,type Ⅱ of intertrochanteric fracture. Methods The clinical data of 21 cases of Jensen Evans and type II femoral intertrochanteric fractures who were treated by headless compression cannulated screw minimally invasive treatment were retrospectively annalyzed.In all patients,male in 6 cases,female in 13 cases;63 -86 years old,average 71.79 years.According to the Evans -Jensen norm and curative effect evaluation according to the improved Harris standard.Results Follow -up time was 6 -18 months (mean 12.15 months),two of them respectively in the six months after death from acute myocardial infarc-tion (ami)and 11 months of cor pulmonale,the rest of 17 cases of bony healing.Harris,the average score (67 -95points)at the time of the last follow -up,including 14 cases of optimal good in 2 cases,fair 1,poor in 1 case,was 88.89%.Conclusion Headless compression cannulated screw percutaneous minimally invasive treatment of Evans-Jensen Ⅰ,type Ⅱ intertrochanteric fractures has the advantangtages of minimally invasive,reliable fixation,high rate of fracture healing,allowing early hip not functional exercise load conditions and obtaining ideal therapeutic effect.
4.Investigation on influence factors of gernal surgeon growth

Objective To investigate the general surgeon growth time, and to probe into the factors affecting the surgeon growth. Methods 72 surgeons who worked more than 15 years in the field of general surgery in 12 hospitals of Gansu Province were investigated, focusing on education, resident rotation train-ing, teachers' qualification, refresher training, the frequency of intra-hospital technical exchange, and scale of hospital employed. SPSS 18.0 statistical software was applied to analyze the data. t test was used in com-parison between groups, and multiple groups were compared by using single factor analysis of variance. Results The surgeon growth time was (9.84±1.51) years averagely. In the process of growing up, the growth time of the general surgeons who experienced hospital resident rotated training, refresher training, intra-hos-pital technical exchange was significantly shorter than that of no-related-experienced surgeons. The higher the education, the higher the quality of the teachers and the higher the level of medical institutions, the shorter the doctor's growth time. Conclusion In the same medical institutions, the resident rotated training, the teacher's qualification, higher hospital training, and the frequency of intra-hospital technical exchange have the positive function to the surgeon's general growth, so in making young doctor training plan we should consider the value of the above-mentioned factors.
5.Clinical outcome of modified posterolateral approaches for the treatment of posterolateral tibial plateau fractures

Objective To observe the clinical outcome of modified posterolateral approaches for the treatment of posterolateral tibial plateau fractures. Methods From January 2012 to January 2015,13 patients with posterolateral tibial plateau fractures were identified in the Second Hospital of Changzhou Affiliated to Nan?jing Medical University. All the posterolateral fractures were fixated with a plate. The knee function was evaluated at the last follow?up by the Hospital for Special Surgery( HSS) Score,and the occurrence of postoperative compli?cations were observed. Results Thirteen patients were followed up for an average of 14. 4 months,all patients obtained bony union. There were no significant differences regarding the mean HSS score among 3 months, 6 months and 1 year post?operation((71. 6±1. 5) points, (76. 4±1. 6) points, (83. 2±1. 1) points;P=0. 154) . There were no significant differences regarding the tibial plateau angle and posterior slope angle on radiograph?ies of inside and outsideamong immediate,6 and 12 months postoperation((83. 7±1. 7)°,(84. 3±1. 5)°,(85. 1 ±1. 4)°,(85. 4±1. 5)°;(7. 7±1. 5)°,(7. 7±1. 4)°,(7. 7±1. 3)°,(7. 6±1. 8)°;P=0. 223,0. 191). No com?plications associated with the approach were observed and there was no infection,no malunion or fixation failure either. Conclusion In the treatment of posterolateral tibial plateau fractures,the modified posterolateral approa?ches can result in excellent fracture reduction under direct visualization and allow for posterior buttress plating.

In recent years, monotherapy and combination therapy with immune checkpoint inhibitors (ICIs) have achieved good efficacy in a variety of malignancies from solid tumors to lymphomas and have become a standardized and systematic treatment modality for many cancers. However, there is still a lack of studies on the safety of ICIs in hepatitis B virus (HBV)-infected patients with malignancies, and early studies have reported HBV reactivation due to ICI antitumor therapy in clinical practice. With reference to related literature, this article reviews the recent clinical trials and application of ICIs in cancer patients with chronic viral infection and clarifies the efficacy and safety of ICIs in this special population, in order to provide a reference for clinical medication.
7.H2S attenuates sepsis-induced cardiomyopathy by regulating the Xc -/ GPX4 pathway in ferroptosis

Objective :
To investigate whether NaHS,a hydrogen sulfide donor,can improve myocardial injury in sepsis by inhibiting oxidative stress and activating the Xc -/ GPX4 signaling pathway in ferroptosis.
Methods :
Lipopolysacc-haride(LPS) induced H9c2 in rat cardiomyocytes to form an in vitro model of myocardial injury in sep- sis,which was divided into Control group,LPS group and LPS + NaHS group.The kits were applied to detect the changes of cardiomyocyte viability,Fe2 + ,LDH and CK-MB,determine the levels of oxidative stress indexes GSH and MDA,detect the changes of cellular ROS and mitochondrial membrane potential levels by fluorescent probes, and detect the expression levels of ferroptosis regulatory proteins SLC7A11 and GPX4 by Western blot.
Results:
Compared with the Control group,H9c2 cell viability decreased,Fe2 + concentration increased ,GSH ,MDA and ROS levels increased,mitochondrial JC-1 monomer increased ,expression levels of ferroptosis regulatory proteins SLC7A11 and GPX4 decreased,and cell damage increased after LPS stimulation (P<0. 05) .Compared with the LPS group,NaHS attenuated LPS-induced H9c2 cell injury and elevated Fe2 + concentration,decreased the level of LPS-induced oxidative stress in H9c2 cells ,and increased the expression levels of ferroptosis regulatory proteins SLC7A11 and GPX4 (P<0. 05 ) .
Conclusion
The mechanism by which NaHS attenuates myocardial injury in sepsis may be related to the inhibition of oxidative stress and activation of the Xc -/ GPX4 signaling pathway in fer- roptosis.
