1.Effect of Occupational Therapy on the Function of Upper Limbs and Complications in Stroke Patients

ObjectiveTo observe the clinical effect of occupational therapy (OT) on the function of upper limbs and complications in stroke patients with hemiplegia.Methods99 stroke patients were randomly divided into the OT group (52 cases treated with OT and routine rehabilitation) and control group (47 cases treated without OT). Fugl-Meyer Assessment (FMA) and evaluation of hand function were performed before and two months after treatment. The complications of patients in two groups were also compared.ResultsAfter treatment, the functions of upper limbs and hand of patients in the OT group were superior to those in the control group ( P<0.05). The numbers of cases with complications in the OT group were significantly less than that in the control group ( P <0.01).ConclusionOT can improve the functions of upper limbs and hand and abate complications in stroke patients.
2.Safety and efficacy of loteprednol for ocular inflammation: a meta-analysis

AIM: To evaluate the safety and efficacy of loteprednol for controlling of ocular inflammation. METHODS: Data from 8 relevant monographs were retrieved by means of computerized and manual search. The combined analysis of the data was assessed in terms of clinical and statistical criteria, and the data were extracted and synthesized by using the statistical techniques of meta-analysis. Treatment effects and safety of loteprednol were through counting as risk difference between treatment and control groups. The estimates of pooled risk differences were computed according to a random-effects model. RESULTS: A total of 1 660 patients were included in this meta-analysis. Pooled risk differences of intraocular pressure elevation were 1 % (95 % CI, -1 %, 3 %) compared with placebo, -5 % (95 % CI, -9 %, 0 %) compared with prednisolone, respectively. Compared with placebo, pooled risk differences of inflammation improvement were: 31 % (95 % CI, 22 %, 40 %) of postoperative inflammation, 28 % (95 % CI, 19 %, 37 %) of seasonal allergic conjunctivitis, and 26 % (95 % CI, 18 %, 35 %) of giant papillary conjunctivitis, respectively. Compared with prednisolone, pooled risk differences of inflammation improvement of acute anterior uveitis was -15 % (95 % CI, -25 %, -4 %). CONCLUSION: Loteprednol is a safe and effective corticosteroid in controlling ocular inflammation such as postoperative inflammation, seasonal allergic conjunctivitis, giant papillary conjunctivitis, and acute anterior uveitis.
3.A gross investigation of dermatosis of hospitalized air force pilots

Objective To elucidate the characteristics of dermatosis in air force pilots and to improve its prevention and treatment scientifically. Method We carried out an investigation on 288 air force pilots hospitalized for dermatosis over the past 20 years. Results The results showed that 57.99% of the patients studied were admitted to hospital due to psychoimmunodermatosis, with high person-time (61.39%) and significantly longer average hospitalization time than that of the patients with the other two kinds of dermatoses. With improvement in flight skill and intensity as well as prolongation of flight time, the incidence and number of relapse of psychoimmunodermatosis in pilots were correspondingly increased. Conclusion The results indicate that among various dermatoses, psychoimmunodermatosis should become the focus of prevention and treatment as far as air force pilots are concerned. Besides drug treatment, we should also put emphasis on health education and psychological persuasion.
4.No significant effect of iodinated contrast after single percutaneous coronary intervention on thyroid function and auto-antibodies

The effect of iodinated contrast on thyroid function and auto-antibodies after a single percutaneous coronary intervention (PCI) was explored.In 96 patients with randomly selected PCI,serum TSH,T3,T4,thyroid peroxidase antibody (TPOAb),thyroglobulin antibody (TgAb) were determined preoperatively as well as by 3-6 months postoperatively.After PCI,the level of TSH,T3,T4,showed no significant changes; TgAb,TPOAb which were positive preoperatively,after 3-6 months were statistically increased(P<0.05).Iodinated contrast in PCI has no significant influence on thyroid function and auto-antibodies after 3-6 months.
5.Effect of glycoprotein (transmembrane) nonmetastatic melanoma protein B on the proliferation and migration of as well as melanogenesis in melanoma cells

Objective To estimate the effect of glycoprotein (transmembrane) nonmetastatic melanoma protein B (GPNMB) on the proliferation and migration of as well as melanogenesis in melanoma cells.Methods The expression of GPNMB was detected by immunofluorescence assay in two melanoma cell lines M14 and G-361,as well as in primary human melanocytes.Then,the three kinds of cells each were classified into three groups:experimental group treated with small interfering RNA targeting GPNMB (GPNMB-siRNA),negative control group treated with the negative control siRNA,blank control group remaining untreated.Methyl thiazolyl tetrazolium (MTT) assay,transwell invasion assay and spectrophotometry were performed to evaluate cell proliferation activity,invasion potential and melanin levels,respectively.Statistical analysis was done using Student's t test.Results GPNMB was expressed in both melanoma cells and melanocytes.The transfection with GPNMB-siRNA down-regulated the mRNA and protein expressions of GPNMB in,and markedly suppressed the proliferation and migration of,melanoma cells.In detail,the proliferative activity (expressed as the absorbence value at 570 nm) of M14 and G361 cells was reduced by 35% and 40% respectively,the migration activity of M14 and G361 cells by 49% and 51% respectively,and the melanin levels in melanocytes,M14 cells and G361 cells by 73%,82% and 69% respectively,in the experiment group compared with those in the blank control group.Conclusions The siRNA-mediated silencing of GPNMB could effectively inhibit the proliferation of,invasion of and melanogenesis in melanoma cells,which suggests that GPNMB plays critical roles in the initiation and progression of melanoma.

The chemical constituents from lipophilic parts in the roots of Angelica dahurica var. formosana cv. Chuanbaizhi were studied in this paper. The compounds were separated and purified by repeated column chromatographic methods on silica gel and HPLC, and the chemical structures of compounds were determined by spectral data analyses. Twenty-nine compounds were obtained and identified as isoimperatorin (1), β-sitosterol (2), imperatorin (3), bergapten (4), osthenol (5), xanthotoxin (6), isoimpinellin (7), dehydrogeijerin (8), phellopterin (9), isodemethylfuropinarine (10), 7-demethylsuberosin (11), alloimperatorin (12), xanthotoxol (13), isooxypeucedanin (14), alloisoimperatorin (15), demethylfuropinarine (16), 5-hydroxy-8-methoxypsoralen (17), oxypeucedanin methanolate (18), pabulenol (19), byakangelicin (20), marmesin (21), (+) -decursinol (22), heraclenol (23), oxypeucedanin hydrate (24), marmesinin (25), ulopterol (26), erythro-guaiacylglycerol-β-ferulic acid ether (27), threo-guaiacylglycerol-β-ferulic acid ether (28), and uracil (29). Compounds 5, 8, 11, 18, 21-23, and 26-28 were obtained from the roots of title plant for the first time.

9.Comparison of the efftec between eccentric fixation and intramedullary fixation for treatment of intertrochanteric fractures.

OBJECTIVETo compare the efficacy between eccentric fixation and internal fixation for treatment of intertrochanteric fractures of femur,to provide a theoretical basis for the selection of the treatment method of the intertrochanteric fractures of femur.
METHODSFrom February 2007 to January 2010,82 patients with femoral intertrochanteric fracture were treated by internal fixation including 39 cases of eccentric fixation involving 23 males and 6 females, aged from 41 to 81 years old with an average of (62.68±10.69), using the DHS or proximal femoral locking plate; 43 cases of intramedullary fixation involving 15 males, 28 females,aged from 43 to 78 years old with an average of (62.60±8.37),using PFN or PFNA fixed. The surgical incision length, operative time, blood loss and postoperative Harris score between two groups were compared.
RESULTSThe wound of two groups were primary healing without operative complications. All cases received follow-up for an average time of 18.3 months (12 to 28 months). The incision length, operative time and blood loss had a statistically significant difference between two groups (P<0.05). Harris scores of hip joint function at 1 month after operation had statistically significant difference between two groups (P<0.05), and Harris scores at 12 months after operation had no statistical significance difference between two groups. The rate of excellent and good was 89.7% in eccentric fixation group and 90.7% in intramedullary fixation group,the difference was not statistically significant (t=0.0613, P>0.05). In eccentric fixation group, there was 1 case of fracture nonunion with DHS loose and ensuing hip varus deformity. In intramedullary nail fixation group, there was no anti-rotation out,distal intramedullary nail of femoral refracture occurred in 1 case.
CONCLUSIONTwo treatment methods for the treatment of femoral fractures had a good therapeutic effect,but the intramedullary fixation had shorter operative time and less blood loss than the eccentric fixation,it prior to apply to osteoporosis and unstable femoral intertrochanteric fractures.

10.Study on the antioxidative effect of losartan at increasing dose in diabetic nephropathy

Objective To understand the situation of oxidative stress among diabetic nephropathy (DN) patients and observe the antioxidative effect of losartan at increasing dose in DN patients. Methods Thirty type 2 DN patients who neither smoked and nor took antioxidants were selected. The study began with an initial 4-6 weeks screening-treatment. Eligible patients then received losartan 50 mg/d daily for 8 weeks followed by losartan 100 mg/day daily for an additional 8 weeks. Blood glucose and blood pressure were closely monitored over the whole study period. All patients were followed up every other weeks, their 24-hour urine samples,fresh urine and venous blood sample were collected to measure urinary protein and creatinine excretion, urinary 8-OHdG, SOD, TAOC and MDA excretion , serum SOD, TAOC , MDA and other blood biochemistry parameters. Urinary 8-OHdG was determined by capillary electrophoresis and liquid phase chromatography. Results The total 24 hours urinary 8-OHdG excretion and the serum MDA concentration were higher than the normal values. The serum and urine SOD concentrations were lower than the normal values. There was an improvement in urinary 8-OHdG,serum and urine SOD, serum and urine MDA levels with losartan therapy. Compared with losartan 50 mg/d, the antioxidative effect of losartan 100 mg/d was more noticeable. Obvious decrease in 24-hour proteinuria on exposure to losartan was found, without severe adverse effect. Conclusions Oxidative stress damage is active in DN patients. Losartan has antioxidative effect on DN patients. Compared with losartan 50 mg/d, the antioxidative effect of losartan 100 mg/d is more marked, without increasing side effect. Losartan's antioxidative effect may be involved in its beneficial mechanisms on DN.
