The study was guided by the scientific development concept in order to gradually improve the efficiency of health services and achieve a comprehensive , coordinat-ed and sustainable development of the hospital .
6.Association between vitamin D receptor gene polymorphisms and pediatric Crohn's disease in China: a study based on gene sequencing.

OBJECTIVETo investigate the association between TaqI, BsmI, and ApaI polymorphisms of vitamin D receptor (VDR) gene and pediatric Crohn's disease (CD) in China.
METHODSNineteen children with CD were selected as a case group, and 122 healthy children who underwent physical examination were selected as a control group. Serum 25-hydroxyvitamin D3 [25(OH)D3] levels were measured using ELISA. The TaqI, BsmI, and ApaI polymorphisms of VDR gene were determined by gene sequencing, and the two groups were compared in terms of genotype and allele frequencies.
RESULTSThe case group had significantly lower serum 25(OH)D3 levels than the control group (17.3±2.4 ng/mL vs 26.9±2.1 ng/mL; P<0.05). There were no significant differences in the frequencies of genotypes and alleles of TaqI, BsmI, and ApaI polymorphisms between the case and control groups (P>0.05).
CONCLUSIONSChildren with CD have low serum 25(OH)D3 levels. TaqI, BsmI, and ApaI polymorphisms of VDR gene may not be associated with susceptibility to CD among the Chinese population.

7.Prevention and treatment of stress ulcer bleeding in patients with extensive burns.

OBJECTIVETo review the measures for prevention and management of stress ulcer bleeding in patients with extensive burns.
METHODSA retrospective study was conducted in 1093 extensively burned patients to understand the incidence of stress ulcer bleeding in comparison with the results according to the literature reports.
RESULTSThe incidence of stress ulcer bleeding was 3.2% in these patients, significantly lower that reported in literatures (6.8%, n=1023) (P<0.01).
CONCLUSIONComprehensive measures should be taken in extensively burned patients, such as shock resuscitation, early feeding, adequate infection and wound management, use of antacid, painkiller and oxygen therapy, which may help reduce the risks of stress ulcer bleeding.

8.Retrograde locked intramedullary nail for the treatment of middle or distal fractures of humeral shaft.

OBJECTIVETo study therapeutic effects of retrograde locked intramedullary nail for the treatment of middle or distal fractures of humeral shaft.
METHODSFrom October 2002 to February 2005, 21 patients with middle or distal fractures of humeral shaft were treated with AO retrograde locked intramedullary nail. Among the patients, 13 patients were male and 8 patients were female, ranging in age from 23 to 56 years, with an average of 34.5 years. Nineteen patients were fresh fractures, and 2 patients were old fractures. Eleven patients had middle fractures of humeral shaft, and other 10 patients had distal fractures of humeral shaft. Two patients with old fractures were non-union and treated with autoallergic bone grafting; 5 patients combined with radial nerve injury underwent exploration. The therapeutic effects were evaluated according to Constant-Murley shoulder score and Mayo elbow score.
RESULTSAll the patients were followed up, ranging from 18 to 24 months, with an average of 20.8 months. The fractures were healed except 1 patient with old fracture and 1 patient with extremely distal acute fracture. The shoulders and elbows had satisfactory recovery except 1 patient with old fracture. The mean Constant-Murley score was (83.6 +/- 13.4) points (50-97), 12 patients got an excellent result, 5 good, 4 fair and no bad; the mean Mayo elbow score was (88.6 +/- 9.8) points (65-95), 13 patients got an excellent result, 5 good, 3 fair and no bad.
CONCLUSIONThe retrograde locked intramedullary nail offers an ideal treatment method for middle and distal fractures of humeral shaft. Strict operative indication, precise surgical techniques and stable locking are the keys to a successful treatment results.

9.Correlation between NT-proBNP and plasma interleukin-15 level in patients with chronic congestive heart failure

Objective :To explore change of plasma interleukin 15 (IL-15) level and its correlation with N terminal pro B-type natriuretic peptide (NT-proBNP).Methods :A total of 205 patients diagnosed as CHF were sequentially en-rolled as CHF group ,and another 52 healthy adults undergoing physical examination simultaneously were regarded as healthy control group .According to NYHA cardiac function class ,CHF group was further divided into class II group (n=55) ,class III group (n=87) and class IV group (n=63).Plasma IL-15 level was measured and compared among all groups ,and correlation among plasma level of NT-proBNP ,left ventricular ejection fraction (LVEF) and plasma IL-15 level were analyzed in CHF patients .Results : Compared with healthy control group ,there was signif-icant rise in plasma NT-proBNP level [ (71 ± 33) pg/ml vs.(5884 ± 1379) pg/ml] ,and significant reduction in LVEF [(57 ± 7)% vs.(31 ± 8)% ] in CHF group ,P=0.001 both .Plasma IL-15 level of CHF group was significant-ly higher than that of healthy control group [(18.35 ± 6.42) ng/L vs.(7.78 ± 3.11) ng/L] , P=0.001 ;compared with class II group ,there was significant rise in plasma IL-15 level [ (10.24 ± 3.61) ng/L vs.(20.17 ± 6.35) ng/L vs.(21.94 ± 7.32) ng/L] in class III and IV group , P=0.001 both ,and there was no significant difference be-tween class III and class IV group (P=0.187).Pearson correlation analysis indicated that plasma IL-15 level was significant positively correlated with plasma NT-proBNP level ( r= 0.172 , P= 0.038).Conclusion :Plasma IL-15 level of CHF patients is significantly higher than that of healthy subjects ,and significant positively correlated with NT-proBNP level ,and it has certain relationship with heart failure severity ,so it may become a new marker for pre-dicting CHF .
10.Effects of Leptospira interrogans infection on the activation of NLRP3 in THP-1 and J774A. 1 cells

Objective To analyze the effects of Leptospira interrogans ( L. interrogans) infection on the activation of NLRP3 in THP-1 and J774A. 1 cells and to further understand the mechanism of inflam-mation caused by L. interrogans in different hosts. Methods Human mononuclear macrophage cell line (THP-1) and murine mononuclear macrophage cell line (J774A. 1) were infected with L. interrogans strain 56601. The expression of NLRP3 at mRNA and protein levels were measured by using real-time RT-PCR and flow cytometry analysis, respectively. The NLRP3-mediated secretion of IL-1β, IL-18 and IL-33 was detec-ted by ELISA combined with the NLRP3 inhibitory test. Results Compared with the normal cells, the ex-pression of NLRP3 at mRNA level in L. interrogans-infected THP-1 cells was respectively increased by 4. 05, 0. 34, 0. 33, 0. 06 and 1. 66 times at the time points of 1 h, 2 h, 4 h, 12 h and 24 h after infection ( P<0. 05), while that in L. interrogans-infected J774A. 1 cells was respectively increased by 12. 98, 16. 19, 10. 68, 5. 8 and 0. 57 times (P<0. 05). The expression rates of NLRP3 protein in THP-1 and J774A. 1 cells respectively increased from 9. 26% to 94. 01%, 89. 24%, 31. 80%, 19. 74%, 11. 28% and from 18. 71%to 58. 78%, 43. 64%, 36. 42%, 76. 46%, 85. 21% at the time points of 1 h, 2 h, 4 h, 12 h and 24 h af-ter L. interrogans infection (P<0. 05). The level of IL-1β in L. interrogans-infected THP-1 cells was 73. 07 pg/ml, 939. 24 pg/ml, 939. 24 pg/ml, 843. 22 pg/ml and 851. 06 pg/ml at the time points of 1 h, 2 h, 4 h, 12 h and 24 h, respectively (P<0. 05), while the level of IL-1β in L. interrogans-infected J774A. 1 cells began to rise at the time point of 12 h from 191. 17 pg/ml to 254. 4 pg/mL at the time point of 24 h (P<0. 05). The level of IL-18 in L. interrogans-infected THP-1 cells was 913. 89 pg/ml, 808. 19 pg/ml, 483. 54 pg/ml, 204. 19 pg/ml and 189. 09 pg/ml at the time points of 1 h, 2 h, 4 h, 12 h and 24 h, re-spectively (P<0. 05), while the level of IL-18 in L. interrogans-infected J774A. 1 cells increased at the time point of 24 h, which was 113. 37 pg/ml (P<0. 05). A slight increase in the level of IL-33 was detected in L. interrogans-infected J774A. 1 cells at the time points of 12 h and 24 h to 201. 14 pg/ml and 155. 68 pg/ml, respectively (P<0. 05), but no significant change was detected in L. interrogans-infected THP-1 cells (P>0. 05). Results of the inhibitory test showed that the up-regulation of IL-1β , IL-18 and IL-33 in THP-1 and J774A. 1 cells were effectively inhibited by the specific inhibitor of NLRP3. Conclusion NL-RP3 inflammasome was activated and involved in the production of specific inflammatory cytokines IL-1βand IL-18 in both THP-1 and J774A. 1 cells after L. interrogans infection, but the inflammatory cytokines induced by L. interrogans infection varied in different cells. L. interrogans induced earlier and higher level of IL-1βand IL-18 production in human macrophages than in murine macrophages.
