1.Relationship between self-management behaviors and family functions among elderly patients with diabetes in community

Objective To explore the relationship between self-management behaviors and family function among elderly pa-tients with diabetes in community in order to provide evidence for community nursing of diabetes.Methods A total of 670 elderly patients with diabetes were selected with convenience sampling household survey method and investigated with the family Adapta-tion,Partnership,Growth,Affection,and Resolve(APGAR)and the Summary of Diabetes Self-Care Activities(SDSCA).Results The family functions total score of the elderly patients with diabetes in community was (6.95±2.91),the self-management behav-iors total score was (20.06±5.80),among the self-management behaviors the score for exercise was the highest and blood glucose monitoring was the lowest;family function had remarkable positive relation with the total score of self-management behaviors a-mong elderly patients with diabetes in community(r=0.156,P <0.05).Conclusion There is a correlation between the self-man-agement behaviors and family function,good family function is conducive to the improvement of self-management behaviors for the elderly patients with diabetes.
3.Effect of glycoprotein (transmembrane) nonmetastatic melanoma protein B on the proliferation and migration of as well as melanogenesis in melanoma cells

Objective To estimate the effect of glycoprotein (transmembrane) nonmetastatic melanoma protein B (GPNMB) on the proliferation and migration of as well as melanogenesis in melanoma cells.Methods The expression of GPNMB was detected by immunofluorescence assay in two melanoma cell lines M14 and G-361,as well as in primary human melanocytes.Then,the three kinds of cells each were classified into three groups:experimental group treated with small interfering RNA targeting GPNMB (GPNMB-siRNA),negative control group treated with the negative control siRNA,blank control group remaining untreated.Methyl thiazolyl tetrazolium (MTT) assay,transwell invasion assay and spectrophotometry were performed to evaluate cell proliferation activity,invasion potential and melanin levels,respectively.Statistical analysis was done using Student's t test.Results GPNMB was expressed in both melanoma cells and melanocytes.The transfection with GPNMB-siRNA down-regulated the mRNA and protein expressions of GPNMB in,and markedly suppressed the proliferation and migration of,melanoma cells.In detail,the proliferative activity (expressed as the absorbence value at 570 nm) of M14 and G361 cells was reduced by 35% and 40% respectively,the migration activity of M14 and G361 cells by 49% and 51% respectively,and the melanin levels in melanocytes,M14 cells and G361 cells by 73%,82% and 69% respectively,in the experiment group compared with those in the blank control group.Conclusions The siRNA-mediated silencing of GPNMB could effectively inhibit the proliferation of,invasion of and melanogenesis in melanoma cells,which suggests that GPNMB plays critical roles in the initiation and progression of melanoma.
4.Effect of Occupational Therapy on Hemianesthesia and Hand Function after Stroke

Objective To explore the effect of occupational therapy (OT) on hemianesthesia and hand function after stroke. Methods 50 stroke patients with hemianesthesia were divided into OT group (n=25) and control group (n=25). The OT group accepted OT combined with routine therapy and the control group accepted routine therapy only. They were assessed with upper extremity sense of Fugl-Meyer Assessment (FMA) and evaluation of hand function before and after treatment. Results The scores of FMA and hand function improved significantly in both groups 2 months after treatment, and improved more in the OT group than in the control group (P<0.05). Conclusion OT can improve the sense and the hand function in stroke patients with hemianesthesia
5.Effect of Occupational Therapy on Post Stroke Depression

Objective To explore the effect of occupational therapy (OT) on the depressive mood and activity of daily living (ADL) in patients with post stroke depression (PSD). Methods 42 patients with PSD were divided into OT group (n=21) and control group (n=21).Both 2 groups received routine therapy, and OT group received ADL training, selective recreation and sport and appreciation of music in addition.They were evaluated by modified Bathel index (MBI) and Hamilton Self-rating Scale for Depression (HAMD) before and after treatment.Results The scores of MBI increased and the scores of HAMD decreased significantly after 4 weeks in both 2 groups (P<0.001), especially in the OT group (P<0.01). Conclusion OT can improve not only ADL, but also the depressive mood in patients with PSD.
6.Multiple Trichodiscoma:the First Case Report in China

Objectives To report the first case of multiple trichodis coma in China.A 31-year-old man presented with multiple,broomcorn grain to r ed bean-sized,skin-colored,firm papules on the right lower extremity for 15 years,and similar lesions on the left knee and lower leg for about one year.T he lesions were asymptomatic.Methods and Results Histopathology showed acanth osis of epidermis,proliferation of fine reticulate collagen fibers and depositi on of focal mucinous proteins in upper dermis.Small blood vessels and nerve fib ers were increased,and proliferation of elastic fibers was localized in the hai r follicles and around some rete ridges.Proliferated hair follicles were seen i n the margins of the lesions and extended down in a collar-like pattern.Ultra structurally,Merkel cell-axon complex was located in the overlying basal lami na of the epidermis.Myelinated nerve fibers were seen in the upper dermis.Bloo d vessel alterations were found under electron microscopy,some basal laminae of blood vessels exhibited laminated structure,proliferation of fibrous component,and thickened wall of blood vessels were observed.Conclusions The disease is rare.It is a hamartoma originated from hair disk.
7.Comparison of the efftec between eccentric fixation and intramedullary fixation for treatment of intertrochanteric fractures.

OBJECTIVETo compare the efficacy between eccentric fixation and internal fixation for treatment of intertrochanteric fractures of femur,to provide a theoretical basis for the selection of the treatment method of the intertrochanteric fractures of femur.
METHODSFrom February 2007 to January 2010,82 patients with femoral intertrochanteric fracture were treated by internal fixation including 39 cases of eccentric fixation involving 23 males and 6 females, aged from 41 to 81 years old with an average of (62.68±10.69), using the DHS or proximal femoral locking plate; 43 cases of intramedullary fixation involving 15 males, 28 females,aged from 43 to 78 years old with an average of (62.60±8.37),using PFN or PFNA fixed. The surgical incision length, operative time, blood loss and postoperative Harris score between two groups were compared.
RESULTSThe wound of two groups were primary healing without operative complications. All cases received follow-up for an average time of 18.3 months (12 to 28 months). The incision length, operative time and blood loss had a statistically significant difference between two groups (P<0.05). Harris scores of hip joint function at 1 month after operation had statistically significant difference between two groups (P<0.05), and Harris scores at 12 months after operation had no statistical significance difference between two groups. The rate of excellent and good was 89.7% in eccentric fixation group and 90.7% in intramedullary fixation group,the difference was not statistically significant (t=0.0613, P>0.05). In eccentric fixation group, there was 1 case of fracture nonunion with DHS loose and ensuing hip varus deformity. In intramedullary nail fixation group, there was no anti-rotation out,distal intramedullary nail of femoral refracture occurred in 1 case.
CONCLUSIONTwo treatment methods for the treatment of femoral fractures had a good therapeutic effect,but the intramedullary fixation had shorter operative time and less blood loss than the eccentric fixation,it prior to apply to osteoporosis and unstable femoral intertrochanteric fractures.

8.Application value of dose reduction techniques (MinDose) in dual - source CT coronary artery angiography

Objective To evaluate the radiation dose and image quality of MinDose techniques in dual-source CT coronary artery angiography.Methods 120 consecutive patients undergoing cardiac CT scans were randomly assigned into 2 groups: Group A1 with the tube current reduced to 20% of the normal tube current outside the pulsing window,and Group A2 with the tube current reduced to 4% of the normal tube current outside the pulsing window (MinDose).The image quality,noise,volume CT dose index (CTDIvol),and effective dose (E) of these two groups were evaluated.Results The mean score of imaging quality of Group A1 was (4.3 ± 0.3 ),not significantly different from that of Group A2 [(4.5 ±0.4),t=0.16,P>0.05].The value of CTDIvol of Group A1 was (40.2 ±9.6) mGy,significantly higher than that of Group A2 [( 36.4 ± 9.1 ) mGy,t = 3.2,P < 0.05].The E value of Group A1 was (9.1± 2.2) mSv ,significantly higher than that of A2 [( 8.1 ± 1.9) mSv,t = 2.7,P < 0.05].Conclusion Application of the MinDose technique not only reduces the radiation dose to the patient,but also meets the requirement of diagnosis.
9.Preliminary study of vulvar and vaginal intraepithelial neoplasias

Objective:To detect the clinical features of vulvar and vaginal intraepithelial neoplasias(VIN and VAIN,respectively).Methods:Total 148 women were performed vulvar or vaginal coloposcopy-directed biopsy pathology tests,from Sep.2004 to Dec.2007.Results:Among 148 women,vulvar or vaginal histologic results were vulvar cancer for 1,VIN or VAIN 2,3 for 23,VIN or VAIN for 16,condyloma for 61,vulvitis and vaginitis for 47.Eighty-five percent(33/39) women with VIN or VAIN 2,3 were more than 30 years old.Compared to women with cervical intraepithelial neoplasia(CIN),women with VIN or VAIN were older.The rate of high-risk HPV DNA in women with vulvar or vaginal lesions was 84%(84/100).VAIN occurred mainly in the upper vagina(90%,69/75).VIN or VAIN often accompanied or followed CIN or cervical cancer(79%,31/39),and VIN or VAIN 2,3 often accompanied or followed CIN 2,3 or cervical cancer(70%,16/23).Conclusion:Our data suggest that women with high-risk HPV infection are at risk of developing VIN or VAIN 2,3.The vulva and vagina should be carefully inspected by colposcopic examination at the time of colposcopy for any abnormal findings.
10.Clinicopathological characteristics of patients with gastrointestinal malignant tumor in some parts of Hainan province

Objective To analyze the clinicopathological characteristics of patients with gastrointestinal malignant tumor in some parts of Hainan province and alien patients outside of Hainan province. Methods Three hundred and fifteen patients who were diagnosed by pathological confirmation were selected in Hainan Branch of PLA General Hospital from Jun 2012 to June 2016, clinicopathological parameters including age, gender, tumor location, cancer cell differentiation, body mass index (BMI), body surface area (BSA), value of preoperative tumor markers, TNM stages, microsatellite state, human epidermal growth factor receptor 2 (Her-2) staining, Ki-67 index and risk factors were collected and compared between local and alien patients. Results In contrast with patients outside of Hainan province, a significant lesser BMI [colorectal cancer: (23 ± 3) kg/m2 vs. (24 ± 4) kg/m2, t=-3.69, P< 0.05; gastric cancer: (21.50 ± 3.15) kg/m2 vs. (23.00 ± 3.61) kg/m2, t=-2.11, P=0.04] and BSA [colorectal cancer:[(1.73 ± 0.18) m2 vs. (1.82 ± 0.18) m2, t=-3.46, P<0.05;gastric cancer:(1.73 ± 0.16) m2 vs. (1.81 ± 0.18) m2, t=-2.09, P=0.04] were found in local patients. Local patients with colorectal cancer were much more younger [(57 ± 13) years old vs. (62 ± 13) years old, t=-2.30, P=0.02], with corresponding elevated positive rate of preoperative CEA (χ2=4.56, P= 0.03), T stages (χ2 = 8.31, P= 0.04) and TNM stages (χ2= 11.19, P= 0.01), however, no such difference was detected in gastric cancer patients (all P>0.05). Conclusions Patients with gastrointestinal cancer in some parts of Hainan province, in particular, colorectal cancer patients, present some exceptional clinicopathological features, which are marked by younger age at diagnosis, low level of BMI and BSA. In addition, preoperative level of CEA would be of important for these patients.
