1.Orthogonal Test for Optimization of Qinshuang Particle Spray Granulation Process

This study was aimed to optimize Qinshuang particle best spray granulation process conditions. The or-thogonal test method was used with grain yield and baicalin transfer rate as the inspection indexes. The main in-fluence factors of Qinshuang particle spray granulation process were investigated. The results showed that the best technological parameters are atomization pressure of 1 800 Pa, inlet temperature at 90°C - 100°C, outlet temper-ature at 50 ° C - 60 ° C , and the extract relative density of 1 . 10 ( 60 ° C ) . It was concluded that the reasonable pro-cess conditions make particles with stable quality, which provide the experimental basis for industrial production.
2.Correlation between blood-brain barrier damage and depression in patients with cerebral small vessel disease

Objective To investigate the correlation of blood-brain barrier (BBB) damage and depression in patients with cerebral small vessel disease (CSVD).Methods Consecutive patients with CSVD admitted to hospital were enrolled prospectively.The patients completed head MRI and cerebrospinal fluid (CSF) examination after admission.The BBB damage degree was evaluated by using albumin CSF/serum ratio (Q-Alb).At 3 months after onset,the depression was assessed according to the Hamilton depression scale (HAMD) and the American Diagnostic and Statistical Manual of mental disorders,4th edition (DSM-Ⅳ).The correlation between the BBB damage and depression in patients with CSVD was analyzed,Results A total of 130 consecutive patients with CSVD were enrolled,including 58 (44.62%) had depression within 3 months.There were significant differences in the proportions of patients with lacunar infarction (43.10% vs.26.39%;x2 =4.008,P =0.045),leukoaraiosis (75.86% vs.58.33%;x2 =4.408,P =0.036),and cerebral microbleed (27.59% vs.12.50%;x2 =4.707,P =0.030),and baseline National Institutes of Health Stroke Scale (NIHSS) scores (5.02 ± 2.51 vs.3.60 ± 2.43;t =3.256,P =0.001),Fazekas scales of deep white matter (2.35 ± 1.00 vs.1.56 ± 1.05;t =4.358,P <0.001) and the proportion of Q-AIb category (x2 =6.852,P =0.033) between the depression group and the non-depression group.Multivariate logistic regression analysis showed that the baseline NIHSS scores (odds ratio [OR] 1.248,95% confidence interval [CI] 1.027-1.517;P =0.026),leukoaraiosis (OR 14.786,95% CI 1.776-123.111;P=0.013),Fazekas scales of deep white matter (OR 1.847,95% CI 1.210-2.819;P=0.004),and Q-Alb (OR 30.417,95% CI 3.662-252.643;P =0.004) had significant independent correlation with depression.Conclusions The BBB damage is independently associated with depression in patients with CSVD.
3.Efficacy and reliability of a five-level pediatric emergency triage system

Objective To formulate a five-level pediatric emergency triage standard and evaluate the efficacy and reliability of it in determining severity of emergency pediatric patients.Methods According to the assessment methods in Pediatric Advanced Life Support recommended by American Heart Association and American Academy of Pediatrics,we formulated a five-level pediatric emergency triage standard based on the situation of our hospital and analyzed the data one year before and after the application of it.Results Before and after the application of the triage standard,the average satisfaction rate of emergency patients were (81.28 ± 3.97) % and (94.13 ± 4.62) %,and there was significant difference (P < 0.01) ; the proportion of whom became worse during waiting time were 1.83% (628/34275) and 0.04% (16/36 187),and there was significant difference (P <0.01) ;the average waiting time of emergency admission patients were (12.71 ± 2.32) min and (3.34 ± 1.95) min,and there was significant difference (P <0.01) ;the misjudgment rate of severity were 3.78% (1 296/34 275) and 0.57% (205/36 187),and there was significant difference (P < 0.05).Conclusion The five-level triage standard is objective,easy to master,and suitable for pediatric triage.It can quickly sort out critical cases from emergency pediatric patients,which can improve the effectiveness of emergency service,make use of medical resources rationally and somewhat solve the problem of overcrowding.

Objective To evaluate the clinical feasibility of localization CT enhanced image replacing plain CT scan image for target delineation and dose calculation.Methods Forty cases of NPC were collected and divided into two groups with different concentrations of contrast agents.The contours of planning target volume (PTV) and organs at risk (OARs) of each case were delineated in the plain scan image,and the contours of PTV and OARs were copied to the enhanced image.Two plans based on the plain scan image and the enhanced image were designed in the planning system of Eclipse.The dose distribution and OARs and MU were compared between the groups.Results No statistical differences were found in the dosimetry of PTV,OARs and MU (P>0.05).Conclusion The image intensifier has little effect on the dose calculation of Eclipse for NPC.In the radiotherapy for NPC,the localization CT enhanced image can be used to replace the plain CT scan image for target delineation and dose calculation.
6.Classification and clinical study of plasma cell mastitis under breast duct endoscope.

OBJECTIVETo evaluate the clinical significance of breast duct endoscope for plasma cell mastitis.
METHODSBreast duct endoscope was performed on 250 patients with nipple discharge. The characters of 95 cases of plasma cell mastitis under the breast duct endoscope were analyzed and studied.
RESULTSAmong the 95 cases with plasma cell mastitis, 21 were in type I, 43 in type II, 18 in type III and 13 in type IV. Eighty-three cases were treated with no-operation treatment, and 45 completely released, 38 partly released. There were 5 cases with middle to serious atypical hyperplasia in 12 cases treated with operation, that were all confirmed by pathological examination.
CONCLUSIONIn patients with plasma cell mastitis, clear diagnosis can be made through breast duct endoscope, and the breast duct with lesions could be positioned. It is a necessary assistant method for patients to avoid biopsy or decrease the biopsy range.

7.Clinical observation of gefitinib in treatment of female patients with adenocarcinoma of lung WU

Objective To evaluate the efficacy and adverse effects of gefitinib in the treatment of fe- male patients with advanced adenocarcinoma of lung who had failed to previous chemotherapy.Methods These patients received 250mg of gefitinib orally,once daily until disease progression or development of intol- erable toxic reaction.They were evaluated one month after treatment and every other month thereafter.Results Among the 27 evaluable patients,there were 1 CR(3.7%),11 PR(40.8%),10 SD(37.0%)and 5 PD(18.5%). The overall response rate was 44.5%(95% CI 29%~68%);and 22 patients(81.5%)gained profit(CR+PR+ SD)from the clinical therapy(95% CI 62%~94%);the mean TTP was 7.2 months.Symptomatic improvement rate was 80.0%.The main adverse effects were mild rash and diarrhea.Conclusion gefitinib has significant efficacy in the treatment of female patients with advanced tung cancer who had failed to previous chemother- apy.Adverse effects are mild.gefitinib is a suitable therapy for these patients.
8.Long-acting gonadotropin-releasing hormone analogue in treatment of idiopathic central precocious puberty in girls.

OBJECTIVETo investigate the efficacy and side-effects of long-acting gonadotropin-releasing hormone analogue (GnRHa) in treatment of idiopathic central precocious puberty (ICPP) in girls.
METHODSThirty six ICPP girls were treated with GnRHa. The secondary sexual characteristics, uterus volume, ovary volume, follicle development, bone age/chronological age (BA/CA), predicted adult height (PAH), serum inhibitor A (INHA) and inhibitor B (INHB), body mass index (BMI) and growth rate were compared before and after treatment. Bone age was assessed using the Greulich-Pyle method, PAH was calculated by the Payley-Pinneau method, serum INHA and INHB were measured by ELISA, and serum LH and FSH were measured by chemoluminescence.
RESULT(1) Breast development was reduced after 3 to approximately 6 months of treatment, and no continued development were observed. In 12 girls the breast development returned from Tanner II to B1. (2) Both uterus and ovary volume were decreased after 6 months [(3.28 +/-2.20)ml comrade with (1.27 +/-0.69)ml and (3.62 +/-1.94)ml compared with (1.24 +/-0.50)ml, respectively]; the follicle was reduced or even disappeared; and the serum INHA and INHB were decreased from Log(0.93 +/-0.35)ng/L and Log(1.95 +/-0.37)ng/L to Log(0.60 +/-0.32)ng/L and Log(1.46 +/-0.32)ng/L, respectively. (3) BA/CA was decreased from 1.40 +/-0.20 to 1.26 +/-0.15; PAH was increased from (149.12 +/-4.04)cm to (152.84 +/-3.72)cm after one-year treatment; BMI showed no significant changes (16.04 +/-1.68 compared with 15.93 +/-1.69).
CONCLUSIONGnRHa can effectively inhibit the development of sex gland and the secondary sexual characteristics, stabilize or delay bone maturation, improve the predicted adult height, and has no observed side-effects in the short-term treatment for ICPP girls.

9.Psychological behavior of girls with idiopathic central precocious puberty before and after treatment with gonadotropin-releasing hormone analogue.

OBJECTIVETo observe the psychological behavior of girls with idiopathic central precocious puberty (ICPP) before and after treatment by gonadotropin-releasing hormone analogue (GnRHa).
METHODSRaven's Standard Progressive Matrices(SPM), Achenbach's Child Behavior Checklist (CBCL), Self-Esteem Scale (SES), and Body-Esteem Scale (BES) were used to assess the psychological behavior in the ICPP girls before and after GnRHa treatment, as well as in control girls. The serum levels of dehydroepiandrosterone (DHEA) and dehydroepiandrosterone sulfate (DHEAS) were measured by ELISA before and after GnRHa treatment.
RESULT(1) The SES and BES scores in ICPP were significantly lower than those of controls(P <0.05). The CBCL scores in depressed, withdrawn,aggressive and somatic complaint assessment were significantly higher in ICPP group than those of control group. (2) The SES score, the body strength scores for BES 12 months after treatment were significantly higher than those pretreatment (P <0.05). Serum DHEA levels in ICPP group and control group were Log(0.77 +/-0.36)microg/L and Log (0.28 +/-0.22) microg/L respectively, with a significant difference (P <0.01). Serum DHEA and DHEAS of ICPP 3 months after treatment were decreased from Log(0.83 +/-0.35)microg/L and Log(2.27 +/-0.30)microg/L to Log(0.68 +/-0.44)microg/L and Log (2.11 +/-0.43)microg/L (both P <0.05). The serum DHEA and DHEAS levels 12 months after treatment were Log(0.78 +/-0.30)microg/L and Log(2.40 +/-0.34)microg/L, there was no significant difference before and after treatment (P >0.05). (3) The SES score,the weight concern and body strength scores for BES were negatively correlated with serum DHEA and DHEAS levels in precocious puberty girls (r=-0.492,-0.356,-0.202 and -0.216, all P <0.05). The nine CBCL factors were not correlated with serum DHEA levels.
CONCLUSIONPrecocious puberty girls are prone to lower self-esteem and less confidence, which are correlated with the increase of serum DHEA levels. There is more frequency to be depressed, withdrawn, aggressive and complaining in these girls, however, which are not correlated with serum DHEA levels. GnRHa may reverse the problem of psychological behavior in ICPP girls.

10.Relationship of plasma ghrelin and adenohypophyseal hormone levels in female precocious puberty.

OBJECTIVETo investigate the relationship of plasma ghrelin and adenohypophyseal hormone levels in female precocious puberty.
METHODSA total of 84 patients aged from 6 to 9 years were enrolled in this study. They were divided into idiopathic central precocious puberty (ICPP) and premature thelarche(PT)groups according to their secondary sexual characteristics, bone age, volumes of uterus and ovary, and results of GnRH test. Plasma ghrelin levels were measured by radioimmunoassay. ACTH, TSH, PRL, GH, LH and FSH were measured by chemoluminescence technique.
RESULTSGhrelin levels in ICPP group were Log (2.42+/-0.26) ng/L, which were significantly lower than those in PT group and controls [Log (2.62+/-0.21) ng/L and Log (2.58+/-0.44) ng/L, respectively, P<0.05]. However there was no significant difference between PT group and controls(P>0.05). Ghrelin levels of ICPP girls with Tanner III were Log (2.31+/-0.24) ng/L, significantly lower than those of ICPP girls with Tanner II [Log (2.53+/-0.24) ng/L, P<0.05]. By bivariate correlation analysis, ghrelin levels in precocious puberty girls were negatively correlated with ACTH, PRL and LH15, LH30 and LH60 in GnRH test(r=-0.248, -0.235, -0.445, 0.405, 0.398, respectively, P<0.05). No significant correlation was found between ghrelin and GH, LH0(-2), FSH0(-2), and FSH15, FSH30 and FSH60 in GnRH test.
CONCLUSIONICPP girls have lower plasma ghrelin levels, which are decreased with the development of Tanner stage. The plasma ghrelin levels are negatively correlated with ACTH, PRL and LH.
