1.Controlled continuous curvilinear capsulorhexis in short axial length and shallow anterior chamber eyes

AIM:To investigate the efficacy of controlled continuous curvilinear capsulorhexis(CCC) technique in short axial length and shallow anterior chamber eyes.METHODS:Sixty-eight patients(68 eyes) with short axial length and shallow anterior chamber were included.The routine CCC technique was used in 32 cases (32 eyes) and controlled CCC technique was used in 36 cases (36 eyes).The success rate and complication were compared between two groups. RESULTS:The success rate of the routine technique group and controlled technique group was 53. 13% and 86.11% respectively. Incomplete CCC leading to posterior capsule tears was 9.38% and zero in two groups respectively.CONCLUSION: Controlled CCC technique can increase the success rate and reduce complications in short axial length and shallow anterior chamber eyes.KEYWORDS:phacoemulsification; continuous curvilinear capsulorhexis; complication
3.Clinical observation of gefitinib in treatment of female patients with adenocarcinoma of lung WU

Objective To evaluate the efficacy and adverse effects of gefitinib in the treatment of fe- male patients with advanced adenocarcinoma of lung who had failed to previous chemotherapy.Methods These patients received 250mg of gefitinib orally,once daily until disease progression or development of intol- erable toxic reaction.They were evaluated one month after treatment and every other month thereafter.Results Among the 27 evaluable patients,there were 1 CR(3.7%),11 PR(40.8%),10 SD(37.0%)and 5 PD(18.5%). The overall response rate was 44.5%(95% CI 29%~68%);and 22 patients(81.5%)gained profit(CR+PR+ SD)from the clinical therapy(95% CI 62%~94%);the mean TTP was 7.2 months.Symptomatic improvement rate was 80.0%.The main adverse effects were mild rash and diarrhea.Conclusion gefitinib has significant efficacy in the treatment of female patients with advanced tung cancer who had failed to previous chemother- apy.Adverse effects are mild.gefitinib is a suitable therapy for these patients.
4.Application of free flap pedicled with supracarpal cutaneous branch of ulnar artery in repairing of finger replantation.

OBJECTIVETo evaluate clinical application and clinical outcomes of free flap pedicled with supracarpal cutaneous branch of ulnar artery in repairing of finger replantation with skin defect.
METHODSFrom April 2007 to March 2013,25 patients affected by finger amputation with skin defect were replanted and repaired by free flap pedicled with supracarpal cutaneous branch of ulnar artery. Among them, 18 patients were male and 7 were female,with an average age of 31.5 years old (ranged 16 to 58). The time of trauma to admission ranged from 45 to 210 min (averaged 105). Fifteen patients were complete separted, and 10 patients were non-complete separated. The area of flaps ranged from 3.5 cm x 2.0 cm to 4.5 cm x 3.0 cm, and the vessels were anastomosed through end-to-end. The functional evaluation standard of finger replantation was used to evaluate the postoperative function.
RESULTSTwenty-four cases were finally survived. Two flaps occurred vascular crisisin within 48 h after operation, one of which was survived after anti-vasospasm treatment and changing dressing,another was replanted finger for failed to survive. One had infection and healed after changing dressing. Twenty-four cases were followed up from 3 to 38 months with an average of 16.5 months. The appearance and texture of flaps were satisfactory, and the superficial senses of pain and touch were recovered,and two-point discrimination was 5.5 to 11 mm (averaged 7.4 mm). According to functional evaluation standard finger replantationissued by Hand Surgery Association of Chinese Medical Association, 8 cases got excellent results, 14 good and 2 poor.
CONCLUSIONThe free flap pedicled with supracarpal cutaneous branch of ulnar artery can be used in complex finger replantation with skin and vessels defect, which can extend operation indications, recover function and appearance for maximum.

5.Long-term effect of repeated febrile convulsions on seizure susceptibility in immature rats.

OBJECTIVETo elucidate the long-term effect of repeated febrile convulsions (FC) on seizure susceptibility in immature rats.
METHODSWarm-water-immersion rat FC model was developed with SD rats 22 days of age, 15 attacks of seizures were induced in the rats every other day, and some of the rats were left un-stimulated for 3 months, then were re-stimulated. Seizure phenomenon was observed, including the latency and the duration of seizures and the temperature of rats. Hippocampal neuron loss and mossy fiber sprouting were detected by thionine staining and Timm staining.
RESULTSAfter the 15th bath, the latency, the duration of seizures, and the temperature of rats were respectively (4.3 +/- 0.8) min, (5.5 +/- 2.9) min, (42.2 +/- 0.7) degrees C. Three months later, on re-stimulation, in 14 of 19 rats with previous FC experience seizures occurred while in only one of 13 non-FC rats seizure occurred and lasted for 8.5 min. Three months later, the latency, the duration of seizures, and the temperature of rats were respectively (5.4 +/- 0.6) min, (19.3 +/- 5.1) min, and (42.4 +/- 0.4) degrees C (4 rats with status epileptics were not included). The incidence of seizures on re-stimulation in rats of FC group (74%) was significantly higher than that in non-FC group (8%) (chi(2) = 13.50, P < 0.01), and the duration of seizures [(19.3 +/- 5.1) min] was significantly longer than those induced in early life [(5.5 +/- 2.9) min] (t = 10.49, P < 0.01). After the 15th bath, no significant change was demonstrated in rats of different groups. While 3 months later, prominent neuron loss was observed in hippocampal CA(3) region in rats with previous FC experience (P < 0.01). Significant mossy fiber sprouting phenomenon was detected after the 15th bath and 3 months later (P < 0.05).
CONCLUSIONRepeated FC in early life enhances long term susceptibility of rats to seizure.

6.Effect of a compound Danshen preparation in preventing long-term gastric lipid emulsion administration-induced nonalcoholic steatohepatitis in rats.

OBJECTIVETo investigate the preventive effects of a compound Danshen preparation (DSC) on long-term gastric lipid emulsion administration-induced nonalcoholic steatohepatitis in rats.
METHODSTwenty-seven 3-month-old SD rats were randomized equally into 3 groups and subjected to daily intragastric administration for 20 weeks of distilled water (control), lipid emulsion at 5 ml/kg (model group), and lipid emulsion plus DSC at 5.0 g/kg (DSC treatment group). After blood glucose (BG) determination, the rats were sacrificed for measurement of serum TC, TG, HDL-c, AST, and ALT, and the liver was weighed and pathologically examined.
RESULTSCompared with the control group, the rats in the model group showed significantly increased BG, TC, LDL-c, arteriosclerosis index (AI), AST, ALT, liver weight, and liver index (P<0.01) and decreased HDL-c (P<0.01), while TG remained unchanged. Fatty degeneration, hydropic degeneration and necrosis with inflammatory cell infiltration were observed in the liver of the rats in the model group. Compared with the model group, the rats in DSC groups showed decreased BG, AI (P<0.01), liver weight, liver index, AST, and ALT (P<0.05) and increased HDL-c, with milder pathological changes in the liver.
CONCLUSIONSLong-term gastric perfusion of lipid emulsion causes lipid metabolic disorder and nonalcoholic fatty liver disease in rats characterized by increased TC and decreased HDL-c. DSC can significantly increase HDL-c and provide partial protection of the liver against the damages by the lipid emulsion.

7.Effect of Danshen root compound on blood lipid and bone biomechanics in mice with hyperlipemia-induced osteoporosis.

OBJECTIVETo observe the effects of Danshen root compound (DSC) on blood lipid and bone biomechanics in mice with hyperlipemia-induced osteoporosis.
METHODSForty Kunming mice were randomized into 5 equal groups, and were given intragastric administration with distilled water (control), lipid emulsion (LE) at the daily dose of 5 ml/kg, LE plus simvastatin, LE plus DSC at 5.0 g/kg (DSC-L group), and LE plus DSC at 10.0 g/kg (DSC-H group), respectively. Serum TC, TG, and HDL-c levels and left femur hydroxyproline, calcium and phosphate contents were measured in the rats, with the right femur taken for bone biomechanical test.
RESULTSCompared with those in the control group, serum TC, LDL-c and AI of the mice increased and HDL-c, Hyp and bone calcium decreased significantly (P<0.01) with lowered bone biomechanical properties. Compared with those of the LE model group, AI decreased and HDL-c increased significantly in DSC-L and DSC-H groups (P<0.01), and the bone biomechanics in DSC-H group was improved.
CONCLUSIONLong-term intragastric administration of lipid emulsion causes lipid metabolic disorder and induces osteoporosis due to hyperlipemia in mice. DSC can significantly increase HDL-c and partially prevent the occurrence of osteoporosis in mice.

8.Effect of glutamine on patients with systemic inflammatory response syndrome by strengthening parenteral nutrition

Objective To explore the effect of glutamine on the nutritional status and inflammatory response by strengthening parenteral nutrition in patients after general surgery .Methods Forty patients with systemic inflammatory response syndrome ( SIRS ) in our hospital were selected , and all patients were randomly divided into treatment group and control group, with 20 cases in each group.Patients in treatment group were given glutamine to strengthen parenteral nutrition , while those in the control group received amino acid solution for parenteral nutrition .The treatment continued for one week .The data of level of hemoglobin , albu-min, transferrin and immunoglobulin (IgG, IgA, and IgM), as well as nutrition risk screening score and acute physiology , age and chronic health evaluation score in two groups before and after parenteral nutrition were recorded and compared .Results After parenteral nutrition , the data of hemoglobin ,albumin and transferrin in the treatment group were (116.5 ±9.2), (35.7 ±2.4) and (66.3 ±2.4) g? L-1, respective-ly, and (102.5 ±8.6), (30.8 ±2.2) and (59.6 ±2.1) g? L-1 in control group (P<0.05);after parenteral nutrition, the data of IgG, IgA, and IgM levels in the treatment group were significantly higher than those in the control group , and nutrition risk screening score and acute physiology and chronic health score were significantly lower than those in the control group (P <0.05).Conclusion The effect of glutamine parenteral nutrition on SIRS patients is significant , and it can im-prove patients′nutritional status and the immune function recovery .
9.Construction of recombinant human CD59 using ICAM-2 promoter for endothelial-specific expression in xenotransplantation.

OBJECTIVETo construct a recombinant human CD59 gene containing intercellular adhesion molecule-2 promoter for high level endothelial-specific expression in xenotransplantation.
METHODSICAM-2 promotor fragment and CD59-intron 1 fragment were produced by PCR from the human blood genome, and then clone these fragments into a pcDNA3-CD59 eukaryotic expression vector which was followed by digestion with the specific restricted endonuclease (for example: EcoRI, Hind III). The ICAM-2 promoter and CD59-intron 1 fragments were identified by PCR, and sequencing. The recombinant was then transfected into pig aorta endothelial cells with Lipofection, and the expression was measured by flow cytometer.
RESULTSProducts of the sequences measured were in accord with the frames of the gene bank. The expression of the protein of this recombinant was positive.
CONCLUSIONThe CD59 recombinant gene is constructed successfully, providing a basis for transgenic research.

10.Distribution of human papillomavirus types in Shenzhen women.

OBJECTIVETo investigate the status of genital infection as well as distribution of types of human papillomavirus (HPV) in women in Shenzhen and provide population data for the future vaccine intervention on cervical cancer.
METHODSWomen with age between 15 and 59 years were selected in cluster stratified sampling from Huaqiaocheng community, Nanshan district, Shenzhen and received a population-based cervical cancer screening. After consent, every woman was interviewed by using questionnaire and tested by liquid-based cytology and HPV DNA (hybrid capture 2 and gene chips typing) separately.
RESULTSTotally 1 137 women were screened. The rate of high risk HPV of hybrid capture 2 test (14. 0% ) was higher than gene chips typing test (9. 8%) (chi(2) = 27. 198, P < 0. 001) ; the consistency of the two tests was acceptable ( kappa = 0. 498, P < 0. 001). The rates of low risk HPV types and other types of gene chips typing test in this population were 1. 9% and 0. 2% respectively. The percentages of HPV 16, 18 and 58 in HPV positive women were 29. 7% , 18. 9% and 18. 9%. The rates of different age group of low risk HPV were 1. 4% (17-34), 1. 7% (35-44) and 3. 2% (45-59) , respectively.
CONCLUSIONSHPV 16, 18, and 58 are the most popular types in the study population. The differences of infection rates of high risk HPV are due primarily to the variation of HPV16 distribution among age-specific population. The chances of being affected by low risk HPV will increase with age.
