1.Influence of early nursing intervention on recovery of upper limb edema after radical mastectomy

Objective To investigate the early nursing intervention on recovery of upper limb edema after radical mastectomy, and subjective comfort of patients and satisfaction degree with the service quality of nursing staff. Methods 70 patients according to different methods of nursing intervention were randomly divided into the control group and the observation group with 35 patients in each group. The observation group was given comprehensive care, including psychological care, raising affected limb, strengthen the training arm function, conducting various forms of aerobic motion, pressure gloves and massage, hot packs, physical therapy, the use of elastic bandages, guidance and other aspects of hospital care. The control group only received routine care. The nursing effect, subjective comfort degree, satisfaction degree with the service quality, time of edema alleviation and disappearance and hospitalization time of two groups were compared. Results Compared with the control group, nursing effect of the observation group was better,subjective comfort of patients and the satisfaction degree with the quality of service significantly improved,and the time of edema alleviation and hospitalization time was significantly shorter, the difference was statistically significant. Conclusions Early implementation of nursing interventions for patients with limb swelling after radical operation of breast cancer can improve patients satisfaction degree, promote the recovery of upper limb edema, reduce hospital stay.
2.ApoE genotyping by real-time fluorescence PCR

Objective To establish a new effective method by using real-time polymerase chain reaction (PCR) to detect single nucleotide polymorphism (SNP) typing for rapid identifying apolipoprotein E alleles.Methods To determine alleles of human apolipoprotein E genetic polymorphism at Cys112Arg locus was detected by PCR melting curve analysis with fluorophore SYBR Green I. In order to increase the speciality of SNP assays, high fidelity Taq polymerase was used. The reliability of SNP typing was validated by comparison with the results of direct DNA sequencing.Results Each sample was determined by double tubes, and two melting curves were analysis. As compared the Tm value of samples with the Tm of standard substance, the apoE genotype of samples was determined. The apoE genotype of 30 samples were E3/3 (27/30) and E3/4 (3/30) respectively, which was accordant with the results of PCR-RFLP and DNA sequencing.Conclusion The presented allelic assay was specific, easy to operate and applicable for discrimination of apolipoprotein E genotyping of human blood.
3.Vessel analysis of diabetic retinopathy based on image segmentation

Objective To enhance the clinical diagnosis efficiency and accuracy of diabetic retinopathy (DR) to facilitate its early finding and diagnosis.Methods Image segmentation method was used to process and analyze DR vessel.Clinical images by fundus fluorescein angiography (FFA) were used as the objects for the research on DR vessel segmentation and extraction.Results Image processing and analysis gained higher definition and accuracy in segmenting the normal retinal image and DR vessel when they were compared with the fundus image and FFA image.Conclusion The method proposed enhances the early diagnosis of DR by processing and analyzing the retinal vessel,and thus contributes to the increased living quality of the patient.
4.Establishment and Application of 18-STR Database in Han Population from Zhejiang Province

[Objective] To construct a database for the genetic polymorphism of 18 STR loci (D8S1179, D21S11, D7S820, CSFIPO, D3S1358, TH01, D13S317, D16S539, D2S1338, D19S433, VWA, TPOX, D18S51, D5S818, FGA, PentaE, PentaD, SE33) in Hart population from Zhejiang province. To investigate the application of 18 STR loci in the field of paternity testing and prenatal diagnosis. [Methods] Fluorescent dye labeling multiplex STR-PCR, capillary electrophoresis and DNA sequencer GeneScan were adopted in genotyping 598 unrelated samples collected from Han population in Zhejiang province. 18-STR database was established and analyzed. Population comparison was conducted between Han population in Zhejiang province and 8 other population. 15-STR and 18-STR identification system were compared in 497 paternity testing cases. [Results] We observed the distribution of 18 STR loci in Han population meet Hardy-Weinberge equilibrium and was different from other 8 population (X~2 test, P>0.05). Statistical results showed that the heterozygosis (He) ranged from 0.630 to 0.942. The combined power of discrimination was>0.9999999999. Compared with 15-STR identification system, higher paternity index scores and higher exclusion rate were obtained with 18-STR identification system in dual-case paternity test and mutation identification. One trisomy 21 fetus was found in a prenatal paternity test case which had two characteristic genotypes in 2 STR loci of D21S11 and Penta D. [Conclusions] The 18 loci were relatively highly genetic polymorphic in Zhejiang Han population and could be used for paternity testing. Some STR loci could be used in prenatal diagnosis for aneuploidy.
5.Value of HBsAb Positive and Detection of HBV-DNA in Selection of Blood Source and Organ Transplantation

OBJECTIVE To study the HBV-DNA level and its replication with only HBsAb positive for transplantation. METHODS Serial serum samples were studied with the quantitative determination of HBV-DNA by a quantitative PCR assay and determination of HBsAb by ELISA. RESULTS The positive rate of HBV-DNA was 8.4% in the patients with only HBsAb positive and that was 11.11% in the patients who had not been injected with vaccine;the level of HBV-DNA in 11 samples was less than 10~4 copies/ml and five samples showed the amount between 10~4-10~6 copies/ml.The positive rate of HBV-DNA was 8.4% in the patients with only HBsAb positive and that was 11.11% in the patients who had not been injected with vaccine;the level of HBV-DNA in 11 samples was less than 10~4 copies/ml and five samples showed the level between 10~4-10~6 copies/ml. CONCLUSIONS Patients with only HBsAb positive are still infective,which should be paid attention in transplantation.
6.Practice and thinking of foreign staff in pre-service training in ETC

The medical contingent of Chinese PLA to libera operated the China Ebola treatment center(ETC) with 100 beds,and employed 80 Liberian health workers according to the task requirements and the normal operation demand.Under consideration of current health education situation and epidemic prevention requirements,combined with experiences in the fight against SARS,the training content includes the Ebola prevention and control knowledge,isolation of patients and safe burial,malaria prevention and control knowledge,hospital disinfection technology,psychological intervention.Training methods diversified,with theoretical teaching,site visit,on-site demonstration and training and video guide.With the pass of the simulation training evaluation model in all staff,the expected goals of pre-service training have been achieved.
7.The lipid-associated membrane proteins derived from Mycoplasma pneumoniae strains induced the expression of HO-1 in THP-1 cells

Objective To investigate the effects of lipid-associated membrane proteins ( LAMPs) derived from Mycoplasma pneumoniae ( M.pneumoniae) strains on the expression of heme oxygenase 1 ( HO-1) in a human monocyte cell line (THP-1).Methods THP-1 cells were in vitro cultured with different concentrations of LAMPs for different times.The cytotoxicity of LAMPs to THP-1 cells was analyzed by using lactate dehydrogenase ( LDH) releasing test.The expression of HO-1 at protein and mRNA levels were de-tected by Western blot and real-time RT-PCR, respectively.The enzymatic activity of HO-1 protein was ex-amined by colorimetric assay.THP-1 cells stimulated with PBS and LPS were set up as the negative and pos-itive controls, respectively.Results A significantly enhanced LDH releasing rate was observed in THP-1 cells treated with 10 μg/ml of LAMPs.The expression of HO-1 at protein and mRNA levels in THP-1 cells were induced by LAMPs in a dose-dependent and time-dependent manner.The highest level of HO-1 protein was detected in THP-1 cells treated with 5.0 μg/ml of LAMPs.The transcriptional levels of HO-1 induced by LAMPs were significantly elevated at 3 h, peaked at 9 h and were decreased at 12 h.The expression of HO-1 protein in THP-1 cells was enhanced after 8 h of treatment with LAMPs and a significant decrease was observed at 20 h after reaching peaks at 12 h and 16 h.The activity of HO-1 protein was significantly en-hanced along with the increased expression of HO-1 protein.Conclusion The LAMPs derived from M.pneumoniae strains induced the expression of HO-1 at mRNA and protein levels.Moreover, the enzyme activity of HO-1 protein was enhanced in LAMPs treated THP-1 cells.

10.Expression of CD226 mRNA in the peripheral blood mononuclear cells of patients with systemic lupus erythematosus

Objective To investigate the expression level of CD226 mRNA in the peripheral blood mononuclear cells (PBMCs) of patients with systemic lupus erythematosus (SLE) and explore the relation between the gene expression and disease activity, and the relation between the gene expression and Gly307Ser polymorphism of CD226 was also examined. Methods CD226 gene was measured with real-time polymerase chain reaction (qRT- PCR) in PBMCs. The expression levels of CD226 gene in PBMCs were compared between 90 SLE patients and 30 healthy individuals. One-way ANOVA and pearson correlation were used for statistical analysis. Results The expression level of CD226 in the PBMCs of SLE patients (6.8±1.1) was significantly decreased compared to healthy individuals (26.5±6.7) (P<0.01), while there was no association between mRNA level and genotype (P>0.05). No correlation between ESR, CRP, ANA, SLEDAI scores, C3 and the expression level of CD226 gene was discovered. Conclusion In Hubei Chinese Han population, CD226-Gly307Ser locus is associated with the development of SLE, while T allele does not impact the expression of CD226 gene, thus the role of CD226 gene in autoimmune diseases should be explored in the future.
