1.Comparison of the efftec between eccentric fixation and intramedullary fixation for treatment of intertrochanteric fractures.

OBJECTIVETo compare the efficacy between eccentric fixation and internal fixation for treatment of intertrochanteric fractures of femur,to provide a theoretical basis for the selection of the treatment method of the intertrochanteric fractures of femur.
METHODSFrom February 2007 to January 2010,82 patients with femoral intertrochanteric fracture were treated by internal fixation including 39 cases of eccentric fixation involving 23 males and 6 females, aged from 41 to 81 years old with an average of (62.68±10.69), using the DHS or proximal femoral locking plate; 43 cases of intramedullary fixation involving 15 males, 28 females,aged from 43 to 78 years old with an average of (62.60±8.37),using PFN or PFNA fixed. The surgical incision length, operative time, blood loss and postoperative Harris score between two groups were compared.
RESULTSThe wound of two groups were primary healing without operative complications. All cases received follow-up for an average time of 18.3 months (12 to 28 months). The incision length, operative time and blood loss had a statistically significant difference between two groups (P<0.05). Harris scores of hip joint function at 1 month after operation had statistically significant difference between two groups (P<0.05), and Harris scores at 12 months after operation had no statistical significance difference between two groups. The rate of excellent and good was 89.7% in eccentric fixation group and 90.7% in intramedullary fixation group,the difference was not statistically significant (t=0.0613, P>0.05). In eccentric fixation group, there was 1 case of fracture nonunion with DHS loose and ensuing hip varus deformity. In intramedullary nail fixation group, there was no anti-rotation out,distal intramedullary nail of femoral refracture occurred in 1 case.
CONCLUSIONTwo treatment methods for the treatment of femoral fractures had a good therapeutic effect,but the intramedullary fixation had shorter operative time and less blood loss than the eccentric fixation,it prior to apply to osteoporosis and unstable femoral intertrochanteric fractures.

2.The crosstalk between autophagy and apoptosis: where does this lead?

Recent advances in the understanding of the molecular processes contributing to autophagy have provided insight into the relationship between autophagy and apoptosis. In contrast to the concept of "autophagic cell death," accumulating evidence suggests that autophagy serves a largely cytoprotective role in physiologically relevant conditions. The cytoprotective function of autophagy is mediated in many circumstances by negative modulation of apoptosis. Apoptotic signaling, in turn, serves to inhibit autophagy. While the mechanisms mediating the complex counter-regulation of apoptosis and autophagy are not yet fully understood, important points of crosstalk include the interactions between Beclin-1 and Bcl-2/Bcl-xL and between FADD and Atg5, caspase- and calpain-mediated cleavage of autophagy-related proteins, and autophagic degradation of caspases. Continued investigation of these and other means of crosstalk between apoptosis and autophagy is necessary to elucidate the mechanisms controlling the balance between survival and death both under normal conditions and in diseases including cancer.

3.Studies on the Screening of a Cyanide-Degradation Strain and Its Cyanide-Transformation Characteristics*

A bacterial strain DN25, effective on cyanide-degradation, was isolated from contaminated soil and identified as Alcaligenes sp. on the basis of phenotype analysis and 16S rDNA sequence analysis. It showed great tolerance to the cyanide, which can grow in the medium containing 500mg CN -/L. The suitable condition for the cell growth and boitransformation was pH8.0 and 30oC and the transformation rate for 500mg CN - /L could achieve 99% in 10 h. It has also been found that the screened strain had the ability of K 4Fe(CN) 6 transformation with 96% of transformation rate at 12 h for the concentration of 500 mg CN /L.
4.Diagnosis Value of Pericardium Crescent Sign under CT in Patients with Non-Calcified Constrictive Pericarditis

[Objective]To investigate the value of"pericardium crescent sign"in the diagnosis of non-calcified con-strictive pericarditis in CT.[Methods]Twenty-seven patients with non-calcified constrictive pericarditis and forty-six cardiac tumors confirmed by surgical pathology were analyzed retrospectively. All patients underwent non-enhanced and enhanced CT scan preoperatively.Pericardial morphology,density,enhancement features,appearance of atrium,ventri-cle and inferior vena cava of non-calcified constrictive pericarditis were analyzed. Pericardial morphology of cardiac tumors was also analyzed.[Results]Of the 27 patients with non-calcified constrictive pericarditis,18 patients developed"pericardium crescent sign",accounting for 66.7%,ventricular varying degrees of deformation in 13 cases,accounting for 48%;venous dilatation in 26 cases,accounting for 93%;atrial enlargement in 7 cases,accounting for 26%. In 46 patients with cardiac tumors,only 1 patient had a similar"pericardial crescent"sign.For the diagnosis of constrictive pericarditis using pericardial crescent sign,the sensitivity was 66.7%,the specificity was 97.8%,Youden index was 0.64. The area under the ROC curve was 0.82([0.71-0.94],P<0.001).[Conclusion]"Pericardium crescent sign"is important CT features of non-calcified constrictive pericarditis. It has high specificity for differentiating non-calcified constrictive pericarditis from pericardial tumor imaging.

ORF3 and partial ORF1 regions were amplified with RT-PCR f rom two patients (T1 and T11)infected with new genotype of hepatitis E Virus. Th e PCR products were cloned and sequenced. The results showed that G-C rich regi on in ORF3 was deleted when amplified with normal PCR reaction. However, PCR rea ction containing G-C melt solution can overcome this problem. The sequence anal ysis showed that T1 and T11 belong to a new genotype of HEV which differs from g enotype I,II and III reported.T1 and T11 have 79%~82%, 80%~81% and 83%~85% id entical to genotype I,II and III respectively.

Objective To observe the effect of berberine on visfatin expression in 3T3-L1 adipocytes,and to explore the mechanism underlying berberine improving insulin resistance.Methods Visfatin mRNA and protein expressions were assayed by RT-PCR and Western blot after incubation of berberine at different concentrations and durations with 3T3-L1 adipocytes.Results In 3T3-L1 adipocytes,visfatin mRNA expression was increased at 0- 10?mol/L berberine with the maximum effect at 10?mol/L,being 4.96 folds of control group.While as the berberine concentrations were further increased above 10?mol/L,the visfatin mRNA level decreased.After incubation with berberine for 3 h,the visfatin mRNA level began to increase,and reached the peak when incubation for 12 h(4.57 fold of basal group).Then the mRNA level decreased,though it was still higher than that in control group after 48h intervention.5,10, 20?moL/L berberine could increase visfatin protein expression by 1.13,2.46,2.34 folds respectively as compared with control group(P
8.Therapeutic effect of far lateral approach on the lesions located ventral to cranial-cervical junction.

OBJECTIVETo investigate the administration of far-lateral approach in lesions located anterior or anterolateral to brain stem and upper cervical spinal cord.
METHODSTwenty-three patients underwent far lateral approach, including 12 retrocondylar approach, 5 partial transcondylar approach, 3 transfacetal and partial transcondylar approach, 2 transtubercular approach and 1 complete transcondylar approach.
RESULTSTotal tumor removal was achieved in 15 patients, subtotal removal in 5 patients, 3 vertibral artery aneurysms were clipped successfully, 3 patients were given occipitalcervical fusion. There was no operative mortality. The most frequent complications were lower cranial nerve deficit, CSF leakage, injury to vertibral artery, and ischemia of brain stem, cerebellum or spinal cord. No patient presented clinical instability of the occipitocervical junction after surgery.
CONCLUSIONSThe far-lateral approach is an ideal approach to structures located ventral to cranial-cervicle junction. But some of the surgical steps are technically difficult and carry some degree of risk. The choice of approach depends on the pathological feature and degree of exposure required for effective surgical treatment. Bone removal should be quantified for individual lesion. The approach may be limited to less aggressive steps, while still achieving significant exposure and surgical space.

9.Observation on therapeutic effect of the third lumbar transverse process syndrome treated with acupotomy and blood pricking therapy.

OBJECTIVETo explore the better treatment for the third lumbar transverse process syndrome.
METHODSNinety cases were randomly divided into an acupotomy group, a blood pricking group and a combined therapy group, thirty cases in each group. In acupotomy group, three points such as the third lumbar transverse process point were released with acupotomy, and then Tuina was applied. In blood pricking group, blood pricking therapy was applied at Qihaishu (BL 24) or Weizhong (BL 40). In combined therapy group, both of releasing with acupotomy and blood pricking were applied. The therapeutic effects of three groups were evaluated by of simplified Chinese version of the Oswestry Disability Index (SCODI) and Visual Analogue Scale (VAS) before, immediately after treatment and 1 week after treatment.
RESULTSAfter treatment, compared with scores sofore treatment, VAS scores were all reduced (both P < 0.05), and it in combined therapy group was lower than those in other groups (both P < 0.05); SCODI scores showed no significant differences in groups (both P > 0.05) except in combined therapy group (P<0. 05). One week after treatment, compared with scores immediately after treatment, SCODI scores and VAS scores were reduced in each group (all P < 0.05), and the scores in combined therapy group were lower than those in other two groups (both P < 0.05).
CONCLUSIONReleasing with acupotomy and blood pricking are the effective therapies for the third lumbar transverse process syndrome, can remarkably relieve the pain and improve movement function of lumbus, and the therapeutic effect is more obvious.
