1.The diagnostic imaging and interventional therapy of hepatic angiomyolipoma associated with tuberous sclerosis

Objective To evaluate imaging diagnosis, differential diagnosis, surgery and interventional therapy of hepatic angiomyolipoma associated with tuberous sclerosis.Methods Clinical features, imaging appearances and interventional therapy for 2 cases of HAML in TS were retrospectrively analyzed. Results The features of HAML in TS are as following:(1) Two cases were female with no history of hepatitis and hepatocirrhosis, liver function test and alpha feto protein were normal. (2) the tumor was shown as a multiple hyperechoic mass with sharp margin on ultrasonography; well defined, fat density and intensity mass on computerized tomography (CT); hypervascular mass with early drainage of hepatic vein and tumor staining on angiography. (3) One case underwent interventional therapy. After 42 month follow up, the tumor was found to be diminished in size. Another case was under monitored with only the enucleating of left renal mass. The hepatic tumor showed no change after 10 months.Conclusions The imaging appearances of HAML in TS are relatively characteristic. Superselective arterial embolization is an effective treatment for HAML in TS.

The chemical constituents from lipophilic parts in the roots of Angelica dahurica var. formosana cv. Chuanbaizhi were studied in this paper. The compounds were separated and purified by repeated column chromatographic methods on silica gel and HPLC, and the chemical structures of compounds were determined by spectral data analyses. Twenty-nine compounds were obtained and identified as isoimperatorin (1), β-sitosterol (2), imperatorin (3), bergapten (4), osthenol (5), xanthotoxin (6), isoimpinellin (7), dehydrogeijerin (8), phellopterin (9), isodemethylfuropinarine (10), 7-demethylsuberosin (11), alloimperatorin (12), xanthotoxol (13), isooxypeucedanin (14), alloisoimperatorin (15), demethylfuropinarine (16), 5-hydroxy-8-methoxypsoralen (17), oxypeucedanin methanolate (18), pabulenol (19), byakangelicin (20), marmesin (21), (+) -decursinol (22), heraclenol (23), oxypeucedanin hydrate (24), marmesinin (25), ulopterol (26), erythro-guaiacylglycerol-β-ferulic acid ether (27), threo-guaiacylglycerol-β-ferulic acid ether (28), and uracil (29). Compounds 5, 8, 11, 18, 21-23, and 26-28 were obtained from the roots of title plant for the first time.

4.Development of a new special ambulance for airfield

Objective To develop a special ambulance for airfield rescue of pilots in danger or offering medical care for pilots in flight.Of course the ambulance can also be used to rescue the wounded daily or in the war.Methods The ambulance owned a cross-country motorcar chassis and bearing carriage.A luffing extension-jib was installed on top of the carriage with a telescopic nacelle.The medical carriage owned a generator,air-conditioner,launder & antisepsis facilities,telescopic medical table,Air Force medical facilities for first aid and so on.It could simultaneously treat 2 injured pilots in lying position.Oxygen outputs were equipped to sustain Persons in carriage.Result The maximal speed of the ambulance was 95 km/h.The time of simulated rescue was about 3 minutes in the maxium height.Conclusion Without new staff established,the ambulance can adapt to any road and is suitable to war field.It can arrive at the location of flight accident quickly and rescue the pilot rapidly.It meets the needs of medical support in flight,war-time medical care in airport and mobile accompanying support.
5.Mismatch negativity in chronic tension-type headache with and without medication-overuse

Background: It is unknown if medication-overuse headache, clinically similar to chronic tension-type
headache, has pre-attentive problems which may be related to pain or substance abuse. Methods:
Auditory frequency deviance elicited mismatch negativity was recorded from 22 patients with chronic
tension-type headache, 26 with medication-overuse headache from underlying chronic tension-type
headache and 41 healthy volunteers as controls. Their depression and anxiety scores were noted.
Results: There were no signifi cant differences in the N1 latency or amplitude to both standard and
deviant stimuli for the different groups. However, the latency and amplitude of mismatch negativity were
signifi cantly shortened and reduced at Fz, Cz, and Pz in medication-overuse headache as compared to
chronic tension-type headache and normal subjects. Anxiety levels were elevated in chronic tensiontype
headache and medication-overuse headache subjects compared to healthy controls but were not
correlated with mismatch negativity latency or amplitude in a given group.
Conclusions: In medication-overuse headache subjects, the shortened mismatch negativity latency
indicates quick involuntary attention switching to auditory change, while its reduced amplitude
indicates poor accuracy in discriminating early stimuli, which may be related to medication overuse
rather than to the head pain experienced.
6.Adjuvant function of guilu erxian glue cataplasm in treating carcinoma of the large intestine patients with myelosuppression after chemotherapy: a clinical observation.

OBJECTIVETo observe the clinical effect of Guilu Erxian Glue Cataplasm (GEGC) on carcinoma of the large intestine patients with myelosuppression after chemotherapy, and further to confirm its efficiency and safety.
METHODSTotally 60 patients with carcinoma of the large intestine were randomly assigned to two groups. Meanwhile, they all accepted FOLFIRI chemotherapy. Patients in the treatment group were additionally applied at Shenque (RN8), exchanging once per every other day, for 14 successive days. Patients in the control group took placebos with the same dose and dosage as the treatment group. The blood cell counts (WBC, NE, and PLT) were detected before chemotherapy, at day 7, 10, and 14. The TCM symptoms integrals, Karnofsky performance score (KPS), liver and kidney functions were observed before chemotherapy, at day 7 and day 14. Adverse skin reactions were observed each day. And the usage of hematopoietic growth factors was recorded.
RESULTS(1) The KPS score at day 7 was more stable in the treatment group than in the control group; the WBC and NE counts in the peripheral blood at day 14 were higher in the treatment group than in the control group; and TCM symptoms integrals at day 14 was lower in the treatment group than in the control group, all with statistical difference (P < 0.05). (2) Compared with the control group, the PLT count was higher in the treatment group than in the control group, the usage of rhG-CSF and antibiotics was less in the treatment group than in the control group, all with no statistical difference (P > 0.05). (3) No obvious adverse reactions such as liver injury, renal injury, or skin allergy were observed.
CONCLUSIONSAdjuvant treatment of GEGC could improve carcinoma of the large intestine patients with myelosuppression to some extent. No relevant adverse reactions were found.

7.Correlation between F-waves of electromyography and spasm in patients with spinal injury

BACKGROUND: After the damage of central nervous system, F-waves of electromyography is the valuable way for detecting tendon reflex and muscular tension below injured segment. The increase of spasm, stiffness and muscular tension induced by upper motor neuron injury can revoke change of F-waves.However, change of F-waves after spinal injury, and relationship between Fwaves and spasm of lower limb after injury are still unclear at present.OBJECTIVE: To analyze the correlation of minimal latency, occurrence rate and every index of dispersion-time degree of F-waves with post-injured spinal cord spasm in patients with spinal injury.DESIGN: Case-control observation.SETTING: Department of Rehabilitative Medicine, Tongji Hospital Affiliated to Tongji Medical College, Huazhong University of Science and Technology.PARTICIPANTS: Totally 29 patients with traumatic spinal cord injury,who were hospitalized at Department of Rehabilitative Medicine, Tongji Hospital Affiliated to Tongji Medical College, Huazhong University of Science and Technology between June 2002 and March 2004, were selected as spinal injury group. Another 29 normal healthy volunteers were enrolled as normal control group in the same period. METHODS: Spasm level of both lower extremities in patients with spinal injury was graded with Ashworth scale to assess flexion of both coax, flexion of knee joint and Ashworth grade of dorsiflexion of ankle. F-waves was detected with Kepoint1.5 type EMG instrument produced by Denmark.Minimal latency, maximal latency and occurrence rate of F-waves of tibial nerve of both lower extremities were recorded. The discrete-time degree (difference value of maximal latency of F-waves and minimal latency of Fwaves) and mean occurrence rate of F-waves were calculated.MAIN OUTCOME MEASURES: The difference of each index between patients with spinal injury and normal persons was compared and the correlation of spasm with discrete-time degree, occurrence rate and minimal latency of F-waves in patients with spinal injury was analyzed.RESULTS: A total of 29 patients and 29 normal persons were involved in the result analysis. ①The discrete-time degree of F-waves in patients with spinal injury was higher than that in the normal persons, which had significant difference [(9.2±1.9), (6.7±1.0) ms, P < 0.000 1]. ②The occurrence rate of F-waves in patients with spinal injury was lower than that in normal persons, which had significant difference [(84.5±6.2)%, (89.5 ±5.7)%, P < 0.05]. ③The minimal latency of F-waves in patients with spinal injury was higher than that in normal persons, which had insignificant difference ( P > 0.05). ④The discrete-time degree of F-waves in patients with spinal injury had positive correlation linearly with score of spasm Ashworth (r=0.790 31, P< 0.000 1). The occurrence rate of Fwaves in patients with spinal injury had positive correlation linearly with score of spasm Ashworth (r=0.742 03, P < 0.000 1 ). The minimal latency of F-waves in patients with spinal injury had insignificant correlation with score of spasm Ashworth (r=0.081 68, P > 0.05).CONCLUSION: The discrete-time degree and occurrence rate of F-waves can be used as sensitive index to evaluate electrophysiology in patients with spinal injury so as to assess the level of spasm in patients with spinal injury.
8.Practice and reflection on the international remote pathology consultation

Justified the necessity of remote pathology consultation in China, and described the basic approach of such consultation in terms of the conditions, organizational framework, specialists and consultation process of the hospital. on the basis of benefit in pationts, the consultation helps the development of the pathology department and other specialist departments at the same time, builds initially a pathology quality control system, and accelerates the multi-discipline diagnosis and treatment approach. Expect to encourage contemplation on international remote pathology consultation in an effort to improve such a practice for the benefit of patients.
9.Effects of ziprasidone and risperidone on cognitive inhibition function through visual pathway of patients with paranoid schizophrenia

ObjectiveTo explore the effects of ziprasidone and risperidone on cognitive inhibition function through visual pathway of patients with paranoid schizophrenia.MethodsIn the open-label,flexible-dosage trial,124 patients with paranoid schizophrenia were randomly divided into ziprasidone group (120-160 mg/d)and risperidone group(4-8 mg/d) for treatment of 8 weeks.They were assessed with computerized Color Word Test (CWT) and Continuous Performance Test(CPT) through visual pathway for cognitive inhibition function,Positive and Negative Syndrome Scale for efficacy on baseline and 8th weekend.ResultsAfter treatment with ziprasidone,the error number (3.12 ± 5.23 ),the time per correct answer( ( 1.92 ± 1.38 ) s) of CWT,as well as the doubledigit mistaken number (2.31 ± 3.76)and the three-figure mistaken number( 3.15 ±2.80) of CPT reduced more than those before medication ( (4.60 ± 6.80),( 2.74 ± 1.52 ) s,(3.85 ± 3.62 ),(4.42 ± 3.53 ) ) (P < 0.05 ).In risperidone.group,the double-digit mistake number of CPT(3.39 ± 3.59) after pharmacotherapy reduced more than that before pharmacotherapy(4.23 ± 3.88) (P< 0.05).After treatment the time per correct answer of CWT and the mistaken numbers of CPT in ziprasidone group were less than those in risperidone group(P< 0.05 ),meanwhile,the scores of PANSS in two groups were significantly lower than those before treatment (P < 0.01 ).ConclusionIt is effective for ziprasidone and risperidone to improve the function of cognitive inhibition on patients with paranoid schizophrenia,but there is more dramatic for ziprasidone in short-term treatment.
10.Nutritional index and immune function in esophageal cancer patients with early enternal nutrition

One hundred and seventeen patients with esophageal squamous cell carcinoma were treated with radical esophagectomy from January 2011 to June 2011,among whom 63 cases received early enteral nutrition (study group) and 54 received parenteral nutrition (control group).There were no differences in serum levels of albumin (ALB),prealbumin (PAB) and transferrin (TRF) before surgery between two groups.Serum PAB and TRF levels in study group were significant higher than those in control at d8 after surgery(P < 0.05),and also higher than those at d1 after surgery (P < 0.05).There were no differences in immune indexes in two groups before surgery,the IgA and IgM levels were rising at d8 compared those at d1 after surgery.The total lymphocytes,CD3,CD4,CD8 and CD4/CD8 in study group at d8 were significant higher than those in control group and those at d1 after surgery.The study shows that early enteral nutrition can improve nutritional index and immune function,and to promote rapid rehabilitation in esophageal cancer patients after surgery.
