Objective To evaluate the value of ultrasound (US) in assessment knee joint inflammation in patients with juvenile rheumatoid arthritis(JRA).Methods US scans of the knees obtained in 30 children at clinically active stage; JRA was compared with those obtained in 30 healthy children and 10 JRA patients in clinical remission.Results Changes in synovial membrane thickness and presence of fluid in suprapatellar bursa showed statistically significant differences between JRA patients with active disease and the other subjects.Alterations in contour of the articular cartilage were demonstrated in 10 knees of patients with JRA.Conclusion US is a simple sensitive and reliable methods for the assessment and monitoring of knee joint involvement in JRA.
2.Value of high-dose dobutamine stress echocardiography combined with two-dimensional strain imaging in early diagnosis of coronary artery disease

Objective To investigate the value of high-dose dobutamine stress echocardiography combined with two-dimensional strain imaging in early diagnosis of coronary artery disease. Methods Highdose dobutamine stress echocardiography was performed to 28 patients with suspected coronary artery disease. All wall movements were observed during resting condition and at all stress levels,respectively;the peak systolic longitudinal strain in each endomyocardial segment of left ventricular was measured; the sensitivity and specificity between visual method and two-dimensional strain imaging in diagnosing myocardial ischemia with high-dose dobutamine stress echocardiography were compared. The average peak systolic longitudinal strain was calculated against control group, coronary artery disease group during ischemia segments and non-ischemia segments, and a comparison was made inside each group as well as against the other groups. The area under receiver operating characteristic curve of the peak systolic longitudinal strain was used to predict the sensitivity and the specificity of myocardial ischemia. Results With dobutamine dose of 40 μg·kg-1 · min-1 ,wall motion abnormalities were diagnosed in 6 patients (20 segments) through visual method, myocardial ischemia was found in 15 patients (148 segments) through computing the peak systolic longitudinal strain. Inside the coronary artery disease group during ischemic segments,the majority of peak systolic longitudinal strain was significantly reduced ( P<0.05) compared to the non-ischemic segments and the control group. In diagnosing myocardial ischemia in high-dose dobutamine stress echocardiography, the sensitivity of visual method and two-dimensional strain imaging were 35.3% and 88.2%(P<0.01), specificity 100% and 100%(P>0.05), and accuracy 60.7% and 92.8% (P<0.01). The cutoff value of the peak systolic longitudinal strain was less than or equal to 14.9%, its sensitivity and specificity in predicting myocardial ischemia were 83.3% and 91.7%,respectively. Conclusions High-dose dobutamine stress echocardiography combined with two-dimensional strain imaging can increase the sensitivity of detecting myocardial ischemia and detect concealed myocardial ischemia. High-dose dobutamine stress echocardiography combined with two-dimensional strain imaging can be used in early diagnosis of coronary artery disease.

Coprinus comatus cultivated on cotton seed hull medium decomposed lignocellulose straggly and was high of absolute biological efficiency. Lignocellulose is the main carbon source for the fruiting stage of the fungus. There existed the positive correlation between the degradation rates of the cellulose and hemicellulose in the medium and the activities of extracellular CMCase (carboxymenthelcel lulase), FPase (filter paper cellulase) and HCase (hemicellu-lase), there also existed the positive correlation between the degradation rate of the lignin in the medium and the activity of extracellular laccase, but no correlation between the degradatio rate of the lignin in the medium and the activity of peroxidase. The activity of extracellular amylase was comparatively high at mycelial growth stage, and the protease activity peek was at teh time when the fruitbody matured.
4.The value of myocardial contrast echocardiography combined with dobutamine stress echocardiography in early diagnosis of coronary artery disease

Objective To investigate the value of myocardial contrast echocardiography(MCE)combined with high-dose dobutamine stress echocardiography(DSE)in the early diagnosis of coronary artery disease(CAD).Methods The dobutamine stress MCE and SonoVue contrast infusion were performed before an elective percutaneous coronary intervention in 38 patients with suspected CAD.The total and regional perfusion were scored as normal or abnormal and attributed to the three main epicardial coronary arteries using a 16-segment left ventricular model.Results An intermediate stress level was obtained in 22(58%)patients,and 9(24%)patients were obtained with peak stress.Twenty seven of 38 patients were diagnosed as CAD by quantitative coronary angiography.A perfusion defect was detected in 89% of the patients at peak stress,compared to 37% at baseline,there was significant difference(χ2=15.565,P<0.01).ConclusionsThe MCE combined with DSE can increase the sensitivity of myocardial ischemia detection.As a new non-invasive method,MCE combined with DSE could be used in the early diagnosis of CAD.
7.Controlled clinical trials on the Pizhen and massage for the treatment of thoracic lateral cutaneous nerve entrapment syndrome.

OBJECTIVETo investigate the clinical effects of Pizhen for the treatment of thoracic lateral cutaneous nerve entrapment syndrome.
METHODSFrom March 2004 to April 2008, among the 320 patients with thoracic lateral cutaneous nerve entrapment syndrome, 160 patients (105 patients were male and 55 patients were female, ranging in age from 26 to 66 years, with an average of 46.00 +/- 0.79 years) were treated with Pizhen for 3 to 7 days, and 160 patients (98 patients were male and 62 patients were female, ranging in age from 19 to 64 years,with an average of 47.00 +/- 0.35 years) were treated with massage for 1 to 2 months. The therapeutic effects of the two groups were compared based on changes of tension test of local soft tissues, local pain and tenderness.
RESULTSAll the patients were followed up and the duration ranged from 7 to 12 months, averaged in 10.3 months. Among the patients in Pizhen group, 119 patients got an excellent result, 27 good, 12 fair, and 2 poor, and the tension test of local soft tissues decreased from preoperative (3.68 +/- 0.28) to postoperative (2.13 +/- 0.35). In the control group, 73 patients got an excellent result, 38 good, 40 fair, and 9 poor, the tension test of local soft tissues decreased from preoperative (3.59 +/- 0.22) to postoperative (3.17 +/- 0.19). The therapeutic effects and tension test of local soft tissue in the two groups had statistical differences (P < 0.01). The therapeutic effects of Pizhen group was better than that of the control group, and the postoperative tension test of local soft tissues of patients in Pizhen group was lower than that of the control group.
CONCLUSIONThe clinical effect of Pizhen is better than the massage for the treatment of thoracic lateral cutaneous nerve entrapment syndrome. The Pizhen is effective to decrease the pressure around nerve, relax the adhesion, so as to eliminate the stimulation and compression of sensory nerve endings to relieve the pain quickly. But massage need longer course of treatment. So Pizhen therapy is safe, good and easily to practice, which is better than massage therapy.

8.Comparative study on the Pizhen and acupuncture for the treatment of supraclavicular nerve-entrapment syndrome.

OBJECTIVETo investigate the clinical effects of Pizhen for the treatment of supraclavicular nerve-entrapment syndrome.
METHODSFrom March 2004 to April 2006, among the 200 patients of supraclavicular nerve-entrapment syndrome, 100 patients (75 patients were male and 25 patients were female, ranging in age from 25 to 65 years, with an average of 45 years)were treated with Pizhen, 100 patients (60 patients were male and 40 patients were female, ranging in age from 16 to 63 years, with an average of 35 years) were treated with Acupuncture. The therapeutic effects of the two groups were compared.
RESULTSAll the patients were followed up and the duration ranged from 6 to 12 months, 9.6 months in average. Among the patients in Pizhen group, 79 patients got an excellent result, 12 good, 8 fair, and 1 poor, and the tension exponent of soft tissues decreased from preoperative 3.56 +/- 0.35 to postoperative 2.11 +/- 0.41. In the control group, 43 patients got an excellent result, 21 good, 17 fair, and 19 poor, the tension exponent of soft tissues decreased from preoperative 3.61 +/- 0.21 to postoperative 3.13 +/- 0.56. The therapeutic effects and tension exponent of soft tissue in the two groups had statistical differences (P < 0.05 and P < 0.01).
CONCLUSIONThe clinical effect of Pizhen is better than the the acupuncture for the treatment of supraclavicular nerve-entrapment syndrome. The therapy of Pizhen is safe, good and easily to practice.

The osmotic pressure of ammonium sulfate solutions has been measured by the well-established freezing point osmometry in dilute solutions and we recently reported air humidity osmometry in a much wider range of concentration. Air humidity osmometry cross-validated the theoretical calculations of osmotic pressure based on the Pitzer model at high concentrations by two one-sided test (TOST) of equivalence with multiple testing corrections, where no other experimental method could serve as a reference for comparison. Although more strict equivalence criteria were established between the measurements of freezing point osmometry and the calculations based on the Pitzer model at low concentration, air humidity osmometry is the only currently available osmometry applicable to high concentration, serves as an economic addition to standard osmometry.
