1.Analysis of human brucellosis surveillance in Weinan city of Shaanxi province in 2009

Objective To survey and analyze characteristics of brucellosis epidemic in Weinan city of Shaanxi province for the purpose of setting up prevention and control measures for the disease. Methods According to "The Executing Plan for the Work of Surveying Brucellosis Disease in Shaanxi Province", 35 villages(towns) of designated monitoring locations and 24 villages (towns) of randomized monitoring locations in five countries of Weinan were chosen to survey brucellosis disease. The five countries were Chengcheng, Dali, Heyang, Tongguan and Hancheng. High risk populations with a history of contacting livestock and livestock products aged between 7 and 60 underwent clinical and serology examination[rose bengal plate agglutination test(RBPT) and standard tube agglutination test(SAT)]. All manipulation methods and judging standards were in accord with the "Diagnostic Standard for Brucellosis" (WS 269-2007). Results In the designated monitoring location, a total of 8664 people at high risk were investigated, among whom 1407 people were tested by RBPT test and 27 people were positive,the positive rate was 1.92%(27/1407); 27 people were tested by SAT test and 27 people were positive, the positive rate was 100% (27/27); 25 people were diagnosed and the diagnosis rate was 92.59%(25/27). In the randomized monitoring location, a total of 3464 people at high risk were investigated, among whom 411 people were tested by RBPT test and 3 people were positive, the positive rate was 0.73%(3/411 ), 3 people were tested by SAT test which were all positive and made a definite diagnosis. Twenty-eight new cases were made a definite diagnosis and its incidence was 2.06 in a hundred thousand(28/1 361 618). Conclusions The infection of human brucellosis in Weinan city stays at higher level. The governments should increase input for the monitoring,investigating and disinfecting to prevent the disease from increasing and outspreading.
2.Effects of family intervention on the life quality of children with tourette syndrome

Objective To explore and evaluate the effect of family intervention on quality of life of children with tourette syndrome.Methods Total of 200 children with tourette syndrome were randomly divided into observation group and control group.The observation group received conventional treatment and family intervention including family education,family visits and education guidance to build a harmonious family environment and healthy lifestyle,while the control group only received the conventional treatment and care.The therapeutic effects of both groups were assessed by Yale Global Tic Severity Scale (YGTSS),PedsQLTM 4.0 universal core scales score and Children' s subjective quality of life questionnaires.Results One year later,the frequency,intensity,complexity,interference,damage of spasm in the observation group were significantly lower than those of the control group (t =14.370,20.612,17.677,24.386,11.643,13.972,respectively;P <0.01 ).The average scores of families report in the observation group in physical function,mental function,social function,role function were (90.78 ± 11.32 ),( 88.80 ± 14.43 ),( 76.78 ± 11.30 ),( 66.80 ± 14.53 ),respectively,which were higher than (80.10 ± 14.23 ),(58.10 ± 13.32),(58.50 ± 14.32),(52.80 ± 13.15 )in the control group,and the differences were statistically significant (t =5.873,17.563,10.677,7.139,respectively;P < 0.01 ). Each dimension score of children self-evaluation in the observation group was significantly higher than that in the control group (P < 0.001 ).There were significant differences in family life,companion contact,life environment,self-cognition,depress experience,anxiety,except the dimension of emotional body( P <0.01 ).Conclusions Family intervention can significantly improve the quality of life and the quality of the population for children with tourette syndrome.
3.Effect of early nursing intervention on the health of cesarean section of infants

Objective To investigate nutrition and nursing intervention on the health effects of caesarean section of infants.Methods 1 206 cases of cesarean section on infants were randomly divided into two groups.The observation group included 606 cases and the controlled group included 600 cases.The cases of the observation group came back into the wards after skin contact with mother,and were given the milk from mother,and then were given milk (which has biological active peptides)every 2 to 3 hours.The skin contact and balance training were given before feeding.The cesarean section children were followed up to 2 years old which was based on the regularly intellect development and education outlines.600 cases of the controlled group were fed by traditional methods,based on the routine observation.Results The neurobehavioral (NBNA) of observation group on visual,hearing,touch and ability of adaption assessment after 30 days birth were better than the controlled group,the values in observation group were ( 1.89 ±0.3),( 1.90 ±0.46),( 1.95 ± 0.63 ) ; and those in the control group were ( 1.54 ± 0.15),( 1.53 ± 0.40) and ( 1.56 ± 0.35),difference had statistical significance ( t =8.886,5.472,4.861,respectively ; P < 0.01 ).Two groups were significantly difference on the short impacls,25.92% in observation group and 62.83% in the control group,which had a significant difference (x2 =11.392,P<0.01).Two groups were significantly difference on the long impacts,which had significant difference ( x2 =4.076,6.041,4.076,4.969,respectively; P < 0.05) ; The development quotient of 3,6,12,18,24 months of the observation cesarean children was better than the controlled cesarean children,which had a significant difference (t=5.503,3.833,6.861,7.514,7.693,respectively; P<0.01).Conclusions Nursing intervention was a safe and effective method which can improve the life quality and it was simple,easy and worth promoting.
4.Application of the potential development of premature children by medication treatment joint early nursing intervention

Objective To explore the usage of Shi Jie Yin (Monosialotetrahexosylganglioside) with early comprehensive nursing intervention on the potential development of premature children.Methods 320 cases of premature children were randomly divided into intervention group and control group.The intervention group with 160 cases was given Shi Jie Yin with early rich environment intervention and comprehensive rehabilitation functional training in addition to conventional treatment.Control group only received conventional treatment,parental guidance and assessments.After discharging,children in both groups were followed up till 5 years old according to the 0 ~3 years outline of intellectual development in children and education programs.Results Scores of intervention group were significantly higher than the control group in behavioral and general condition according to NBNA [ (9.32 ± 0.33) and (5.94 ± 0.83 ) for intervention group,(6.35 ± 1.35 ) and (4.39 ± 0.42) for the control group; t =9.573,3.590,respectively; P < 0.0 1 ].In the month with completed neurological and psychological developments,children in control group showed significantly less raising head at the supine position,upright of head,turn over,conscious pronouncing and laughing,grabbing toys with fingers,sitting steady with free hands and crawling than those of the intervention group (t =3.29,4.27,3.62,2.98,4.62,4.52,4.51,respectively ;P <0.01 ).The CDCC scale monitoring of the two groups included development index (MDI) and physical development index (PDI).Intervention group had higher scores in both indexes than the control group[ (95.46 ± 12.35 ) and ( 98.87 ± 12.20) vs ( 79.12 ± 1 4.71 ) and ( 84.03 ± 1 3.37 ) ; t =5.761,6.832,respectively;P < 0.01 ].Comparing two groups of the development of intelligent energy region and the total developmental quotient,the intervention group was significantly higher than the control group ( P <0.01 ).For retardation and the incidence of complications,the intervention group was significantly lower than the control group ( P < 0.05 ).Conclusions Application of Shi Jie Yin with early comprehensive nursing intervention in the potential development of premature children,together with comprehensive system of rehabilitation functional training,adequate treatment and follow-up treatment could reduce the incidence of cerebral palsy and mental retardation and complications,promote the overall development potential of premature children,and improve the quality of survival of premature children and population quality.
5.A novel mutation of 428delG in DAX-1 gene causing X-linked adrenal congenital hypoplasia.

OBJECTIVETo investigate the clinical features and to identify the DAX-1 gene mutation in a Chinese kindred with X-linked adrenal hypoplasia congenital(AHC).
METHODSClinical data and peripheral blood samples were obtained from the affected individuals and their relatives. The genomic DNA was isolated from whole blood. Four pairs of primers were used to amplify the two exons of the DAX-1 gene, and PCR products were purified and sequenced directly. Sequencing results were compared to the human DAX-1 sequence in the public database.
RESULTSA novel hemizygous frameshift mutation (428delG) in exon 1 of the DAX-1 gene was found in both patients (the index case and his cousin). Some clinical features such as the age of onset were different although these 2 patients carried the same mutation. Three females in the family, including the mothers of the 2 patients and their grandmother were carriers of this mutation. No such mutation was detected in other healthy persons in the family.
CONCLUSIONThe result suggested that X-linked AHC in the kindred was caused by a novel mutation of 428delG in the DAX-1 gene, and the same mutation can give rise to variable phenotypes.

6.Application of side to side anastomosis between the lesser curvature of stomach and jejunum in laparoscopic Roux-en-Y gastric bypass.

OBJECTIVETo evaluate the application of side-to-side anastomosis of the lesser curvature of stomach and jejunum in laparoscopic Roux-en-Y gastric bypass (LRYGB).
METHODSClinical data of 29 patients with type 2 diabetes mellitus (T2DM) undergoing side to side anastomosis of the lesser curvature of stomach and jejunum in LRYGB from May 2012 to November 2012 in Department of General Surgery, Beijing Tiantan Hospital, Capital Medical University were analyzed retrospectively.
RESULTSAll the procedures were successfully completed without conversion to laparotomy. The side-to-side anastomosis of the lesser curvature of stomach and jejunum avoided the laparoscopic suture. No gastrojejunostomy anastomotic bleeding, fistula, obstruction and other complications occurred after operation and no complications of gastrojejunostomy anastomosis were found during a follow up of 1 to 7 months.
CONCLUSIONSSide-to-side anastomosis of the lesser curvature of stomach and jejunum in LRYGB can manipulate the size of anastomosis accurately and avoid the laparoscopic suturing. It is simple and easy to learn.

7.Observation on therapeutic effect of the third lumbar transverse process syndrome treated with acupotomy and blood pricking therapy.

OBJECTIVETo explore the better treatment for the third lumbar transverse process syndrome.
METHODSNinety cases were randomly divided into an acupotomy group, a blood pricking group and a combined therapy group, thirty cases in each group. In acupotomy group, three points such as the third lumbar transverse process point were released with acupotomy, and then Tuina was applied. In blood pricking group, blood pricking therapy was applied at Qihaishu (BL 24) or Weizhong (BL 40). In combined therapy group, both of releasing with acupotomy and blood pricking were applied. The therapeutic effects of three groups were evaluated by of simplified Chinese version of the Oswestry Disability Index (SCODI) and Visual Analogue Scale (VAS) before, immediately after treatment and 1 week after treatment.
RESULTSAfter treatment, compared with scores sofore treatment, VAS scores were all reduced (both P < 0.05), and it in combined therapy group was lower than those in other groups (both P < 0.05); SCODI scores showed no significant differences in groups (both P > 0.05) except in combined therapy group (P<0. 05). One week after treatment, compared with scores immediately after treatment, SCODI scores and VAS scores were reduced in each group (all P < 0.05), and the scores in combined therapy group were lower than those in other two groups (both P < 0.05).
CONCLUSIONReleasing with acupotomy and blood pricking are the effective therapies for the third lumbar transverse process syndrome, can remarkably relieve the pain and improve movement function of lumbus, and the therapeutic effect is more obvious.

8.Efficacy Observation of Obtaining yin from yang Method in the Treatment of Xerostomia After Radiotherapy in Patients with Nasaopharyngeal Carcinoma

OBJECTIVE To observe the clinical effect of obtaining yin from yang method in the treatment of xerostomia after radiotherapy in patients with nasaopharyngeal carcinoma.METHODS 102 patients with nasaopharyngeal carcinoma after radiotherapy received in outpatient and inpatient department of our hospital from June 2016 to June 2017 were randomly divided into two groups.On the basis of the formula of the control group,guizhi (Ramulus Cinnamomi) and paojiang (Rhizoma Zingiberis Praeparatum) were added in for the treatment group to obtain yin from yang.Three courses later,degree of dry mouth score and irritant saliva flow in both groups were compared before and after the treatment.RESULTS Degree of dry mouth score and irritant saliva flow in both groups were improved compared with those before the treatment (P<0.05,P<0.01),and the treatment group was better than the control group (P <0.01).The comprehensive effective rate of the treatment group was 88.23%,which was better than that of the control group (66.66%,P <0.05).CONCLUSIONS Obtaining yin from yang method has significant effect for xerostomia after radiotherapy in patients with nasaopharyngeal carcinoma.It can obviously improve the degree of dry mouth,and has a certain protecting effect for salivary glands.
9.An epidemiological survey of alcoholic liver disease in Zhejiang province.

OBJECTIVETo describe the brief survey of alcohol intake and the incidence of alcohol liver disease in Zhejiang province.
METHODS18,237 requested persons aged over 18 years were selected by multi-stage stratified cluster sampling in Zhejiang province. Questionnaire about alcohol consumption, hepatic ultrasonic scan and detection of hepatic enzymes and markers of HBV and HCV were carried out. Daily alcohol intake more than 40g (including equal to 40g/d) was essential for the diagnosis of alcoholic liver disease.
RESULTSAmong the 18,237 persons (male 12,042, female 6195), the average daily alcohol intake was (17.7 +/- 27.2) g. The incidence of alcoholic liver disease in Zhejiang province was 4.34% (male 6.36%, female 0.36%) in the whole population. Four subtypes were separated as alcoholic cirrhosis, alcoholic fat liver, alcoholic hepatitis and mild alcoholic injury in liver with the corresponding incidence of 0.68%, 0.94%, 1.51% and 1.21% separately.
CONCLUSIONAlcoholic liver disease is found to be a common disease in Zhejiang province, indicating an urgent need for the public education on alcohol abuse and the treatment on related health problems

10.Primary lymphoma of breast: a clinicopathologic, immunophenotypic and prognostic study of 21 cases.

OBJECTIVETo investigate the clinicopathologic and immunophenotypic features of primary breast lymphoma (PBL) and to discuss the diagnosis of the tumor.
METHODSTwenty-one cases of PBL with follow up data were clinically reviewed. The histologic diagnosis of the tumor was based on the updated WHO Classification of tumors of hematopoietic and lymphoid tissues (2008). Immunohistochemistry was performed by SP method and antibodies selected were as follows: CD20, CD3epsilon, CD10, Bcl-6, MUM-1, CD5, Bcl-2, CD23, CD10, cyclin D1, CD43 and Ki67.
RESULTS(1) All 21 patients were female and the median age of patients was 48 years. The right and the left breasts were involved in 11 (52.4%) and 10 patients (47.6%), respectively. According to Ann Arbor staging system, 20 cases were stage I-II (95.2%), and the remaining case was stage IV (4.8%). For the international prognostic index (IPI), 19 cases were score 0-1, and 2 cases were score 2-3. For ECOG score, 19 cases were 0, and the remaining 2 cases were 1. (2) Histologically, all 21 cases (100%) were DLBCL. Immunohistochemically, the frequency of antigen expression was as follows: CD20 (100%), MUM-1 (14/21, 66.7%), bcl-6 (5/21, 23.8%), CD10 (0), bcl-2 (13/21, 61.9%), CD5 (1/21, 4.8%); Ki-67 index: 10 cases (47.6%) were less than 59%, with the expression of seven cases (33.3%) being 60% - 89%, and more than 90% in the remaining four cases (19.1%). The median Ki-67 index was 60%. All the cases were considered non germinal center B-cell-like type of DLBCL. (3) Follow-up data was available in 64% of the cases. One, two and five-year survival rates were 11 cases, 7 cases and 3 cases, respectively.
CONCLUSIONAll the cases of PBL in the current study were DLBCL, non germinal center B-cell-like type, and a diagnosis of PBL can only be established after excluding breast involvement in systemic lymphoma.
