1.Efficacy of sublingual immunotherapy with Dermatophagoides farinae drops on patients with allergic rhinitis of different symptom severity.

OBJECTIVE:
To evaluate the efficacy of sublingual immunotherapy (SLIT) with Dermatophagoides farinae drops for allergic rhinitis (AR) of different symptom severity.
METHOD:
This retrospective analysis to receive SLIT treatment of 143 cases of patients with allergic rhinitis, according to the severity of disease symptoms divid- ed into two groups, moderate group (62 patients) and severe group (81 patients). Before SLIT and after SLIT for half year, 1 year and 1. 5-2.0 years, the TNSS, TMS and sign scores of patients with allergic rhinitis were evaluated.
RESULT:
The TNSS, TMS and sign scores had continuously improved significantly after SLIT for half year, 1 year and 1.5-2.0 years in two groups as compared with baseline (P < 0.05). Before SLIT, TNSS and sign scores of severe group had a significantly higher level than moderate group (Z = 10.40, 2.40, P < 0.05), while TMS of two groups had no significant differences (Z = 0.00, P > 0.05). Half year after SLIT treatment, in two groups for sign scores, there were significant differences (Z = 3.32, P < 0.05), and there were no significant differences for TNSS (Z = 1.58, P > 0.05) and TMS (Z = 0.37, P > 0.05). 1 and 1.5-2.0 years after SLIT, there were no significant differences in two groups for TNSS, TMS and sign scores (P > 0.05).
CONCLUSION
SLIT with Dermatophagoides farinae drops for 1.5-2.0 years is effective in the patients with allergic rhinitis of different symptom severity. And equivalent efficacy could be achieved for different symptom severity.

2.The effect of LPC on the pacemaker current I(f) in ischemic myocardium and the influence of ISO on it.

AIMTo observe the effect of LPC on the pacemaker current I(f) in ischemic myocardium and if the effect could be reversed by ISO.
METHODSBy using two microelectrode voltage clamp technique to measure and compare the amplitude of I(f) of ischemic myocardium in the presence of LPC and LPC add ISO.
RESULTSIschemia decreased the amplitude of I(f) at all membrane potential levels. Adding LPC 2 x 10(-5) mol/L to the ischemia-like solution, the amplitude of I(f) decreased further (n = 5, P < 0.05), it means that LPC aggravated the inhibitory effect of "ischemia" on the pacemaker activity. Adding LPC 2 x 10(-5) mol/L and ISO 1 x 10(-6) mol/L together to the ischemia-like solution, the amplitude of I(f) increased significantly at membrane potential -90 mV to - 120 mV (n = 8, P < 0.05) compared with ischemia condition, but still did not reach the levels before ischemia.
CONCLUSIONIn acute myocardial ischemia condition, toxic metabolite LPC accentuated its inhibitory effect on pacemaker current I(f), a local release and accumulation of catecholamine could not completely reverse their inhibitory effect.

The effects of pioglitazone (PIO) and tumor necrosis faetor-?(TNF-?) on two kinds of adiponectin receptor (AdipoR) mRNA expression were observed in 3T3-L1 adipocytes by RT-PCR.AdipoR mRNA expression was up-regulated during 3T3-L1 preadipocyte differentiation;PIO could increase the AdipoR mRNA level expressed in undifferentiated and differentiated 3T3-L1 adipocytes in a time-and dose-dependent manner,and TNF-?had no influence on the expression of AdipoR mRNA.
4.Prognostic Value of 99mTC-MIBI SPECT Exercise Myocardial Imaging in Patients with Suspected or Diagnosed Coronary Artery Disease

ObjectiveTo examine the prognostic value of 99mTC-MIBI single-photon emission computed tomography (SPECT) in patients with suspected or diagnosed coronary artery disease (CAD).Methods116 patients with suspected or diagnosed CAD were examined with 99m TC-MIBI SPECT exercise and rest myocardial imagings, and followed by investigation of cardiac events.Results Follow-up information was complete in 106 patients. Over an average follow-up of 25±8 months, cardiac events occurred in 11 patients. Univariate analyses showed that age, old myocardial infarction, exercise peak heart rate, the change in heart rate from rest to exercise peak, exercise peak systolic blood pressure, the change in systolic blood pressure from rest to exercise peak, abnormal myocardial imaging, the number of myocardial perfusion defect and ischemic volume rate (IVR) were the effective predictors of subsequent cardiac events. Multivariate stepwise Logistic regression analyses revealed that the size of IVR was the only variable with independent predictive value for occurrence of subsequent cardiac events. IVR more than 25% was associated with a significantly increased frequency of subsequent cardiac events (relative risk ratio was 5.9, P<0.005). IVR less than 25% or even normal myocardial perfusion imaging was associated with a very low-risk of cardiac event and a very good prognosis.Conclusion 99mTC-MIBI SPECT myocardial imaging is the most effective noninvasive technique that predicted the occurrence of cardiac event in patients with suspected or diagnosed CAD. According to the size of IVR, high-risk and low-risk patients can be effectively identified.
5.Pharmacokinetic and clinical study of FK506

Objective To explore the relationship of the whole blood concentrations of FK506 with the occurrence of acute rejection after renal transplantation and further, with the toxicity of this medication. Methods Ten patients with renal transplantation were given FK506-based immunosuppressive regimen 24 h after the operation. Heparinized blood samples were obtained after the initial oral administration of the drug for the pharmacokinetic evaluation. The FK506 dosage, the episodes of acute rejection and the toxicity of the medication were recorded in detail. Results t(peak) was (1.4797±1.2174) h,t1/2 beta was (10.8065±12.0799) h, and the mean residence time (MRT) was (8.0535±4.4750) h. Within the first postoperative month, 2 patients experienced acute rejection and 2 others developed diabetes which necessirare insulin therapy. The blood concentrations of the drug were not significantly different between patients with acute rejection and those without, but were significantly different between diabetic and non-diabetic patients. Conclusions FK506 is an effective prophylactic immunosuppressant for renal transplantation. The whole blood concentrations, in a certain range, are not corresponding to the episode of acute rejection. There was a significant tendency for side effects with increase of the dose concentrations, and small dose low concentrations of Fk506 can achieve good effect and reduce its toxicity.

AIM: To investigate the effect of huayu xiaoliu fang, a Chinese medicine, on the cell cycle of human lung carcinoma cell line by serologic pharmacological method. METHODS: PGLH7 cells were incubated with rabbit serum containing huayu xiaoliu fang at different doses obtained by serologic pharmacological method. MTT assay was used to calculate the proliferation inhibition rate. The target cells were harvested to analyze the cell cycles by flow cytometry. 8.Gastrointestinal pacemaking for gastric dynamic disorders.

OBJECTIVETo investigate the efficacy and safety of gastrointestinal pacemaker in treatment of gastric dynamic disorders.
METHODSSixty patients with more than two symptoms as postprandial fullness, epigastric pain,epigastric fullness, belch, nausea, hypercoria,anorexia,vomiting and at least for twelve weeks were divided into three groups randomly. Thirty patients were treated with gastrointestinal pacemaker for ten days(pace maker group); fifteen patients were treated with gastrointestinal placebo machine for ten days (control group); fifteen patients received treatment with cisapride 5 mg three times daily, thirty minutes before meal (cisapride group) for ten days. Electrogastrogram(EGG) was performed on before and after treatments, the mean frequency and mean amplitude of EGG, the clinical symptoms and adverse reactions were observed.
RESULTSTotal efficacy of pace maker group, control group and cisapride group was 90.0%, 46.6% and 86.7%, respectively. The score of symptoms was significantly decreased after treatment with gastrointestinal pacemaker and cisapride (P<0.05). Before and after meal EGG exam showed that average frequency was reversed to normal level after treatment with gastrointestinal pacemaker.
CONCLUSIONGastrointestinal pacemaking can relieve symptoms and improve the average frequency of EGG in patients with gastric dynamic disorders.

9.Status assessment on rural environmental hygiene in Pukou District of Nanjing in 2017

Objective To ascertain the level and dynamic changes of environmental health hazards in the rural areas of Pukou, Nanjing, to objectively evaluate the situation of rural environmental sanitation, providing basis and support for formulating policies and measures by the higher authorities. Methods According to the requirements of the rural environmental sanitation monitoring program of Jiangsu Province in 2017, 5 streets, 20 administrative villages were selected as monitoring points and 100 families used as monitoring objects.The monitoring data were obtained through interviews, access data, field records and laboratory tests. Results Centralized water supply was realized by 20 monitoring points.The rate of collection and centralized disposal of domestic waste was 95.00%.The sewage disposal sites were mainly discharged into sewage treatment plants and harmless sanitary toilets, accounting for 41.00% and 42.00% respectively.The coverage rate of sanitary toilets was 95.54%.Those who did the work of killing rats, flies and mosquitoes accounted for 40.00% and those who did the work of killing cockroaches 25.00%.And 98.00% of the families used fuel for gas, 98.00% of the families never burned straw in the fields, 88.00% of the families did not use the film, and the membrane recycling rate was 91.67%.It was found that 100% of the households drank boiled water, and 90.00% of the respondents developed the habit of washing hands before and after meals.The detection rate of Ascaris eggs was 20.00% in 20 soil samples, and the contents of Pb, CD and Cr were all within the normal range. Conclusion Although some achievements have been made in the rural environmental sanitation in Pukou, the management of pollution of rural sewage, waste and feces should be further strengthened, and the dangerous factors that affect the health of rural residents should be decreased and four pests removal activities actively carried out.
