1.Arthroscopic synovectomy for rheumatoid arthritis in early and middle stages

Objective To compare the efficacy of arthroscopic synovectomy between early and middle stage rheumatoid arthritis. Methods A total of 34 cases of rheumatoid arthritis (42 knee joints) either in early stage (22 cases, 24 joints) or in meddle stage (12 cases, 18 joints) underwent arthroscopic synovectomy. The bipolar radiofrequency was used for the synovial debridement and hemostasis in 32 knee joints. Postoperatively, routine anti-rheumatoid drugs were administrated. Follow-up observations in the two groups included the evaluation of knee function, the erythrocyte sedimentation rate (ESR), the C-reaction protein (CRP), and the rheumatoid factor (RF). Results The rate of excellent or good results was 91.7% in early stage (22/24) and 66.7% in middle stage (12/18), without statistical significance between the two groups ( ? 2=2.705, P =0.100). Conclusions Arthroscopic synovectomy is effective for the treatment of rheumatoid arthritis in both early and middle stage. Bipolar radiofrequency is helpful for the complete elimination of the synovium, the prevention of joint hematoma, and the rehabilitation of joint function.
2.The effect of bipolar radiofrequency technique in the treatment of patellofemoral malalignment

Objective To study the early effect of arthroscopic bipolar radiofrequency technique in the treatment of patellofemoral malalignment. Methods 42 patients(47 knees) who were confirmed to be patellofemoral malalignment by radiographic methods, underwent arthroscopic bipolar radiofrequency chondroplasty and lateral retinacular release. The cartilage abnormality were classified according to the Outerbridge grade. 34 cases(37 knees) were followed up and mean period was 18.3 months. Patients were assessed before and after surgery using the Kujala patellofemoral score. Patellofemoral alignment were evaluated through MRI in 23 of 37 knees. The change of Cac (congruence angle of cartilage), Tac(tilt angle of cartilage), LPDc (lateral patellofemoral distance of cartilage) and LPFac(lateral patellofemoral angle of cartilage) were measured. Results The patellofemoral joint score was improved significantly in OuterbridgeⅠ,Ⅱand Ⅲ grade patients(6 knees ofⅠgrade: 60.83?3.54 preoperatively and 82.50?9.22 postoperatively, P0.05). 7.The protection of clopidogrel in a rat model of decompression sickness

Objective Platelet aggregation, activation induced by bubbles is the main cause of decompression sick-ness.Clopidogrel(Clo) can decrease platelet aggregation through inhibiting the bind of fibrinogen and ADP .This study is designed to find if Clopidogrel can paly a protective role in decompression sickness and explore the intervention mechanism . Methods Totally 111 male SD rats divided into 3 groups:normal control group (n=20), decompression sickness(DCS) group(n=46), and DCS+Clo(Clopidogrel)treated decompression sickness (DCS+Clo)group(n=45).The rats in DCS and DCS+Clo group were placed in chamber and compressed to 1.5 MPa at speed of 2t/4 , the time of compression and res-idence was 4.5 min totally, then decompressed to surface at a speed of 3 m/s.The mortality and behavioral of rats were ob-served within 30 min post decompression .The pathology and the wet/dry ratio of lung , WBC and platelet counts in periph-eral blood, the expression of activated platelets , and immunohistochemical detection of lung tissue CD 41 expression were also been tested .Results We found Clo reduces the DCS mortality risk ( mortality rate:11/45 in DCS+Clo group vs 28/46 in DCS group, P<0.01).Clo reduced the lung injury, the wet/dry ratio of lung, the accumulation of platelet and leu-kocyte in lung , the WBC counts and activated platelets in peripheral blood .Conclusion Clo can play a protective role in decompression sickness through reducing post-decompression platelet consumption and activation , decreasing the activation of leukocytes .
8.Expression levels of interferon-?-inducible protein 30 genes in CD4~+T cells of systemic lupus erythematosus patients

Objective To investigate the expression levels of interferon-?-inducible protein 30 genes in CD4~(+) T cells of systemic lupus erythematosus(SLE) patients and to evaluate the relations between the gene expression levels and the disease activities.Methods The clinical data of 23 SLE patients and 10 normal controls were collected.Peripheral blood was drawn and CD4~(+)T cells were separated by using immunomagnetic cell sorting system(MACS).Total RNA was extracted and reversely transcribed into cDNA.Screening and identification of IP-30 in LongSAGE Tags from a SLE patient was done by generation of longer cDNA fragments from serial analysis of gene expression(SAGE) tags for gene identification.Results IP30 expression levels of the total SLE patients were significantly higher than those of normal controls(P
9.Characteristics and Clinical Features of Positive Response for Head - Up Tilt Test in Children

Objective To study the characteristics and clinical features of positive response during head - up tilt test (HUT) in children. Methods Forty- nine cases of syncope and aura of syncope were included in the study. All of them underwent HUT (tilt angle 60 degree,lasting 45 minutes). During the test,children' blood pressure,heart rate.electrocardiography and clinical symptoms and signs were dynamically observed. Results The common syncope aura symptoms were headache, dizziness, chest distress, difficulties in breathing, pale, perspiration, blurred vision, auditus depression and symptoms related to digestive symptoms. Among the 28 positive cases, 7 cases were of vaso - inhibitory pattern (25%), with a decrease in blood pressure and an increase in heart rate, 3 cases inhibitory pattern (11%), with a decrease in heart rate but withoilt'blood pressure changes, 4 cases mixed pattern (14 %), with decreases in heart rate and blood pressure obviously, 10 eases POTS (36%), with an increase in heart rate by 30 bpm or up to 120 bpm within 10 minutes while testing, and 4 cases orthostatic hypotension (14%), with a decrease in blood pressure within 3 minutes. Conclusion HUT can be regarded as the main tool in differential diagnosis of unexplained syncope in children.
