OBJECTIVETo observe clinical efficacy in treating cutaneous deficiency of traumatic shank amputation stump with full-thickness skin graft combined with vacuum sealing drainage.
METHODSFrom September 2009 to December 2012, 15 patients with cutaneous deficiency of traumatic shank amputation stump were treated with full-thickness skin graft combined with vacuum sealing drainage. Among patients, there were 11 males and 4 females with an average age of 41.5 (ranged from 25 to 62) years old. Ten cases were caused by traffic accident and 5 cases were caused by heavy object, 9 cases on left and 6 cases on right. Six patients with smashed wound were treated with debridement and amputation, combined with vacuum aspiration in-emergency; 9 patients caused by infection and necrosis were treated with debridement and amputation, combined with vacuum aspiration, and full-thickness skin graft were performed at stage II. The skin defect area of residual limbs ranged from 40 cm x 20 cm to 25 cm x 15 cm.
RESULTSAll patients were followed up from 3 months to 1 year. Full-thickness skin graft of residual limbs were survived, and obtained satisfactory walking function with prosthetic. Residual skin increased thicken, wearproof without rupture and pain.
CONCLUSIONFull-thickness skin graft combined with vacuum sealing drainage in treating cutaneous deficiency of traumatic shank amputation stump could reserve the length of residual limbs, increase survival rate of skin graft with less scar of survival skin, get good wearability and it is conducive to prosthetic wear. It is a simple and easy treatment method.

Objective To analyse the neuropsychological changing of nonfluent aphasia frontotemporal dementia (nf-FTD) with the FDG-PET, MRI and fMRI. Methods The language and other cognitive function were dynamically estimated by standard neuropsychological assessment which modified by Chinese. Compared with FDG-PET and fMRI, the relationship between the neuropsychologieal changing and the cortex function was studied. Results All the 5 patients met the criteria of nf-FTD by Association for Frontotemporal Dementias, with core syndrome and other supptted syndrome. And still they had the good word reading, calculation by pen, and the un-speechmemory. And they also had the related well living ability. They all had the related cortext metabolism support. Conclusions nf-FTD patients preserve the word reading, visuapacial ability and calculation by pen. These can help us to give the effective advise about the patients training and caring and to give them the chance to show their real social skill.

A 57-year-old man was admitted to hospital with diarrhea for 10 months and dizziness for 4 months. The patient had 1-2 liters watery stool per day, without pyogenic blood or abnormality in gastroenteroscopy examination. The level of hemoglobin and albumin was generally normal, and fasting test was positive. At the same time, he was accompanied with hyperalgesia of lower limbs and orthostatic hypotension. After the discussion of multiple disciplinary teams, the patient was diagnosed with amyloidosis by sural nerve biopsy, myocardial MRI, and the assays of urine immunoelectrophoresis and serum free light chain. Light chain amyloidosis was confirmed after excluded the diagnosis of familial amyloidosis. The patient was improved after courses of chemotherapy with melphalan and dexamethasone.

Objective To study the effect of positive lymph node number on the survival of patients with esophageal carcinoma. Methods From July 1995 to July 2005, a total of 11,447 resected lymph nodes were obtained from 1140 patients who underwent curative resection of the primary tumor with systematic lymphadenectomy at Shanxi cancer hospital. The survivals were analysed by life tables and Kaplan-Meier methods, the related factors of lymph node metastasis were assessed by Chi-square test. Results The number of positive lymph nodes was negatively related to survival rates of esophageal carcinoma. According to the number of lymph nodes resected (≥8 nodes versus <8 nodes), there was significant difference in metastatic lymph node ratio. Conclusion The number of positive lymph node can reflect the prognosis of patients better. The authors suggest that the modification of the tumor-lymphnode-metastasis (TNM) staging classification (TNM) to include the number of positive lymph nodes in the N1 category.

Aeromonas hydrophila CGMCC 0911 possessing type I polyhydroxyalkanoate (PHA) synthase gene (phaC) only accumulate copolyesters consisting of 3-hydroxybutyrate (3HB) and 3-hydroxyhexanoate (3HHx), abbreviated as PHBHHx, from lauric acid as sole carbon source but not from glucose. The gene encoding type I PHA synthase was interrupted by insertion of a chloramphenicol resistance gene (Cm). Conjugation of suicide plasmid pFH10 transformed A. hydrophila CGMCC 0911 into a recombinant organism with the disrupted type I PHA synthase gene (phaC:: Cm) , through an in vivo homologous recombination process, type I phaC of A. hydrophila genome was replaced by the disrupted phaC, and Cm gene was integrated into the genome of A. hydrophila, resulting in type I phaC-negative mutant, which was proved by DNA sequencing. Results of GC analysis showed that this mutant could not accumulate PHBHHx again but accumulate medium-chain-length (mcl) PHA from lauric acid or glucose as carbon source, clearly indicating the existence of another type I PHA synthase in the wild type A. hydrophila. It will play its function and accumulate mcl PHA only when type I PHA synthase was inactivated. into the genome of A. hydrophila, resulting in type I phaC-negative mutant, which was proved by DNA sequencing. Chinese Journal of Oncology 2004;26(3):173-176
OBJECTIVETo evaluate the significance of extended radical resection in the treatment of pancreatic head cancer and its indication.
METHODSBetween Jan. 1995 and Dec. 1998, 56 patients with pancreatic head cancer were retrospectively reviewed, among whom 35 were treated by the Whipple operation and 21 received the extended radical resection during the same interval.
RESULTSThere was no significant difference between the mortality and morbidity rate of complication, though with more patients having higher clinical stages in the extended radical resection group. The 1-, 2- and 3-year survival rates were 84.8%, 62.8%, 39.9% in the extended radical resection group and 70.8%, 47.6%, 17.2% in the Whipple operation group with significant difference between the two groups. The total mortality rate was 51.4% in Whipple group and 42.9% in extended radical resection group with significant difference between the two. The 3-year cumulative rate of death from local recurrence decreased from 37.4% in the Whipple group to 23.8% in the extended radical operation group. Patients who survived for more than 3 years were only those in clinical stage (SC)1 in the Whipple group whereas they were found both in patients who had had CS1, CS2 lesions and also in some who had CS3 lesions in the extended radical resection group.
CONCLUSIONThe extended radical operation does benefit patients with pancreatic head carcinoma in CS1, CS2 and in a part of CS3 without too extensive exrtra-pancreatic invasion.
