Objective To evaluate the efficacy and safety of intravenous esmolol, amiodarone and diltiazem for controlling rapid ventricular rate in patients with atrial fibrillation (AF) during anesthesia period. Methods Ninety AF patients with rapid atrial ventricular rate ( ≥ 120 beats/min) in anesthesia period were randomly divided into 3 groups ( n = 30 each: group Ⅰ patients were treated with intravenous group Ⅱ patients were treated with intravenous amiodarone (loading dose: 3 mg/kg for 10 minutes, followed with intravenous infusion of 1 mg/min); group Ⅲ patients were treated with intravenous diltiazem ( 0. 25 mg/kg for 5 minutes). The heart rate, blood pressure, rhythm were recorded before treatment, at 5, 10,15, 30, 60 and 90 min after treatment. The reacting time, side effects including hypotension, bradycardia, nausea, vomiting, dizziness, etc, were analyzed. Results The mean reacting time was significantly shorter in group Ⅰ (4.3 ±2. 1)min than in group Ⅱ (19.2 ±8.5) min and in group Ⅲ (8.5 ±3.4) min (P<0. 05 ). The mean reacting time in group Ⅲ was significantly shorter than in group Ⅱ ( P < 0. 05 ). The total effective rate were similar among the groups ( 86. 7%, 90.0% and 83.3% with a mean decrease in heart ventricular rate by 42.4%, 42% and 41.9% of the baseline level in group Ⅰ , group Ⅱ and group Ⅲ,respectively). The incidence of total side effect was significantly lower in group Ⅱ ( 10% ) than in group Ⅰ ( 16. 7% ) and group Ⅲ (20%, P < 0. 05). Conclusions Intravenous esmolol, amiodarone and diltiazem are all equally effective and safe on controlling rapid ventricular rate in patients with atrial fibrillation during the anesthesia period. Esmolol use is associated with the shortest mean reacting time and amiodaron use is associated with the lowest total side effect rate in this patient cohort.

Objective To propose a deep learning-based retinal image quality classification network, FA-Net,to make convolutional neural network ( CNN) more suitable for image quality assessment in eye disease screening system. Methods The main network of FA-Net was composed of VGG-19. On this basis, attention mechanism was added to the CNN. By using transfer learning method in training, the weight of ImageNet was used to initialize the network. The attention net is based on foreground extraction by extracting the blood vessel and suspected regions of lesion and assigning higher weights to region of interest to enhance the learning of these important areas. Results Total of 2894 fundus images were used for training FA-Net. FA-Net achieved 97. 65% classification accuracy on a test set containing 2170 fundus images, with the sensitivity and specificity of 0. 978 and 0. 960,respectively,and the area under curve(AUC) was 0. 995. Conclusions Compared with other CNNs,the proposed FA-Net has better classification performance and can evaluate retinal fundus image quality more accurately and efficiently. The network takes into account the human visual system ( HVS) and human attention mechanism. By adding attention module into the VGG-19 network structure, the classification results can be better interpreted as well as better classification performance.

Propolis is a bee wax rich in various phytocomponents and traditionally used to treat various ailments. Propolis is reported to possess an array of biological properties including anti-inflammatory, antioxidant, anti-cancer, and anti-diabetic as well as cardioprotective, hepatoprotective, renoprotective, and derma protective activities. A plethora of studies confirmed that propolis is effective against various types of cancer including head and neck, lung, liver, brain (glioma), pancreas, kidney, prostate, skin (melanoma), breast, oral, esophagus, gastric, colorectal, and bladder cancers. However, many researchers have demonstrated that propolis displays potent chemoprotective/chemopreventive or anti-cancer activity against only a few types of cancers like oral, gastrointestinal, dermal (melanoma), breast, and prostate cancers. Therefore, this mini-review only summarizes the chemopreventive/chemotherapeutic activities of propolis and its updated underlying mechanisms. Taken together, propolis displays potent chemoprotective or anti-cancer effect due to the presence of various phytocomponents which contribute to pro-apoptotic, cytotoxic, anti-proliferative (cell cycle arrest), anti-metastatic, anti-invasive, anti-angiogenic and anti-genotoxic or anti-mutagenic properties along with antioxidant, immunomodulatory, and anti-inflammatory functions. Hence, propolis could be used as an adjuvant for treating various cancers along with standard chemotherapeutic drugs. However, many large-scale clinical studies are needed to justify such applications.

OBJECTIVETo investigate the early diagnosis and treatment for trauma around the knee with popliteal vascular injury.
METHODSA retrospective analysis was employed to analyze the clinical data from 15 patients (9 males and 6 females were with a mean age of 39.2 years old, ranging from 26 to 62 years old) with fracture or dislocation around the knee with popliteal vascular injury from January 2007 to January 2013. Combined with clinical symptoms and signs, oxygen saturation monitors, color ultrasound, DSA angiography and interventional surgery were used to determine the vascular injury. The knee fracture and dislocation were fixed with hybrid external fixation and plate-screw fixation, respectively. Then, the blood circulation was reconstructed by thrombectomy, repair and autologous vein graft for individual injured vascular. The average total operation time, average hospitalization days, predictive salvage index (PSI), average blood transfusion amount, average medical expenses and infection cases were recorded to determine the effect of early diagnosis and treatment.
RESULTSThere was one patient with death, 8 patients with amputation, and 6 patients with successful repair surgery for popliteal artery, anterior tibial and posterior tibial arteries. These six patients with surviving limbs were followed up for an average of 28.3 months (ranged, 12 to 60 months). Among the 6 successful patients, the joint function of 4 patients was good and excellent.
CONCLUSIONThe trauma around the knee with popliteal vascular injury is characterized by complex and serious injury, easy misdiagnosis and loss diagnosis, poor prognosis and high risk of amputation. The early diagnosis of trauma around the knee with popliteal vascular injury should depend on the mechanism of trauma, local anatomical characteristics of injury site, clinical presentations and appropriate auxiliary examinations. The appropriate indications for limb salvage and amputation should be used to achieve more effective clinical results.

Objective To explore the effects of benzalkonium bromide and citalopram on the corneal epithelium and corneal thickness of mice using optical coherence tomography angiography (OCTA).Methods Together 60 mice were randomly divided into 5 groups (group A,B,C,D and E;n =12),with group A left untreated, group B receiving PBS eye drops, group C given benzalkonium bromide eye drops, group D undergoing intraperitoneal administration of citalopram suspension, and group E treated with combination of benzalkonium bromide eye drops and citalopram suspension. After 2 weeks,OCTA was applied for corneal subarea, followed by measurement of the thickness of corneal epithelium and full-thickness of the cornea of all mice, and then the mean values were calculated. Results The thickness of corneal epithelium and fullthickness of the cornea was (66 ±7) μm and (141 ± 11) μm in the group A,(66 ± 8) μm and (140 ± 12) μm in the group B and D,(73 ± 10) μm and (141 ± 14) μm in the group C,(76 ± 12) μm and (141 ± 15) μm in the E group, respectively. And there was no significant difference in the thickness of corneal epithelium and full-thickness of the cornea before treatment and 2 weeks after treatment in the group A,B and D (all P > 0.05),but both variables were markedly thickened in group C and E 2 weeks after treatment, and the difference was statistically significant (all P <0.05).Moreover,the increased levels on the both variables in the group E was higher than those in the group C 2 weeks after treatment, and the difference was statistically significant (both P < 0.05).The average thickness of corneal epithelium and full-thickness of the cornea in the group C and E were significantly thickened after treatment, and the difference was statistically significant (all P < 0.05).The average values of both variables in the group C and E were obviously larger than those in the group A,and the difference was statistically significant (all P < 0.05).Conclusion Citalopram alone has no significant effects on the corneal thickness by OCTA,whereas both the thickness of corneal epithelium and fullthickness of the cornea tend to thicken by benzalkonium bromide treatment, which has a synergistic effect on corneal thickening with citalopram.
