Objective To explore the principle and methods of normative minimally invasive surgery treatment on hypertensive intracerebral hemorrhage. Methods 49 cases of hypertensive intracerebral hemorrhage were treated by microsurgery with small incision. Results 46 cases survived and 3 cases died after surgery. 42 cases were followed up. Adopting GOS, 19 cases were normal, 12 cases were disable and could care for self, 5 cases were severely disable, sober and could not care for self, 3 cases were vegetable survival, and 3 cases were dead. Conclusion Application of minimally invasive surgery idea and normative treatment according to the patients' condition can obviously improve the efficacy of patients with hypertensive intracerebral hemorrhage.

Objective To investigate effects of hypertensive agents on regional cerebral blood flow (rCBF)and prognosis in patients with hypertensive intracerebral hemorrhage (HICH). Method Thirty patients withFILCH were divided into group A,B and C according to mean arterial pressure(MAP) with various ranges of <10%, 10 ~ 20%, and > 20%, respectively. Patients of three groups were checked with single photon emissioncomputed tomography (SPECT),and computerized tomography of head successively, 24 hours, 3 ~ 5 days, and 12~ 15 days after admission for the assessment of therapeutic effects. The rtes of the decrease in rCBF compared be-twecn groups were analyzed with t test. Neurologic function defect scale, activities of living scale and number of re-bleeding compared between two groups were analyzed by X2 test, respectively. Results There were statisticallysignificant differences in the rate of the decrease in rCBF. The neurologic function defect scale and activities of liv-ing scale at 24 hours, 3 ~ 5 days, and 12 ~ 15 days after admission compared between group C and group A, andbetween group C and group B showed significant differences ( P < 0.05). There were statistically significant differ-enees in rebleeding during acute phase of HICH between group A and group B or group C (P < 0.05).Conclusions If the systolic blood pressure is above 185mmHg or diastolic blood pressure over 95mmHg,it is ra-tional and safe to lower the MAP by about 15% with hypotensive gents so as to minimize the impact on regionalcerebral blood flow.

Purpose To investigate the clinicopathological features, treatment and prognosis of perivascular epithelioid cell tumor ( PEComa) of the head and neck. Methods Two cases of PEComa were analyzed by studying clinical data, histopathologic and immu-nohistochemical ( EnVision) changes and the related literatures were reviewed. Results Case 1 was a 26-year-old woman with a re-current larynx tumor, and case 2 was diagnosed in a 56-year-old man with a left nasal cavity mass. Their tumors were well-demarcated, and ranged from 2. 5 to 3. 5 cm in diameter. Microscopically, the tumors were composed of epithelioid cells arranged in nets, organoid pattern. The tumor cells had abundant pale eosinophilic granular to clear cytoplasm, vesicular nuclei, and mild atypia. The stroma was rich in capillaries, sinusoidal vasculature. There was mitotic activity and focal necrosis. By immunohistochemistry, the tumor cells were diffusely or focally positive for vimentin (2/2), HMB-45(1/2), Melan-A(2/2), SMA(2/2), Calponin(2/2), desmin(1/2), and TFE3(1/2). They were negative for CK, CD10, S-100, CgA, Syn, and MyoD1. Conclusion PEComa of the head and neck is rare. Definite diagnosis depends upon the comprehensive analysis of histopathology and coexpression of melanocytic and myoid mark-ers. The differential diagnosis include paraganglioma, malignant melanoma and metastatic carcinoma. Few PEComas of the head and neck behaved in an aggressive fashion with distant or loco-regional metastases and died of disease-related causes, it should be regarded as tumors with a malignant potential.

Objective To discuss the clinical features and treatment measures on patients with medium volume of supratentorial hypertensive cerebral hemorrhage in the elderly. Methods The clinical data of 57 patients with medium volume of supratentorial hypertensive cerebral hemorrhage aged ≥ 70 years were analyzed retrospectively. Results All of 57 patients,15 cases accepted medical treatment,42 cases were treated with minimally invasive operation for 1-26 (4.6 ± 6.5) h after onset.Three cases died in hospital,7 cases were transferred or gave up,6 cases lived as plant,8 cases were severely maimed,11 cases were moderately maimed,22 cases [38.6%(22/57)] recovered favorably. The main clinical characteristics of elderly patients:long history of hypertension, blood pressure fluctuation; atypical symptoms of increased intracranial pressure; more complications such as arrhythmia and pulmonary infection; most of family members of patients were pessimistic to treatment. Conclusions All factors should be thought over about the treatment of elderly patients with medium volume of hypertensive cerebral hemorrhage.Individual treatment is reasonable.Do not emphasize on early operation, minimally invasive operation can be helpful for patients.

VE: Chitosan and sodium tripolyphosphate were used as Complex matrix material for preparing pellets loading sodium diclofenac and its properties were studied. METHODS Chitosantripolyphosphate sodium polyelectrolyte was prepared according to the principle of static electricity polymerization. Its properties and structure characteristics were further investigated. The preparation process, effective factors and the optimal condition for the pellets loading sodium diclofenac were studied. RESULTS IR indicated that the structure of compound contained -NH3+-O-P group. DTA demonstrated that polyelectrolyte had an exothermic peak. There was no interaction between the drug and expedient. SEM showed that the surface of the pellets was regular, dense and the structure of the surface wasn't consistent with the inner. The pellets prepared by this method were uniform, round, well-distributed, hardy, good-mobility and its average diameter was about 10mm. CONCLUSION Chitosan-tripolyphosphate sodium polyelectrolyte could be used as a good matrix material for preparing pellets.

Objective To investigate the effect of porous silk fibroin scaffolds (PSFSs) combined with chondroitinase ABC (ChABC)for treatment of rats with spinal cord injury (SCI).Methods After exposed to T9 spinal cord transection injury,96 SD rats were divided into control group,PSFSs group,ChABC group, and PSFSs plus ChABC group according to random number table.BBB scoring system was used to evaluate hindlimb motor function in rats.Immunohistochemistry and Western blot analysis were performed to detect expression levels of neurofilament-200 (NF-200),glial fibrillary acidic protein (GFAP),and growth associated protein-43 (GAP-43) of the injured spinal cord.Immuno-fluorescence staining was carried out to evaluate regeneration of nerve fiber. Results BBB score improved in PSFSs group (8.1 ± 0.8),ChABC group (9.0 ± 1.1),and PSFSs plus ChABC group (13.7 ± 1.3) compared with control group 4 weeks after injury (5.3 ±0.7,P <0.05).Immunohistochemistry showed higher integral absorbance (IA) values of NF-200 and GAP-43 in those treatment groups, but smaller GFAP-positive area was observed compared with control group (P < 0.05).Immuno-fluorescence staining indicated more GAP-43 growth at injury sites in PSFSs plus ChABC group in contrast with other 3 groups.Western blotting showed levels of NF-200,GFAP,and GAP-43 differed among groups (P < 0.05).Conclusion PSFSs combined with chondroitinase ABC transplantation can enhance axonal regeneration, inhibit glial scar proliferation and hence promote motor function recovery.
