Because of the uniqueness and complexity of medical facilities that provide medical care,even people who sincerely agree to principles of medical ethics would consciously fall away from certain principle of medical ethics. Although such deviation is mere expedient, enormous impact is imposed on the principles of medical ethics. Contingency in moral choices, being an expedient under specific circumstances, is inherently necessary and justifiable. However, temporary deviation does not negate a principle. When there are conflicts of principles, contingency in medical moral choices requires that the moral values of different medical behaviors be judged and choices be made. Appropriate contingency in medical moral choices depends on the rational beliefs of medical staff and the right judgment about moral values of different medical plans.

The relation between pituitary-thryoid axis and physical development in 42 cases of endemic cretinism in Qinghai province was studied with 38 normal subjects as controls. The crania and the pituitary enlargement in cretins were revealed by CT scan. It was found that in cretins the physical development was retarded, the crania were smaller than those of the controls, and the serum TSH levels were increased, particularly in the myxedematous ones. White matter lesions on neuroimaging are common in the normal aged and in patients with several cognitive disorders. Its pathological features include demyelination, axonal loss and lacunar infarction, and the pathophysiological mechanisms may be chronic cerebral ischemia caused by hypoperfusion and disturbance of cerebral blood flow disorders. Vascular white matter lesiom may cause different degrees of cogntive disorders, but the exact the pathological subtype of dementia associated with white matter lesions remains uncertain.

ObjectiveTo observe influence of psychological nursing on perioperative irritated response of patients undergoing gynecological surgeries. MethodsOne hundred and twenty gynecological surgical pa-tients were randomly divided into the psychological nursing group and the control group, each group included 60 cases. Patients in the control group were given routine nursing. In addition of routine nursing, patients in the psychological nursing group were given perioperative psychological nursing intervention. The change of psycho-logical reaction, heart rate and blood pressure of the two groups were compared. ResultsCompared with the control group, number of patients with 3 to 4 grade of anxiety and terror in the psychological nursing group was significantly lower. Compared with that of preoperative day, heart rate and blood pressure in the control group evidently increased 5 minutes after entering the operation room, while in the psychological nursing group, except heart rate increasing, blood pressure was more stable. ConclusionsPsychological nursing can alleviate anxi-ety of patients undergoing gynecological surgeries, maintain normal hemodynamics and promote early recovery of patients.

Objective To observe the clinical effects of topiramate together with low-dose ACTH and prednisone、vitamine B6、gamma globulin on infantile spasms. Methods 134 infantile spasms patients were selected and randomly divided into control group and treatment group, each group 67 patients. The control group was treated with sodium valproate. The treatment group received low-dose ACTH and prednisone、topiramate、vitamine B6 、gamma globulin. The two groups all completed a therapeutic period of 3 months. Results The effect of the treatment group compared with the control group has significantly statistical difference (P ＜ 0.05) ;the result of the treatment group'electroencephalogran compared with that of the control group also has significants difference (P ＜ 0.05). Conclusion The therapeutic effect of low-dose ACTH and prednisone、 topiramate、 vitamine B6、 gamma globulin together on infantile spasms was much better than just taking sodium valproate only.

The clinical evaluation system for therapeutic effects of tumor, which focuses on survival period and tumor response rate, has been proved not very proper for evaluating the therapeutic effects of traditional Chinese medicine (TCM). Quality of life (QOL) has been introduced into the effective evaluation system of TCM and integrative medicine because of its similarity in concept and extension with TCM at theory basis, syndrome differentiation-treatment, and evaluation of therapeutic effects. But there are no QOL questionnaires which can evaluate the therapeutic effects of TCM and integrative medicine precisely. This article discussed the necessity and significance of developing QOL questionnaire special for TCM.

Objective To investigate the curative effect of Suhuangzhike capsules and Desloratadine Citrate Disodium used in chronicity allergic pharyngits .Methods Branched 147 patients into two sets ,the observation group with 85 patients treated with Suhuangzhike capsules and Desloratadine Citrate Disodium ,the control group with 62 pa-tients treated with Desloratadine Citrate Disodium only .Results The total effective rate of the observation group was 97.6%.The total effective rate of the control group was 75.8%,with statistical significance(χ2 =14.25,P<0.05). Follow up in 6 ~8 weeks,relapse rate of the observation group was 14.1%,relapse rate of the control group was 37.1%,with statistical significance(χ2 =10.53,P<0.05).Conclusion Suhuangzhike capsules and Desloratadine Citrate Disodium has good curative effect used in chronicity allergic pharyngits ,which can be decreased relapse .

Objective To explore the efficacy of self-designed waist pillow combined with plastic toilet in bedridden patients in emergency department.Methods From August 2013 to February 2014,90 bedridden patients in emergency department were divided into the observation group 1,the observation group 2 and the control group by random digits table with 30 cases in each group.Patients in the observation group 1 lied in the suprine position,and elevating head of bed 15 to 30 degrees,with pillow under the waist,and plastic toilet under the buttocks.Patients in the observation group 2 used the same method as that of the observation group 1,except that they did not use waist pillow.The control group lied in the suprine position,only used plastic toilet.The defecation and other related items were compared between the three groups.Results The observation group 1 was better than those of the observation group 2 and the control group in preventing pressure ulcers,urine leakage and backflow,comfort degree,worrying degree,defecation difficulty degree.Conclusions Application of waist pillow and plastic toilet in bedridden patients in emergency department could reduce the skin damage,and let patients defecate smoothly,which was in accordance with the connotation of comfort nursing.
