3.Therapeutic effect of topiramate on infantile spasms

Objective To observe the clinical effects of topiramate together with low-dose ACTH and prednisone、vitamine B6、gamma globulin on infantile spasms. Methods 134 infantile spasms patients were selected and randomly divided into control group and treatment group, each group 67 patients. The control group was treated with sodium valproate. The treatment group received low-dose ACTH and prednisone、topiramate、vitamine B6 、gamma globulin. The two groups all completed a therapeutic period of 3 months. Results The effect of the treatment group compared with the control group has significantly statistical difference (P < 0.05) ;the result of the treatment group'electroencephalogran compared with that of the control group also has significants difference (P < 0.05). Conclusion The therapeutic effect of low-dose ACTH and prednisone、 topiramate、 vitamine B6、 gamma globulin together on infantile spasms was much better than just taking sodium valproate only.
4.Clinical observation for traditional chinese fumigating to spasticity of cerebral palsy

Objective To explore the efficacy of traditional Chinese fumigating's treatment effect on infant's spasticity of cerebral palsy. Methods 92 patients with infant's spasticity of cerebral palsy were randomly divided into two groups: each group 46 patients. The control group received ordinary functional training and tuina therapies, and the treatment group except receiving functional training and tuina therapies ,they were given traditional Chinese fumigating additional,both groups were treated for 3 months. Results The muscular tension relieving extent in the treatment group is much better than that in the control group,and compared with the control group,there is significant statistical difference( t = 3. 11 ,P <0. 05) ;The overall efficacy was 86. 96% for the treatment group and 69. 57% for the control group. Between the two groups there were significant differences( x2 = 4. 09 ,P < 0. 05). Conclusion Traditional Chinese fumigating could obviously relieve the muscular tension and improve clinical efficacy, and had a good clinical prospect.
5.The curative effect investigate of Draft type frontal sinus drainage used on 42 patients with prosopantritis

Objective To investigate the curative effect of DrafⅠtype,Ⅱa/b-type frontal sinus drainage used on 42 patients(75 sides)with prosopantritis.Methods A summary analysis of 42 patients(75 sides)with prosopantritis who lined DrafⅠtype,Ⅱa/b-type frontal sinus drainage according to dissect characteristics during January 2011 to October 2012.Results CT of the 42 patients shows that there are 75 sides with prosopantritis,Based on the analysis of anatomical features performed DrafⅠtype,Ⅱa/b-type frontal sinus drainage to these patients,efficiency was 82.67%.Follow-up in half a year,restenosis and atresia again of 21 sides frontal sinus ostium was found.Conclusion Draf drainage has a good curative effect with small creation,few bleed and few recur-rence in chronic prosopantritis.
6.Low-frequency electrical stimulation can improve ability in the activities of daily living soon after stroke

Objective To assess the efficacy of low-frequency electrical stimulation (LES) in improving ability in the activities of daily living (ADL) of subjects after a recent stroke. Methods In a randomized controlled trial, 37 subjects with a first stroke were randomly divided into an electrical stimulation (ES) group and a control group. In the ES group (n = 19 ) electrodes were applied on the motor points of the tibialis anterior, the peroneal longus and the peroneus brevis muscles. Stimulation current was applied at an intensity set to cause full ankle extension. The stimulus pulse was a symmetric biphasic wave with frequency of 30 Hz, a pulse width of 200 μs, a duty cycle of 5 s on and 5 s off ramped up and down for 1 s each and amplitude up to 90 mA. The current amplitude was adjusted according to each subject's comfort. Stimulation lasted for 30 min/d, 5 d/week for 3 weeks. All subjects in the 2 groups also received a standard rehabilitation program. Evaluations included the Fugl-Meyer motor assessment (FMA) for the lower extremity and the modified Barthel index (MBI) for ADL ability. Measurements were recorded before treatment and after 2 and 3 weeks of treatment. Results Before treatment there was no significant difference between the two groups in terms of age, time post-stroke, stroke severity or the baseline measurements. After 2 and 3 weeks of treatment, the FMA scores in the ES group were significantly higher than those of the control group. The MBI scores in the ES group were also significantly higher than in the control group. The average MBI transferring, walking and stair climbing scores in the ES group were all significantly higher than those in the control group after either 2 or 3 weeks. Conclusion Three weeks of LES can improve ADL ability soon after stroke.
7.Influence of psychological nursing on perioperative irritated response of patients undergoing gyne-cological surgeries

ObjectiveTo observe influence of psychological nursing on perioperative irritated response of patients undergoing gynecological surgeries. MethodsOne hundred and twenty gynecological surgical pa-tients were randomly divided into the psychological nursing group and the control group, each group included 60 cases. Patients in the control group were given routine nursing. In addition of routine nursing, patients in the psychological nursing group were given perioperative psychological nursing intervention. The change of psycho-logical reaction, heart rate and blood pressure of the two groups were compared. ResultsCompared with the control group, number of patients with 3 to 4 grade of anxiety and terror in the psychological nursing group was significantly lower. Compared with that of preoperative day, heart rate and blood pressure in the control group evidently increased 5 minutes after entering the operation room, while in the psychological nursing group, except heart rate increasing, blood pressure was more stable. ConclusionsPsychological nursing can alleviate anxi-ety of patients undergoing gynecological surgeries, maintain normal hemodynamics and promote early recovery of patients.
8.Surgical treatment for complications of congenital retinoschisis.

This study examined the clinical features of complications of congenital retinoschisis and the clinical efficacy of vitreoretinal surgery in the treatment of these complications. The clinical efficacy of surgical treatments was retrospectively analyzed in 10 patients with congenital retinoschisis (10 eyes) complicated with rhegmatogenous retinal detachment (n=5), vitreous hemorrhage (n=2) and macula- involving schisis (n=1). All the patients suffered foveal and peripheral schisis. They were treated with scleral buckling (n=1) or vitrectomy (n=9). After the surgical treatment, the retina was reattached in patients with rhegmatogenous retinal detachment; the refractive media became transparent in those with vitreous hemorrhage; the visual acuity in 80% of patients was improved; no remarkable progression of schisis was found; no severe operative complications occurred. It was concluded that vitreoretinal surgery in the treatment of complications of congenital retinoschisis is safe and effective, and helps improve and maintain the visual function.
9.The effect of Atorvastatin on the levels of serum IL-6 、IL-8 and TNF-α in patients with acute cerebral infarction

Objective To study the effect of atorvastatin on the levels of serum IL-6、IL-8 and TNF-α in patients with acute cerebral infarction.Methods 120 patients with ACI were randomly divided into the atorvastatin therapy group(n = 60) and the conventional therapy group(n = 60).At the basis of conventional therapy,the Atorvatatin therapy group was treated with Atorvastatin 20mg/d.Both groups were treated for 4 weeks.The levels of serum IL-6 、IL-8 and TNF-α were measured before and after treatment.A healthy coatrol group which approximately matched the experimental group included 35 persons.Results The levels of serum IL-6 、IL-8 and TNF-α in ACI patients were higher than control group,(P<0.05).No difference of IL-6、 IL-8 and TNF-α were found between the atorvastatin group and the control group before treatment(P>0.05).Both two groups after 4 weeks treatment were significantly lower than those before treatment(all P<0.05),and the level of serum IL-6 、IL-8 and TNF-α in Atorvastatin therapy group were significantly lower than that in conventional therapy group(P<0.05).Conclusion The IL-6、 IL-8 and TNF-α played an important role in the pathogenesis of ACI.Atorvastatin could significantly decrease the level of serum IL-6 、IL-8 and TNF-α.Atorvastatin had anti-inflammatory effects and neuroprotection role.
10.Vascular white matter lesions and its impact on cognitive function

White matter lesions on neuroimaging are common in the normal aged and in patients with several cognitive disorders. Its pathological features include demyelination, axonal loss and lacunar infarction, and the pathophysiological mechanisms may be chronic cerebral ischemia caused by hypoperfusion and disturbance of cerebral blood flow disorders. Vascular white matter lesiom may cause different degrees of cogntive disorders, but the exact the pathological subtype of dementia associated with white matter lesions remains uncertain.
