1.Induction of Rat Liver gamma-Glutamyl Transpeptidase by Bile Acid Load.

BACKGROUND/AIMS: In order to elucidate the possible mechanism of increase of y-glutamyl transpeptidase (y-GTP) activity in cholestatic liver and serum was studied. METHOD: Rats were divided into eight groups: Normal, sham operated control, bile duct obstruction (BDO) alone (BDO group), BDO plus taurocholic acid (TCA) injection (BDO plus TCA group), BDO plus tauroursodeoxycholic acid (TUDCA) injection (BDO plus TUDCA group), choledoco-caval shunt (CCS) operation (CCS groups), CCS plus TCA injection (CCS plus TCA group), and CCS plus TUDCA injection (CCS plus TUDCA group). Y-GTP activity was determined in the serum and liver cytosolic, mitochondrial and microsomal preparations isolated from above experimental rats. The values of Km and Vmax in this hepatic enzyme was measured. RESULT: the activities of liver cytosolic and microsomal y-GTP showed a significant increase in the CCS group. The activities of liver cytosolic, mitochondrial and microsomal y-GTP showed a significant increase in the BDO group. And the activity of serum y-GTP showed a marked increase in teth CCS and BDO poups. However, y-GTP activities in the serum and in liver microsomal prepatation rose more rapidly in the BDO group tban CCS. Y-GTP activity in liver cytosolic and microsomal preparatians, and its Vmax value incmmxl significantly in both CCS plus TCA group, and BDO plus TCA group than each control group, such as CCS and BDO group. On the other hand, the values of Km of the hepatic subcellular y-GTP did not change in the all experimental groups. Sennn y-GTP activity increased significantly in both CCS plus 7CA group, and BDO plus TCA group than each control group. However, these serum and hepatic enzyme activities did not change in both CCS plus TUDCA group and BDO plus TUDCA group. CONCLUSIONS: The above results suggest that 7CA stimulates biosynthesis of the y-GTP in the liver. And the elevations of the serum enzymes activity thought to be caused by increase of hepatocyte membrane permeability by a physical property (detergency) of TCA, which cause the enzyme to leak into the blood in large quantities.

2.Two Cases of Keratosis Palmaris et Plantaris.

We described here-in a 36-year-old male patient (case 1) with a severe form of keratosis palmaris et plantaris(Thost-Unna type) which was diffuse hyperkeratosis with hyperhidrosis on the entire surface of the palms and soles. His son, 8-year- old boy, showed symmetrical distribution of hyperkeratotic patches on the inner sides of both great toes(case 2). Therefore, it was suggested this genodermatosis occured in two generations of a family. The biopsy taken from the right palm of case 1 showed features of epidermal hyperplasia with hyperkeratosis, hypergranulosis and acanthosis. We tried the oral Vit. A therapy(50,000 unit, daily) in case 2. The hyperkeratotic lesions became slightly improved in 4 months after therapy.

3.Syphilis Prevalence in Teagu Hope Village.

Serologic tests for syphilis, including VDRL and TPHA tests, and Physical examination were carried out on 1, 605 subjects who lived in Taegu Hope Village, a social welfare center, from April, 198R to May, 1985. We prepared this study comparing with previous reports for healthy individuals and prostitutes. The results were as follows, The reactive rate of VDRL. qualitative test was 11. 6% in total, 10. 3%, in men and l3. 5%. in women. The reactive rates of VDRL. qualitative tests were highest in 7 th decade as 28, 7% and followed by 6 th decade as 16. 7% the decacile as 16. 0% 8 th decade as 14,3% 4th decade as 8. 8% and 3rd decade as 5. 9%, 3 In ]Hg sero-positive subjects, VDRL quantitative tests were done, and 35. 5% were reacted with 1: 8 or high and 29. 0% had one or more syphilitic lesions. 4 In 54 subjects who had one or more syphilitic lesions, 19 had chancres, 17 had alopecia syphilitica, 7 had rnaculopapular syphilid, 7 had condylomata lata. 3 had papular syphilid, and 1 had maculopapular syphilid and alopecia syphilitica. The biological false positive rate was 10. 1,4 in 138 VDRL reactive subjects, using 1;he TPHA test standard.

4.Cauda Equina Syndrome after Laminectomy of Lumbar Spine with Application of Autogenous Free Fat Graft: A Case Report.

Although epidural fibrosis after laminectomy is considered to be the cause of pain in a number of patients, the exact relationship of postoperative scar tissue and symptoms remains controversial. However it is generally accepted that epidural fibrosis after surgical decompression of neural tissue has to be avoided. To diminish the likelihooa that such a scar will form, fat grafts have been used to create an interpositional membrane. Compression of a nerve after the use of a fat graft is rare; postoperative cauda equina syndrome, resulting from compression by a grafted fat. We report the case of a patient who had this complication.

5.Rotational Acetabular Osteotomy for the Dysplastic Acetabulum

For the management of adult dysplastic hips with early degenerative arthritis, we can prevent further progress of secondary degenerative changes of the hip by a Rotational Acetabular Osteotomy, which effectively corrects the preoperatively inadequate acetabular coverage of the femoral head. Biomechanical advantages of this procedure are, l. An adequate coverage of tl; femoral head then enlarges the actual weight-bearing surface area, and so diminishes the resultant force per unit area of the acetabular weight-bearing surface and femoral head. 2. The resultant forces are also diminished by a medial shift of the femoral head. 3. The shearing force is decreased by the reduction of roof obliquity. 4. Because of the rotation of the original articular surface, remodeling the post-operative acetabular articular surface as Chiari operation is not necessary, and so can be performed even on elderly patients.

10.Neurofibroma Associated with Alopecia.

Neurofibromas are benign tumors composed of a complex proliferation of neuromesenchymal tissue with residual nerve fibers. There are several distinct types of neurofibromas: cutaneous, subcutaneous, nodular plexiform, and diffuse plexiform. To our knowledge, none of these have previously been described in association with alopecia in the literature. We present a case of neurofibroma of the scalp which is associated with alopecia.
