2.The Relationship of Somatic Symptoms and Self-esteem in Adolescence.

OBJECTIVES: The aims of this study were to develop the Korean version of Adolescent Symptom Checklist(ASC) which was devised by Thomas et al.(1988), to investigate descriptive data of somatic symptoms which our adolescents reported and to find the correlation between somatic symptom and self-esteem. And furthermore we compared our results with those of foreign countries. METHOD: The subject group was composed of 794 students who go to commercial or academic high school in kwangju city. They completed self rating questionnaires which include ASC, Self-esteem scale and somatization and depression subscales of SCL-90-R. The descriptive statistics, reliability and validity of ASC, means of somatic symptom and self-esteem for each group and correlation of somatic symptom and self-esteem were examined. We would consider an adolescent as belonging to high risk group if their score on the ASC was one or more standard deviations above the mean. RESULTS: Seventeen point eight percent of all subjects were in high risk group and the pacentage was higher than 11-15% of foreign countries. Sixty nine point four percent had visited to hospital due to their somatic symptoms during the past 12months and 26.2% were treated by doctors recently. Internal consistency(Cronbach a .89) was high. Inter-item correlations were all positive and ranged from .09 to .58 with a mean of .29 and item-total correlations varied from .35 to .60 with a mean of .48. The ASC was significantly correlated with the somatization and depression subscales of SCL-90-R. Somatic symptoms of female group were much more frequent than those of male group. The higher their grade is, the more somatic symptoms they complained. There were signiticant negative correlations between somatic symptoms and self-esteem in all groups. CONCLUSION: By the result of this study, we suggest that ASC can be used as a tool for evaluation of adolescent's somatic symptoms. We showed that the frequency of somatic symptoms in adolescents were higher in females than in males, and were higher in upper grade than in lower grade. We also showed that the more somatic symptoms they complained, the lower self-esteem they had.

4.Air Shadow Containing Mass Lesion in the Chest.

A 70-year-old female Patient was admitted due to the impression of lung abscess via local clinic. Chest P-A showed air shadow containing huge mass lesion in the right lower lung field, pericardio phrenic angle. But the patient complained of only mild upper respiratioy tract infection symptoms and the laboratory tests were within normal limits. Barium enema and chest C-T taken right after barium enema were performad in suggesting of the diaphragmatic hernia. Barium tilled transverse colon and associated omentum were in the right anterior hemithorax surrounded by the hernial sac. The mass lesion shown in the chest P-A was compatible with the diaphragmatic hemia, Morgagni type. Operative reduction of the herniated bowel and simple closure of the diaphragm was performed.

6.Clinical consideration between the type of pneumonia and cold agglutinin titer, and mycoplasma antibody titer caused by mycoplasma pneumonia in children.

The authors analysed 261 cases of mycoplasmal pneumonia in children who were admitted to the pediatric department of Seoul Adventist Hospital between July 1986 and June 1991. The result obtained were as follows; 1) Yearly distribution of the cases showed high in 1987 and 1991, but no significant difference was noted in seasonally or monthly distribution. 2) The peak incidence of age was between 2 and 6 years of age, and the sex ratio of boy to girl was 1.1:1. 3) pulmonary manifestations were asthma in 21 cases (8.0%), atelectasis in 5 cases (1.9%), emphysema in 3 cases (1.2%), pleural effusion in 16 cases (6.1%), otitis media in 4 cases (1.2%) and sinusitis in 9 cases (3.4%). 4) Extrapulmonary complication were hepatitis in 53 cases (20.3%), skin rash in 9 cases (2.4%), proteinuria in 6 cases (2.3%), and hematuria in 4 cases (1.5%). 5) On the chest X-ray examination, the most common type of pneumonia was interstitial pneumonia (94 cases, 39.8%), and unilateral involvement was common (85.6%), and the most common involvement was right lower lobe (83 cases, 41.4%). 6) The relationship between the type of pneumonia and cold agglutinini titer, and between the type of pneumonia and Mycoplasma antibody titer were not found (P>0.05). 7) In the 16 cases of pleural effusion, The type of pneumonia was lobar, lobular (9 cases), bronchopneumonia (7 cases), and interstitial type was not present. The site of pneumonia was left (6 cases), right (9 cases), and 1 case showed bilatrality. The extent of pleural effusion was mild (10 cases), moderate (3 cases), and severe (3 cases). The result of pleural fluid exam in severe cases was all exudate. 8) The mean duration of admission was most common from 6 to 10 days. the mean duration of admission by the type of pneumonia was lobar, lobar (10.71+/-3.40), interstitial (8.78+/-2.14), and bronchopneumonia (8.83+/-2.47). 9) Both mycoplasma antibody test and cold agglutinin test were carried out in 185 cases and sensitivity of mycoplasma antibody test was 55.1%.

7.Knowledge of orthodontics and malocclusion.

Orthodontists want why the patients want orthodontic treatment. Demand and need for orthodontics is very important factors of cooperation and management. Chief complaint would be different according to the race and culture. Our results show that in the control group, Korean female adolescents[chief complaint is cowding(30.6%), spacing(26.5%), upper anterior protrusion(18.4%), anterior cross bite(8.2%) in order. Korean female adolescents considered anterior cross bite is more nonacceptable than spacing and anterior protrusion. The purpose of orthodontics seemed to enhance esthetic, dental health, and to improve oral function.

8.Congenital Pseundarthrosis of the Tibia: 3 Cases Report

Three cases of congenital pseudarthrosis of the tibia were treated at Department of Orthopedic Surgery, Seoul national University Hospital during last four years. In all cases, many Cafe-au-lait spots and subcutaneous nodules were shown on the whoIe body skin, which are suggestive of neuro-fibromatosis. Case 1, A thirteen years old boy was treated with plate and screw fixation with massive iliac cancellous and cortical graft. In a follow-up of three and one half years, solid bony union has been obtained with successful result. Case 2, A 5 years old boy was treated with Boyds dual cortical graft with plate and screw fixation. In a follow-up to eight months, bony union were satisfactory. Case 3, A six months old boy who is young brother of case 2, was treated with Charnleys intramedullary nailing and iliac cancellous and cortical graft. The patient is still in the Iong leg cast, six months postoperatively. The evidence of familial and hereditary factor in pedigree is elicited on case 2 and 3. This congenital pseudarthrosis of the tibia in the sibling is regared as the first report in Korean literature.

9.Study on the Biological Characteristics of Cultured Osteoblasts Derived from Alveolar Bone.

Osteoblasts from alveolar bone may have an important role in the bone regeneration for periodontium, but their culture and characterization are not determined yet. The purpose of this study was to investigate the biological characteristics of primary explant cultured osteoblasts(PECO) from alveolar bone. Osteoblasts were isolated and cultured from alveolar socket of extracted tooth in children. To compare the characteristics, osteoblasts and gingival fibroblasts were cultured with DMEM at 37degrees C, 5% CO2, 100% humidity incubator, and human fetal osteoblasts cell line(hFOB1) were cultured with DMEM at 34degrees C, 5%, CO2, 100% humidity incubator. To characterize the isolated bone cells, morphologic change, cell proliferation and differentiation were measured. Morphology of PECO was small round body or cuboidal shape on inverted microscope and was similar with hFOB1. PECO became polygonal shape with stellate and had an amorphous shape at 9th passage in culture. PECO had significantly higher activity than that of gingival fibroblasts and hFOB1 in alkaline phosphatase activity. The expression of osteocalcin and bone sialoprotein in PECO was notably increased when compared with hFOB1 and gingival fibroblasts. These result indicated that PECO from alveolar bone in children has an obvious characteristics of osteoblast, may be applied for the regeneration of bone.

10.Treatment of Torsion Dystonia with Motor Point Block Using Phenol : A case report.

Dystonia is an abnormal movement characterized by sustained muscle contractions, frequently causing twisting and repetitive movements or abnormal postures. Torsion dystonia is characterized by torsion spasms of muscle contraction, which distorts the limbs and trunk into dystonic postures. We present a case of a patient with torsion dystonia who was recalcitrant to oral medication or even surgical operations. This patient was treated with motor point block using 5% phenol solution. Using electromyographical guidance, phenol was injected into the paraspinal and upper extremity muscles, respectively. He showed reduction of dystonia and improvement of functional abilities. Motor point block using phenol can be considered as a tool of the management for patients with torsion dystonia. MDVAQ@yumc.yonsei.ac.kr
