2.Bilateral Segmental Neurofibromatosis with Partial Unilateral Lentiginosis: Case Report with Review of the Literature.

Bilateral segmental neurofibromatosis is a rare disease characterized by bilateral neurofibromas, with or without pigmented lesion, or unilateral neurofibromas with contralateral pigmented lesion, limited to a body segment. Partial unilateral lentiginosis is characterized by numerous lentigines localized to a body segment, often corresponding to one or more dermatome. Bilateral segmental neurofibromatosis combined with partial unilateral lentiginosis is very rare, and to our knowledge, only 2 cases have been reported in English literature. We herein report another case of bilateral segmental neurofibromatosis with partial unilateral lentiginosis in a 46-year-old woman.

5.Bilateral Elastofibroma: Report of a case.

Elastofibroma is a rare benign tumor-like condition manifesting as firm and spherical mass with poorly circumscribed margins of fibroelastic tissue, occuring in the subscapular region or the chest wall of elderly persons. It is not a true neoplasm but rather a reactive or degenerative process causing abnormal elastogenesis. It is unilateral in the majority of cases and the right side is affected more commonly than the left. We report a case of bilateral elastofibromas removed from both subscapular regions of a 73-year-old female farmer. She was presented with tender masses on the bilateral subscapular areas for seven years. Microscopically, it consisted of a mixture of intertwining broad eosinophilic collagen bundles and elastic fibers associated with a few fibroblasts and mature fat cells. The elastic fibers had a degenerated beaded appearance or were fragmented into serrated globules in a linear arrangement.

7.Time Trend in Height, Weight, BMI and Waist Circumference of Korean Adolescents; From the Korean National Health and Nutrition Examination Survey (KNHNES), 1998, 2001 and 2005.

PURPOSE: The mean height and weight had been increasing rapidly in Korean adolescents. The purpose of this study was to analyze body physique (height, weight, BMI and waist circumference) of Korean adolescents, using KNHNES data. METHODS: The data were obtained from the KNHNES from each year of 1998, 2001 and 2005. Total number of 4164 adolescents (2140 boys, 2024 girls) aged 10 to 19 years were included. Height, weight, and waist circumference were measured and the mean value of body physique was analyzed by sex and age. RESULTS: For height of 10 yr-old boys, there was no difference between 1998, 2001 and 2005, but the height of 13 yr-old boys was increased more than 3 cm in 2005, compared to 1998 and 2001. For height of 10 yr-old girls, more than 2 cm was increased, but, there was no significant difference after the age of 14 year in each 1998, 2001 or 2005. Height of the age of 19 was 173.7 cm in boys and 161.3 cm in girls in 2005, showing no distinct change compared to 1998 or 2001. Weight and BMI showed significant increase in both sexes and all-aged groups for older than the age of 11 years in 2005, compared to 1998 or 2001. Weight increase was more distinct in boys compared to girls. For boys, waist circumference of 2005 were higher than 1998, but lower than 2001. For girls, waist circumference of 2005 were higher than 1998 and 2001 especially in the late teens. CONCLUSION: Rapid increase in height was shown in the early teenage whereas final height showed no significant difference during 1998-2005. The mean weight and BMI have been increased dramatically in 2005, compared to 1998 or 2001 in boys and girls. This phenomenon was shown more remarkably in boys, compared to girls. These findings emphasize importance of obesity management, especially on boys.

8.A Study on the Propensity of Koreans in Choosing Dementia Care Settings.

OBJECTIVE: This study was to investigate the factors that influence the propensity of Koreans in choosing care settings of dementia patients. METHODS: This study analyzed the data from the '2014 Nationwide Survey on Dementia Awareness of Koreans' that was conducted by the National Institute of Dementia. Korean's perception of care burden for dementia was measured with grading on its types. Also its influences on preference for care between facilities and homes were evaluated using multivariate analysis with socio-demographic characteristics. RESULTS: In terms of preferred care settings, respondents preferred facilities over homes in case of themselves and their family, respectively 77.5% and 68.2%. The preference for facilities was significantly influenced by the respondents' age for both themselves and their family. Additionally, the perception of relatively higher emotional and physical burden compared to economic burden significantly influenced preference for facilities for their family. CONCLUSION: Improving public awareness and setting-up a practical social supporting system are needed to reduce emotional and physical burden as well as economic burden of dementia. Furthermore, building up an appropriate and safer communities for dementia patients and their caregivers is much demanded for reducing their burdens.

9.Clinical impact of applying strategic programming in patients with implantable cardioverter‑defibrillators beyond reducing inappropriate shocks

Background:
The incidence of inappropriate shocks remains high at 30% in patients with implantable cardioverter-defibrillators (ICDs). This retrospective study sought to examine the efficacy of strategic programming (ICD programming with a long detection interval and high-rate cutoff) in reducing electrical storm, inappropriate shocks, and unexpected hospital visits in patients with ICDs with/without cardiac resynchronization therapy with defibrillator (CRT-Ds).
Methods:
This was a single tertiary center retrospective study, evaluating the clinical outcomes, especially regarding inappropriate therapies in patients with ICDs or CRT-Ds. Enrolled patients underwent ICD or CRT-D implantations from January 2008 to May 2016. Clinical information was attained by a thorough chart review.
Results:
We analyzed 155 defibrillator patients from January 2008 to May 2016 (124 patients had ICDs and 31 had CRT-Ds). Since we adopted this strategic programming as a default programming from 2015 implanted ICDs and CRT-Ds, we divided the patients into two groups: devices implanted before 2015 (group A, n = 94) versus implanted after 2015 (group B, n = 61). During a median of 1289 days of follow-up, electrical storms occurred in three patients (eight events) in group B versus 11 (28 events) in group A (P = 0.18); appropriate therapies were delivered in 27 patients (56 events) in group A versus 7 (15 events) in group B (P = 0.72); inappropriate therapies were delivered in 15 patients (21 events) in group A versus 1 with 1 episode in group B (P = 0.03); and 5 unexpected hospitalizations occurred in four patients in group B versus 36 in 24 patients in group A (P = 0.02).
Conclusion
The clinical application of strategic programming reduced inappropriate shocks and unexpected hospitalizations in ICD and CRT-D patients.
10.Body Composition of Children and Adolescents with Insulin-dependent Diabetes Mellitus.

Bodycomposition measurement is useful in the diagnosis of pathology, assessment of disease process and response to treatement in many endoclonologic and metabolic diseases. The Techniques used currently are mostly indirect, often expensive, difficult and time-consuming. A new method for estimation of body composition, infrared interactance, is rapid, safe, noninvasive, and may be useful in research and clinical studies. Body composition was assessed in 51 children and adolescants with insulin-dependent diabetes mellitus by infrared interactance method using a Futrex 5000A body fat content analyzer. Percentage body fat of pubertal girls was 26.1+/-7.6%, significantly greater than prepubertal girls (17.9%, 0<0.01) and pubertal boys (19.9%, p<0.05). Body fat content was increased through puberty in girls. but not increased in boys. Our data do not explain why body fat of pubertal boys was not increased. There was no significant association of insulin dose and HbA1C level with percentage body fat. Thus, we had to attempt measure calorie intake, activity level and diet composition. Our data revealed that dietary carbohydrates may prompt fat deposition, but energy balance does not explain the differences between pubertal girls and boys. In conclusion, Body fat increased through puberty in girls, but the etiology of differences in pubertal girls and boys is not clear, and in view of potential adverse effect of fat depostion of long-term health, further larger case-contolled study will be necessary.
