Bodycomposition measurement is useful in the diagnosis of pathology, assessment of disease process and response to treatement in many endoclonologic and metabolic diseases. The Techniques used currently are mostly indirect, often expensive, difficult and time-consuming. A new method for estimation of body composition, infrared interactance, is rapid, safe, noninvasive, and may be useful in research and clinical studies. Body composition was assessed in 51 children and adolescants with insulin-dependent diabetes mellitus by infrared interactance method using a Futrex 5000A body fat content analyzer. Percentage body fat of pubertal girls was 26.1+/-7.6%, significantly greater than prepubertal girls (17.9%, 0<0.01) and pubertal boys (19.9%, p<0.05). Body fat content was increased through puberty in girls. but not increased in boys. Our data do not explain why body fat of pubertal boys was not increased. There was no significant association of insulin dose and HbA1C level with percentage body fat. Thus, we had to attempt measure calorie intake, activity level and diet composition. Our data revealed that dietary carbohydrates may prompt fat deposition, but energy balance does not explain the differences between pubertal girls and boys. In conclusion, Body fat increased through puberty in girls, but the etiology of differences in pubertal girls and boys is not clear, and in view of potential adverse effect of fat depostion of long-term health, further larger case-contolled study will be necessary.

4.Effects of Isoflavones on Bone Mineral Density and Bone Mineral Content in Ovariectomized Rats.

A recent study reported that a diet rich in isoflavones is beneficial for bone formation in growing rats. It therefore seemed desirable to find out whether the beneficial effect of isoflavones in ovariectomized rats could also be reproduced with same amount of isoflavones which used for growing rats. To study the effect of isoflavones, an equal amount of isoflavones which used for growing rats, on bone mineral density and bone mineral content in ovariectmized rats were performed. Forty female Sprague-Dawley rats (body weight 210+/-5 g) were divided into two groups, ovariectomy and sham groups, which were each randomly divided into two subgroups that were fed casein and casein supplemented with isoflavones diets for 9 weeks after operation. All rats were fed on experimental diet and deionized water ad libitum for 9 weeks. Bone mineral density (BMD) and bone mineral content (BMC) were measured using PIXImus (GE Lunar Co, Wisconsin) in spine and femur. Serum alkaline phosphatase activity (ALP) and osteocalcin and urinary DPD crosslinks value were measured as markers of bone formation and resorption. The results of this study indicate that body weight gain and food intake were higher in ovariectomy groups than in sham groups regardless of diets. Serum Ca concentration was lower in ovariectomy groups than in SHAM groups. Serum ALP, osteocalcin, and crosslink value were increased in ovariectomy groups. Spine BMD/weight, femur BMD/weight, and femur BMC/weight of ovariectomy groups were significantly lower than SHAM groups after 9 weeks. However, isoflavones supplemented group in ovariectomy groups, serum ALP and osteocalcin concentrations, spine BMD/weight and spine BMC, femur BMD/weight and femur BMC/weight were significantly increased after 9 weeks. In conclusion, the beneficial effect of isoflavones on bone in ovareiectomized rats was shown on 9 weeks after feeding with an equal amount of isoflavones supplementation which used for growing rats.

5.Skin Organ Culture Model for Evaluation of Melanin Pigmentation.

BACKGROUND: Skin organ culture is widely used as a tool to investigate skin biology or skin disease. OBJECTIVE: The objective of the present study was to develop an ideal skin organ culture model for evaluation of melanin pigmentation. METHODS: An air-liquid interface and submerged method were used. The histology of the cultured skin was studied with H&E stain. To examine the epidermal pigmentation, Fontana-Masson stain and NKI/beteb stain were performed. Pigment modifiers (arbutin, LY294002) were applied to the culture medium for 3 days as an air-liquid interface culture. RESULTS: The general architecture of the skin was well maintained for 5 days. The melanin pigment decreased during culture without change of the number of melanocytes. As expected from previous reports, the effect of pigment modifiers (arbutin, LY294002) on cultured skin was demonstrated. CONCLUSION: The results indicate that this skin organ culture model is useful in evaluating the melanin pigmentation

6.A Case of Leiomyoma of the Uterus and Vagina.

The leiomyoma of the vagina is relatively rare benign tumor, but most common mesencymal tumors in adult woman. There does not seem to be a correlation between the existence of vaginal leiomyoma and uterine or other leiomyomas. But, recently we experienced a case of vaginal leiomyoma arising in the lateral vaginal wall and concomitant uterine leiomyoma. Now we present it with a brief review of the case and its literatures.

Ovarian myoma is a rare mesenchymal tumor, accounting for only 1% of benign ovarian neoplasm. These tumors only originate from smooth muscle on the walls of blood vessels in the cortical stroma, in the corpus luteum and in the ovarian ligaments at the point of attachment to the ovary. Most patients are asymptomatic and the tumors are usually found incidently during operations or at autopsies. Two cases of ovarian myoma experienced in our hospital recently are reported with a brief review of the literatures."

Leflunomide is a disease-modifying antirheumatic drug that has been available in Korea since 2003. Leflunomide induced interstitial pneumonitis has been reported as an adverse effect in other countries but not in Korea. A 57-year-old woman was treated with leflunomide since she had been resistant to methotrexate, hydroxychloroquine and sulfasalazine. She developed high fever, dyspnea, and non-productive cough 3 months after the administration of leflunomide. She was diagnosed leflunomide-induced interstitial pneumonitis based on history, physical, laboratory, radiologic and pathologic findings. The patient was treated by prednisolone 1 mg/kg/day with cholestyramine 24 g/day, resulting in dramatic improvement. Here we report a case of leflunomide induced pneumonitis treated successfully with high dose steroid.

9.Becker's Nevus with Underlying Neurofibroma.

Becker's nevus is a common and acquired focal epidermal melanotic hypermelanosis characterized by a large, unilateral patch showing hyperpigmentation and hypertrichosis on the shoulder, back or chest of a man around puberty. In some cases, it is associated with hamartomatous proliferation of pilosebaceous or dermal component including smooth muscle, connective tissue and nerve sheath. According to these findings, some authors suggested that Becker's nevus could be an organoid hamartoma accompanied by variable proliferation of epidermal and dermal components. We herein report a case of Becker's nevus associated with underlying neurofibroma in a 30-year-old man, which might be another evidence supporting that suggestion.

10.A 36 Year-Old Primivida Diagnosed with Primary Lung Cancer.

A 36-year-old primivida, at 29 weeks of pregnancy with no smoking history, was admitted to the hospital complaining of cough for 3 days. Chest X-rays revealed atelectasis of the right upper lobe, and a transbronchial lung biopsy confirmed primary lung adenocarcinoma. After consulting with obstetricians and neonatologists, we wanted to deliver the child and treat the mother with chemoradiotherapy. But as she was adamantly opposed to treatment until fetal lung maturation was complete, we planned to delay the birth until 34 weeks, deliver the baby by caesarian section, and then treat the mother. However, maternal hypoxia and fetal distress resulted in an emergency delivery at 30 weeks. After delivery, we treated the mother's brain metastases with radiation therapy and systemic cisplatin-pemetrexed, but she deteriorated and expired 95 days after the diagnosis. Lung cancer during pregnancy is a rare disease and raises many medical and ethical issues in deciding the best course of therapy. We describe our clinical approach and review the potentially challenging features of managing a pregnant patient with lung cancer.
