1.Nutrient Intake according to Weight Gain during Pregnancy, Job Status, and Household Income.

The objective of this study was to investigate the association of nutrient intake and pregnancy outcome mediated by weight gain during pregnancy, job status, and household income. Maternal age, educational level, self-reported pre-pregnancy weights, educational level, and household income were collected from the women at 2 months postpartum. For each offspring, weight at birth, length at birth, and gestational age were collected. Participants were asked to report the frequency of consumption of foods between 28–42 weeks into the pregnancy. Diet was assessed by using a validated 106-item semi-quantitative food-frequency questionnaire (SQFFQ) and women were asked portions and quantities based on pictures, food models, and measuring tools such as cups or teaspoons. Results showed that women who gained below the recommended weight gain during pregnancy, within, and over were 25.3%, 38.7%, 36.0%, respectively. In comparison to weight gain and the offspring's length and weight at birth, the offspring of mothers with a lower weight gain had a higher length. Energy, protein, vitamin B2, vitamin C, calcium, and potassium were significantly lower at employed group. We did not observe a significant difference between birth characteristics and maternal nutrient intake by income. Infants with a higher ponderal index at birth were born to women with a higher pre-pregnancy body mass index (BMI).

4.The Relationship of Somatic Symptoms and Self-esteem in Adolescence.

OBJECTIVES: The aims of this study were to develop the Korean version of Adolescent Symptom Checklist(ASC) which was devised by Thomas et al.(1988), to investigate descriptive data of somatic symptoms which our adolescents reported and to find the correlation between somatic symptom and self-esteem. And furthermore we compared our results with those of foreign countries. METHOD: The subject group was composed of 794 students who go to commercial or academic high school in kwangju city. They completed self rating questionnaires which include ASC, Self-esteem scale and somatization and depression subscales of SCL-90-R. The descriptive statistics, reliability and validity of ASC, means of somatic symptom and self-esteem for each group and correlation of somatic symptom and self-esteem were examined. We would consider an adolescent as belonging to high risk group if their score on the ASC was one or more standard deviations above the mean. RESULTS: Seventeen point eight percent of all subjects were in high risk group and the pacentage was higher than 11-15% of foreign countries. Sixty nine point four percent had visited to hospital due to their somatic symptoms during the past 12months and 26.2% were treated by doctors recently. Internal consistency(Cronbach a .89) was high. Inter-item correlations were all positive and ranged from .09 to .58 with a mean of .29 and item-total correlations varied from .35 to .60 with a mean of .48. The ASC was significantly correlated with the somatization and depression subscales of SCL-90-R. Somatic symptoms of female group were much more frequent than those of male group. The higher their grade is, the more somatic symptoms they complained. There were signiticant negative correlations between somatic symptoms and self-esteem in all groups. CONCLUSION: By the result of this study, we suggest that ASC can be used as a tool for evaluation of adolescent's somatic symptoms. We showed that the frequency of somatic symptoms in adolescents were higher in females than in males, and were higher in upper grade than in lower grade. We also showed that the more somatic symptoms they complained, the lower self-esteem they had.

6.Clinical impact of applying strategic programming in patients with implantable cardioverter‑defibrillators beyond reducing inappropriate shocks

Background:
The incidence of inappropriate shocks remains high at 30% in patients with implantable cardioverter-defibrillators (ICDs). This retrospective study sought to examine the efficacy of strategic programming (ICD programming with a long detection interval and high-rate cutoff) in reducing electrical storm, inappropriate shocks, and unexpected hospital visits in patients with ICDs with/without cardiac resynchronization therapy with defibrillator (CRT-Ds).
Methods:
This was a single tertiary center retrospective study, evaluating the clinical outcomes, especially regarding inappropriate therapies in patients with ICDs or CRT-Ds. Enrolled patients underwent ICD or CRT-D implantations from January 2008 to May 2016. Clinical information was attained by a thorough chart review.
Results:
We analyzed 155 defibrillator patients from January 2008 to May 2016 (124 patients had ICDs and 31 had CRT-Ds). Since we adopted this strategic programming as a default programming from 2015 implanted ICDs and CRT-Ds, we divided the patients into two groups: devices implanted before 2015 (group A, n = 94) versus implanted after 2015 (group B, n = 61). During a median of 1289 days of follow-up, electrical storms occurred in three patients (eight events) in group B versus 11 (28 events) in group A (P = 0.18); appropriate therapies were delivered in 27 patients (56 events) in group A versus 7 (15 events) in group B (P = 0.72); inappropriate therapies were delivered in 15 patients (21 events) in group A versus 1 with 1 episode in group B (P = 0.03); and 5 unexpected hospitalizations occurred in four patients in group B versus 36 in 24 patients in group A (P = 0.02).
Conclusion
The clinical application of strategic programming reduced inappropriate shocks and unexpected hospitalizations in ICD and CRT-D patients.
7.Effectiveness of Nursing Management Information Systems: A Systematic Review.

OBJECTIVES: The purpose of this study was to review evaluation studies of nursing management information systems (NMISs) and their outcome measures to examine system effectiveness. METHODS: For the systematic review, a literature search of the PubMed, CINAHL, Embase, and Cochrane Library databases was conducted to retrieve original articles published between 1970 and 2014. Medical Subject Headings (MeSH) terms included informatics, medical informatics, nursing informatics, medical informatics application, and management information systems for information systems and evaluation studies and nursing evaluation research for evaluation research. Additionally, manag* and admin*, and nurs* were combined. Title, abstract, and full-text reviews were completed by two reviewers. And then, year, author, type of management system, study purpose, study design, data source, system users, study subjects, and outcomes were extracted from the selected articles. The quality and risk of bias of the studies that were finally selected were assessed with the Risk of Bias Assessment Tool for Non-randomized Studies (RoBANS) criteria. RESULTS: Out of the 2,257 retrieved articles, a total of six articles were selected. These included two scheduling programs, two nursing cost-related programs, and two patient care management programs. For the outcome measurements, usefulness, time saving, satisfaction, cost, attitude, usability, data quality/completeness/accuracy, and personnel work patterns were included. User satisfaction, time saving, and usefulness mostly showed positive findings. CONCLUSIONS: The study results suggest that NMISs were effective in time saving and useful in nursing care. Because there was a lack of quality in the reviewed studies, well-designed research, such as randomized controlled trials, should be conducted to more objectively evaluate the effectiveness of NMISs.

A procedure descried by Fontan and Baudet in 1971 successfully bypassed the right ventricle in Tricuspid Atresia patients, after then many modification of the Fontan operation had been described and applied to many cyanotic complex heart patients. Forty patients with a variety of cardiac malformation underwent the Fontan operation at Seoul National University hospital (September 1978 to June 1986). The age at operation ranged 2 months to 18 years. Each number of cases according to basic cardiac anomaly was as follows; 17 in Tricuspid Atresia, 17 in Univertricular Heart, 2 in Double outlet of Right Ventricle, 2 in Transposition of the Great Arteries and 2 in Criss-cross heart. Total mortality rate after the Fontan operation was 50%. There was only one late death (>30 days). Mortality rate under 4 years of age (67%) was higher than that between 4 and 18 years of age (40%). we observed a significantly higher mortality for patients who, in the immediate postoperative period, had central venous pressure greater than 25cm H2O. 45% among survivals did not require further medication. Although mortality rate after the Fontan operation is much higher than that in the foreign literature, operative mortality will decline with the increased expirence of surgeon and the effective patients selection.

9.The Role of CD34 in the Immunophenotyping Panel of Leukemia.

BACKGROUND: CD34 is expressed by early hematopoietic (myeloid & lymphoid) progenitor cells, endothelial cells, and bone marrow stromal cells. Recent studies have suggested that the expression of the hematopoietic antigen CD34 is predictive of outcome. But the functional importance of CD34 antigen has not yet been fully elucidated, and CD34 antigen has not yet been included in the routine immunophenotyping panel. We designed this experiment to evaluate the significance of including CD34 antigen in the routine immunophenotyping panel. MATERIALS AND METHODS: We have included CD34 antigen in the immunophenotyping panel from 1994 to 1997. Seventy eight patients with acute leukemia and 7 patients with chronic leukemia and CML-blast crisis were analyzed by direct immunofluorescence using monoclonal antibodies. RESULTS: CD34 was positive in 64% (50/78) of cases with acute leukemia and 100% (3/3) of cases with CML-blast crisis, but negative in 100% (4/4) of cases with chronic lymphocytic leukemia. Seventeen (63%) of 27 patients that belonged to AML FAB subtype M0, M1, M2, M4, M5, M7 were CD34-positive, but all of the 9 patients (100%) that belonged to AML FAB M3 were CD34-negative. Thirty-one patients of CD34-positive non-T-ALL belonged to immunologic group II, III, or IV, and all 2 patients of CD34-positive T-ALL belonged to immunologic group I. There are no significant correlations of CD34 with the remission rate and the survival rate of acute leukemia. CONCLUSIONS: CD34 facilitates the rapid diagnosis of acute promyelocytic leukemia (APL) different from the other AML subtypes in clinical course and therapeutic plan. It is also helpful for the immunologic grouping of ALL, and differentiation of chronic leukemia from CML-blast crisis or acute leukemia. We concluded that it is desirable to include CD34 in the routine immunophenotyping panel.

10.Benign Cystic Mesothelioma.

Benign cystic mesothelioma (BCM) is a rare mesothelial lesion that forms multicystic masses in the upper abdomen, pelvis and retroperitoneum. Although it is categorized as a benign lesion, it has a tendency to recur. It is uncertain whether the nature of this lesion is reactive or neoplastic, but many articles support the conclusion that it is reactive rather than neoplastic. The majority of cases were associated with a history of a previous abdominal or pelvic operation, or an evidence of endometriosis or a pelvic inflammatory disease, or a combination of these findings. In a 26-year-old woman we experienced a case of BCM which was incidentally discovered at cesarean delivery revealing multilocular thin and translucent walled cysts in the pelvic cavity. Microscopic examination revealed a thin cyst wall that was composed of fibrous connective tissue and lined by internal stratified and external nonstratified single cuboidal epithelia.
