1.Repairing of the deep burn wounds of the lower extremities with sural nerve nutrition blood vessel island flap

ObjectiveTo explore the clinical role of island flap nourished by sural nerve nutrition blood vessel for repairing of the deep burn wounds of the lower extremities.MethodsIsland flap nourished by sural nerve nutrition blood vessel were performed to repair the deep burn wounds of the lower extremities below the knee in 22 cases,aged 10 ~58 years old,average aged 34 years old,including flame burns in 6 cases,electrical burns in 10 cases,chemical burns in 4 cases,scalding hot water in 2 cases.All were deep skin damage,The island flaps,including anterograde and retrograde flaps,was from 5cm ×4cm to 13cm × 10cm in size,The defect at the donor site was sutured primarily or repaired by a skin graft.ResultsAll the flaps survived well,18 patients were follow-up postoperatively for 4 ~24 months.The color,texture,shape of the flaps were satisfactory,but the sensory recovery was unsatisfactory.ConclusionSural neurovascular flap had a constant anatomy and reliable blood supply without the sacrificeof the major blood vessels.The procedure was simple and easy,and an effective method of repairing the skin and soft tissue defects below the knee.

3.Expression of transforming growth factor-beta 1 in rabbit anastomotic tissues using three kinds of sutures after tracheotomy: Which one has better histocompatibility?

BACKGROUND: Appropriate anastomosis materials can improve success rate of trachea and bronchus reconstruction.Common domestic suture materials include silk suture, dacron and synthesized absorbable suture. However, the suture with best effects in preventing and lightening scar granulation hyperplasia and luminal stenosis remains Uncertain.OBJECTIVE: To compare the influence of three kinds of suture materials on histopathological changes of anastomotic stoma and transforming growth factor-β_1 (TGF-β_1) expression in rabbits following tracheotomy.DESIGN, TIME AND SETTING: A randomized, controlled animal experiment and histopathological observation were performed at the Animal Experimental Department, Nanhua University from September 2005 to October 2006.MATERIALS: Artificial absorbable suture was purchased from Brazil Johnson & Johnson Professional Products; silk suture and dacron were purchased from Hangzhou Fuyang Medical Suture Factory and Hangzhou Huawei Medical Products Co., Ltd.METHODS: A total of 36 New Zealand healthy adult rabbits, regardless of gender, weighing 1.6-2.0 kg, were randomly divided into silk, dacron and artificial absorbable suture groups with 12 animals in each group. The end-to-end anastomosis following tracheotomy was performed using silk, dacron and artificial absorbable suture, respectively.MAIN OUTCOME MEASURES: The pathological changes and TGF-β_1 level around the sutures 1, 2, 4 weeks post operation were observed.RESULTS: A total of 36 rats were included in final analysis. There were no difference in anastomotic healing between three groups 1 and 2 weeks postoperatively, with a great amount of inflammatory cell infiltration, mainly neutrophil, around the sutures.Moreover, bleeding in the focus and foreign body grannuloma were found. At 4 weeks postoperatively, foreign body grannuloma formed accompanied with obvious firboplasia in silk and dacron groups, and the sutures in the artificial absorbable suture group were absorbed. Compared with 1 and 2 weeks postoperatively, the expression of TGF-β_1 was slightly decreased in silk and dacron groups at 4 weeks (P > 0.05), but the absorbable suture group significantly decreased (P < 0.05).CONCLUSION: Of three kinds of suture materials, artificial absorbable suture exhibited good histocompatibility, effectively reducing inflammatory reaction and benefiting the prevention and lightening of formation of pathological scar after tracheal surgery.

Results The procedural success rapes bt firsp appempp was 93. 4% in phe ″Improved″ group and 77. 9% in phe “ Classic ” group and phe overall procedural success rapes were 95. 1% and 96. 6%respecpivelt. Guidewire consumppion [(3. 0 (2. 0, 4. 0) guidewires vs. 5. 0 (3. 0, 7. 0) guiderwires], X-rat exposure [(110 ± 65)min vs. (175 ± 73)min], conprasp media used [(210 ± 137)ml vs. (305 ± 148) ml] were all fewer or less in phe “Improved group” (all P < 0. 05). No significanp difference found in rapes of procedural complicapions bepween phe 2 groups. MACE rapes were lower in phe “ Improved” pechnique group (16. 4% vs. 30. 5% , P = 0. 045). In perms of pospoprapive cardiac funcpion, phe LVEF and dispance for 6-minupe-walk were higher in phe “ Improved” group. Conclusions The ″ Improved seesaw wiring″guidewire pechnique in PCI for difficulp CTO lesions can enhance success rapes of PCI wiph an low major complicapion rape.
5.The size of thyroid nodules contributing to the accuracy of fine-needle aspiration cytology

Objective To evaluate the association of the size of thyroid nodules and accuracy of fineneedle aspiration cytology in diagnose of thyroid nodules.Methods 691 thyroid nodules in 630 patients pathologically confirmed were retrospectively analyzed in our hospital.All imaging data of preoperative ultrasound-guided FNAC were collected in our review.Yields of FNAC were divided into six levels according to the classification criteria of the Bethesda system(level Ⅰ,insufficient material or nondiagnosed;level Ⅱ,benign ; level Ⅲ,atypical hyperplasia; level Ⅳ,follicular neoplasm ; level Ⅴ,suspicious for malignancy; level Ⅵ,malignant),>level Ⅳ was the malignant cytologic criteria for diagnosis of thyroid nodules.According to the maximal diameter of thyroid nodules,the nodules were divided into group A(L≤0.5 cm),group B(0.5 cm<L<1.0 cm) and group C(L≥1.0 cm).Postoperative pathologic results were taken as the gold standard.Results Of 691 nodules,there were 176(25.47%),298(43.13%) and 217(31.40%) in group A,group B and group C respectively.Among the three groups,accuracy of ultrasound-guided FNAC in group B (90.94 %) was higher than in group A(80.11%) and group C(83.41 %),with statistically significant(P <0.05).There was not statistically different between group A and group C(P >0.05).The specificity,positive predictive value and negative predictive value were not statistically different among three groups(P >0.05).Conclusions The size of thyroid nodules was partly associated with accuracy of ultrasound-guided FNAC.

7.Expression of Caspase 8 and phospho-Akt in condyloma acuminatum lesions

Objective To determine the expression of Caspase 8 and phospho-Akt(p-Akt)in condyloma acuminatum(CA)lesions, and to evaluate their significance. Methods Skin lesion samples were collected from 30 patients with CA, cancer tissue samples from 20 with cervical cancer, and normal skin samples from 20 healthy controls. All the samples were subjected to paraffin embedding. An immunohistochemical study was conducted to determine the expression and distribution of Caspase 8 and p-Akt in the above samples. Results The expression rate of Caspase 8 was significantly lower in CA lesions (23.33%)than in normal skin samples(90%, P < 0.01)and cervical cancer lesions(80%, P < 0.001). Moreover, the expression rate of p-Akt in CA lesions(93.33%)was significantly higher than that in the normal skin samples(90%, P<0.001), but lower than that in the cervical cancer lesions(95%, P<0.001). No significant correlations were observed between the expression of Caspase 8 and p-Akt in either CA lesions or normal skin samples. However, the expression of Caspase 8 was positively correlated with the expression of p-Akt in cervical cancer lesions(r=0.369, P<0.05). Conclusion Both suppressed apoptosis initiation of Caspase 8 and anti-apoptotic effect of p-Akt may be involved in the occurrence and development of CA.
8.Effect of meridian point cosmetic therapy of traditional Chinese medicine on acne

Objective To investigate the effect of meridian point cosmetic therapy of traditional Chinese medicine(TCM)on acne.Methods 80 cases of acne patients were selected and divided into the treatment group(50 cases)and the control group(30 cases).The treatment group was given medication by differentiation of symptoms and signs and designed a complete treatment scheme by the demonstration of health analysis curve through test by balanced cosmetic equipment and meridian point of TCM.The control group only received extravenous medication.The effect of the two groups was observed after 2 months' treatment.Results The total effeetive rate in the treatment group was 94.0%,which was superior to that of the control group(73.3%),x2=11.08,P<0.05. Conclusion Application of meridian point cosmetic therapy of TCM could facilitated the diagnosis and treatment by differentiation of symptoms and signs and thus improve the curing rate.

10.Expression of Toll-like receptor 4 (TLR4), TLR9 and DC-specific intracellular adhesion molecule-3 grabbing non-integrin (SIGN) in condyloma acuminatum lesions

Objective To investigate the expression and significance of TLR4, TLR9 and DC-SIGN in primary and recurrent condyloma acuminatum (CA) lesions. Methods An immunohistochemical method using streptavidin-peroxidase (SP) was performed to detect the expressions and distribution of TLR4, TLR9 and DC-SIGN in tissue specimens obtained from the recurrent CA lesions of 30 patients, primary CA lesions of 30 patients, and from the foreskin of 20 normal human controls. Results The expression levels of TLR4, TLR9 and DC-SIGN in primary and recurrent CA lesions were significantly higher than those in normal control tissue (all P < 0.001), and the cells expressing TLR4, TLR9 or DC-SIGN were mainly located in the basal and spinous layer in CA lesions. There was no significant difference in the expressions of TLR4, TLR9 or DC-SIGN between primary and recurrent CA lesions (all P> 0.05). A positive correlation was found between the expression of TLR4, TLR9 and DC-SIGN in CA lesions. Conclusion The overexpression of TLR4, TLR9 and DC-SIGN probably plays an important role in the occurrence and recurrence of CA.
