2.Is 99mTc-MDP Mammoscintigraphy Useful in Patients with Breast Mass Lesions ?.

PURPOSE: Piccolo et al. advocated the usefulness of Tc-99m-MDP breast scan in differential diagnosis of breast mass with high accuracy. But there are little or no follow-up studies about it as we know. We studied Tc-99m-MDP uptake pattern and lesions/background ratio in patients complaining palpable breast lesions to evaluate the clinical usefulness of Tc-99m-MDP scan. MATERIALS AND METHODS: Total 34 patients were studied with physical examination, mammogram and Tc-99m-MDP scan prospectively. Anteroposterior and both lateral view of breast were obtained 5 minutes after iv injection of 740 MBq Tc-99m-MDP. Breast uptake pattern of Tc-99m-MDP was analyzed by a grade system: 0=no uptake, grade 1=bilateral diffuse uptake, grade 2=asymmetric faint uptake, grade 3=focal hot uptake. 20 cases were pathologically confirmed by excision biopsy or aspiration biopsy. 14 cases were normal in physical examination and mammo-grain. RESULTS: Pathologic results showed 7 carcinomas, 6 benign solid tumors, and 7 fibrocystic changes. Grade 3 pattern of Tc-99m-MDP uptake was noted in 4/7 carcinomas, 3/6 benign solid tumors, and 1/7 fibrocystic changes. Grade 2 pattern was 2/7, 0/7, 3/7 respectively. The average L/B ratio was 1.66 in carcinomas, 1.68 in benign solid masses, 1.20 in fibrocystic diseases, 1.05 in normal patients. L/B ratio was higher in carcinoma and benign mass groups than in fibrocystic change and normal control groups(p=0.005). But there was no statistical difference between L/B ratio of malignant mass group and benign mass group. CONCLUSION: Tc-99m-MDP scan is not suitable to routine clinical use for breast mass diagnosis. It might be used in limited conditions when whole body bone scan is planned.

4.Dose Effects of Epinephrine to Intrathecal Morphine for Postoperative Analgesia.

BACKGROUND: Epinephrine itself interacts with adrenergic receptors to block pain transmission pathways. Intrathecal co-administration of morphine and epinephrine results in a near maximal increase in the nociceptive threshold. This study was designed to investigate the influence of different dosages of epinephrine on the postoperative pain relief through intrathecal morphine in lower extremity surgery. METHOD: Thirty patients, ASA physical status I or II, undergoing orthopedic surgery of lower extremities were selected and randomly divided into two groups as follows: Group I (n=15); bupivacaine, morphine 0.2 mg with epinephrine 0.12 mg. Group II (n=15); bupivacaine, morphine 0.2 mg with epinephrine 0.2 mg. With patients lateral in the decubitus position, a 25-gauge spinal needle was introduced into the subarachnoid space. Patients were turned to the supine position, and sensory levels were checked by pin-prick tests at the midclavicle level , and the degree of motor block by Bromage scale every 5 minutes. The two segment regression time was measured every 20 minutes. During the postoperative day and the second day, their analgesic effects were evaluated by use of the visual analogous scale (0~10). The side effects were also evaluated. RESULTS: There were no difference between both groups in sensory level and two-segment regression time. Motor block in 5, 10, 15 minutes was more intense in group II. Also, there were no significant difference in the analgesic effects and the incidence of side effects, such as nausea or pruritus. CONCLUSION: There were no significant dose effects of epinephrine added to intrathecal morphine for postoperative pain relief undergoing lower extremities surgery.

6.Clinical Study of Total Hip Prosthesis Replacement: Report of 69 cases

The goals of hip arthroplasty have been to eliminate pain, decrease deformity, increase morbility and obtain stabiIity. Currently total hip replacement is the best avaliable procedure to accomplish these goals. A prospective study of total hip arthroplasty was begun at the orthopedic dept of Kyung Hee Medical College in 1974. Between May, 1974 and June, 1977. 69 total hip replacements of the Charnley type, the Trapesoidal-28 type and the Muller type were peformed by the authors. Of the 60 Patients involved in this study, 9 had bilateral operations. The Charnley type Prosthetic device was used in 56 cases, the Trapesoidal-28 type in 7 cases and the Muller type in 6 cases. Complications included wire breakage, dislocation, loosening, femur shaft perforation, nonunion of greater-trochanter, acetabular protrusion, transient femoral nerve palsy, femur shaft fracture, infection, socket lateralization, ectopic bone formation and death. In 59 patients of the 60 patients, 53 patients felt that they were definitely improved, 5 patients felt their condition was unchanged and 1 patient felt her condition was worse.

7.Characterization of microsatellite markers covering chromosome 1 in the Korean and Japanese populations.

Microsatellite markers are considered to be very promising genetic markers for genetic linkage analysis. The majority of the markers are as informative as in Caucasians but there are significant ethnic differences in the genetic variations. In order to investigate the genetic variations in the Korean and Japanese populations and their ethnic differences, 51 microsatellite marker loci spanning the whole human chromosome 1 were arranged from a commercially available set (ABI PRISM Linkage Mapping Set-HD5, Applied Biosystems, Foster City, CA, USA), and then determined the allelic frequencies and heterozygosities for these marker loci in the 96 unrelated Korean subjects and 96 unrelated Japanese subjects. Of all 51 markers tested, significant differences were observed when microsatellite allele frequency pattern of Korean was compared with those of Caucasian, while this pattern was highly similar between Korean and Japanese populations. Our data indicate that an extensive verification of public microsatellite markers in a particular population study should be undertaken prior to their linkage studies. Moreover, this information should facilitate genetic linkage studies of various hereditary diseases, especially in the Koreans and Japanese.

8.A photoelastic study on the stress distribution of the upper anterior teeth when retract with high pull J-hook headgear.

This study was designed to inveatigate the stress intensity and distribution produced by 1mm activation of retraction archwire with 0degrees, 7degrees, 14degrees torque and application of high pull J-hook headgear during retraction of four maxillary incisors using the photoelatic stress analysis. The photoelastic model was made with a PL-3 type epoxy resin which was substituted by alveolar bone portion. Each retraction archwire was fagricated from .020" x .025" stainless steel wire which had vertical loops in 7mm height and hook for high pull J-hook headgear between central and lateral incisors. The high pull J-hook headgear was applied 35 degree backward and upward to occlusal plane with 200mg per each side. The findings of study as follows : 1. In case of 0degrees torque, the stress was distributed from cervical 1/3 to apex of root of central and lateral incisors which were the froms of arc mode. When the high pull J-hook headgear was applied, the stress disrtrbuted by following by arc mode was presented from cervical 1/2 to apex of roots of central and lateral incisors. And the stress distributed by following the root surface was presented from alveolar crest to cervical 1/2 of central and lateral incisors. The stress between apecies of central and lateral incisors was presented also. 2. In case of 7degrees torque, the stress distributed by arc mode was presented from cervical 1/2 to apex of roots of central and lateral incisors. And the stress distributed by following the root surface was presented from alveolar crest to cervical 1/2 of roots of central and lateral incisors. When the hight pull J-hook headgear was applied, the stress distributed by following the root surface was presented more apically than without headgear. The stress between apecies of central and lateral incisors was presented also. 3. In case of 14degrees torque, the stress distributed by following the root surface was presented from alveolar crest to apex of roots of central and lateral incisors. When the high pull J-hook headgear was applied, the stress distributed by following the root surface was presented stronger than without headgear. The stress between apecies of central and lateral incisors was presented also.

9.A Novel Human Brainstem Map Based on True-Color Sectioned Images

Background:
Existing atlases for the human brainstem were generated from magnetic resonance images or traditional histologically stained slides, but both are insufficient for the identification of detailed brainstem structures at uniform intervals.
Methods:
A total of 319 sectioned images of the brainstem were selected from whole-body axial sectioned images, then coronal and sagittal sectioned images were reconstructed from the horizontal images. The fine and detailed structures were annotated in PowerPoint slides, then the volume model was produced and some white matter fibers were traced using MRIcroGL.
Results:
In this study, a novel brainstem atlas based on sectioned images was generated that shows the true color and shape, as well as the accurate location of the nuclei and tracts;it reveals the striking contrast between gray and white matter, as well as fine structures.In total, 212 structures, including nuclei and tracts, were annotated in axial, coronal, and sagittal plane views of sectioned images (48-bit true color; 0.2 mm intervals, 0.06 mm × 0.06 mm pixel size). To verify the accuracy of the annotations, a volume model of the brainstem was constructed for independent observations of the three planes.
Conclusion
In this paper, we describe several interesting structures included in the atlas. By depicting the fine structures of the human brainstem in detail, this atlas allows comprehensive understanding of the complicated topographies of the brainstem. As such, it will be of value for neuroanatomy education and research, in addition to enriching the literature on the human brain.
10.A Pilot Study on the Development and the Effect of a Cognitive-Behavioral Intervention for Undergraduates at Mental Health High Risk

Objectives:
: The purpose of this study was to develop a cognitive-behavioral intervention for college students at high risk for mental health and to investigate its effect on mental health outcomes.
Methods:
: The program was developed to reduce depressive symptoms and adverse outcomes while promoting resilience and positive effects. It consisted of eight sessions in a small group format for eight weeks. Thirty subjects at high risk for mental health participated in the program. Outcomes on the depressive symptoms, resilience, satisfaction with life, negative affect, and positive affect were evaluated at pre-and post-intervention.
Results:
: Depressive symptom scores reduced significantly post-intervention compared to pre-intervention (t=3.51, p=0.002). Resilience scores (t=-3.65, p=0.001) and satisfaction with life scores (t=-3.00, p=0.006) increased after intervention than pre-intervention. Positive affect scores decreased post-intervention (t=2.28, p=0.031). There was no significant difference between pre-and post-intervention negative affect scores (t=-0.94, p=0.356).
Conclusions
: Present study demonstrated that group-based cognitive-behavioral intervention could be helpful for college students at high risk for mental health to reduce mental health symptoms and improve healthy protective factors.
