Xanthoma disseminatum (XD) is one of cutaneous nonhistiocytosis X, and characterized by multiple, widely distributed red, brown, yellowish papules and nodules that tend to coalesce. 5.Comparision of Effects and Complications of Diprivan (ICI Pharm Co.) and Pofol (Dong - guk Pharm Co.).

BACKGROUND: Propofol is a rapidly acting alkylphenol compound. Pofol is another alkylphenol compound that was made by Dong-guk Pharm Co. in Korea. This study was performed to evaluate the efficacy and complications of pofol in comparison with diprivan. METHOD: Fourty three unpremedicated patients were randomly allocated to receive one of two i.v. anesthetic agents(pofol and diprivan). We assessed the induction dose, the time to loss of consciousness, time to opening eyes on command, changes of vital sign and various complications of the two drugs. RESULT: There were no differences in induction dose and maintenance dose between pofol group (P) and diprivan group (D). The induction times of P were shoter than those of D and the recovery times of P longer than those of D. Both groups produced a significant fall in mean arterial pressure(MAP) and heart rate(HR) but there were no differences in changes of vital sign between two groups. There was no difference in incidence of pain between D and P (83% Vs 80%). Even though the characters of adverse events to P and D, the incidence rate was not different between two groups. CONCLUSION: There is no differences in efficacy and complication of pofol in comparison with diprivan except differences in induction times and recovery times between two groups. So we can use the pofol as the i.v. anesthetics as diprivan and further evaluations in induction times and recovery times are needed.

6.Cutaneous Metastasis from Internal Malignancy.

We reviewed clinical records and histologic materials of 96 cases of metastatic skin cancer in Seoul National University Hospital from January 1973 to July 1986. The metastatic skin cancers of male were twice as many as that of female and they occurred most frequently in male at their fifties, female, forties. The most frequent primary cancers in men were carcinoma of the stomach, lung, colorectum and pancreas; in women, carcinoma of the breast, stomach, lung and uterine cervix. Cutaneous metastases from the carcinomas of the liver, uterine cervix and stomach which were many in Korea were smaller in number than that from the carcinomas of the lung and breast. Metastatic lesions were recognized before the primary tumor relatively often in carcinoma of the lung, pancreas and rarely in carcinoma of the uterine cervix, stomach. Though localizations of cutaneous metasta.sis were widespread over all body surfaces, regional areas of primary tumor were predisposed to metastasis. Metastatic carcinoma usually produced nonspecific nodules in the skin. Less commonly, some lesions of metastasis showed inflammatory or sclerotic plaque. The histologic findinga of cutaneous metastases generally mimicked that of primary tumors; nevertheless, in most instances, it was not possible to recognize the original tumor from a histologic examination of metastatic skin cancer and it was possible to classify the metastatic carcinoma only as an adenocarcinoma, a squamous cell carcinoma or an undifferntiated carcinoma.

7.Histopathologic Evaluation of Linear Lichen Planus and Lichen Striatus.

BACKGROUND: Linear lichen planus (LLP) and Lichen striatus (LS) are rare disorder that can be confused because they can share similar clinical and histopathologic features. OBJECTIVE: The purpose of this study was to evaluate the characteristic differences and common features between the two disorders histopathologically. METHODS: We reviewed the clinical records of patients who had been diagnosed as LLP or LS in our dermatology clinic during the 15-year period between 1985 and 1999. We classified twenty seven cases, which were differentiated from other possible linear dermatoses, into LLP and LS on the basis of clinical features, and then compared them histopathologically, and appreciated the characteristic differences or common features of the two disorders. RESULTS: In cases diagnosed as LLP clinically, epidermal changes were mainly composed of hyperkeratosis (78%), acanthosis (78%), basal degeneration (78%), granular layer thickening (67%) and saw-toothed appearance of rete ridges (44%). In dermis, colloid bodies (78%), band-like inflammatory cell infiltration with pigmentary incontinence (78%) were strik-,ting findings. In cases with clinical features of LS, parakeratosis (50%), dyskeratotic cells scattered in the epidermis (61%) and intercellular edema (39%) were noted in the epidermis. Dermal cellular deposits were focally band-like infiltration(89%), more frequently perivascular in-filtration (83%) and often involved deep dermis (50%), hair follicles (44%) and eccrine glands (22%) in contrast to LLP. CONCLUSION: This study presents a comparative histopathologic features of LLP and LS. Three cases of LLP with overlapping histopathologic features suggest the possibility that there may be an intermediate form between either end of a spectrum, LLP and LS.

9.A Case of Subcutaneous Juvenile Xanthogranuloma.

Juvenile xanthogranuloma is a congenital or perinatal tumor, 1 to 2 cm in diameter, usually located on the head. The extracutaneous lesions can occur on the eye, the lung, the epicardium, the oral cavity or the testicles. Subcutaneous form of juvenile xanthogranuloma has been reported very rarely in the literature. We report a unique case of a subcutaneous juvenile xanthogranuloma that showed 4 × 4 cm sized plaque and located on the extremity of 9-year-old girl.

10.Transvaginal Sonographic Evaluation of Uterine Wall Thickness on Prior Cesarean Scar.

OBJECTIVES: Estimation of the anterior lower uterine segment (LUS) thickness difference who underwent prior cesarean delivery measured with transvaginal sonography at or after 36 gestational weeks (sonographic thickness) and a ruler during elective cesarean section (operation thickness) Methods: One hundred sixty women who underwent prior cesarean delivery had the thickness of their LUS measured with transvaginal sonography at or after 36 gestational weeks. The LUS thickness was measured with a ruler during elective cesarean section. We compared group I whose LUS was fairly well visualized 4 cm or more from the uterine cervix to group II which had less than 4 cm. RESULTS: The mean sonographic thickness of LUS was 1.7 0.8 mm and that of operation thickness was 1.9 0.5 mm. The mean difference of the two (thickness difference) was 0.5 0.5 mm. In 31.3% the sonographic thickness was same as the operation thickness and in 70.7% of the total with 0.5 mm or less difference, the sonographic thickness could be regarded as accurate. The thickness difference with the sonographic thickness with 2 mm or more was smaller than those with 1 mm or less (0.4 0.5; 0.6 0.6) and that of group I was smaller than that of group II (0.4 0.4; 0.9 0.6) (p< .05). CONCLUSION: The thickness difference was 0.5 0.5 mm and it was smaller when the LUS thickness is 2 mm or over, clearly visible 4 cm or over from the cervix.
