1.A Case of Microscopic Polyangiitis with Acute Myocardial Infarction.

Microscopic polyangiitis is a systemic small-vessel vasculitis, which may involve multiple organs, but cardiac involvement is relatively rare. We report a case of microscopic polyangiitis with multiple organ involvement, in which myocardial infarction was the early manifestation of the disease. A 53-year-old man presented with sudden papulovesicular eruptions and swellings on the face, posterior neck, dorsa of both hands and fingers, and with diffuse erythematous patches on the back. He had suffered from renal dysfunction, arthralgia, and hypertension for more than 8 years. He had been admitted to the department of cardiology for acute myocardial infarction and had suffered from recurrent gastrointestinal bleeding, renal failure, acute pancreatitis and sepsis during the admission. Histopathologically, small-vessel leukocytoclastic vasculitis with out granuloma was seen. Direct immunofluorescence showed no immune deposite. A high serum level of P-ANCA was detected by ELISA.

2.The Effect of Clonidine Added to Bupivacaine on Intercostal Nerve Block for Postoperative Pain Control.

BACKGROUND: The addition of clonidine to local anesthetics for regional block has been shown to increase the duration of anesthesia and analgesia. This study was designed to determine whether the addition of clonidine to bupivacaine would produce an extension of the analgesic effect after intercostal nerve block (ICNB). METHODS: After informed consent, 30 ASA 1 or 2 patients undergoing appendectomy under general anesthesia were randomly divided into two groups. Before induction of anesthesia, ICNB using a posterior approach was performed with 15 ml of 0.25% bupivacaine plus epinephrine 1:200,000 with (Group BEC; n = 15) or without (Group BE; n = 15) clonidine 75 microgram. Analgesia was assessed by cold testing at 1/min intervals until cold sensation decreased. The duration of analgesia (time between injection and onset of pain) was recorded. We also recorded the visual analogue scale (VAS) of pain, the number of supplemental analgesics, heart rate and blood pressure, and side effects over 24 hours postoperatively. RESULTS: The onset time, duration of analgesia, number of analgesics, and heart rate and blood pressure were comparable in both groups. VAS scores were significantly lower in Group BEC than in Group BE at 12, 16, and 20 hours postoperatively. CONCLUSIONS: The addition of clonidine to bupivacaine with epinephrine may be a useful adjunct and can prolong the duration of analgesia after ICNB without significant side effects.

4.The Effects of a Pubovaginal Sling Procedure, with Martius Labial Fat Pad Interposition, after a Female Urethral Diverticulectomy.

PURPOSE: We evaluated the effects of a pubovaginal sling (PVS), with a Martius labial fat pad interposition (MLFI), on patients with a female urethral diverticulum. MATERIALS AND METHODS: 23 consecutive cases with a female urethral diverticulum were retrospectively reviewed. The pre-operative evaluations included the history, physical examination, voiding cystourethrogram, cystoscopy, transperineal ultrasound and urodynamic studies to investigate the urethral function and the concomitant stress urinary incontinence. The treatment consisted of a transvaginal diverticulectomy alone, or with a PVS only, or combined with a MLFI. RESULTS: A transvaginal diverticulectomy only was performed, using a 3-layer closure, in nine patients (39.1%) and a concomitant PVS with a diverticulectomy in eight (34.8%) with stress urinary incontinence. A MLFI, combined with the above procedure, was performed in six patients (26.1%) with large defects of the urethra following the resection of diverticulum. No women having undergone a diverticulectomy with, or without, a PVS and MLFI had an urethrovaginal fistula, recurrent diverticulum or postoperative incontinence. CONCLUSIONS: In complicated cases, a simultaneous transvaginal diverticulectomy, pubovaginal sling and Martius labial fat pad interposition can be performed safely and effectively.

Recently, computed tomography (CT) guided nerve block has improved results of the block and made it safer, particularly when neurolytic drugs are injected for the relief of intractable abdominal pain caused by malignancy or chronic pancreatitis. We report a case of CT-guided alcohol block of the celiac plexus with the anterior approach in a patient with a common bile duct carcinoma.

8.Knowledge of orthodontics and malocclusion.

Orthodontists want why the patients want orthodontic treatment. Demand and need for orthodontics is very important factors of cooperation and management. Chief complaint would be different according to the race and culture. Our results show that in the control group, Korean female adolescents[chief complaint is cowding(30.6%), spacing(26.5%), upper anterior protrusion(18.4%), anterior cross bite(8.2%) in order. Korean female adolescents considered anterior cross bite is more nonacceptable than spacing and anterior protrusion. The purpose of orthodontics seemed to enhance esthetic, dental health, and to improve oral function.

9.Congenital Pseundarthrosis of the Tibia: 3 Cases Report

Three cases of congenital pseudarthrosis of the tibia were treated at Department of Orthopedic Surgery, Seoul national University Hospital during last four years. In all cases, many Cafe-au-lait spots and subcutaneous nodules were shown on the whoIe body skin, which are suggestive of neuro-fibromatosis. Case 1, A thirteen years old boy was treated with plate and screw fixation with massive iliac cancellous and cortical graft. In a follow-up of three and one half years, solid bony union has been obtained with successful result. Case 2, A 5 years old boy was treated with Boyds dual cortical graft with plate and screw fixation. In a follow-up to eight months, bony union were satisfactory. Case 3, A six months old boy who is young brother of case 2, was treated with Charnleys intramedullary nailing and iliac cancellous and cortical graft. The patient is still in the Iong leg cast, six months postoperatively. The evidence of familial and hereditary factor in pedigree is elicited on case 2 and 3. This congenital pseudarthrosis of the tibia in the sibling is regared as the first report in Korean literature.
