A particular problem associated with osseointegrated implants is the fact that the implants lack a periodontal ligament. Thereby, marginal inflammation around an implant may cause more serious bone loss than does marginal inflammation around teeth with a periodontal ligament. In addition, osseointegrated implants are ankylosed and do not haute the same mobility as natural teeth with a periodontal ligament. Implants with a periodontal ligament would eliminate these problems. In order to explore the possibility of producing a periodontal ligament around titanium dental implants, a study of the attachment of cultured periodontal ligament cells to titanium was carried out. Periodontal ligament cells obtained from premolar teeth of individuals undergoing tooth extraction for orthodontic reasons were cultured on titanium-coated epon blocks. Sections of the blocks were cut perpendicular to the surface of the cell layer. Transmission electron microscopy of the periodntal ligament cells/titanium interface showed that there was no evidence of attachment at the cultured periodontal ligament cells titanium interface. The microfilaments, commonly located adjacent to the titanium surface, run mostly parallel to the titanium surface. The study showed that cultured periodontal ligament cells did not create an attachment structure on a titanium surface similar to that of natural teeth.

BACKGROUND AND OBJECTIVES: One of problems for the management of horizontal semicircular canal benign paroxysmal positional vertigo (HSC-BPPV) is the difficulty of determining the affected ear using Ewald's second law. The purpose of this study is to develop the new "Bow and Lean Test (BLT)" to determine easily the affected ear of HSC-BPPV and evaluate its efficiency. MATERIALS AND METHOD: We compared the efficiency between the classical method and BLT in 26 patients with HSC-BPPV. The classical method is based on Ewald??s second law comparing the intensity of nystagmus or symptoms in head roll test. BLT is based on the direction of both "bowing nystagmus" and "leaning nystagmus" at head's bowing and leaning state on sitting position. The affected ear is the same direction of bowing nystagmus in canalolithiasis and the same direction of leaning nystagmus in cupulolithiasis. RESULTS: In 26 patents (15 canalolithiasis, 11 cupulolithiasis), 3 (11.5%) patients did not show a prominent affected ear in the classical method, and 7 (26.9%) patients showed the different affected ear between two methods. All 10 patients were successfully treated with just one trial of barbecue rotation based on the affected ear in BLT. Three patients did not show any bowing or leaning nystagmus. The side with canal paresis in all 4 patients, who showed significant canal paresis in bithermal caloric tests, was equal to the affected ear based on BLT. CONCLUSION: "Bow and Lean Test" (also called "Choung's test") is a new method which can easily determine the affected ear of HC-BPPV.

BACKGROUND: Rotavirus activity in Korea has been reported beginning in October, peak in November, continuing in winter and ending in spring. But the peak month and the incidence of rotavirus seems to be changed recently. So we investigated the trends of rotavirus activity for the last 10 years in Hanyang University Hospital (HUH). Also latex agglutination test was compared with automated enzyme-linked fluorescent immunoassay for the detection of rotavirus in stool specimens. METHODS: Stool specimens (3,636 from HUH, 1989-1998; 1,171 from Hanyang University Kuri hospital, HUKH, 1996-1998) from pediatric patients with acute diarrhea were tested for rotavirus,. Sixty specimens were tested by latex agglutination test (Sliders Rota-kit 2, bioMerieux Vitek, France) and enzyme-linked fluorescent immunoassay (VIDAS Rotavirus, bioMerieux Vitek, France) according to the instructions from the manufacturer. RESULTS: The annual incidence of rotavirus diarrhea from 1989 to 1998 was 47%, 32%, 33%, 25%, 26%, 24%, 24%, 17%, 17%, 14%, respectively. Positive rate of rotavirus was 25% for the 10year period in HUH, 20% for the recent 3 years in HUKH. Peak month was November (46%) in the first 5 year, but November incidence decreased to 17% in the last 5 year, and the peak moved to January, February, and March (34%, 35%, 33%, respectively). Epidemic period was from October to February during the first 5 year, but from December to April during the last 5 year period. The agreement rate of the two methods was 90% and VIDAS Rotavirus showed significantly higher sensitivity compared to Sliders Rota-kit 2. CONCLUSIONS: The incidence of rotavirus diarrhea decreased gradually for the last 10 years and the peak month of rotavirus activity was changed from November to January, February, and March. The VIDAS Rotavirus was more sensitive than the Sliders Rota-kit 2 for the detection of rotavirus in stool.

The objective of this study was to investigate the association of nutrient intake and pregnancy outcome mediated by weight gain during pregnancy, job status, and household income. Maternal age, educational level, self-reported pre-pregnancy weights, educational level, and household income were collected from the women at 2 months postpartum. For each offspring, weight at birth, length at birth, and gestational age were collected. Participants were asked to report the frequency of consumption of foods between 28–42 weeks into the pregnancy. Diet was assessed by using a validated 106-item semi-quantitative food-frequency questionnaire (SQFFQ) and women were asked portions and quantities based on pictures, food models, and measuring tools such as cups or teaspoons. Results showed that women who gained below the recommended weight gain during pregnancy, within, and over were 25.3%, 38.7%, 36.0%, respectively. In comparison to weight gain and the offspring's length and weight at birth, the offspring of mothers with a lower weight gain had a higher length. Energy, protein, vitamin B2, vitamin C, calcium, and potassium were significantly lower at employed group. We did not observe a significant difference between birth characteristics and maternal nutrient intake by income. Infants with a higher ponderal index at birth were born to women with a higher pre-pregnancy body mass index (BMI).

Pigmented villonodular synovitis (PVNS) is a relatively rare condition. The disorder almost always involves a single joint and the knee is most commonly affected. We report on three cases of localized PVNS that involved the patella fat pad and synovium. Diagnostic and therapeutic arthroscopies were performed, and typical findings of localized PVNS were found. Complete resection of the lesions were performed arthroscopically. Arthroscopy can be used as an effective diagnostic and therapeutic tool for identification and resection of intraarticular localized PVNS of the knee.

Purpose: The aim of this study was threefold, to longitudinally examine the risk of habitual smoking onset in adolescents, to delineate the effects of neighborhood characteristics and friends’ smoking status on the habitual smoking onset, and to investigate whether the association between friends’ smoking status and habitual smoking onset was moderated by neighborhood characteristics. Methods: This study conducted multilevel discrete-time survival analysis, using cohort data from the 3rd to 6th waves of the Korean Child and Youth Panel Survey, which excluded habitual smokers, matched with 2010 census data on respondents’ residence. Results: Habitual smoking onset risk increased from the 8th to the 11th grade, and then slightly decreased from the 11th to the 12th grade. Friends’ smoking status (B = 0.60, p < .001), smoking rate (B = 0.06, p = .038), and the number of tobacco outlets in the respondents’ neighborhood (B = 0.51, p = .003) were positively associated with habitual smoking onset risk. Furthermore, the association between friends’ smoking status and habitual smoking onset risk was moderated by the number of tobacco outlets in the neighborhood. Specifically, the association was stronger in neighborhoods with more tobacco outlets (B = 0.58, p = .048). Conclusion Friends’ smoking status and living in neighborhoods that are more susceptible to smoking increase the risk of habitual smoking. The number of tobacco outlets in the neighborhood enhances the peer effect of adolescent’s smoking behavior. Therefore, policies or interventions designed to reduce youth’s tobacco use should focus on not only on reducing peer smoking, but also restricting smoking by adults and the number of neighborhood tobacco outlets.

Nephrolithiasis is one of the most common disease seen in the Emergency Department. Among the patients who visited in the Emergency Department with acute abdominal pain, many people were suspected urolithiasis. Because of limitation of diagnostic method on Emergency Department we planned this study in order to confirm the our accuracy of diagnosis and the possibility of misdiagnosis. During 5 months from April 1996 to August 1996, the 189 patients was suspected the urolithiasis. Among this patient, 51 cases was excluded due to incomplete study. After performing P/ex, U/A KUB in the Emergency Department, they were followed out patients department of Urology and performed Intravenous Pyelogram. The sensitivity and specificity for KUB were 84.5% and 94.0%, respectively. The positive predictive value was 93.8%, the negative predictive value was 93.8%. Diagnostic accuracy of urolithiasis in our Emergency Department is 89.1%. There was no relationship between urinary occult blood and KUB or IVP. There was no relationship between degree of occult blood and diagnosis of urolithiasis. For differential diagnosis of urolithisis, emergency ultrasound and IVP will be available. So combination of this diagnostic methods will improve accuracy and safety of diagnosis and treatment of urolithiasis.
