1.Effects of Dietary Supplement Containing Kurozu Concentrate on Blood Pressure, QOL and TCM Constitution

Objectives: The present study was examined the effects of dietary supplement containing Kurozu concentrate on Blood Pressure, QOL and Traditional Chinese Medicine (TCM) Constitution. Methods: Forty subjects with slightly high blood pressure were assigned randomly to 2 groups (active group and placebo group). Kurozu-containing foods or the placebo was administered to the subjects daily for 12 weeks. At week 0, 6 and 12, blood samples were collected, physical measurements were taken, and the quality of life (SF-36v2) and TCM constitution were examined. Results: Significant differences on blood pressure and TCM constitution between the groups were observed at week 12. The change of QOL between the groups was observed significantly at week 6. Conclusion: These findings suggest that foods containing Kurozu have hypotensive action and improve effects on constitutional problems. In addition, it was confirmed that foods containing Kurozu are safe to ingest.
2.Changes in the Expression Levels of Stress Proteins in Rat Skeletal Muscles Due to Heat Stress Exposure.

The expression levels of heat shock proteins after heat stress on rat slow soleus and fast plantaris muscles were examined and compared during a recovery period following 1 h of heat stress. The left hindlimbs of adult male Wistar rats (n=15) were carefully inserted into a stainless steel can and subjected to heat stress for 1 h by raising the air temperature inside the steel can to 54-58t with a flexible heater so as to bring the muscle temperature up to 42°C. The muscles of the contralateral right hindlimb served as the control. The expression levels of HSP 60, HSP 72, and HSC 73 were analyzed by Western blotting after 0, 2, and 4 h of recovery following 1 h of heat stress. In the soleus muscle, all of the HSP levels analyzed were significantly increased during 0-4 h of recovery. On the other hand, heat stress had no effect on the expression levels of HSPs, except HSP 60, in the plantaris muscle during recovery after 1 h of heat stress. These results suggest that the slow soleus muscle has a higher ablility to respond quickly to heat stress than the fast plantaris muscle.
3.The Open Clinical Tests for the Safety of Kurozu-containing Food

Two different types of clinical safety test of Kurozu-containing food were done. Under the informed consent, following tests were conducted, Test 1; Ten healthy adults were orally administrated 10 capslues/day of Kurozu for 4 weeks, Test 2; Ten patients with hepatitis C and 10 patients with mild hepatic dysfunction were orally administrated 2 capslues/day of Krozu for 4 weeks. In both the Test1 and Test2, Vital sign including Blood pressure etc, blood cell count, blood chemical analysis, and urinalysis were performed. In both tests, there were no abnormal findings. Furthermore, no side effect was observed. In conclusion, safety of Kurozu-containing food was confirmed.
4.The Excessive Consumption Test for the Safety of Black Sesame Extract

The excessive consumption test for the safety of black sesame extract (BSE) was conducted. Eleven healthy adults (male 6,female 5) were orally administered 5 tablets/day of BSE for 4 weeks. There were no abnormal changes in blood and urinary analysis. In addition, no serious adverse effects were observed. These results indicate that the excessive dose of BSE is safe for healthy adults.
5.Association of Continuous Vertebral Bone Bridges and Bone Mineral Density with the Fracture Risk in Patients with Diffuse Idiopathic Skeletal Hyperostosis

Methods:
We examined the computed tomography scans from the thoracic vertebra to the sacrum used to diagnose DISH in 140 patients (98 men and 42 women; average age, 78.6 years). We compared patients who did (n=52) and did not have (n=88) fractures at the continuous vertebral bodies fused by bone bridges. The relationship between the vertebral fractures and the maximum number of vertebrae that are bony cross-linked with contiguous adjacent vertebrae (max VB) from the thoracic vertebra to the sacrum or from the lumbar vertebra to the sacrum and proximal femur BMD were analyzed using a logistic regression model.
Results:
We found that after adjusting for the confounding factors, higher max VB, both from the thoracic vertebrae to the sacrum and the lumbar vertebrae to the sacrum, was associated with a higher risk of vertebral fractures. This difference was statistically significant. The risk was higher when only the lumbar vertebrae to the sacrum was considered (thoracic vertebrae to the sacrum: odds ratio, 1.21; p<0.05; lumbar vertebrae to the sacrum: odds ratio, 2.78; p<0.01). Moreover, low proximal femur BMD in DISH patients raises the fracture risk (odds ratio, 0.47; p<0.01).
Conclusions
Many continuous vertebral bone bridges, especially those that extend to the lumbar spine and low proximal femur BMD, are risk factors for fracture in DISH patients.

Secretory immunoglobulin A (s-IgA) in saliva constitutes the first-line barrier to the entry of pathogens into the body, implying its critical role in mucosal immunity.To examine the effect of a shark liver oil (SLO)-containing diet on salivary s-IgA concentration in healthy male and female adults, 42 subjects were assigned to either placebo or 6 weeks of a 2,400 mg SLO-containing diet (1,500 mg as SLO) and assessed in a randomized, double-blind, placebo-controlled, parallel group trial.Salivary s-IgA concentration significantly increased at week 6 in the SLO group (P = 0.033), but not in the placebo group.Moreover, there was a significant difference between groups in the magnitude of change from baseline to week 6.No intervention-related adverse event or abnormal changes of laboratory test parameters were observed throughout the study period.In conclusion, an SLO-containing diet increases salivary s-IgA in healthy adults.
7.Development of a Format to Record Clinical Experiences of Trainees during the Initial Two-year Period of Postgraduate Clinical Training; A Study of Self-evaluation by Jichi Medical School Graduates.

A self-reporting questionnaire was designed and sent to our Jichi Medical School graduates each year since 1980, in order to investigate clinical competence. The format was designed based on several major reports concerning postgraduate clinical training in primary medical care. We found that more than 70% of Jichi Medical School graduates trained in the multi-specialty rotation style in general hospitals that were certified by the Ministry of Health and Welfare. Our results also suggested that these graduates obtained a relatively high level of clinical competence in performing physical examinations, basic laboratory testing, clinical procedures, and various treatments.

A self-reporting questionnaire was designed and sent to our Jichi Medical School graduates each year since 1980, in order to investigate clinical competences attained in certain technical items. The responses from graduates of classes 1984 through 1987 were analysed, and the questionnaire was found to be highly reliable and moderately valid. The coefficient variable was 0.942, and the relevant variable was 0.934 using the half-split method. The content was clear, because items in the questionnaire were selected from published official reports. In the construction validity, 4 meaningful groups of items were found by factor-analysis. Yearly analysis using such a questionnaire may clarify some of the problems experienced by trainees and training hospitals, and would encourage the trainees to undergo a process of selfevaluation.
9.Expression of Hyaluronidase-4 in a Rat Spinal Cord Hemisection Model.

STUDY DESIGN: Examination of hyaluronidase-4 (Hyal-4) expression in a rat spinal cord hemisection model. PURPOSE: To determine the status of Hyal-4 expression after hemisection of the spinal cord, and the relationship between its expression and that of chondroitin sulfate proteoglycans (CSPGs). OVERVIEW OF LITERATURE: CSPGs are expressed at the site of spinal cord injury and inhibit axon regeneration. Administration of exogenous chrondroitinase ABC (ChABC), derived from bacteria, digested CSPGs and promoted axonal regrowth. Using a rat hemisection model, we have demonstrated peak CSPGs levels at by 3 weeks after injury but then decreased spontaneously. Could there be an endogenous enzyme similar to ChABC in the spinal cord? It has been suggested that Hyal-4 is involved in CSPG degradation. METHODS: A rat hemisection model was prepared and spinal cord frozen sections were prepared at 4 days and 1, 2, 3, 4, 5, and 6 weeks post-cordotomy and stained for CSPGs and Hyal-4 and subjected to Western blotting. RESULTS: CSPGs appeared at the injury site at 4 days after hemisection, reached a peak after 3 weeks, and then decreased. Hyal-4 was observed around the injury site from 4 days after cordotomy and increased until after 5-6 weeks. Double staining showed Hyal-4 around CSPGs. Western blotting identified a band corresponding to Hyal-4 from 4 days after hemisection. CONCLUSIONS: Hyal-4 was expressed in a rat hemisection model in areas surrounding CSPGs, and as its peak was delayed compared with that of CSPGs. These results suggest the involvement of Hyal-4 in the digestion of CSPGs.

10.Effects of Dietary Supplement Containing Kurozu Concentrate on Visceral Fat Accumulation

Objective: The present study was done to examine the effect of dietary supplement containing Kurozu concentrate on human visceral fat accumulation. Material and Methods: Sixteen subjects(BMI≧25) were orally administrated Kurozu concentrate for 8 weeks(low dose group: 500 mg/day, high dose group 1000 mg/day). Before and at weeks 8, hematological analysis, urinalysis and CT-scan to estimate the visceral fat accumulation were conducted. Results: In high dose group, following results were obtained; visceral fat decreased significantly(p=0.033), HDL-cholesterol level elevated significantly (p=0.034) and systolic blood pressure decreased significantly(p=0.004). In addition, no serious symptoms or abnormal hematological values were observed in both group.Conclusions: The dietary supplement containing Kurozu concentrate may be useful to prevent a metabolic syndrome.
