1.Parosteal Ossifying Lipoma of Femur: A Case Report.

Parosteal lipoma is a rare and generally asymptomatic benign tumor of mature adipose tissue that is located in direct apposition to the external surface of the bone. These tumors are occasionally associated with reactive changes in the underlying bone. The reactive bone formation is generally restricted to the base of the tumor, that is, near the periosteum. We recently experienced a case of parosteal lipoma in the proximal femur, which displayed exuberant bone and cartilage formation, and this led us to a misdiagnosis of chondroblastic osteosarcoma on the initial biopsy. We report here on this case with a special emphasis on making the differential diagnosis from osteosarcoma.

2.Reconstruction of the Orbital Wall with the Galeal- frontalis-pericranial-calvarial Bone Flap.

The orbital wall resection with removal of tumor in patient who has malignant tumor in paranasal sinuses or nasal cavity causes enophthalmos, dystopia, or severe facial deformities. The methods of reconstruction of orbital wall have been used skin graft, muscular sling, parietotemporal fascial calvarial bone flap, and free flaps. The parietotemporal fascial calvarial bone flap has been used as a pedicle flap, but it can not be used in reconstruction of medial wall of the orbit because it has a short vascular pedicle. Therefore we designed the galeal-frontalis- pericranial-calvarial bone flap which is a new pedicled bone flap for reconstruction of orbital wall. We used this flap in 2 patients who have orbital wall defect following resection of malignant tumor arising from the nasal cavity or maxilla. The outcome was gratifying in all two patients. We think that this flap is a new useful method in reconstruction of the orbit.

3.Neurilemmoma of the Masseteric Nerve in the Masseter Muscle: A Case Report.

A case is reported of neurilemmoma of the right masseteric nerve in the masseter muscle. We experienced a 34-years-old male patient who had neurilemmoma in the masseter muscle caused by chronic irritation. Special stain using S-100 immunoperoxidase was useful in establishing the diagnosis in this case. The most important aspect of this case is the anatomical site of the neurilemmoma; this tumor was originated from the masseteric nerve and is the first case reported in the masseter muscle. The patient underwent surgical excision of the tumor and no recurrence occured.

4.Cryopreservation of Testicular Spermatozoa using Mouse Zona Pellucida in Intracytoplasmic Sperm Injection Program.

The survival rate and motility recovered after cryopreservation of testicular spermatozoa in testicular sperm extraction (TESE)-ICSl program is low. The purpose of this study was to assess the availability and efficiency of mouse empty zona pellucida in cryopreserving human TESE spermatozoa. Mouse empty zonae pellucidae were obtained by extraction of cytoplasm with or without cytochalasin B treatment. Motile sperm from proven-fertile donor and two azoospermic patients after TESE were individually inserted into empty zona pellucida and cryopreserved. Two to five days after cyropreservation, the frozen sperm were thawed and the rates of recovery and motility were observed. The ooplasmic extraction rates of control (N=80) and cytochalasin B treated oocytes (N=80) were 94.0% and 96.2%, respectively (p>0.05). The post-thaw recovery rates of spermatozoa and rates of motility recovery of ejaculate (N=70) and testicular (N=70) sperm were 97.1%, 97.1% and 95.7%, 94.3%, respectively (p>0.05). The results of this study showed that the mouse zone pellucida is useful for cryostorage of single testicular spermatozoa.

5.Can Periodic Health Examinations Modify Risk Factors of Coronary Heart Disease?.

BACKGROUND: The management of risk factors of coronary heart disease is highly needed because the prevalence of ischemic heart disease is increasing in Korea. This study was aimed to evaluate the effects of periodic health examination on risk factors of coronary heart disease using Framingham Point Scores. METHODS: The medical records of 343 clients who underwent periodic health examination five times from January 1996 to December 2001 at one medical center in Seoul were reviewed. The five risk factor scores (age, total cholesterol, smoking, HDL-cholesterol, systolic blood pressure), total risk scores, absolute risk and the proportion of the high risk group and the obesity group were analyzed for all the periodic health examinations. RESULTS: For the five periodic health examinations, age risk factor score increased in men and women. Scores of risk factors such as smoking, total cholesterol, HDL-cholesterol and systolic blood pressure decreased in men only (P<0.0001). The total risk score (P<0.0001) and the absolute risk (P=0.0002) increased in men. The total risk score (P<0.0001) and the absolute risk (P=0.3238) increased in women with no changes of other risk factors except increased age score and decreased HDL-cholesterol score. The proportion of the high risk group (P=0.5406) and the obesity group (P=0.0762) were very small and not changed. CONCLUSION: The risk of coronary heart disease as not reduced with the five periodic health examinations. Therefore, the total risk score and the absolute risk did not decrease. Continuous risk factors management must be emphasized after periodic health examination for absolute risk reduction.

6.Motor Innervation and Nerve Pathway of Glabellar Expression Muscles.

We studied the motor innervation and the nerve pathway of glabellar expression muscles and a kind of glabellar skin wrinkles. Electric stimulation and surface tracing of the temporal and zygomatic branches of the facial nerve was performed using an electric nerve stimulator on both sides of 14 healthy volunteers. Micro- dissection of four cadaveric heads were also performed to confirm the results of nerve stimulation test. The corrugator supercilii muscle is innervated by the temporal branch of the facial nerve. The depressor supercilii muscle, the medial head of the orbicularis oculi muscle, and the procerus muscle are innervated by the zygomatic branch. The temporal branch to the corrugator supercilii muscles travels in a curvilinear pattern superior to the upper margin of the eyebrow. The zygomatic branch to the glabellar expression muscles travels in a curvilinear pattern inferior to the inferior orbital rim in the proximal portion and in a cobwebby pattern medial to the medial orbital rim in the distal portion. In cadaveric study, the zygomatic nerve branches off in the inferomedial orbital portion, and its fine branches enter into the procerus muscle, the depressor supercilii muscle, and the orbicularis oculi muscle. The surface pathway of the motor nerve to the glabellar expression muscles is described. We think that the zygomatic and temporal branches both should be blocked simultaneously to improve glabellar skin wrinkles completely.

7.A Study Evaluating the Effect of Telephone Counselling on Smoking Performed by a Nurse Cessation: A Preliminary, Randomized, Controlled Trial.

BACKGROUND: Smoking is well known as an important preventable risk factor contributing to mortality and morbidity. Telephone counseling might be one of adjunctive interventions for smoking cessation. We evaluated whether telephone counseling performed by a nurse could improve smoking cessation rate. METHODS: Study subjects were 152 male smokers who visited family practice in one tertiary hospital between November 2001 and January 2002 and agreed to participate in this study. After completing a self- administered questionnaire, the subjects received self-help materials and were randomly assigned into either an intervention or control group. A well-trained nurse provided telephone counseling only to experimental group at 8th and 17th week of follow-up. The 25-week smoking cessation rates were checked in both groups by telephone call. RESULTS: There were no significant differences in socio- demographic and smoking-related characteristics between intervention, control, and non-participants groups. Intention-to-treat smoking cessation rate at 25th-week of follow- up was 21.1% in control, 25.0% in intervention group and was not statistically different from each other. Smoking cessation rate according to the baseline stage of change toward quit smoking was not statistically different from each other. CONCLUSION: The telephone counseling performed by a nurse was not significantly effective for improving smoking cessation rate in this study.

8.Effect of Thymol and Linalool Fumigation on Postharvest Diseases of Table Grapes.

Several postharvest diseases of table grapes (Vitis vinifera) occur during storage, and gray mold rot is a particularly severe disease because the causal agent, Botrytis cinerea, grows at temperatures as low as 0degrees C. Other postharvest diseases, such as those caused by Penicillium spp. and Aspergillus spp., also often lead to deterioration in the quality of table grapes after harvest. The use of plant essential oils such as thymol and linalool, to reduce postharvest diseases in several kinds of fruits, including table grapes and oranges, has received much attention in European countries. However, to the best of our knowledge there has been no report of the use of thymol fumigation to control gray mold in table grapes in Korea. Thymol (30 microg/mL) and linalool (120 microg/mL) significantly inhibited mycelial growth and conidia germination of B. cinerea. The occurrence rate of gray mold rot of B. cinerea and other unknown fungi was significantly reduced by fumigation with 30 microg/mL thymol in several table grape cultivars, such as Campbell early, Muscat Bailey A, Sheridan, and Geobong. In this study, fumigation with 30 microg/mL thymol, had no influence on the sugar content and hardness of grapes, but reduced fungal infection significantly. This suggests that 30 microg/mL thymol could be utilized to reduce deterioration of grapes due to gray mold and other fungal infections during long-term storage.

9.The Treatment of Pressure Sore Using Hydrogel and Hydrocolloid Dressing.

Pressure sores are a common complication of hospitalized patients. However, It is often impossible to correct surgically because the general conditions of these patients are poor. It is known that the hydrogel has a powerful autolytic effect by providing moist environments and facilitates wound healing and hydrocolloid dressing is also known to promotes granulation tissue formation and epithelialization. The patients were treated with hydrogel(Purion gel(R), Coloplast A/S, Denmark) and hydrocolloid dressing (Comfeel Plus Transparent Dressing(R), Coloplast A/S, Denmark) after surgical debridement of pressure sores progressed to stage III(n=2) and IV(n=7). This combination treatment could facilitate to debride the nectrotic tissue and promote granulation tissue formation epithelialization simultaneously. We could achieve complete healing of pressure sores using the combination treatment without requiring surgical correction. In conclusion, hydrogel in combination with hydrocolloid dressing is effective in acheiving complete healing of progressed pressure sores.

10.Early Experience of Iron-Intern Usage during Modified Radical Neck Dissection.

PURPOSE: Modified radical neck dissections (MRND) in papillary thyroid cancer surgery usually leave a long incision in the neck, causing cosmetic problems. We introduce a new surgical approach to MRND, with minimal lateral extension of transverse collar incisions, using the Iron-Intern. METHODS: Between Jan 2008 and Dec 2008, 135 patients with papillary thyroid cancer who had undergone MRND were enrolled in this study. Total thyroidectomy and central node dissection with Kocker transverse collar incision with minimal additional lateral extension were performed before MRND. MRND were performed including level II, III, IV and V. Among 135 patients, we performed MRND with the Iron-Intern in 70 patients (Group I) and 65 patients who had MRND performed without the Iron-Intern (Group II) were included as a control group. In Group I, the Iron-Intern, with some modifications, was applied during the dissection of the level II area. The postoperative outcomes of the patients were analyzed and compared retrospectively. RESULTS: The median hospital stay was significantly lower in Group I compared to Group II (P=0.003). The median operation time was significantly higher in Group I than Group II (P=0.002). The median numbers of harvested central and lateral lymph nodes were 10, 29 (group I) and 8, 23 (group II), respectively (P=0.073, P=0.148). Median levothyroxine off thyroglobulin levels were 1.2 and 1.8 in Group I and II, respectively (P=0.110). CONCLUSION: MRND, with minimal lateral extension using the Iron-Intern, can be easily and safely performed, leaving only minor scars in the neck.
