Free Tissue Flaps*

A 44-year-old man was cutting an outer plate of a ship, at a depth of 25 m below sea level. Following a sudden explosion, he was discovered unconscious and was carried to the surface by other divers. There was no evidence of vital signs upon arrival at the hospital. Postmortem computed tomography, which was performed prior to autopsy, revealed massive pneumocephalus in the brain, pneumohemothorax, diffuse lung contusions with multiple traumatic lung cysts, air-fluid level in the cardiac chamber of the chest, and pneumoperitoneum in the abdomen. Postmortem external examination showed a circular abrasion on the jaw, diffuse subcutaneous emphysema, and contusion in the right upper arm. An internal examination revealed intravascular air bubbles in all four chambers of the heart, and diffuse pulmonary trauma including contusion, laceration, and multiple traumatic cysts. Blast injury to the chest, and air embolism due to the underwater explosion were established as the underlying cause of death.

PURPOSE: Since 1997, private postnatal care facilities (San-hu-jo-ri-won in Korean) have emerged to take the role of the family. As a result, neonates are now exposed to many people and are very vulnerable to infection. However, there has been no study on the influence of postnatal care facilities on neonatal infection. The aim of this study was to determine the risk factors of neonatal infection in full-term babies in Korea. MATERIALS and METHODS: We followed up 556 pregnant women and their babies for 4 weeks after their births at 2 hospitals in Seoul and Daejeon from October 2004 to September 2005. Among 512 full-term babies, 58 had infectious diseases. To determine the risk factors for infection, 53 infected neonates at 4-28 days of life and 413 healthy neonates were compared. RESULTS: The incidence of neonatal infection at 4 to 28 days after birth was 10.5%. After adjusting the related factors, the number of siblings (OR = 2.05, 95% CI = 1.13-3.71 for 1 or more) and postnatal care facilities or home aides (OR = 1.91, 95% CI = 1.07-3.45) were significant risk factors. Formula or mixed feeding (OR = 1.66, 95% CI = 0.91-3.04) increased the risk of neonatal infection but it was not statistically significant. CONCLUSION: When the newborns had siblings, stayed at postnatal care facilities, or were cared for by home aides, the risk of neonatal infections significantly increased. Further research on the feeding effect on neonatal infection and evaluation of prevention efforts are needed.

A case of Wegener's granulomatosis is described, with special attention focused on the typical histologic findings and involvement of both middle ear and lung. The patient is a 37-year-old man presented with four-month history of cough and sputum. He had a past history of surgery of both ears because of otitis media followed by left facial palsy. Chest radiographs showed variable sized ill defined nodules in both lower lobes with internal airspace consolidation. Histologic preparations of the open lung biopsy specimens demonstrated a diffusely scattered palisading micro and macrogranulomas with central focus of neutrophils and necrotic collagen surrounded by histiocytes, histiocytic giant cells. Fibrinoid necrosis involved blood vessels and lung parenchyma. Chronic inflammation, diffuse granulation tissue formation and irregular fibrosis are also found in the lung parenchyma. The histologic findings of middle ear which was previously biopsied showed scattered palisading ill defined microgranulomas mixed with fibrotic tissue.

Churg and Strauss established the clinicopathologic features of allergic angiitis and granulomatosis. The condition is rare and is characterized by involvement of vessels of all sizes(including veins and venules), especially the pulmonary vessel. intra- and extravascular granuloma formation, eosinophil infiltration of tissue and an association with asthma and peripheral eosinophilia. Churg Strauss syndrome (CSS) involves the GI tract, the skin, the heart, the lungs etc. We report a case of CSS in 38 year old male patient, who presented acute peritonitis and who had had 4-year history of bronchial asthma. An emergency laparotomy revealed multiple impending perforations in small intestine. A skin lesion and neuropathy developed postoperatively. Microscopic exammination of the appendix and the skin lesion showed thrombs in the lumens of vessels, vasculitis and eosinophilic infiltration to the lesion and the vessel wall.

BACKGROUND: In an effort to use cigarette warning labels as a means for reducing smoking rates, this study examined current cigarette warning messages and newly designed pictorial warning labels. METHODS: From June 15, 2005, to June 22, 2005 we conducted an e-mail survey of 1,200 civilians to examine the effect of cigarette warning labels. For statistical analysis, we used chi-square test with SPSS 11.0 software. RESULTS: Among all the respondents, 78.7% indicated that current cigarette warning labels did not have any effect. However, most of the respondents indicated that pictorial warning labels are effective in raising public awareness (84.6%) or knowledge (81.5%) of the harmful effects of cigarettes. They also responded that pictorial warning labels would help them quit smoking (75.9%) or keep them from buying cigarettes (64.1%). Over 80% of the respondents and over 70% of the smokers responded that they would welcome the use of pictorial warning labels as an anti-smoking measure. CONCLUSION: The results of this study showed that current cigarette warning labels are not effective in curbing smoking rates. Also, the results suggest that current cigarette warning labels need to be improved if they are to be used as an effective anti-smoking measure.
