1.The Effect of NutriPlus Program among 1-5 Year Children in Daejeon Area - The Improvement in Nutritional Status of Children and Nutrition Knowledge and Attitude of Parents -.

This study investigated the effect of nutritional improvement of 1-5 year children participated in NutriPlus program. The program was carried out at Public health Center, Daedeok-gu, Daejeon from March 2009 to April 2011. The subjects were selected among applicants for low-income family financing of the government and included 90 children and their parents. they were divided into 2 groups (less than 6 months and more than 6 months of participation in the study). We analyzed the effects of NutriPlus program and nutrition education in both study groups. The results of this study were summarized as follows: The number of children with less than 10th percentile in height and weight and weight for height was decreased. The anemia prevalence rate was reduced from 64.4% to 8.9%. NAR value of subjects increased for energy, protein, calcium, iron, vitamin A, riboflavin, niacin and vitamin C. The education program improved knowledge and attitude of the parents. In nutrition knowledge according to the income level, there were greater changes in the group of less than 100% compared to the subsistence. In nutrition attitude according to the education level, there were greater changes in the group of middle school graduates than in high school graduates or college graduates. There were no difference between less than 6 months group and more than 6 months group in improvement of hemoglobin level, anthropometric data and nutrients intakes. Based on these results, we conclude that providing supplementary food to children and nutrition education brought positive effects on growth of the children.

2.Ichthyosis Hystrix Treated by Vitamin A acid Ointment Application.

Ichthyosis hystrix is a localized variant of Bullous Congenital Ichthyosiform Erythroderma by Broq, and also, localized form of Epidermolytic hyperkeratosis according to Frost and Van Scott's classification of ichthyosi-form dermatoses. Clinically it is generally considered to be widespread systematized epidermal nevus, and demonstrating an autosomal dominant pattem of inheritance, and characterized by verrucous scale at or shortly after birth with particular involve- ment of flexural areas. A case of 21 year old girl with ichthyosis hystrix was reported, The skin lesion showed multiple rice to pea sized linear verrucous papules on left forearm, neck, axilla and upper chest. The skin biopsy was done, the finding shows remarked laminated hyperkeratosis, acanthosis, papillomatosis, and vacuoliization on malpighian layer. Diagnosis was confirmed by clinicaI apperance and histopathologicaI finding. The method of treatment is 0. 05% Vitamin A acid ointment application and show marked improvement.

3.Acute Myocarditis Complicated by Permanent Complete AV Block Associated with Salmonella Group B Gastroenteritis.

Acute infectious myocarditis in children can be caused by many pathogens, including bacteria, viruses, fungi and protozoa. Patients may be asymptomatic or may have 'flu-like symptoms' to life-theratening arrhythmias or sudden death. We herein report a case of acute myocarditis complicated by complets and permanent AV block, associated with Salmonella group B gastroenteritis in a previously healthy 5-year-old boy. Presenting symptoms were dyspnea, cyanosis, vomiting, and diarrhea. Electrocardiography on admission showed very slow ventricular escape rhythm at 37 beats/minute. He was treated with antibiotics, inotropic agents, ventilatory support, and transvenous temporary pacemaker. Ten days later, permanent dual chamber pacemaker was implanted. All three consecutive stool cultures done after admission grew Salmonella group B. His ventricular function recovered rapidly and completely and he became fully active 2 months after the onset. However, his conduction system was totally destroyed and at last follw up evaluation 2 years after the onset, he is still totally dependent on the pacemaker without any escape rhythm.

5.Expressions of p53 and MIB-1 in Glandular Lesions of the Uterine Cervix.

The glandular lesions of the uterine cervix can be classified into endocervical glandular dysplasia (EGD), adenocarcinoma in situ (AIS) and adenocarcinoma, but the diagnostic criteria and the continuity of endocervical glandular lesions are still controversial. The aim of this study was to evaluate the significance of immunohistochemical findings of p53 and MIB-1 in the discrimination and the continuity of EGD, AIS and adenocarcinoma. The materials for the study included 11 cases of adenocarcinoma, 7 cases of AIS, 12 cases of high grade EGD, and 19 cases of low grade EGD. Also included were eleven benign glandular lesions (5 cases of tuboendometrial metaplasia, 3 cases of mesonephric remnant, 3 cases of microglandular hyperplasia). A strong reaction of more than 5% of the glandular epithelial nuclei was interpreted as positive for p53 protein. MIB-1 expression was analyzed semiquantitatively as negative, 1 , 2 , 3 , depending on the percentage of positive nuclei (less than 1%, 1~9%, 10~39%, > or = 40%, respectively). p53 protein expression was found in 3 (27.3%) out of 11 cases of adenocarcinoma, and 2 (28.6%) out of 7 cases of AIS. But all of high and low grade EGD cases were negative. High MIB-1 labelling index (> or =10%) was found in all adenocarcinoma cases and in 3 (42.9%) out of 7 cases of AIS. But only 2 (17.7%) out of 12 cases of high grade EGD showed high MIB-1 labelling index, and all of low grade EGD and benign lesions showed negligible MIB-1 positivities. In summary, MIB-1 labelling index might be valuable in the discrimination of malignant glandular lesions and endocervical glandular dysplasia from benign lesions, but p53 expression could be a useful parameter in the discrimination of malignant glandular lesions from endocervical glandular dysplasia and benign lesions.

6.Changes of A2D Time as an Index of Diastolic Function of the Left Ventricle.

A2 D time, a time interval from aortic second heart sound to the D point of mitral valve echogram, was reported as a valuable index of the left ventricular relaxation in its early diastole. It was suggested, however, that A2D time is influenced by variable factors rather than single factor. This study was performed to evaluate the influences of several on the A2D time and to extend our understandings in A2D time. For this purpose, A2D time was measured in 4 groups whose hemodynamic states were different, i.e.31 normal subjects (group A), 32 patients with essential hypertension without decompensation (group B), 10 normotensive patients with clinically full blown congestive heart failure (group C), and 11 patients with hypertensive heart failure (group D), and the mean values of each group were compared between the groups. Concordantly systolic hemodynamic parameters were observed and the relationship of A2D time and each of these parameters were observed. A2D time was consistently influenced by the level of left ventricle impedance and it is well correlated with parameters representing left ventricular systolic performances. In hypertensive subiects, A2D time was increased before the deterioration of the left ventricular systolic function but shortened after clinical heart failure. These findings suggest that A2D time may be of value in longitudinal follow-up of the left ventricular function in the hypertensive patients even before the development of clinical heart failure.

7.The effect of transforming growth factor-beta on the expression of CD8 in the CTLL-2 cell line.

BACKGROUND: Transforming growth factor-beta(TGF-beta) has multiple regulatory effects on cells of the immune system, and it has been suggested that differentiation of lymphoid cells is influenced by low concentra tions of this cytokine. OBJECTIVES: The aim or this study was to investigate the role of TGF-beta in regulation of T cell growth and differentiation, and to compare this effect with that of other cell signals known to be important in T cell ontogeny. METHODS: We used the CTLL-2 cell line in the presence of IL-2. Surface phenotype expression was analysed to see whether these cells could be switched to the other subtype of cells. RESULT: Treatment of CTLL-2 cells with TGF-beta resulted in dose dependent growth inhibition and morphological changes. Curing routine passage, less than 5% of cells were CD8alpha positive, whereas 38% of cells expressed CD8alpha when treated with IL-2 plus TGF-beta. However, TPA plus calcium ionophore, IFN-gamma, or TNF-alpha caused no significant changes in the proportion of CD8 cells. CONCLUSION: Our results show that this experiment can be a useful model for investigating CD8 precursor potentials in populations of CD4-CD8-(double negative) cells, and such a model may offer a way to study the molecular regulation of CD8 gene expression.

Malignant fibrous histiocytoma, which is interesting because of its pleomorphic nature, is a rare soft tissue sarcoma possibly arising from histiocyte. The tumors are usually situated in the skin or subcutaneous tissues, especially of the extremities, but they may also involve deeper structures, such as the retroperitoneum or genital organs. Herin we report a case of malignant fibrous histiocytoma in the retroperitoneal space and discuss the pathologic and clinical features.

10.A case of disseminated superficial porokeratosis in immunosuppressed kidney transplant recipient.

Porokeratosis is an uncommon autosomally dominant inher ted disorder. Clinically, it is characterized by nonhealing plaques that develop most comnorly on the limbs. We report a case of disseminated superficial porokeratosis in immunosuppre sed kidney transplant recipient. An abrupt and extensive eruption of porokeratosis was observed in a 46-year-old man 7 months after renal transplantation, while being treated with cyclosporin A and prednisone. The histological features were essentially the same as the typical cornoid lamella. Immunosuppression may exacerbate or initiate the developm nt of porokeratosis in patients predisposed to alterations of cutaneous growth dynamics.
