1.MR Imaging Findings of Clonorchiasis.

OBJECTIVE: The purpose of this study was to evaluate the MR spectrum and MR cholangiographic imaging findings of clonorchiasis. MATERIALS AND METHODS: We reviewed 26 patients with confirmed clonorchiasis by either stool tests (n=24) or surgery (n=2). MR imaging was performed on a 1.5 T system (GE Medical Systems, Milwaukee, WI) with a torso coil. Axial T1-and T2-weighted, gadolinium-enhanced dynamic images and MR cholangiography were obtained. Image analyses were used to identify abnormalities of the intrahepatic and extrahepatic bile ducts and the presence of hepatobiliary malignancy. All MR examinations were reviewed by the consensus of two abdominal radiologists. RESULT: Intrahepatic bile duct abnormalities were seen in 23 (89%) of the 26 patients. The most common finding was mild dilatation of the intrahepatic bile duct which was found in 21 (81%). "Too many intrahepatic ducts" were found in 16 (62%), wall enhancement and thickening in 21 (81%) and filling defects and ductal stricture in the intrahepatic bile duct in 6 (24%) and 3 (12%) patients, respectively. Extrahepatic ductal dilation was found in 5 (19%) and 9 (35%) revealed hepatobiliary malignancy. CONCLUSION: MR imaging revealed various findings of clonorchiasis, including dilatation, wall enhancement, stricture of the intrahepatic ducts and filling defect within the intrahepatic bile duct.

3.Development of A New Course "Medicine : Science and Practice " in Premedical Program.

As a part of study on curricular renovation in the premedical program, a new course entitled "Medicine : Science and Practice" was developed in an otherwise traditional premedical education setting wi th hopeful linkage of two objectives aiming for effective delivery of traditional academic contents from individual disciplines and drawing of a strong motivation toward future medical carrier development. The course was divided into two parts : a series of lectures at the phase I (the second semesters of Year I) and a subsequent session of student-centered seminars in small group session at the phase II (the first semester of Year II ). The topics in lectures and seminars were selected among the more practical, contemporary and hot medico-social issues being specifically designed to meet the educational needs/concepts of "motivation for future medical carrier development", "students learn from students versus self-instruction" and "active participation in learning process". Integration of basic sciences/cultural subjects to professional medical sciences/practice was more emphasized together with increase of attention to the humanity and socio-econonmic courses by which cultivation of problem-solving ability is possible instead of the present science-focused approach. The small group study was persistently encouraged in phase II to develop their confidence in successfulness of self learning. The course was also specifically designed to promote the habit of critical thinking on current medical issuse, to stimulate learning of general educational disciplines by usage of medical phenomena. The details of curricular development and operational strategies were described with examples.

4.Advantage of the Presence of Living Dermal Fibroblasts within Restylane(R) for Soft Tissue Augmentation.

For the correction of facial wrinkles and skin contour defects, commercially prepared nonanimal stabilized hyaluronic acid(Restylane(R)) is now widely used. Although this suspension has been shown to be relatively safe and convenient to administer, varying degrees of resorption have required repeated percutaneous injection. Therefore, this study was undertaken to evaluate the feasibility of Restylane(R) combined with cultured human dermal fibroblasts to enhance the persistence of the injected implant. For the experimental group, fibroblasts from the dermis of healthy adults were isolated and cultivated. Five hundred thousand fibroblasts suspended in 200mul of Dulbecco's phosphate buffered saline(DPBS) were then dispersed in 200mul of Restylane(R) to form a human fibroblast mixed Restylane(R) that measured 400mul in volume. For the control group, 200mul of DPBS without fibroblasts was mixed with 200mul of Restylane(R) These implants were injected subdermally into the back of athymic nude mice at six sites; control group into the left three sites and experimental group into the right three sites. Twelve nude mice were injected to form total of 36 injections per group. The nodular swellings that resulted from the injections were excised wide enough to include skin beyond the swelling points down to panniculus carnosus layer using 5 mm punches, and the weights were measured at 1, 2, 4, 8, 12, and 16 weeks after the injections. Histologic comparisons were also made to confirm the presence of human collagen in the fibroblast mixed Restylane(R) group using immunohistochemical study with anti-human collagen type I polyclonal antibody. The mean weight of the control group nodules decreased throughout the examination period. The mean weight at 16th week was 60% of the weight at first week. On the other hand, the mean weight of the experimental group nodules decreased over the first 2 weeks only. Beyond 2 weeks, there was no further significant weight change. The mean weight at 16th week was 91% of the weight measured at first week. Histologic examination of the control group taken at each examination period exhibited negative immunohistochemical staining for human collagen. That of experimental group exhibited positive staining after 2 weeks, indicating the presence of human collagen. These results indicated that Restylane(R) mixed with cultured human dermal fibroblasts could be successfully injected as living grafts for long-term retention of the implants.

5.System for Collecting Biosignal Data from Multiple Patient Monitoring Systems.

OBJECTIVES: Biosignal data include important physiological information. For that reason, many devices and systems have been developed, but there has not been enough consideration of how to collect and integrate raw data from multiple systems. To overcome this limitation, we have developed a system for collecting and integrating biosignal data from two patient monitoring systems. METHODS: We developed an interface to extract biosignal data from Nihon Kohden and Philips monitoring systems. The Nihon Kohden system has a central server for the temporary storage of raw waveform data, which can be requested using the HL7 protocol. However, the Philips system used in our hospital cannot save raw waveform data. Therefore, our system was connected to monitoring devices using the RS232 protocol. After collection, the data were transformed and stored in a unified format. RESULTS: From September 2016 to August 2017, we collected approximately 117 patient-years of waveform data from 1,268 patients in 79 beds of five intensive care units. Because the two systems use the same data storage format, the application software could be run without compatibility issues. CONCLUSIONS: Our system collects biosignal data from different systems in a unified format. The data collected by the system can be used to develop algorithms or applications without the need to consider the source of the data.

7.Plasma Glucose, Insulin and C-Peptide in Essential Hypertension.

BACKGROUND: High blood pressure is prevalent in obesity and diabetes, especially noninsulin dependent diabetes mellitus, and both conditions are insulin resistant state. METHOD: To test whether resistance to insulin-stimulated glucose uptake and hyperinsulinemia are involved in the pochogenesis of hypertension, author measured glucose, insulin and C-Peptide reponse after oral glucose loading in 52 cases of essential hypertension and 62 cases of normal controls who had been admitted to the ward of internal medicine, Pusan National University Hospita. RESULTS: Basal plasma glucose, insulin and C-Peptide levels in control subjects were 92.1+/-36.8mg/dl, 8.7+/-5.5microu/ml and 2.2+/-1.8ng/ml and in hypertensive subjects were 95.7+/-32.6mg/dl, 12.2+/-5.3microu/ml and 2.9+/-1.6ng/ml. The basal insulin level was markedly higher than tat of control subjets (p<0.05). The basal glucose and C-Peptide levels in hypertensive patioents were higher than controls but statistically not significant. Plasma glucose levels in time course after glucose load in hypertensive patients showed significantly higher levels in 60,90minutes than controls. Plasma insulin levels in hypertensives in 90 minutes were significantly higher. The C-Peptide levels in hypertensives showed significantly higher in each times 30,60,90,120 minutes than controls. In hypertensive patients, body weight, blood pressure levels and duration of hypertension were not significantly correlated with responses of glucose, insalin and c-peptioce. Hypertensive patients aboce the age of 50 showed significantly higher glucose levels in 60,90,120 minutes than under age of 50. CONCLUSION: These results indicate some tendency of disturbed glucose turnover or insulin-resistant state in essential hypertension. This metabolic disturbance in essential hypertension should be considered in the management of hypertensive patients.

8.How Serious Is Erectile Dysfunction in Men's Lives? Comparative Data From Korean Adults.

PURPOSE: Whereas sexual function has long been assumed to be an important component of adult men's lives, the impact of sexual dysfunction has not been estimated in parallel to other modern disease entities. We compared the seriousness of erectile dysfunction (ED) with that of other diseases by use of self-administered questionnaires. MATERIALS AND METHODS: Between January 2012 and July 2012, 434 healthy male volunteers (group 1) and 263 ED patients (group 2) were enrolled. The questionnaire consisted of the following: "If you must undergo only one disease in all your life, which disease could you select among these items or ED?" The comparative disease entities included hypertension, diabetes mellitus (oral hypoglycemic agent/insulin injection), hemodialysis, myocardial infarction, herpes zoster, chronic sinusitis, chronic otitis media, gastric cancer (early/late), lung cancer (early/late), liver cancer (early/late), and dementia. RESULTS: Group 1 recognized ED as being a more serious disease than hypertension, diabetes mellitus (oral hypoglycemic agent), herpes zoster, chronic sinusitis, and chronic otitis media. In comparison, group 2 recognized ED as being a more serious condition than diabetes mellitus (insulin injection) and dementia (p<0.001 and p<0.001, respectively). In particular, ED was deemed to be more serious than hemodialysis, gastric cancer (early), lung cancer (early), and liver cancer (early) by men in group 2 in their 30s to 40s, and these results were statistically significant compared with the same age subgroups in group 1 (p<0.001, p<0.007, p<0.02, and p<0.007, respectively). CONCLUSIONS: In contrast with their healthy counterparts, Korean men with ED recognized ED as being as serious as hemodialysis, dementia, and early stage cancer, which reflects the severe bother of ED in Korean patients.

10.A Case of Cerebral Syndrome Associated with High-Dose Cytosine Arabinoside Chemotherapy.

Cytosine arabinoside (Ara-C) is a highly active agent in the treatment of acute leukemia and malignant lymphoma. The use of high-dose Ara-C therapy has been advanced as a possible means of overcoming resistance. High dose Ara-C regimens are occasionally associated with cerebellar and cerebral toxicities while toxicities in conventional dose regimens are mainly restricted to the bone marrow and gastrointestinal tract. A 21-year-old male patient with acute lymphoblastic leukemia in second relapse developed cerebral syndrome 10 days after high-dose Ara-C chemotherapy. We present a case of acute cerebral toxicity after high dose Ara-C chemotherapy with relevant literature review.
