1.Analysis of genes involved in the pathogenesis of intracellularly survival bacteria.

Eight bacterial strains were examined whether they have phoP/phoQ genes which were known to be involved in the intracellular survival of Salmonella typhimurium. The phoP/phoQ operon were known to sense the stimuli of the genes involved in the adaptation of the environment. Using 514-basepairs EcoRV DNA fragment of phoP region of Salmonella typhimurium as a probe, dot blot hybridization were performed. Chromosomal DNAs of Klebsiella pneumonia, Pseudomonas aeruginosa, Serratia marscescens, Enterobacter cloacae, Salmonella typhimurium, Escherichia coli, Shigella dysenteriae, and Listeria monocytogenes were examined by DNA hybridization assay. Against our expectation, intracellular pathogen, L. monocytogenes, did not have similar DNA sequences to phoP/phoQ of S. typhimurium, while E, coli, S. dysenteriae, and E. cloacae showed the positive signal even though they were not intracellular pathogens. This result suggested that the phoP/phoQ operon was absent in intracellular pathogenic bacterias other than S. typhimurium. Rather it was found in phylogenetically closer bacterias to S. typhimurium, which were not able to survive in intracellular environment. Some different mechanism, which is not dependent on phoP/phoQ operon, could be involved in the intracellular survival of L. monocytogenes.

The diagnosis of soft tissue osteochondroma should be considered when a well-defined osseous mass is located in the soft tissues. The differential diagnosis includes myositis ossificans, tumoral calcinosis, synovial chondromatosis, and soft tissue osteosarcoma, true osteochondroma which arises from bone. One case of soft tissue osteochondroma in the knee, a lesion of uncertain pathogenesis is reported.

4.A Clinical Study of the Treatment of Ruptures of Deltoid Ligament Associated with Fracture of Distal Part of Fibula

A survey of the literature on the treatment of ruptures of deltoid ligament associated with fracture of distal part of fibula is controversial. Some authors advocated surgical repair of the ruptured deltoid ligament based on the theoretical consideration, while others advocated non-operative treatment based on the clinical consideration. We studied the results in forty-five patients who were treated for disruption of the deltoid ligament and a distal fibular fracture. The length of follow-up in our series was twelve to twenty-four months, with an average of fifteen months. When the fibular fracture was adequately reduced and medial clear space was returned to its normal width, the 91 percents both of patients of being treated with repair of deltoid ligament and patients of being treated without repair had a good or excellent results.

5.The Effect of Alkylating Agent Cyclophosphamide on the Testis of Pubertal Rat.

We examined the effects of the cyclophosphamide on the testis of pubertal rats. The effects of drug on the pubertal testis were compared to the prepubertal and postpubertal experimental groups. The drug was administered intraperitoneally l00mg/kg weekly for 3 weeks and then two and four weeks later the experimental groups were sacrificed.Rats4,7 and 12weeks old were selected as the prepuberty, puberty and adult groups respectively.In the all experimental groups except the prepubertal group which was sacrificed at postinjection 2 weeks, the body weight was significantly decreased compared to the control group. In the pubertal and adult group the testis weights were decreased (puberty 0.94+/-0.07gm at 2 weeks, 1.48+/-0.03gm at 4 weeks, adulthood 1.52+/-0.07gm at 2 weeks, 1.76+/-0.06gm at 4 weeks) very significantly compared to that of the control group(puberty: 1.53+/-0.06 at 2 weeks, 1.7l+/-0.07gm at 4 weeks, adulthood: 1.83+/-0.08gm at 2 weeks, 1.99+/-0.07gm at 4 weeks, p<0.0l). The seminiferous tubular diameter of the puberty group was very significantly decreased (269+/-24um at 2 weeks, 275+/-21um at 4 weeks, p<0.01) and decreased significantly in the adult group( 278+/-20 m at 2 weeks, 289+/-18um at 4 weeks, p<0.05)compared to that of the control group(puberty 2 weeks 291+/-22um, 4 weeks 289+/-25um, adulthood 2 weeks 290+/-22um, 4 weeks 298+/-21um) Our results suggest that the prevention of testicular damage is needed in the anticancer chemotherapy of the reproductive age group because testicular damage is significant during pubertal age.

6.Modest Improvement of Untreated Severe Sleep-Disordered Breathing in the Middle-Aged and Elderly.

OBJECTIVE: It has been reported that untreated sleep-disordered breathing (SDB) deteriorates over time, however this remains contentious. The aim of the present study is to evaluate the clinical course of SDB in middle-aged and older SDB patients, and to identify how relevant factors contribute to the change in SDB severity. METHODS: Baseline and follow-up polysomnographic data of 56 untreated SDB patients (mean age, 61.2±5.71) were obtained retrospectively and the mean interval was 62.4±22.0 months. Subgroup analysis was performed based on the baseline severity, and the factors associated with the course of SDB were analyzed. RESULTS: At the baseline, 13 subjects were simple snorers, 15 had mild to moderate SDB, and 28 were severe SDB patients. While there was no significant change in apnea-hypopnea index (AHI) as a whole, subgroup analysis showed decrease of AHI in severe SDB patients (43.9±10.6 to 35.6±20.0, p=0.009). The change in supine time percent and baseline AHI were associated with the change in AHI (β=0.387, p=0.003; β=-0.272, p=0.037). CONCLUSION: Untreated SDB did not deteriorate over time with modest improvement in severe SDB. A proportion of severe SDB patients might expect decrease in SDB severity irrespective of changes in sleep position or body weight.

7.Acute Hepatitis after General Anesthesia with Enflurane.

We have experienced acute hepatitis after general anesthesia. We guess that acute hepatitis is caused by enflurane anesthesia. We will report this case with references. A 34 year old pregnant woman suffered from preeclamsia. She admitted for emergence repeat cesarean-section due to labor pain. In this operation general anesthesia was performanced with enflurane. The preoperative liver enzyme were normal and other tests were within normal range. She developed high fever, marked elevation of serum glutamic oxaloacetic transaminase(SGOT)/serum glutamic pyrubic transaminase(SGPT)/ alkaline phosphatase(ALP)/total bilirunbin(T-B) on the 2nd postoperative day. She recovered gradually and discharged after 13 day's hospitalization. A possible cause of the acute hepatitis in this case were considered to be the enflurane.

8.Intranodal Palisaded Myofibroblastoma with Desmin Expression : A Brief Case Report.

Intranodal palisaded myofibroblastoma is a rare benign mesenchymal neoplasm of the lymph node. It is characterized by intranodal spindle cell proliferation along with amianthoid fibers and prominent hemorrhage. It has been rarely reported in South Korea. We report here on a case of palisaded myofibroblastoma that arose in the left inguinal lymph node. The tumor mass was well demarcated, and it was composed of a proliferation of benign-looking spindle cells. It showed focal hemorrhage and a fibrous pseudocapsule. The tumor cells displayed little pleomorphism, no mitotic count, and characteristic palisading nuclei and amianthoid fibers. The tumor cells were positive for smooth muscle actin, vimentin, and also for desmin, but they were negative for S-100 protein, supporting the diagnosis of myofibroblastoma.

9.Awareness of occupational hazards and personal protective equipment use among dental hygienists

BACKGROUND: The aim of this study was to evaluate the awareness of occupational hazards and personal protective equipment use among dental hygienists (DHs).METHODS: A total of 271 self-administered questionnaires were obtained from 280 DHs working at dental hospitals or clinics in Daegu and Gyeongsangbuk-do, Korea.RESULTS: The occupational hazards included work involving dust (94.1%), volatile substances (86.0%), noise (97.0%), and light-curing units (96.7%). The proportion of dental hygiene tasks that participants perceived as harmful were 42.4%, 51.7%, 9.2%, and 31.4% in the same order as above. The proportion of participants who used dust-proof masks during work involving dust was 1.1%. Those who wore gas-proof masks and gloves for work using volatile substances were 0.7% and 31.2%, respectively. Participants who used goggles for work involving light-curing units were 31.0%. None of the participants used ear plugs for work involving noise. A total of 22.9% of the participants recognized the Material Safety Data Sheet, while 79.7% had never been educated about harmful work environments.CONCLUSION: When compared to exposure status and perception of occupational hazards, the level of protective equipment use was very low. Extra measures to increase DHs’ use of personal protective equipment are necessary.
