3.Clinical study of segmental vitiligo.

This is an analysia of 126 cases(14.1%) of segmental vitiligo among 892 vitiligo patient who had visited vitiligo special clinie in Severance Hospital. The results are summarized as follow : 1. There were 53 males(42.1%) and 73 females(57.9%). 2. The mean age of onset was 15.4 years, the mean age on the first visit was 19.3 years, and mean duration of the disease was 4.8 years. 3. Mode of onset was single in 86.5% and the disease was table in 57.1% of patients at the visit. 4. The mean percentage of depigmented lesions was 3.3% and less than 5% of body surface area was involved in 86.5% of patients. 5. The most common site of involvement was head and necl(59.6%), especially face(43.7%) and the trigerminal dermatome was most commonly involved. 6. Poliosis was observed in 39.7%. 7. Family history of vitiligo was obtained in 11.1% of patients. There was no precipitating factors in 126 cases prior to development of vitiligo. 8. Koebner phenomenon was found in 4% of patients. 9. Association with diseases of a proven or suggested allergic or immunologic etiology including atopic dermatitis, halo nevus, uveitis, thyroid disease, lopecia areata and premature graying of hair was found in 10.3% of patients.

4.A Study on the Effect of Various Dosage of Khellin on Epidermal Melanocytes and a Comparison with Trimethylpsoralen in Systemic Photochemotherapy.

Ninety black C 57 BL mice were divided into control and photochemotherapy groups. Control group was subdivided into a UVA 1rradiation only subgroup and a khellin only administration subgroup, while the phototherapy group was subdivided into one TMP and two khellin administration subgroups with dosages of 1.5mg/kg, 5mg/kg and 20mg/kg, respectivly. UVA was administered three thimes a week in a dose of 4J/cm for 4 weeks. Skin biopsies were taken at 0, l, 2, 3 and 4 weeks and the split DOPA stain was employed to observe pigment production. The number, size and circumference of the melanocytes were assessed. In summary, our results inclicate that khellin, which is slightly less effective than TMP at the same effective dose, is quite effective for pigment production and the degree of the production is dose related.

5.Two Cases of Prurigo Pigmentosa in Korean Women.

Although nearly 100 cases of prurigo pigmentosa have been reported in Japan, there has been no report in Korea. Recently, we exporienced two cases of prurigo pigmentosa in Korean women of 27 and 20 years of age. Histopathological findings of reddish papules showed exocytosis, inter and intra-cellular edema in the epidermis, liquefaction degeneration of the basal cell layei and perivascular round cell infiltration in the dermis. The specimens from the pigmented lesions show merely incontinence of pigments and mild perivascular round cell infiltration. We treated two patients with oral antihistamines and topical corticosteroids. One patient showed complete regression after 8 months of treatment but the other patient was lost for follow up.

6.Percutaneous Balloon Mitral Vavuloplasty under General Anesthesia in Patient with Mitral Stenosis and Schizophrenia.

Since the introduction of percutaneous mitral valvotomy(PMV) in 1984, PMV was an effecive alternative to surgical commissurotomy in selected patients with severe mitral stenosis. Also PMV was an excellent palliative strategy in such patients who were high risk for operative management or longterm anticoagulation was not feasible. We performed PMV in a patient with very tight mitral stenosis and severe pulmonary hypertension under the general anesthesia because the patient was anticipated to be uncooperative due to mental problem, who was diagnosed as schizophrenia 16 years ago. Robinol was used for premedication and i.v fentanyl was used for maintenance of anesthesia. Inoue balloon was introduced into the LV and gradual ballooning was performed with favorable results. Total anesthesia time and interval from internal jugular vein puncture to the completion of valvuloplasty were 1 hour 45 min and 40 minutes respectively. Hemodynamic variables were improved immediately after intervention and mitral valve area was increased from 0.5cm2 to 1.3cm2.

10.Vasodilator Therapy in Children with Congestive Heart Failure.

Patients with severe congestive heart failure due to rheumatic heart disease generally have both reduced cardiac output and increased pulmonary and systemic venous pressures. A study was therefore made of the use of vasodilator therapy with orally administered hydralazine, which acts on arteriolar resistance vessels and thus reduces afterload. 55 children aged from 4 years old to 15 year old were divided into two groups. Group I received oral hydralazine(4mg/kg/kay in 4 divided doses) in addition to digoxin therapy for mean duration of 10.5 months. Group II received oral digoxin only. The hemodynamic results using M-mode echocardiography, chest X-ray, and EKG between two groups were analyzed. 1) 4(78%) of 55 patients had only mitral valve involvement and another 12 patients had double valve involvement and all the patients belonged to functional class III or IV fo New York heart Association. 2) Left venticular end-diastolic dimensions and the height of R in V6 on EKG were not changed significantly in both groups. 3) Shortening fraction, LPEP/LVET, and RPEP/RVET were markedly improved in both groups, but the degree of improvement in group I was more significant than in group II. 4) 21(80%) of 26 patients in group I showed significant improvement in functional class(t=1.92, 0.05
