1.A Case of Multiple Basal Cell Carcinomas.

The multifactorial etiology of BCC(basal cell carcinoma) are incompletely understood, including factors leading to multiple lesions in some patients. Cases of multiple BCCs reported in the literature have been associated with UV irradiation, inorganic arsenic poisoning, X-ray therapy, hematologic malignancy and several genodermatoses such as nevoid BCC syndrome, albinism, xeroderma pigmentosum. Multiple BCCs presented on the sun-protected areas, lacking predisposing risk factors which have been known, is rare skin condition. However, it suggests that there may be an unknown genetic susceptibility to the development of multiple BCCs. We report a rare case of multiple BCCs, which had developed only on the sun-protected areas in 64-year-old man, without any known predisposing causes.

2.A Study on the Systolic Time Intervals in Normal Children.

The systolic time intervals were measured on 388 normal chidren, ranging from 8 months to 15years of age by using simultaneous recordings of the electrocardiogram, phonocardiogram and carotid or axillary arterial pulse. The statistical methods were applied to study the influence of age, sex and heart rate(HR) on these intervals. The results were as follows. 1. The regression equation of QS2was -1. 60HR+480+/-16 msec in males and -1.68HR+492+/-15 msec in females, respectively. 2. The regression equation of LVET was -1. 13 HR+359+/-10 msec in males and -1.21 HR+369+/-13 msec in females, respectively. 3. The regression equation of PEP was -0.58 HR+133+/-9 msec in males and -0.58 HR+128+/-9 msec in females, respectively. 4. The PEP/IVET ratio was 0.29+/-0.04 in males and 0.30+/-0.06 in females. 5. There was no sex difference in various systolic time intervals, and age had a slight influence but it was negligible for clinical use. 6. According to the regression equations of QS2, LVET, and PEP to the heart rate, QS2, and LVET were found to be more inversely correlated with heart rate than PEP.

3.A Study on the Effect of Various Dosage of Khellin on Epidermal Melanocytes and a Comparison with Trimethylpsoralen in Systemic Photochemotherapy.

Ninety black C 57 BL mice were divided into control and photochemotherapy groups. Control group was subdivided into a UVA 1rradiation only subgroup and a khellin only administration subgroup, while the phototherapy group was subdivided into one TMP and two khellin administration subgroups with dosages of 1.5mg/kg, 5mg/kg and 20mg/kg, respectivly. UVA was administered three thimes a week in a dose of 4J/cm for 4 weeks. Skin biopsies were taken at 0, l, 2, 3 and 4 weeks and the split DOPA stain was employed to observe pigment production. The number, size and circumference of the melanocytes were assessed. In summary, our results inclicate that khellin, which is slightly less effective than TMP at the same effective dose, is quite effective for pigment production and the degree of the production is dose related.

4.Two Cases of Prurigo Pigmentosa in Korean Women.

Although nearly 100 cases of prurigo pigmentosa have been reported in Japan, there has been no report in Korea. Recently, we exporienced two cases of prurigo pigmentosa in Korean women of 27 and 20 years of age. Histopathological findings of reddish papules showed exocytosis, inter and intra-cellular edema in the epidermis, liquefaction degeneration of the basal cell layei and perivascular round cell infiltration in the dermis. The specimens from the pigmented lesions show merely incontinence of pigments and mild perivascular round cell infiltration. We treated two patients with oral antihistamines and topical corticosteroids. One patient showed complete regression after 8 months of treatment but the other patient was lost for follow up.

Actinomycosis is a chronic suppurative and granulomatous disease characterized by peripheral spread and extension to contagious tissue, rare hematogenous spread, and the formation of multiple draining sinus tracts. In the rare case of hematogenous disseminated Actinomycosis; lesions appear in all parts of the body. Bone involvement, however, is very rare except the jaw and there has been only some reports about the ribs and vertebral columns. To our knowledge, this case represents the first report of documented distal femoral involvement.

7.Q-Switched Ruby Laser in the Treatment of Nevus of Ota.

BACKGROUND: The Q-switched ruby laser has recently shown that it can remove tattoos with-out scarring. Therefore the Q-switched ruby laser is expected to be effective in the treatment of nevus of Ota, which contains pigmented cells in the dermis. OBJECTIVE: The purpose of this study is to evaluate the efficacy of Q-switched ruby laser in the treatment of nevus of Ota. METHODS: Twenty-five patients (6 men, 19 women) with nevus of Ota on the face were treat-ed. The age of the patients ranged from 3 to 62 years. The energy fluence used varied from 6 to 8.5 J/cm². Treatment intervals ranged from 1 to 3 months, and the number of treatments ranged from 2 to 9. Photographs were taken of all patients before and on every return visit. The photographs were then compared with each other. After completion of treatment, we evaluated some parameters including the extent of pigment-free area within the skin lesions (score 0-4), degree of lightening in the remaining skin lesions (score 0-3), and patient's own or their parents' opinions on the result of treatment (score 0-3). The scores of the above three parameters were added and the resulting scores were designated as excellent (score 9-10), good (score 7-8), moderate (score 5-6) and poor (score 0-4). RESULTS: Excellent treatment response (score 9-10) was obtained in 6 of the 25 patients. In all patients disappearance of pigmentation on a 40 percent or larger area was seen. One month after completion of treatment, the remaining lesion size was less than 40 percent of the original lesion in 21 (84.0%) patients. The degree of lightening in the remaining lesion was marked (score 3) in 22 (88%) patients. No scarring was noted following treatment. Transient hyperpigmentation was noted in 7 patients, and transient hypopigmented areas were noted in one patient. CONCLUSION: Q-switched ruby laser appears to be an effective and safe modality for the treatment of nevus of Ota.

8.A Case of Carcinoma Erysipelatoides Originating in an Internal Organ.

We present a case of carcinoma erysipelatoides which or iginated from an internal organ. The patient was a 50 year-old female who had peritoneal carcinomatosis, She presented an erythematous lesion resembling erysipelas on her right breast. Histopathologic findings were consistent with metastatic adenocarcinoma but clinically it was not associated with carcinoma of the breast.

9.Paroxysmal Supraventricular Tachycardia in Infants and Children: Clinical Study of 36 Cases.

Thirty-six cases of paroxysmal supraventricular tachycardia which were observed at the Department of Pediatrics, Seoul National University Hospital from 1976 till October 1982, were analyzed. 1. The sex ratio was 1.8:1, male predominance. Sixteen cases (44.4%) were under 1 year of age and 11 cases (30.6%) were under 4 months of age. 2. Cases without known heart disease were 22(61.6%). Among Known etiology, most frequent one was 7 cases(19.4%) of W-P-W syndrome. 3. The sex ratio of group without known heart disease was 2.1:1, and that of group with known heart disease was 1.3:1. Under 4 months of age, there were more cases without known heart disease(72.7%) and after 4 months of age number of cases with and without known heart disease were similar. 4. Most frequent clinical manifestation was tachydyspnea. hepatomegly, irritability, palipitation, cyanosis and pallor, vomiting, chest pain, edema, abdominal pain, and fever were followed in order of frequency. 5. Congestive heart failure occurred in 22 cases(61%). The incidence was related to the duration of attack of tachycardia, age of patients, and heart rate. The longer the duration of attack and the younger the age of patients and the faster the heart rate, the higher incidence of heart failure was observed. 6. As treatment, vagal reflex stimulation was almost ineffective, and digitalization was effective in most of the cases. Verapamil and electrical cardioversion were tried in small number of cases and proved to be effective. 7. One case (2.8%) expired becase of the supraventricular tachycardia. 8. Eight cases (30.8%) had recurrence. In younger age group, recurrence rate was lower. 9. Digoxin was given in 15 cases for prevention of recurrence and was effective in 12 cases.

10.Histomorphologic Study on the Fate of Autogenous Fascial Graft of the Rabbit under Various Conditions.

The autogenous fascia graft has been used for various surgical purposes because it has good durability, a gliding capacity, elasticity and a high survival rate. However, few studies have reported histological changes of the graft in various situations. This study attempted to determine histomorphologic changes after autogenous fascia graft in various surgical conditions. The results revealed were as follows: 1. The tension applied to grafts plays the key role in the survival rate of autogenous fascia grafts. 2. Rolling the fascia reduced the volume and weight after autogenous graft due to fibrosis and central fatty degeneration. 3. Autogenous fascia graft covering silicone blocks showed its fascial structure and reduction of the formation of fibrous capsule around the silicone.
