Ankylosing spondylitis is a chronic inflammatory disease of unknown etiology that affects spine, sacroiliac joint, and the large joints of the extremities, and lead to fibrous or bony ankylosis and deformity. The rigid ankylosed spine of ankylosing spondylitis is prone to fracture by minimal trauma, and the resulting fractures likely to be unstable and damage to spinal cord or cauda equina. The authors report a case of paraplegia in ankylosing spondylitis after open reduction of the femoral trohanteric fracture.

4.Relationship Between Central Corneal Thickness and Retinal Nerve Fiber Layer Thickness in Glaucomatous Subject.

PURPOSE: To evaluate the correlation between central corneal thickness (CCT) and retinal nerve fiber layer thickness (RNFLT) as determined by optical coherence tomography (OCT) in glaucomatous subjects. METHODS: One hundred eyes diagnosed with glaucoma and 99 glaucoma suspect (GS) eyes were tested by visual field (VF), OCT, and ultrasonic pachymetry. The relationship between CCT and RNFLT measurements was assessed by Pearson correlation analysis. A mixed effect model was employed to determine the relationship between CCT and RNFLT in glaucoma and GS groups. We divided the patients into two groups depending on the thickness of their corneas: Thin (< 553.6 micrometer) and thick (> or = 553.6 micrometer), and compared three parameters: VF mean deviation (MD), pattern standard deviation (PSD), and RNFLT between the two groups. RESULTS: There were no significant correlations between CCT and RNFLT in any participant (R2=0.00, p=0.88). There was no significant relationship between CCT and RNFLT in glaucoma and GS groups (p=0.11, p=0.46). There were no statistically significant differences in MD, PSD, or RNFLT between the thin and thick cornea groups (p=0.38, 0.32, 0.44). CONCLUSIONS: CCT is not significantly associated with RNFLT in glaucoma and GS subjects.

5.A Clinical Evaluation on Adenomyosis at Hysterectomy.

OBJECT: In order to estimate the frequency and risk factors for adenomyosis. METHOD: The clinical records of 1127 women undergoing hysterectomy were retrieved in the Department of Obstetrics and Gynecology, Incheon Christian Hospital, during 7 years, from Jan. 1st 1991 to Dec. 31st 1997. RESULT: The following results were obtained. 1. Adenomyosis was found in 206 of 1127 patients(18.3%). 2. The highest incidence was 41-50 years of age group, an incidence of 55% and mean age group was 46.9 years. 3. Adenomyosis was more frequently observed in parous woman than non-parous woman, such as 8.3% and 91.7%, respectively. 4. Grossly, the size of uterus was enlarged more than 10 weeks gestational size in adenomyosis, an incidence of 62.4%. 5. Pelvic pain, dysmenorrhea and metrorrhagia were common symptom of adenomyosis, an incidence of 26.7%, 25.2% and 19.4%, respectively. 6. Myoma was the most combined disease in adenomyosis, showing the incidence of 53.4%. 7. Combined pelvic endometriosis was not observed in this study. 8. Endometrial findings of adenomyosis showed proliferative phase of normal endometrial cycle in the highest incidence, giving 72.3% of all cases. 9. Preoperative diagnostic accurracy of adenomyosis was 8.2%. CONCLUSION: This results show that deeply understanding of the common symptom and epidemiology of adenomyosis improve the preoperative diagnostic accuracy.

6.1 Case of Xanthogranulomatous Pyelonephritis.

Xanthogranulomatous pyelonephritis is characterized by multiple yellowish nodules and secondary inflammatory changes as necrotic area or abscess cavity grossly, and by infiltration of macrophage, mononuclear cell, and rarely giant cell microscopically. Also, this disease is a rare condition, which has been misdiagnosed as other renal disease such as renal tuberculosis or renal carcinoma of clear cell type in literatures. A case of xanthogranulomatous pyelonephritis is added to the literature.

7.Ipsilateral Cerebral and Contralateral Cerebellar Hyperperfusion in Patients with Unilateral Cerebral Infarction; SPM Analysis.

PURPOSE: Cortical reorganization has an important role in the recovery of stroke. We analyzed the compensatory cerebral and cerebellar perfusion change in patients with unilateral cerebral infarction using statistical parametric mapping (SPM). MATERIALS AND METHODS: Fifty seven (99m)Tc-Ethylene Cystein Diethylester (ECD) cerebral perfusion SPECT images of 57 patients (male/female=38/19, mean age=56+/-17 years) with unilateral cerebral infarction were evaluated retrospectively. Patients were divided into subgroups according to the location (left, right) and the onset (acute, chronic) of infarction. Each subgroup was compared with normal controls (male/female=11/1, mean age=36+/-10 years) in a voxel-by-voxel manner (two sample t-test, p<0.001) using SPM. RESULTS: All 4 subgroups showed hyperperfusion in the ipsilateral cerebral cortex, but not in the contralateral cerebral cortex. Chronic left and right infarction groups revealed hyperperfusion in the ipsilateral primary sensorimotor cortex, meanwhile, acute subgroups did not. Contralateral cerebellar hyperperfusion was also demonstrated in the chronic left infarction group. CONCLUSION: Using (99m)Tc-ECD SPECT, we observed ipsilateral cerebral and contralateral cerebeller hyperperfusion in patients with cerebral infarction. However, whether these findings are related to the recovery of cerebral functions should be further evaluated.

8.18F-FDG PET/CT in POEMS Syndrome.

POEMS syndrome is a rare disorder, also known as Crow-Fukase, PEP or Takatsuki syndrome. The acronym, POEMS, represents polyneuropathy, organomegaly, endocrinopathy, M protein and skin change. However, there are associated features not included in the acronym such as sclerotic bone lesions, Castleman disease, papilledema, thromobocytosis, peripheral edema, ascites, effusion, polycythemia, fatigue and clubbing. In most cases, osseous lesions in POEMS syndrome present as an isolated sclerotic deposit and that reveal as osteosclerotic myeloma. Several cases of 18F-FDG PET in multiple myeloma involvements were reported, but there was no previous literature that reported FDG PET findings in POEMS syndrome. We describe here a 66-year-old patient with POEMS syndrome who underwent 18F-FDG PET/CT image.

10.Influence on a Doctorless Island Residents' Health Care Utilization by Video Telemedicine.

PURPOSE: Video telemedicine system is efficient method of health care delivery that uses communication technologies via the internet and video-conferring to provide real-time video remote health provider, patient, or both. Video telemedicine system has the potential to improve access to the specialty of emergency medicine, particularly in rural and remote settings. Inadequate maintenance of ongoing health issues can result in high-cost care for both acute illness and long-term complications, whereas prevention of even a small percentage of these costs can pay for the cost of a video telemedicine system. METHODS: We visited a doctorless island on September 8th, 2006 and studied the public health status of the citizens, making charts while giving a free medical examination and treatment. We maintained and analyzed the charts for one year and then surveyed citizens to evaluate the effectiveness of video telemedicine system and to query them regarding future remedies. RESULTS: Out of 178 cases using video telemedicine system, 99 cases were the first medical examination and 12 cases were emergent, with 7 cases resulting in transfer to a mainland hospital. Of the patients transferred to the 3rd grade hospital, 41.4% had a heart problem. Thirty-eight cases were newly diagnosed via video telemedicine system, including 12 cases of vertebral disease (31.5%). 74.0% of citizens were satisfied with the video telemedicine system and among them 45.0% indicated satisfaction with the ease of seeing a doctor. After experiencing video telemedicine system, 100% of the citizens were agreed to continue video telemedicine system on the 'doctorless island', so more support and development is needed. CONCLUSION: Evidence suggests that a variety of video telemedicine system techniques can be applied effectively and safely to deliver emergency medicine across a variety of clinical settings. The utility and efficacy of telemedicine is discussed. Patient satisfaction is generally reported as high.
