1.The Accuracy of Self-Reported Weight and Height.

The use of self reported height and weight is very common in epidemiologic studies and clinical settings. In this study we assessed the accuracy of selfreported weight, height and body mass index by comparing with measured data. Data were obtained from 1065 persons(657 men, 358 women) who visited a health care center for routine medical check-up during the period September 1994-February 1995. The correlation between reported and measured height and weight was very strong. There were some discrepancies between two data, however, and men and women differ ed somewhat in their pattern of misreporting. Weight was overestimated by 0.27kg in men and underestimated by 0.09kg in wo men whereas height was overestimated in men(0.88cm) and women(2.15cm). These misreported data resulted in an underestimation of body mass index and low sensitivity for diagnosing overweight in men and women. The size of error was larger in women than in men. So it suggests that the self reported height and weight data could be used as a continuous variable with little error, but misclassification could be occured when the self reported data were used as a categorical variable. Errors in self reported weight and height, BMI were related to a person's over-weight status. The more people overweighed the more they underestimated their weight. Educational level and age, exercise, marri age had no relationship with the accuracy of self reported data. In conclusion, the use of self reported weight and height in epidemiologic studies and clinical settings could result underestimation of obesity, especially in obese female group.

2.In Vitro High-Resolution Sonography of Benign and Malignant Lymph Nodes: A Sonographic-Pathologic Correlation.

PURPOSE: In vitro sonography of lymph nodes was performed to ascertain the differential point between benignancy and malignancy. MATERIALS AND METHODS: Ninty-one lymph nodes were obtained at gastric cancer surgery and pathologically proved as benignancy in 68 and malignancy in 23 cases. Sohographic and pathologic correlation of resected ninty one lymph nodes was done in terms of size, the ratio of longest diameter versus shortest diameter, central hyperechogenicity, and peripheral cortical echo. RESULTS: Sonographic findings of benign lymph node were small size, oval shape(85%), narrow central high echogenicity(44%), and symmetric cortical thickening(41%). Sonographic findings of malignant lymph node were large size, round contour(39%), loss of central high echogenicity(82%), and diffuse increase of cortical thickness(82%). CONCLUSION: The most reliable sonographic criterion for differentiation of benignancy from malignancy was size of a lymphnode.

4.MR Findings of Cystic Acoustic Neurinomas.

PURPOSE: The purpose of this study was to evaluate the spectrum of MR characteristics of cystic acoustic neurinomas and to correlate with pathologic findings. MATERIALS AND METHODS: We retrospectively reviewed the MR findings of 12 patients with cystic acoustic neurinomas. The signal intensity and enhancement pattern of the cystic components as well as the location relative to the tumors were analyzed. RESULTS: A total of 30 cysts were found in 15 acoustic neurinomas;28 were intratumoral cysts and the remaining two were extratumoral cysts. The signal intensity of all 28 intratumoral cysts was higher than that of CSF on Tl-weighted images, and isointense to CSF on T2-weighted images, while the signal intensity of the two extratumoral cysts was same as CSF on both Tl-weighted and T2-weighted images. Rim enhancement was seen in 27 of 28 intratumoral cysts but in none of the two extratumoral cysts. CONCLUSION: We concluded that intratumoral cyst was due to tumoral necrosis, hemorrhage or degenerative change and extratumoral cyst due to secondary arachnoid cyst formation.

5.A Case of Sarcoidosis.

We report a case of sarcoidosis which developed in the skin, lung and eyes in a 50-year-old woman. The skin lesions showed two types of cutaneous manifestations which were subcutaneous nodules on her back and erytrematous papules on the face for several months. Our patient's seurm angiotensin converting enzyme (ACE) level was elevated at 33.5 IU/L(reference range, 8.3-21.4 IU/L) and the purified protein derivative(PPD) skin test had a negative result. On her roentgenographic examinations, we could find multiple nodular densities with hazziness on both her lungs and radionuclides(Gallium 67) uptakes in right lower lung field. The ratio of helper to suppressor T cells was 9 in bronchial lavage. Diagnosis of sarcoidosis was made on the basis of histologic findings of non-caseating granulomas from skin and lung tissue. Treatment was initiated with oral alternate-day prednisone 40mg. The cutaneous lesions and dyspnea improved in 2 and 4 weeks respectively, and serum ACE level returned to normal in 2 months.

6.Characteristics of Retinal Detachment in Pseudophakic Eyes.

To evauate the clinical characteristics of pseudophakic retinal detachments, 20 eyes were reviewed retrospectively. We identified the following characteristics of pseudophakic retinal detachment ; age in period of 40 (35%) and 60(30%), male(65%), rupture of posterior capsule (intraoperative and YAG laser capsulotomy)(50%). 15(75%) of the 20 pseduophakic retinal detachment eyes occured within 24months from the time of cataract surgery. Within 6 months after cataract surgery, the incidence of retinal detachment after posterior capsule rupture was higher than after uncomplicated cataract surgery, but there was no statistical significance. Retinal breaks were not detected in 7 eyes(35%). Retinal breaks were principally located in the upper quadrant in 6 eyes, horsehoe tear and atropic hole were most commonly encountered. More than 3 quadrant retinal detachment was observed in 80% of patients at the time of presentation and macular detachment, in 75%, Anatomic success rates were 85%. All of the failures were due to proliferative vitreoretinopathy.

7.Comparision of Clinical Findings between Phakic Retinal and Pseudophakic Retinal Detachment.

We reviewed our medical records of primary retnal detachment(239 patients, 243 eyes) to compare the clincal findings of phakic retinal detachment(206 eyes) and pseudophakic retinal detachment(37 eyes). Pseudophakic retinal detachment was more common in male(73.0%) than in female(27.0%) and occurred more frequently in aged 60 years or more than phakic retinal detachment(10.4%)(P<0.001). Associated ocular finding such as high myopia(35.0%), peripheral retinal degeneration(23.3%0, retinal tear in fellow eye(5.8%) were more commonly seen in phakic retinal retinal detachment. Rupture of posterior capsule(40.5%), high myopia(18.9%), small pupil(13.5%) were associate with pseudophakic retinal detachment. In pseudophakic retinal detachment, more than 3 quadrant retinal detachment(73.0%) and macular involvement(78.4%) were more common than in phakic retinal detachment(P<0.01), but atropic hole(29.7%), or 2 or more retinal break(10.8%) were less common than in phakic retinal detachment(P<0.01) where the success rate of first operation(59.5%) and the overall success rate(81.1%) were lower in paeudophakic retinal detachment than phakic retinal detachment. The degree of visual recovery was nearly equal between two.

8.Size of Pancreas in Non-insulin-dependent Diabetes Mellitus: A Study Based on CT.

PURPOSE: To evaluate changes of pancreatic size with aging in control subjects and in non-insulin-dependent diabetic patients. MATERIALS AND METHODS: Two groups of non-insulin-dependent diabetic patients were examined; one had been treated with an oral hypoglycemic agent(n=59), and the other with insulin(n=56). The CT findings of 175 patients without clinical evidence of pancreatic disease were included as a normal control. RESULTS: In control subjects, pancreatic size and age correlated. The pancreas was smaller in non-insulin-dependent diabetics than incontrol subjects and smaller in insulin- treated non-insulin-dependent diabetics than in non-insulin treated patients. CONCLUSION: The pancreas was smaller in non-insulin-dependent diabetic patients than in control subjects within the same age range.

10.Perinatal outcome of preterm infants born to severe pregnancy induced hypertension mothers with or without HELLP syndrome.

PURPOSE:The aim of this study was to compare the perinatal outcome of preterm infants born to severe pregnancy induced hypertension (PIH) mothers according to the presence or absence of HELLP syndrome. METHODS:A retrospective analysis was done in 314 premature live infants born to 302 severe PIH mothers who admitted at Chonnam National University Hospital from January 2002 to May 2008. Maternal and neonatal characteristics and their outcome were compared between the groups. Statistical analysis was performed by SPSS program using Student's t test, Chi-square analysis, Fisher's exact test, and logistic regression test. RESULTS:Among 302 severe PIH mothers (single pregnancy 270, twin pregnancy 32), 84 (27.8%) were associated with HELLP syndrome. Among their 334 fetuses (singleton 270, twins 64), total 20 (6.0%) resulted in fetal death and the rate of fetal death was higher in HELLP syndrome group compared to the control group (12.2% vs. 3.4%, P=0.006). Among the 314 live premature infants, 86 (27.4%) were born to severe PIH with HELLP syndrome (study group) and 228 (72.6%) were born to the control group. There were no differences in maternal and neonatal characteristics, maternal death, neonatal morbidity and neonatal death between the groups. However, the duration of mechanical ventilation (12.2+/-14.5 vs. 7.6+/-9.9 days, P=0.019) and hospitalization (25.6+/-12.3 vs. 13.0+/-10.5 days, P=0.013) were longer in the study group. CONCLUSION:Most of severe PIH mothers complicated by preterm birth, and especially combined with HELLP syndrome showed higher fetal and perinatal death than without HELLP syndrome. Moreover, their live-born premature infants needed longer duration of mechanical ventilation and hospitalization. These results indicate that careful fetal monitoring and delivery with the co-operations of obstetricians and neonatologists is essential to improve the perinatal outcome.
