2.Study on the Pigment Producing Effect of Various Concentrations of Trimethylpsoralen in Topical Photochemotherapy.

Topical photochemotherapy is effective for the treatment of vitiligo, However the effectiveness of photosensitizer, 4,5', 8-trimethyl-psoralen is not well known. For thesie reasons, we evaluated the pigment producing effect of various concentrations of TMP in fifty-six male black mice doing photochemotherapy. 1) The number of melanocytes according to the concentrations of TMP: The melanocytes were not increased in the control group. However, melanocytes were increased, in three TMP applied groups in the order of 0.1% 0.3% and 0.5% group at 1,2,3 weeks, respectively but the difference of melanocyte counts in the TMP applied groups was not remarkable at 4 weeks. 2) The area of melanocytes in relation to the concentrations of TMP: The area significantly increased from the second week of UVA exposure in the control group. There was marked increase in the area of the melanocytes in the TMP applied groups compared with those of control group in the order of 0. l%, 0, 3% and 0. 5% respectively. 3) The circumference of the melanocytes in relation to the Concentrations of TMP: Circumference of the control groups increased in proportion to the frequency of UVA exposure. TMP applied groups showed significant increase of the circumference compared that of the control group and generally increased in proportion to the increase in the concentration of TMP.

3.A Study of Combined Methotrexate-UVB Therapy for the Treatment of Psoriasis.

We evaluated the treatment schedule of a 4 week course of methotrexate followed by UVB radiation therapy on 18 patients of generalized psoriasis. The patients received the weekly triple doses of methotrexa,te(MTX), 5 mg given every 12 hours for three doses on weekend for 3 weeks. On 4 th weekend, the doses was halved to 2. 5 mg every 12 hours for three doses, and then stopped. After a 4 week course of methotrexate, total doses 52. 5 mg, UVB treatment was commenced. The results are as follows: 1. 13 patients showed clearing of psoriatic involvolvement over 50, after 4 week course of MTX therapy. 2. Complete clearing of skin lesions was achieved in all 18 patients with the average,"j, 8 times of UVB exposure in trunk and 5 6 times in extremities respect ively. 3. The average dose of UVB exposure at clearance was 187 mJ/cm in trunk lesion and 263. 6 mJ/cm in extremities, The mean cummulative dose of UVB exposure at clearance was 800. 7 mJ/cm in trunk lesion and 994. 3 mJ/cm in extremities. In conculuaion, the brief 4 week course of MTX allows for clearing of psoriasis at relatively low dose of UVR, and thus may reduce the long-term cumrnulative toxicity of both agents.

4.A Case of Sparganosis, Formed Giant Hematoma in Scrotum.

Sparganum infestation in human body was reported over 20 cases in Korea. Infestation of sparganosis in spermatic cord was reported a case in Korea. Three cases of sparganosis at Keu Je island in Korea was reported by Dr. Weinstein, Krawczyc and Peers 1954. We experienced a case of the sparganum infestation in scrotum. The patient, 41 year-old male who was suffering with pulmonary tuberculosis moderate advanced, visited 3rd Army Hospital urologic clinic complained with acute swelling of the scrotum in baby head size abruptly without any specific causes. severe tenderness and walking disturbance. The laboratory findings revealed no specific findings except ascaris ova in stool. Operation revealed massive hematoma formation and proliferated connective tissue where noted several infested parasites in length of 120cm, 75cm, 70cm around the tunica vaginalis in scrotum Extracted parasites revealed milkish-whitish color, flat, fragile, and tendon like glistening appearance. There is not any specific sign and symptoms by parasite infestation on the other organs. Patient had an episode of intake of raw snake as a tonic 9 years ago at Keu Je island in Korea.

5.Sensitivity of Electrodiagnostic Parameters in Patients with Asymptomatic Diabetic Neuropathy.

OBJECTIVE: To determine the sensitivity of electrodiagnostic parameters in the patients with asymptomatic diabetic neuropahty. METHOD: The subjects were 26 patients with asymptomatic diabetic neuropathy and 40 healthy adults as control group. All subjects underwent electrodiagnostic evaluation of the following motor nerves: median, ulnar, tibial, and peroneal. Sensory nerves included: median, ulnar, radial, superficial peroneal, sural, lateral dorsal cutaneous branch of the sural nerve (LDSN) and medial plantar. And other studies were the sural/radial amplitude ratio, LDSN/sural amplitude ratio, peroneal and tibial F-responses, and H-reflex recorded from the soleus muscle. The frequency of abnormal parameters in the patients with asymptomatic diabetic neuropathy was obtained by comparison with the normative limits obtained from the control group. RESULTS: The most frequent abnormal electrodiagnostic parameters were the LDSN onset latency and the amplitude ratio of LDSN/sural (84.6%, respectively) followed by the LDSN peak latency, LDSN amplitude, and medial plantar onset and peak latency (80.8%, respectively). CONCLUSION: We concluded that the LDSN and medial plantar nerve conduction studies are useful for early detection of neuropathy in diabetes mellitus.

7.Diagnosis of Carpal Tunnel Syndrome by Diagnostic Ultrasound.

OBJECTIVE: To evaluate the usefulness of the diagnostic ultrasound (US) to diagnose carpal tunnel syndrome (CTS) and the correlation between electrodiagnosis and US findings. METHOD: Forty hands of 30 patients diagnosed with CTS by electrodiagnosis and 28 hands of 19 controls were examined with US. The 7.5 MHz probe of the US was used to view the median nerve in the carpal tunnel. The short and the long axis and the area at the two points, 2 cm proximal and 1 cm distal to the distal wrist crease were measured. The flattening and compression ratio and the ratio of the area in both groups were analysed. The correlation between the eletrodiagnostic severity and compression ratio were analyzed. RESULTS: The compression ratio of CTS was significantly increased comparing with that of control group. The compression ratio of severe CTS was significantly increased comparing with that of mild and moderate CTS. CONCLUSION: These results suggest that US is useful in diagnosis of CTS.

8.Comparison between Focal Laser Photocoagulation and Intravitreal Bevacizumab Injection for Treating Chronic Central Serous Chorioretinopathy

Purpose:
We compared the efficacy of focal laser photocoagulation and intravitreal bevacizumab injection in the treatment of chronic central serous chorioretinopathy.
Methods:
We conducted a retrospective analysis involving 46 eyes of 46 patients with chronic central serous chorioretinopathy who received focal laser photocoagulation (22 eyes) or intravitreal bevacizumab injection (24 eyes). Changes in best-corrected visual acuity (BVCA), central macular thickness, and rates of complete subretinal fluid (SRF) resolution and recurrence were compared over a 6-month period.
Results:
Although the focal laser photocoagulation and intravitreal bevacizumab injection groups demonstrated a significant reduction in central macular thickness at 6 months (p < 0.001), no statistically significant differences were observed between these groups at the end of the follow-up period. Notably, a gradual improvement in BVCA was observed in both groups following initial treatment, with significant improvement seen at 6 months (p = 0.049 and p = 0.048 for the laser and injection groups, respectively). However, no statistically significant difference in BVCA improvement was observed between the groups. Complete SRF resolution was achieved in 22 eyes in the laser group and 23 eyes in the injection group. Although recurrence of SRF was observed in 9 and 13 eyes in the laser and injection groups, respectively, there was no statistically significant difference between the groups (p = 0.229).
Conclusions
Focal laser photocoagulation and intravitreal bevacizumab injection significantly improved anatomical and visual acuity outcomes in patients with chronic central serous chorioretinopathy. Although focal laser photocoagulation was associated with more rapid visual recovery and a lower recurrence rate compared to intravitreal bevacizumab injection, further studies are needed to evaluate its potential complications.
9.Quantitative Measurement of Current Perception Threshold in Carpal Tunnel Syndrome.

OBJECTIVE: To compare current perception threshold with nerve conduction study, we measured current perception threshold (CPT) in healthy control and patients with carpal tunnel syndrome (CTS). METHOD: Twenty control subjects and twenty patients with CTS were included. Latency and amplitude of median and ulnar motor and sensory nerves were measured. The sensory current perception threshold was measured at the distal interphalangeal joint of third and fifth fingers and the palm with electrical current of 5 Hz, 250 Hz, and 2,000 Hz in frequency. We compared the results of the nerve conduction study with the data of the CPT. RESULTS: We found that measuring of the sensory threshold might detect carpal tunnel syndrome, especially with 2,000 Hz and 250 Hz stimulation and that CPT data correlated to sensory latency and amplitude of the median nerve. CONCLUSION: The sensory threshold test might be useful for diagnosis and follow up test in carpal tunnel syndrome.

10.Gamma Knife Radiosurgery Using Co-Registration with PET-CT and MRI for Recurrent Nasopharyngeal Carcinoma with Previous Radiotherapy : A Single Center 14-Year Experience

Objective:
: We retrospectively evaluated the efficacy of Gamma Knife radiosurgery (GKS) for recurrent nasopharyngeal carcinoma (NPC) in patients who previously underwent radiotherapy, and analyzed the treatment outcomes over 14 years.
Methods:
: Ten patients with recurrent NPC who had previously received radiotherapy underwent stereotactic radiosurgery using a Gamma Knife® (Elekta Inc, Atlanta, GA, USA) between 2005 and 2018. The median target volume was 8.2 cm3 (range, 1.7–17.8), and the median radiation dose to the target was 18 Gy (range, 12–30). The median follow-up period was 18 months (range, 6–76 months). Overall and local failure-free survival rates were determined using the Kaplan-Meier method.
Results:
: The NPCs recurred at the primary cancer site in seven patients (70%), as distant brain metastasis in two (20%), and as an extension into brain in one (10%). The recurrent tumors in seven of the 10 patients (70%) were found on the routine follow-up imaging studies. Two patients presented with headache and one with facial pain. Local failure after GKS occurred in five patients (50%) : two of whom died eight and 6 months after GKS, respectively. No adverse radiation effects were noted after GKS. The 1- and 3-year overall survival rates after GKS were 90% and 77%, respectively. The local failure-free survival rates at 6 months, 1 year, and 3 years after GKS were 80%, 48%, and 32%, respectively. The median interval from GKS to local failure was 8 months (range, 6–12). Univariate analysis revealed that using co-registration with positron emission tomography-computed tomography (PET-CT) and magnetic resonance imaging (MRI) was associated with a lower local failure rate of recurrent NPC (p=0.027).
Conclusion
: GKS is an acceptable salvage treatment option for patients with recurrent NPC who previously received radiation therapy. PET-CT and MRI co-registration for dose planning can help achieve local control of recurrent NPC.
