4.A Case of Sparganosis, Formed Giant Hematoma in Scrotum.

Sparganum infestation in human body was reported over 20 cases in Korea. Infestation of sparganosis in spermatic cord was reported a case in Korea. Three cases of sparganosis at Keu Je island in Korea was reported by Dr. Weinstein, Krawczyc and Peers 1954. We experienced a case of the sparganum infestation in scrotum. The patient, 41 year-old male who was suffering with pulmonary tuberculosis moderate advanced, visited 3rd Army Hospital urologic clinic complained with acute swelling of the scrotum in baby head size abruptly without any specific causes. severe tenderness and walking disturbance. The laboratory findings revealed no specific findings except ascaris ova in stool. Operation revealed massive hematoma formation and proliferated connective tissue where noted several infested parasites in length of 120cm, 75cm, 70cm around the tunica vaginalis in scrotum Extracted parasites revealed milkish-whitish color, flat, fragile, and tendon like glistening appearance. There is not any specific sign and symptoms by parasite infestation on the other organs. Patient had an episode of intake of raw snake as a tonic 9 years ago at Keu Je island in Korea.

6.Combination Chemotherapy with High Dose Cisplatin - Cyclophosphamide in Primary Epithelial Ovarian Cancer.

OBJECTIVE: The purpose of this study was to evaluate the therapeutic efficacy and toxicity of high dose cisplatin-cyclophosphamide combination chemotherapy on patients with primary epithelial ovarian cancer. METHODS: A review of 63 patients previously diagnosed as primary epithelial ovarian cancer after initial operation and histology at Pusan National University Hospital from Jul. 1993 to Jun, 1997 was performed. Patients were received the combination chemotherapy including cisplatin 100mg/m2/day and cyclophosphamide 750mg/m2/day, repeated 6 cycles every 4 weeks. The mean age was 48 years old, and previous surgical procedures were total abdominal hysterectomy and bilateral salpingo-oophorectomy with omentectomy. The patients were classified into FIGO stage and pathologic results. RESULTS: The clinical response rate was 100% in the FIGO stage Ic patients with PC combination chemotherapy, 100% in stage II, 53.5% in stage III, and 25% in stage IV. The overall response rate was 69.8%. The 3-year survival rate according to the treatment groups was 93.3% in stage Ic group, 60% in stage II, 50% in stage III and 0% in stage IV. The mean survival duration was 34.6 months. Hematologic toxicities in cisplatin-cyclophosphamide chemotherapy were neutropenia and anemia. Nausea and vomiting were the most common side effects and occurred in 96.8%. Most of the toxicities were grade 1 and 2. CONCLUSION: The combination chemotherapy with cisplatin-cyclophosphamide is relatively safe and effective method in the treatment of primary epithelial ovarian cancer.

10.A Clinical Analysis of the Scapula Fractures

Fracture of the scapuls is one of the rare bony injuries seen in hospital practice. Authors reviewed the records and the roentgenograms of forty three patients from April 1976 to March 1987. The results were as follows : l. Of the 43 cases, peak incidence was in the middle aged men with high activity, and age range was 30 to 50 years. 2. The most common cause of injury was the motor vehicle accident. 3. Twenty eight cases of the scapular fracture occurred in the body, and eleven had fractures more than two parts in the scapuls. The most common type was the combined fracture of the body and neck. 4. A severe force was required to produce the injury and 79% of the patients had combined injuries of head and chest. 5. Delay in diagnosis was frequent because of the masking effect of the associated injury. 6. Although results of conservative measurements on the scapular fracture are generally good, we feel that treatment on more active line should be undertaken in young and active patients with fracture involving glenoid.
