1.Clinical Observation of Sulfobenzylpenicillin(Sulbencillin) in Urinary Tract Infection.

The effectiveness of Sulfobenzylpenicillin in the treatment of urinary tract infection was evaluated in the Department of Urology, Seoul National University Hospital during the period from May to November, 1973. Our clinical study was performed on the 28 patients who visited the outpatients clinic and admitted to the ward. These patients were studied by bacterioscopy with Gram's method of staining. and culture in blood agar plate and chocolate agar plate In this studies. the causative organisms were divided into 8 groups; E. coli (5), coagulase positive staphylococcus (5), pseudomonas (2), Corynebacterium spp. (1), a-hemolytic etreptococcus (2), N. Gonorrheae (10), Proteus (2) and KIebsiella (1). The dosage schedule was 1 gm. of Sulfobenzylpenicillin at intervals of 12 hours for consecutive 5 days in gonococcal urethritis and nongonococcal urethritis. But in upper urinary tract infections and post-operative infections, it lasted for 7 to 10 days or more. And following results were obtained. Effectiveness on Urological diseases 1) Gonococcal urethritis; Satisfactory clinical results were obtained with Sulfobenzylpenicillin for five days. Results were excellent in 5 cases, good in 4 cases and negative effect in 1 case. 2) Non-gonococcal urethritis; In this group, the therapeutic results were favorable in 90% of all cases. (excellent-4, good-5, fail-l) 3) Urethral stricture and fistula (2 cases); AII cases were responded to the Sulbenicillin. 4) Cystitis including Prostatism; Two of three cases were caused by E. coli and they had a good response to sulbenicillin. 5) Two cases of pyelonephritis and two cases of postoperative infections were studied with sulbenicillin and more than 60% of cases were effective with sulbenicillin. Side effects were not remarkable but in a few cases who received intramuscular injection, pain on the injection site was noticed.In summary, this new semi-synthetic penicillin is considered to be a useful chemotherapeutic weapon against a variety of infection in the urological field including those due to Gram negative bacilli such as pseudomonas and proteus sp.

4.Brain Magnetic Resonance Imaging Findings in Children withCerebral Palsy: Compared between Preterm and Term Groups.

PURPOSE: We used MRI to retrospectively analyze the brain of patients suffering from cerebral palsy. Our aim is to determine MRI's role in the assessment of brain damage, the relationship of gestational age. METHODS: A total of 66 patients(29 preform group and 37 term group), who visited Kang-Dong Sacred Heart Hospital from January, 1994 to July, 1998, were enrolled in this study. RESULTS: Among the 29 in the preform group, 13 patients showed MR images of hypoxic ischemic injury in which periventricular leukomalacid(PVL) and multifocal ischemic necrosis in 12(41.3%) and 1(3.4%) respectively. Neuronal migration disorders were 6(20.8%), other congenital malformations 5(17%) and normal MR images 5(17%) in this preform group. Among the 37 in the term group, 22 patients showed MR images of hypoxic ischemic injury in which selective neuronal necrosis were 11(29.7%), PVL 4(10.8%), focal and multifocal ischemic necrosis 4(10.8%) and status marmoratus 3(8.1%). Neuronal migration disorders were 4(10.8%), other congenital malformations 5(13.5%) and normal MR images 6(16.2%) in the term group. CONCLUSION: MRI provided useful information in a majority of children with cerebral palsy. Hypoxic ischemic injury was significantly different in preform and term groups. PVL was frequent in the preterm group and selective neuronal necrosis was statistically common in the term group.

6.Arterial Priapism: Superselective Embolization using Autologous Blood Clot.

A case of arterial priapism was treated by superselective transcatheteric autologous clot embolization of cavernosal artery 25 days after blunt periheal trauma. No immediate improvement was noted but the priapism subsided on 48th day after the procedure. Normal physiclogic erectlie function was achieved on 53rd day after the procedure, and the period of postprocedure impotence was shortened, as compared with previously reported cases. Autologous blood clot embolization may be an effective treatment method even in delayed priapism, and superselective technique is thought to be more effective to shorten the postprocedure erectlie dysfunction

8.A Case fo Hypertrophic Lichen Planus.

A case of hypertrophic lichen planus was reported and the literature reviewed. A 69-year-old widowed woman displayed generaIized discrete, multiple, round, various sized, flat-topped, hypertrophied papules and nodules on the abdomen, trunk and four extrimies for 42 years. An electron microscopic finding revealed widespread breakdown and thickening of the basal lamina at the dermo-epidermal junction and a degenerating change of the desmosomes. No particular intran uclear particles or bodies were discovered. We suggest that emotional factors and autoimmune mechanism might be the cause of the eruption in this case.

9.Posterior Chamber Intraocular Lens Implantation in front of the Anterior Lens Capsule during Pars Plana Lensectomy and Vitrectomy.

Many patients present simultaneously with visually significant (to the patient and surgeon) lens opacities and vitreoretional disease. Traditional management of this problem has included two operations: vitrectomy followed by cataract extraction or cataract extraction followed by vitrectomy. Visual rehabilitation of aphakic eyes after pars plana vitrectomy is often difficult when anatomic objectives are achieved. Intraocular lens provides more natural and convenient vision. Now, simultaneous cataract extraction and lens implantation during pars plana vitrectomy can be considered. We performed posterior chamber lens insertion in the ciliary sulcus after completion of pars plana lensectomy, vitrectomy, and removal of intraocular foreign body. During pars plana lensectomy, we left the anterior lens capsule and zonules intact. After posterior chamber lens insertion, we removed the central anterior lens capsules from behind the optical component leaving a peripheral rim to support the haptics. The procedure provided good pseudophakic vision.
