3.Effects of 12 Weeks High-Speed Elastic Band Training on Cognitive Function, Physical Performance and Muscle Strength in Older Women with Mild Cognitive Impairment: A Randomized Controlled Trial.

BACKGROUND: Exercise has shown to slow down the progression from mild cognitive impairment (MCI) to dementia. In particular, not only resistance exercise has been verified to improve physical performance and muscle strength, leading to the prevention of fall-related injuries in the frail elderly, but also cognitive function. In this study, we investigated how high-speed elastic band training, applied using a high-speed power training protocol, affects cognition, physical performance and muscle strength of female elderly individuals with MCI. METHODS: Thirty-nine subjects diagnosed with MCI from one senior college using a Korean version of mini-mental state examination (MMSE-K) and a Korean version of the Montreal Cognitive Assessment (MoCA-K) were randomly divided into exercise group (n=20) and control group (n=19). The exercise group completed a 12-week high-speed resistance elastic band exercise program and was encouraged to exercise at home as well during the intervention period. RESULTS: The results showed significant increases in cognitive function, physical performance and muscle strength assessed by digit span forward test (P<0.001), digit span backward test (P=0.003), the Stroop test B (P=0.031), SPPB scores (P=0.010), right knee 60degrees extensor peak torque (P=0.004), 60degrees flexor peak torque (P=0.001), 180degrees extensor peak torque (P=0.020) and grip strength (P=0.025). CONCLUSIONS: Our study showed that participants with MCI who continued the high-speed elastic band training program for 12 weeks showed improvements in cognitive function, physical performance and muscle strength.

4.Relations of Self-Esteem with Paranoia in Healthy Controls, Individuals at Ultra-High Risk for Psychosis and with Recent Onset Schizophrenia.

OBJECTIVES: Some emotional components, such as self-esteem, depression and anxiety, have been reported to be associated with paranoia in non-clinical population and schizophrenia patients. The aim of this study was to investigate the correlation between self-esteem and paranoia in healthy controls, in ultra-high risk for psychosis and schizophrenia patients. METHODS: 34 subjects with recent onset schizophrenia, 36 subjects with ultra-high risk for psychosis, and 44 healthy volunteers participated in this study. A detailed assessment was made of the paranoia, self-esteem, depression, and anxiety. RESULTS: In all three groups, there were a negative correlation between paranoia and self-esteem, and positive correlations between paranoia and depression and anxiety. In healthy control, lower self-esteem showed a trend to predict higher paranoia, and in ultra-high risk for psychosis, this trend tern on statistically significant level, and in recent onset schizophrenia group, this correlation was disappeared. CONCLUSION: The individual who have lower self-esteem showed higher paranoia tendency under delusional level, but after formation of persecutory delusion, the tendency was disappeared. This result supports the hypothesis that persecutory delusions are a defense against negative affective process.

5.The Influence of Halothane Anesthesia on the T - lymphocyte Subpopulations.

There are substantial evidences indiacting that anesthesia and surgery produce significant suppression of immune function. It is generally recognized that immunosuppressive manifestations such as lymphopenia, granulocytosis, decreased transformation to various antigens and impaired neutrophil chemotaxis could be followed by surgical operation. anesthetic agents may also have both direct and hormone-mediated indirect effects on immune systems. Recent evidences indicate that immunosurveillance mechanisms are not only important in the management of emerging infection and malignancy, but also critical to successful elimination of microscopic residual tumor after operation. This study was done to investigate the effeets of halothane anesthesia on T-lymphocyte subpopulations in patients undergoing hysterectomy. Lymphocyte, granulocytes and T-lymphoeyte subpopulations were counted before, during and after anestesia. The results showed that the decrease in the CD4+ cells (helper/inducer T-cells) was bigger than the decrease in the CD8+ cells (suppressor/ cytotoxic T-cells). CD3+ cells (total T-cells) and CD45R+ cells were also decreased. However, these decrease turned to normal values in 5 days. The overall result of this study suggest that anesthesia with halothane may bring temporal suppressive effect on the immunologic responses.

6.Somatosensory Evoked Potential and Nerve Conduction Studies in Diabetic Patients.

OBJECTIVE: This study aims the electrophysiological documentation of possible neurological abnormalities in diabetic patients with or without neuropathy symptoms. METHOD: Forty five diabetic patients, 15 male and 30 female, were included in this study. They were divided into symptomatic and asymptomatic groups and received various electrophysiologic studies including a nerve conduction study, F-wave study and median and tibial SSEP study. The clinical parameters were the clinical symptom and sign of neuropathy, disease duration, complications, HbA1c, and fasting blood sugar. Statistical significances of the parameters were observed between symptomatic and asymptomatic groups. RESULTS: The most sensitive electrophysiologic parameter was the tibial SSEP. For the documentation of diabetic neuropathy, the electrophysiologic study of posterior tibial, median, superficial peroneal and sural nerves were most useful. F-wave study did not reflect the early involvement of proximal nerve segment in diabetic patients. CONCLUSION: Multimodal neurophysiological approaches including a tibial SSEP study rather than the conventional nerve conduction studies can depict a broader and more complete map of the possible abnormalities of diabetic neuropathy.

7.The Clinical Experiences of Induced Hypotension with Halothane and Trimetaphan combined with Halothane for Cerebral Aneurysm Surgery .

Induced hypotensive anesthesia is well known anesthetic method. It reduces bleeding into surgical field and provides better visibility for the surgery of cerebral aneurysm. The most current method for induced hypotensive anesthesia is composed of general anesthesia and ganglionic blockade. This study was performed to compare the clinical experience from hypotensive anesthesia with halothane only to hypotensive anesthesia with trimetaphan and halothane. The results of our study were as follows; 1) There are no different technical difficulties during hypotensive anesthesia between halothane induced cases and trimetaphan used cases. 2) No clinical evidence of direct organic damage because of hypotensive anesthesia was found. 3) The prognosis of postoperative recovery may deeply related to preoperative physical state than anesthesia. To conclude through this experience, induced hypotensive anesthesia is a useful method for the cerebral aneurysmal surgery. The technique of deliberate hypotension is not too difficult to use in ordinary equiped hospital.
